Acetylsalicylic acid for metal stent in malignant distal common bile duct obstruction:A randomized controlled trial

Background:Endoscopic biliary drainage is the treatment of choice for patients with malignant distal common bile duct ***-expandable metal stents have clinical advantages including an increased duration of patency that may be prolonged by acetylsalicylic acid(ASA)*** aim of this study was to investigate whether ASA had a positive effect on the patency of self-expandable metal stents compared with ***:This prospective,multicenter,double-blinded,and randomized placebo-controlled trial was conducted from October 2017 to May 2020 in *** who underwent palliative endoscopic biliary drainage with self-expandable metal stents for malignant distal bile duct obstruction were enrolled,and allocated to ASA treatment or *** study outcomes were the rate of stent dysfunction at 6 months,duration of stent patency,risk factors for stent dysfunction,and any adverse ***:Interim analysis included 24 and 28 patients in the ASA and placebo groups,*** was no significant difference between the ASA and placebo groups in stent dysfunction(25.0%vs.20.7%,P=0.761)or the duration of stent patency(150.97±10.55 vs.158.07±8.70 days,P=0.497).Six patients experienced suspected ASA-related adverse events,and there was one lethal ***:ASA did not prolong stent *** study was terminated early because of the possibility of serious adverse events related to ASA treatment of these patients receiving palliative care.

目的观察γ-酮醛清道夫水杨胺(salicylamine,SAM)对尿毒症ApoE-/-小鼠动脉粥样硬化(atherosclerosis,AS)及其表型的影响。方法ApoE-/-小鼠通过5/6肾切除术建立尿毒症模型,对照组仅行假手术。实验动物分为对照组、尿毒症组及尿毒症SAM干预组,分别以SAM或安慰剂干预6周。干预结束后检测体质量、血压、血脂、肾功能、血清SAM浓度、主动脉AS斑块面积及斑块组分。结果与对照组相比,尿毒症组主动脉AS病变面积明显增大,AS斑块组分中坏死面积增大,巨噬细胞含量增多,胶原含量减少。SAM干预6周,尿毒症小鼠AS损伤面积、AS斑块中坏死面积及巨噬细胞含量减少,胶原含量增加,但不影响体质量、血压、血脂和肾功能,未发生药物蓄积和毒性作用。结论SAM能减轻尿毒症ApoE-/-小鼠加速发展的AS病变并使斑块的稳定性增加,这些作用不依赖于对血压、血脂或肾功能的影响,同时不引起药物蓄积及毒性作用。

Folfirinox chemotherapy prolongs stent patency in patients with malignant biliary obstruction due to unresectable pancreatic cancer

Background:Stent insertion for biliary decompression to relieve jaundice and subsequent biliary infection is necessary for patients with biliary obstruction caused by pancreatic cancer,and it is important to keep the stent patent as long as ***,few studies have compared stent patency in terms of chemotherapy in patients with pancreatic *** study aimed to evaluate the differences in stent patency in terms of recently evolving ***:Between January 2015 and May 2017,161 patients with pancreatic cancer who had undergone biliary stent insertion with a metal stent were retrospectively *** relationship between chemotherapy and stent patency was ***,overall survival according to the treatment,risk factors for stent patency,and long-term adverse events were ***:Median stent patency was 42 days for patients with the best supportive care and 217 days for patients with chemotherapy(conventional gemcitabine-based chemotherapy and folfirinox)(P<0.001).Furthermore,the folfirinox group showed the longest median stent patency and overall survival,with 283 days and 466 days,respectively(P<0.001)despite higher adverse events *** who underwent folfirinox chemotherapy after stent insertion had better stent patency in multivariate analysis(HR=0.26;95%CI:0.12–0.60;P=0.001).Conclusions:Compared with patients who received best supportive care only,patients who underwent chemotherapy after stent insertion had better stent *** prolonged stent patency can be expected for patients with folfirinox than conventional gemcitabine-based chemotherapy.

In this paper, we develop a mathematical model of the COVID-19 pandemic in Burkina Faso. We use real data from Burkina Faso National Health Commission against COVID-19 to predict the dynamic of the disease and also the cumulative number of reported cases. We use public policies in model in order to reduce the contact rate, this allows to show how the reduction of the daily report of infectious cases goes, so we would like to draw the attention of decision makers for a rapid treatment of reported cases.

