Treatment of Helicobacter pylori in surgical practice:A randomised trial of triple versus quadruple therapy in a rural district general hospital

AIM: To compare a lansoprazole-based triple versus quadruple therapy for Helicobacter pylori (H pylori) eradication with emphasis on side effect prof ile,patient compliance and eradication rate at a rural district general hospital in Wales,United Kingdom. METHODS: One hundred one patients with H pylori infection were included in the study. Patients were randomised to receive triple therapy comprising of lansoprazole 30 mg,amoxycillin 1 g,clarithromycin 500 mg,all b.d. (LAC),or quadruple therapy comprising of lansoprazole 30 mg b.d.,metronidazole 500 mg t.d.s.,bismuth subcitrate 240 mg b.d.,and tetracycline chloride 500 mg q.d.s. (LMBT). Cure was defi ned as a negative 13C urea breath test 2 mo after treatment. RESULTS: Seven patients were withdrawn after randomisation. Fifty patients were assigned to LAC group and 44 to LMBT group. The intention-to-treat cure rates were 92% and 91%,whereas the per-protocol cure rates were 92% and 97%,respectively. Side effects were common,with 56% experiencingmoderate to severe symptoms in the LAC group and 59% in the LMBT group. Symptoms of vomiting,diarrhoea and black stools were significantly more common in the LMBT group. Patient compliance was 100% for triple therapy and 86% for quadruple therapy (P < 0.01). One-third of patients in both groups were still taking acid-reducing medications at six-month follow-up. CONCLUSION: One-week triple and quadruple therapies have similar intention-to-treat eradication rates. Certain side effects are more common with quadruple therapy,which can compromise patient compliance. Patient education or modifi cations to the regimen are alternative options to improve compliance of the quadruple regimen.

Differential Effects of Strategies to Improve the Transduction Efficiency of Lentiviral Vector that Conveys an Anti-HIV Protein,Nullbasic,in Human T Cells

Nullbasic is a mutant form of HIV-1 Tat that has strong ability to protect cells from HIV-1 replication by inhibiting three different steps of viral replication: reverse transcription, Rev export of viral m RNA from the nucleus to the cytoplasm and transcription of viral m RNA by RNA polymerase II. We previously showed that Nullbasic inhibits transduction of human cells including T cells by HIV-1-based lentiviral vectors. Here we investigated whether the Nullbasic antagonists huTat2(a Tat targeting intrabody), HIV-1 Tat or Rev proteins or cellular DDX1 protein could improve transduction by a HIV-1 lentiviral vector conveying Nullbasic-Zs Green1 to human T cells. We show that overexpression of huTat2, Tat-FLAG and DDX1-HA in virus-like particle(VLP) producer cells significantly improved transduction efficiency of VLPs that convey Nullbasic in Jurkat cells. Specifically, co-expression of Tat-FLAG and DDX1-HA in the VLP producer cell improved transduction efficiency better than if used individually. Transduction efficiencies could be further improved by including a spinoculation step. However, the same optimised protocol and using the same VLPs failed to transduce primary human CD4^+T cells. The results imply that the effects of Nullbasic on VLPs on early HIV-1 replication are robust in human CD4^+T cells. Given this significant block to lentiviral vector transduction by Nullbasic in primary CD4^+T cells, our data indicate that gammaretroviral, but not lentiviral, vectors are suitable for delivering Nullbasic to primary human T cells.

Atomic Processes in Emission Characteristics of a Lithium Plasma Plume Formed by Double-Pulse Laser Ablation

High resolution spectral analysis of lithium plasma formed by single and double laser ablation has been undertaken to understand the plume-laser interaction, especially at the early stages of the plasma plume. In order to identify different atomic processes in evolving plasma, time resolved spectral emission studies at different inter-pulse delays have been performed for ionic and neutral lithium lines emitting from different levels. Along with the enhancement in emission intensity, a large line broadening and spectral shift, especially in the case of excited state transition Li I 610.3 nm have been observed in the presence of the second pulse. This broadening and shift gradually decrease with increasing time delay. Another interesting feature is the appearance of a multi-component structure in the ionic line at 548.4 nm and these components change conversely into a single structure at the later stages of the plasma. The multi-component structures are correlated with the presence of different velocity （temperature） distributions in non-LTE conditions. Atomic analyses by computing photon emissivity coefficients with an ADAS code have been used to identify the above processes.

This article presents a design of the internal model control （IMC） based single degree of freedom （SDF） fractional order （FO） PID controller with a desired bandwidth specification for a class of fractional order system （FOS）. The drawbacks of the SDF FO-IMC are eliminated with the help of the two-degree of freedom （TDF） FO PID controller. The robust stability and robust performance of the designed controller are analyzed using an example.

