BACKGROUND The Ankle Spacer was developed as a joint-sparing alternative to invasive endstage ***,there are no clinical studies on the Ankle *** To describe the operative technique and the clinical efficacy of the Ankle Spacer for the treatment of multiple,cystic osteochondral lesions of the talus in patients with failed prior operative *** This is a prospective study during which patients were assessed preoperatively,at 2-and 6 wk,and at 3,6,12 and 24 mo *** with multiple,cystic or large(≥15 mm)osteochondral lesions of the talus after failed prior surgery were *** primary outcome measure was the numeric rating scale(NRS)for pain during walking at 2 years *** outcome measures included the NRS in rest and during stair climbing,the American Orthopaedic Foot and Ankle Society Hindfoot Score,the Foot and Ankle Outcome Score,the Short-Form 36 physical and mental component scale,and the Range of Motion(ROM).Radiographic evaluations were conducted to evaluate prosthetic loosening and *** rates and complications were also *** Two patients underwent an Ankle Spacer implantation on the *** NRS during walking improved from 6 and 7 preoperatively to 2 and 2 points postoperatively at 2 years,in patient 1 and 2,*** other patient-reported outcome measures also improved *** were no re-operations nor *** imaging showed no loosening of the implant and no change of implant *** The Ankle Spacer showed clinically relevant pain reduction during walking,improvement in clinical outcomes as assessed with PROMs,and no complications or *** treatment option may evolve as a joint-sparing alternative to invasive end-stage surgeries.

BACKGROUND The use of uncemented cups during total hip arthroplasty(THA)has gained popularity in recent *** Robert Mathys(RM)pressfit cup,an uncemented monoblock implant is expected to preserve bone density due to its composition and external surface,while reducing backside wear with its monoblock *** factors should lead to a high survival rate of the *** To evaluate the mid-term survival and functional outcome of the RM Pressfit cup in a large study *** Between 2011 and 2020,we included 1324 patients receiving a primary THA using the RM pressfit *** clinical follow-up was performed at 2 years postoperatively with the Dutch arthroplasty register used to assess implant status *** for acetabular failure and reason for revision were reported to evaluate implant survival,while the hip disability and osteoarthritis outcome score(HOOS)scores were used to assess functional *** The mean age at surgery was 64.9 *** mean follow-up was 4.6 *** the 1324 THAs performed,13 needed cup revisions within 5 years after index THA:5 due to aseptic loosening,6 due to infection,2 due to dislocation and 2 due to other *** resulted in a 5-year cup survival of 98.8%(95%CI:98.1-99.5).Nine of the cup revisions occurred within the first year after index *** scores increased significantly in all domains during the first year and levelled out during the second *** In the present study,the RM pressfit cup demonstrated excellent clinical outcomes at mid-term follow-up;however,future studies are needed to assess the long-term outcomes of this acetabular implant.

Optimisation of Thermal Comfort of Building in a Hot and Dry Tropical Climate: A Comparative Approach between Compressed Earth/Concrete Block Envelopes

Compressed earth blocks (CEB) are an alternative to cement blocks in the construction of wall masonry. However, the optimal architectural construction methods for adequate thermal comfort for occupants in hot and arid environments are not mastered. This article evaluates the influence of architectural and constructive modes of buildings made of CEB walls and concrete block walls, to optimize and compare their thermal comfort in the hot and dry tropical climate of Ouagadougou, Burkina Faso. Two identical pilot buildings whose envelopes are made of CEB and concrete blocks were monitored for this study. The thermal models of the pilot buildings were implemented in the SketchUp software using an extension of EnergyPlus. The models were empirically validated after calibration against measured thermal data from the buildings. The models were used to do a parametric analysis for optimization of the thermal performances by simulating plaster coatings on the exterior of walls, airtight openings and natural ventilation depending on external weather conditions. The results show that the CEB building displays 7016 hours of discomfort, equivalent to 80.1% of the time, and the concrete building displays 6948 hours of discomfort, equivalent to 79.3% of the time. The optimization by modifications reduced the discomfort to 2918 and 3125 hours respectively;i.e. equivalent to only 33.3% for the CEB building and 35.7% for the concrete building. More study should evaluate thermal optimizations in buildings in real time of usage such as residential buildings commonly used by the local middle class. The use of CEB as a construction material and passive means of improving thermal comfort is a suitable ecological and economical option to replace cementitious material.