Clinical and pathological differences between serum immunoglobulin G4-positive and -negative type 1 autoimmune pancreatitis

AIM: To identify clinical and pathological differences between serum immunoglobulin G4 (IgG4)-positive (SIP) and IgG4-negative (SIN) type 1 autoimmune pancreatitis (AIP) in South Korea. METHODS: AIP was diagnosed by the international consensus diagnostic criteria. The medical records and pathology were retrospectively reviewed and IgG4-positive cells were counted in a high power field (HPF). Type I AIP was defined as a high serum level of IgG4or histological finding. SIN type 1 AIP was defined as a histological evidence of type 1 AIP and a normal serum IgG4 level. The clinical and pathological findings were compared between the two groups. The analysis was performed using Student's t test, Fischer's exact test and Mann-Whitney's U test. A P value of < 0.05 was considered statistically significant. As repeated com- parison was made, P values of less than 5% (P < 0.05) were considered significant. RESULTS: Twenty five patients with definite type 1 AIP (19 histologically and six serologically diagnosed cases) were enrolled. The mean tissue IgG4 concentrations were significantly higher in SIP than SIN group (40 cells per HPF vs 18 cells per HPF, P = 0.02). Among eight SIN patients, the tissue IgG4 concentrations were less than 15 cells per HPF in most of cases, except one. The sensitivity of serum IgG4 was 68% (17 SIP and eight SIN AIP). Other organ involvement was more frequent- ly associated with SIP than SIN AIP (59% vs 26%, P = 0.016). However, the relapse rate and diffuse swelling of the pancreas were not associated with serum IgG4 level. The concentrations of IgG4-positive cells per HPF were higher in SIP than SIN AIP (40 vs 18, P = 0.02). CONCLUSION: The sensitivity of serum IgG4 was 68% in type 1 AIP. High serum IgG4 level was associated with other organ involvement and tissue IgG4 concentration but did not affect the relapse rate in type 1 AIP.

＜正＞We consider a multi-input multi-output （MIMO） communication system equipped with a linear minimum mean-square error （MMSE） receiver. We design a full-rate linear space-time block code （STBC） for blocks of data that span several realizations of a block-static fading channel to achieve high diversity gain. The proposed code structures are necessary and sufficient to minimize the detection error probability. They are information lossless when analyzed from information theory view point. They also enable the detection computations to be partitioned into separate blocks such that the normalized complexity is reduced to little more than that for a single block. The additional complexity grows only linearly with L, the number of blocks for which the code design covers. Analysis shows that the diversity gain achieved by the code also grows linearly with L.

Costing of Malaria treatment in a rural district hospital

Objective: It is the aim of this paper to estimate the provider’s cost of treating paediatric cases of Malaria in a rural African hospital. Furthermore, we intend to give some insights into the possibilities of improving the efficiency of treating children with this disease in order to support policy makers in the resource allocation process. Methods: The cost analysis was done in the district hospital of Nouna, Burkina Faso. Based on a comprehensive cost-of-illness information system, the cost of treating paediatric Malaria in the district hospital in the year 2005 were estimated using a combination of top- down and activity-based costing. It divides the entire treatment process into a set of activities along the clinical pathway and allocates monetary values for the resource consumption to each activity. Results: The average actual provider’s cost were 6.74 US$ for a paediatric outpatient with Malaria, 61.08 US$ for a paediatric Malaria inpatient with anaemia and, respectively 74.29 US$ for a case of paediatric Malaria with neurological affection. 54% of the cost was due to laboratory work. This high unit cost was mainly due to a severe underutilisation of the hospital capacity. The current cost recovery rate per case was between 18% and 43%. It would be between 32% and 73% if the occupancy increased to 80%. Conclusion: The paper demonstrates that detailed costing is possible in a district hospital in rural Africa. The unit cost seems to be extra-ordinary high and the share of laboratory cost is tremendous. However, this is mainly due to a very strong underutilisation of the existing capacities. This fact calls for intensive efforts of the management of the institution to attract more patients by improving the quality of services and in particular the satisfaction of the patients.

Development of Predictive QSPR Model of the First Reduction Potential from a Series of Tetracyanoquinodimethane (TCNQ) Molecules by the DFT (Density Functional Theory) Method

In this work, which consisted to develop a predictive QSPR (Quantitative Structure-Property Relationship) model of the first reduction potential, we were particularly interested in a series of forty molecules. These molecules have constituted our database. Here, thirty molecules were used for the training set and ten molecules were used for the test set. For the calculation of the descriptors, all molecules have been firstly optimized with a frequency calculation at B3LYP/6-31G(d,p) theory level. Using statistical analysis methods, a predictive QSPR (Quantitative Structure-Property Relationship) model of the first reduction potential dependent on electronic affinity (EA) only have been developed. The statistical and validation parameters derived from this model have been determined and found interesting. These different parameters and the realized statistical tests have revealed that this model is suitable for predicting the first reduction potential of future TCNQ (tetracyanoquinodimethane) of this same family belonging to its applicability domain with a 95% confidence level.