Energy absorption potential of concrete floors containing secondary （shrinkage and temperature） reinforcements

This paper experimentally investigates the namely, normal density concrete and structural low-density energy absorption potential of two types of concrete floors, concrete, containing secondary （shrinkage and temperature） reinforcements. The test program considered the following secondary reinforcements： 1） traditional welded-wire steel mesh, 2） steel fiber and 3） poly composite fiber. To estimate the extent to which crushing of floor slab materials would help absorb energy, a series of concrete penetration tests employing patch loading was undertaken on scaled down model slabs. Each concrete-secondary reinforcement combination considered slabs of 50 mm in depth with square plan dimensions ranging from 50 to 500 mm, resulting in a total of 30 test specimens. The first part of the paper discusses the test specimens, the test setup, and the test procedure. The second part of the paper presents the experimental results and establishes the energy absorption of different concrete- secondary reinforcement combinations. Sieve analysis results of the crushed specimens were used to derive a ＂work index＂ value that relates the pulverized particle size distributions to energy inputs. The work index values of concrete-secondary reinforcement systems can be used to assess the energy dissipation potential associated with such floor slabs in buildings undergoing progressive collapse. The results indicate that floors with secondary reinforcements could play an important role in helping arrest global progressive collapse.

This research of study continues personal research and professional practice in the Alternative Medicine (AM) has become an attractive and a popular issue between academicians and researchers in the last decades. AM is mostly used by individuals to support or to substitute the medical treatment. The studies from different disciplines have shown that the AM use is expanding all over the world but the popularity and the types of its use remain unclear. A qualitative study was conducted by treating a patient through several kinds of AM, receiving positive feedback from the patient. The overall data shows very good improvement since the AM becomes one of their parts of life to continue surviving on this earth with lots of challenges. Herbalist and vitamin supplements are the most popular and commonly used alternative medicine methods. Most people had used alternative medicine to improve their health conditions, thus eliminating the side effects of modern treatments. To supersede and recover or heal this chronic symptom, a combination of AM will be great to encounter it such as cancer, psoriasis, epilepsy, viral fever, etc. A combination of Ganotherapy and Knowledge of Holistic Human System enhances the AM era into battling these chronic cases and creates a powerful immune system to protect against it.

A Novel Asymmetric Three-Phase Cascaded 21 Level Inverter Fed Induction Motor Using Multicarrier PWM with PI and Fuzzy Controller

Multilevel inverters are gaining popularity in high power applications. This paper proposes a new ladder type structure of cascaded three-phase multilevel inverter with reduced number of power semiconductor devices which is used to drive the induction motor. The ultimate aim of the paper is to produce multiple output levels with minimum number of semiconductor devices. This paper uses only 11 switches along with 3 diodes and 4 asymmetrical sources to produce an output voltage of 21 levels. The modulation technique plays a major role in commutation of the switches. Here we implement the multicarrier level shifting pulse width modulation technique to produce the commutation signals for the inverter. The proposed multilevel inverter is used to drive the three-phase induction motor. The mathematical modeling of three-phase induction motor is done using Simulink. Furthermore the PI and fuzzy logic controllers are also used to produce the reference waveform of the level shifting technique which in turn produces the commutation signals of the proposed multilevel converter. The controllers are used to control the speed of the induction motor. The effectiveness of the proposed system is proved with the help of simulation. The simulation is performed in MATLAB/Simulink. From the simulation results, it shows that the proposed multilevel inverter works properly to generate the multilevel output waveform with minimum number of semiconductor devices. The PI and fuzzy logic controller performances are evaluated using the results which indicate that with the help of controllers the harmonics has been reduced and the speed control of induction motor is achieved under different loading conditions.

Performance Analysis of PI and Fuzzy Control for Modified LCC Resonant Converter Incorporating Boost Converter

In this paper, the modified LCC type of series-parallel Resonant Converter (RC) was designed and state-space modeling analysis was implemented. In this proposed converter, one leg of full bridge diode rectifier is replaced with Synchronous Rectifier (SR) switches. The proposed LCC converter is controlled using frequency modulation in the nominal state. During hold-up time, the SRswitches control is changed from in-phase to phase-shifted gate signal to obtain high DC voltage conversion ratio. Furthermore, the closed loop PI and fuzzy provide control on the output side without decreasing the switching frequency. The parameter such as conduction loss on primary and secondary side, switching loss, core and copper also reduced. Simultaneously, the efficiency is increased about 94.79 is realized by this scheme. The proposed converter with an input of 40 V is built to produce an output of 235 V with the help of ZVS boost converter [1] even under line and load disturbances. As a comparison, the closed loop fuzzy controller performance is feasible and less sensitive than PI controller.

Three Phase Dual Input Direct Matrix Converter for Integration of Two AC Sources from Wind Turbines

This project proposes a novel dual-input matrix converter (DIMC) which is used to integrate the output of the wind energy to a power grid. The proposed matrix converter is developed based on the traditional indirect matrix converter under reverse power flow operation mode, but with its six-switch voltage source converter replaced by a nine-switch configuration followed by the current source inverter (CSI). Matrix electric power conversion topologies and their switch functions are flexible and are used for specific applications. With the additional three switches, the proposed DIMC can provide six input terminals, which make it possible to integrate two independent AC sources from two independent wind turbines into a single grid tied power electronics interface. Commanded currents can be extracted from the two input sources to the grid. The proposed PI control and modulation schemes guaranteed sinusoidal input and output waveforms as well as reduced THD. The simulation results are provided to validate the effectiveness of the proposed control and modulation schemes for the proposed converter.
