BAcKgROUND:Unexplained stillbirth and the sudden infant death syndrome (sIDs)share some features.A raised maternal serum level of alpha-fetoprotein during the second trimester of pregnancy is a marker of placental dysfunction and a strong predictor o f the risk of unexplained *** is unknown whether alpha -fetoprotein levels also predict the risk of ***:We linked a prenatal -screening database for wo men in western scot-land with databases of maternity,perinatal death,and birth and death certifications to assess t he association between second -trimester levels of maternal serum alpha -feto-protein and the subsequent risk of sI ***:A-mong 214,532women with singleton bi rths,there were114cases of sIDs(incidence,2.7per 10,000births a-mong women with alpha -fetoprotein l evels in the lowest quintile and 7.5per 10,000births am ong those with levels in the highest quintile).When the lowest quintile was used as a referent,the unadjusted odds ra tios for sIDs for the second through fifth quintiles were1.7(95percent confi-dence interval,0.8to 3.5),1.8(95percent confidence interval,0.9to 3.7),2.5(95percent confidence inter-val,1.3to 4.8),and 2.8(95percent confidence inter-val,1.4to 5.4),respectively(P for trend =0.001).The risk of sIDs varied inversely with th e birth -weight per-centile and the gestational age at delivery;after adjustment for these factors,the odds ratios fo r sIDs were 1.7(95percent confidence interval,0.8to3.5),1.7(95percent confidence interval,0.8to 3.5),2.2(95percent confi-dence interval,1.1to 4.4),and 2.2(95percent confi-dence interval,1.1to 4.3),respectively(P for trend =0.01).cONcLUsIONs:There is a direct asso ciation be-tween second -trimester maternal se rumalpha -fetoprotein levels and the risk of sIDs,which may be mediated in part through impaired fetal growth and pr eterm birth.

Objective: The purpose of this study was to develop a predictive model for preeclampsia. study design: This was a prospective screening study for preeclampsia using uterine artery Doppler ultrasound in unselected low-risk singleton pregnancies at community hospitals in the UK (n = 32,157). Logistic regression models were developed and their predictive ability assessed using the area under the receiver operator curve (AROc). Results: six hundred twelve (2.0% ) women developed preeclampsia, and 144 (0.5% ) required early delivery (s). A model using both maternal and ultrasound factors had an AROc of 0.798, which was higher than ultrasound alone (0.729, P ctors alone (0.712, P set disease, the AROc of ultrasound alone (0.922) was not significantly improved by adding maternal predictors (0.945, P = .27). Hepatocellular carcinoma recurrence after liver transplant:An Australian single-centre study

BAcKgROUND Hepatocellular carcinoma(Hcc)is a leading cause of cancer-related deaths *** transplantation(LT)offers the most effective *** recurrence is the strongest risk factor that decreases post-LT survival in patients transplanted for *** rate of Hcc recurrence is generally reported as 8%-20%in the *** predictors of Hcc have already been researched,however,to our knowledge there are no published studies on this topic using Australian *** To determine the rate and identify predictors of Hcc recurrence in a contemporary Western Australian LT *** We performed a retrospective cohort study of all liver transplants in patients with Hcc at sir charles gairdner Hospital between 2006 and *** was collected from various health record databases and included recipient demographics,serum biochemistry,radiology,operation notes,explant histopathology and details of *** survival of Hcc patients post-LT,stratified for recurrence,was calculated by Kaplan Meier *** and multivariate cox regression was used to determine predictors of Hcc recurrence *** Between 1/1/2006 and 12/31/2021,119 patients were transplanted with Hcc.8.4%of subjects developed recurrent Hcc after LT with median follow-up time of 5.4 *** median time to recurrence was 2.9 years±0.75 *** comparing baseline characteristics,a greater proportion of subjects with recurrence had common characteristics on explant histopathology,including>3 viable nodules(P=0.001),vascular invasion(P=0.003)and poorly differentiated Hcc(P=0.03).Unadjusted survival curves showed lower 1-year,3-year,5-year and 10-year survival rates in subjects with Hcc recurrence compared to those without Hcc recurrence(90%vs 92%,70%vs 88%,42%vs 80%,14%vs 76%,respectively;log rank PcONcLUsION Hcc recurrence was low at 8.4%in this contemporary Australian cohort,however it significantly impacted post-LT *** studies are requir

co,Ni,and Zn iso-reticular coordination polymers of 2,5-dihydroxyterphthalic acid(H_(2)dot)can be rapidly obtained under room temperature and aqueous conditions using either sodium or ammonium hydroxide as the bases to deprotonate the *** use of metal bromides and tetrabutylammonium hydroxide(TBAOH)as the base allows the green synthesis of ionic metal-organic frameworks(MOFs),characterized by X-ray diffraction(XRD),Fourier transform infrared(FTIR),scanning electron microscopy(sEM),thermo gravimetric analysis(TgA),H-nuclear magnetic resonance(NMR),elemental analysis,and X-ray photoelectron spectroscopy(XPs),and employed in the pressure,solvent,and additive-free cO_(2)fixation to epoxides by its cycloaddition to epichlorohydrin,exhibiting a cooperative behavior between the MOF and the ionic liquid formed within the framework,achieving TONs of the confined tetrabutylammonium bromide(TBABr)up to 700.

Incidence of brain metastasis in patients with esophageal carcinoma

AIM:To determine the incidence of brain metastasis in a contemporary group of patients with carcinoma of the ***:Retrospective analysis of 53 patients with esophageal carcinoma who received radiotherapy as a component of treatment between 1998 and 2007,including patient and tumor characteristics,and subsequent diagnosis of brain *** association between the histological type of esophageal cancer and the incidence of brain metastasis was assessed using Fisher's exact ***:Forty-four of the fifty-three patients in this study had adenocarcinoma and nine had squamous cell carcinoma,ranging from stage ⅡA-Ⅳ*** treatment was surgery with neoadjuvant chemoradiotherapy (trimodality therapy) in 19% of patients;chemoradiotherapy in 42%;and surgery and adjuvant radiotherapy in 7%.Twenty-five percent of patients inthis study received palliative *** overall incidence of brain metastasis in this cohort was 13%.Adenocarcinoma was the primary tumor histology in all of the patients who developed brain metastasis,representing an incidence of 16% in this *** patients with squamous cell carcinoma received trimodality *** association between histology and brain metastasis was not statistically ***:The incidence of brain metastasis in this contemporary cohort of patients with esophageal carcinoma is higher than previously reported and was confined to those with adenocarcinoma.

The myth of “minima” and “maxima”,the species of Physalis in the Indian subcontinent

The status of the names, Physalis minima L. and P. maxima Mill. (solanaceae), and their alleged presence on the Indian subcontinent are discussed. The issues of nativity and identity of Linnaean Physalis minima are long-debated while the use of the name P. maxima Mill. and its report from India are recent. The available evidence indicates that the name "P. minima L." is misapplied to two different elements, viz., P. angulata L. and P. lagascae Roem. & schult. The name Physalis minima L. may be rejected as nomen confusum, for which the paper provides the primary information. As on today, it is submerged under the synonymy of P. angulata L. The correct name for the widely known P. minima is P. lagascae. The name "P. maxima Mill." applied to the escape and naturalized weed in the Indian subcontinent and elsewhere is to be substituted by P. pruinosa L., a name misapplied to P. grisea (Waterf.) M. Martínez.