CD4^(+)T cells can"help"or"license" conventional type 1 dendritic cells(cDC1s)to induce CD8^(+)cytotoxic T lymphocyte(CTL)anticancer responses,as proven in mouse *** recently identified cDC1s with a transcriptomic imprint of CD4^(+)T-cell help,specifically in T-cell-infiltrated human cancers,and these cells were associated with a good prognosis and response to PD-1-targeting ***,we delineate the mechanism of cDC1 licensing by CD4^(+)T cells in *** CD4^(+)T cells produce IFNβvia the STING pathway,which promotes MHC-I antigen(cross-)presentation by cDC1s and thereby improves their ability to induce CTL anticancer *** cooperation with CD40 ligand(L),IFNβalso optimizes the costimulatory and other functions of cDC1s required for CTL response ***-I-producing CD4^(+)T cells are present in diverse T-cell-infiltrated cancers and likely deliver“help”signals to CTLs locally,according to their transcriptomic profile and colocalization with“helped/licensed”cDCs and tumor-reactive CD8^(+)T *** agreement with this scenario,the presence of IFN-I-producing CD4^(+)T cells in the TME is associated with overall survival and the response to PD-1 checkpoint blockade in cancer patients.

Nonlinear roll damping of a barge with and without liquid cargo in spherical tanks

Damping plays a significant role on the maximum amplitude of a vessel’s roll motion,in particular near the resonant *** is a common practice to predict roll damping using a linear radiation-diffraction code and add that to a linearized viscous damping component,which can be obtained through empirical,semi-empirical equations or free decay tests in calm ***,it is evident that the viscous roll damping is nonlinear with roll velocity and *** liquid cargo motions inside cargo tanks also contribute to roll damping,which when ignored impedes the accurate prediction of maximum roll *** this study,a series of free decay model tests is conducted on a barge-like vessel with two spherical tanks,which allows a better understanding of the nonlinear roll damping components considering the effects of the liquid cargo *** examine the effects of the cargo motion on the damping levels,a nonlinear model is adopted to calculate the damping *** liquid cargo motion is observed to affect both the linear and the quadratic components of the roll *** flow memory effect on the roll damping is also *** nonlinear damping coefficients of the vessel with liquid cargo motions in spherical tanks are obtained,which are expected to contribute in configurations involving spherical tanks.

Effect of viscosity on the avalanche dynamics and flow transition of wet granular matter

The dynamic behaviour of granular flows is important in geo-mechanics and industrial applications,yet poorly *** studied the effects of liquid viscosity and particle size on the dynamics of wet granular material flowing in a slowly rotating drum,in order to detect the transition from the avalanching to the continuous flow regime.A discrete element method(DEM)model,in which contact forces and cohesive forces were considered,was employed to simulate this flow *** model was validated experimentally,using glass beads in a wooden drum and water–glycerol mixtures to tune the liquid *** DEM simulations showed comparable results to the experiments in terms of average slope angle and avalanche *** observed that the avalanche amplitude,flow layer velocity and granular temperature decrease as the liquid viscosity *** effect is more pronounced for smaller sized *** increase in viscous forces causes the flowing particles to behave as a bulk,pushing the free surface towards a convex *** addition,avalanches become less pronounced and the granular flow transitions from the avalanching regime to the continuous *** avalanching flow regime is marked by intermittent rigid body movement of the particulate bed and near-zero drops in the granular temperature,while no rigid body movement of the bed occurs in the continuous flow *** identified the avalanching-continuous flow transition region as a function of a dimensionless granular Galileo number.
