Five years of post-validation surveillance of lymphatic filariasis in Thailand

Background The World Health Organization (WHO) validated Thailand in 2017 as having eliminated lymphatic filariasis (LF) as a public health problem with recommendations for continued surveillance. This article describes measures and progress made in Thailand with post-validation surveillance (PVS) of LF from 2018 until *** The implementation unit (IU) is a sub-village in 11 former LF endemic provinces. Human blood surveys are targeted in 10% of IUs each year. InWuchereria bancrofti areas, filaria antigen test strips (FTS) are used, and inBrugia malayi areas, antibody test kits (Filaria DIAG RAPID) are used. Positive cases are confirmed by thick blood film (TBF) and polymerase chain reaction (PCR). Vector surveys for mosquito species identification and dissection for microfilaria (Mf)/filarial larvae are done in 1% of IUs where human blood surveys are conducted. Human blood surveys using FTS are conducted among migrants in five provinces. Surveillance of cats is done in areas that previously recorded > 1.0% Mf rate among cats. Morbidity management and disability prevention (MMDP) are done every 2 years in LF-endemic areas where chronic disease patients *** From 2018 to 2022, in a total of 357 IUs in 11 provinces, human blood surveys were conducted in 145 IUs (41%) with an average population coverage of 81%. A total of 22,468 FTS and 27,741 FilariaDIAG RAPID were performed. 27 cases were detected: 3 cases ofW. bancrofti in Kanchanaburi province and 24 cases ofB. malayi in Narathiwat province. 4 cases ofW. bancrofti were detected in two provinces through routine public health surveillance. Vector surveys in 47 IUs detectedB. malayi Mf filarial larvae only in Narathiwat province. Chronic LF patients reduced from 114 in 2017 to 76 in 2022. Surveys among 7633 unregistered migrants yielded 12 cases ofW. bancrofti. Mf rate among cats in Narathiwat province declined from 1.9% in 2018 to 0.7% in 2022. Objective:To determine the bioefficacy of plant essential oils on Aedes aegyptiandAnopheles dirus. Repellency was determined by measuring reduction in feeding and mortality. A novelin vitro bioassay apparatus was developed that had a sausage-casing membrane feeding system. Methods:Mixtures of three essential oils were evaluated: turmeric (Curcuma longa), eucalyptus (Eucalyptus globulus), and orange (Citrus aurantium). The oils were mixed in pairs or all together at equal volume for a total of 10% volume and then formulated with 90% virgin coconut oil. Completed formulations were evaluated with and without an additional 5% vanillin. The formulations were applied to the sausage casing membranes and female mosquitoes provided (expose) blood meals (0.5, 1.0, 1.5, 2.0, 2.5, 3.0, 3.5 and 4.0 h) to assess the percentage repellency over time. Results:The strongest repellency was at shorter exposure periods. ForAedesaegypti, the strongest feeding reduction was with the turmeric and eucalyptus combination and with the addition of vanillin (97.6%–99.6%). ForAnopheles dirus, the strongest repellency was when all three oils were combined (98.4%–99.6%). Conclusions:Vanillin increased the effects of repellency and mortality for all formulations and demonstrated an increased potential to enhance the bioefficacy of essential oil repellents. This study also demonstrated anin vitro membrane feeding system that can be used to screen essential oils.

Objective In Thailand,trypanosomiasis or surra outbreaks in pigs has been reported throughout Thailand with varying clinical ***,the aim of this study is to evaluate levels of proinflammatory cytokine(TNF-α, IL-1βand IL-6) and prostaglandin(PGE,PGF) response,clinical and clinico-pathology in pregnant sows after experimental infection with *** the present study,infected sows showed clinical and clinico-pathology signs including abortion,fever,petechial and plaquehaemorrhage,skin rash,anaemia,monocytosis and elevation of AST and ALT *** results demonstrated that transplacental transmission is another route of *** from sows to their fetuses. However,the data on the levels of proinflammatory cytokine(TNF-α,IL-ip and IL-6) response could not differentiatedbetween trypanosomes infected sows and control *** the present study,the alteration of PGE and PGFin experimental sows showed no significative correlation with clinical signs such as fever and ***,the results of CATT/***,Ab-ELISA/*** PCR diagnosis were confirmed parasitologically positive in all infected *** summary,this study indicated that parasitaemia,clinico-pathological signs,serological assays and PCR method are the gold standard for the diagnosis of surra in pigs.

Stir-Fry Chicken with Green Curry Suppresses Inflammatory Gene Expression by Lipopolysaccharide-Induced Murine Macrophages

Inflammatory mediators produced during inflammatory response play an important role on pathological development of several chronic diseases. Although several dietary plants exhibited anti-inflammatory property, their impacts as a whole food has been rarely reported. The aim of the present study is to assess anti-inflammatory activity of an ethanol extract from a whole food namely “ready to eat stir-fry chicken with green curry” consisting of green curry paste, big egg plant, pea egg plant, red chili, kaffer lime and sweet basil as plant-based ingredients. The food extract at 55 - 220 μg/ml was incubated with RAW264.7 murine macrophage cells prior to stimulation with lipopolysaccharide (LPS) for 24 h. Inflammatory mediators [(inducible nitric oxide synthase (iNOS), cyclooxygenase-2 (COX-2), tumor necrosis factor (TNF-α) and interleukine-6 (IL-6)] mRNA and protein were determined by RT-PCR, immunoblot and ELISA respectively. The modulation mechanism by the food extract was observed by measuring the phosphorylated form of mitogen activated protein kinases (MAPKs) and inhibitor kappa B alpha (IκB-α). The ready to eat stir-fry chicken with green curry extract significantly suppressed LPS-induced iNOS, COX-2, IL-6 and TNF-α gene expression in a dose-depend- ent manner without cytotoxicity. The suppressive effect was modulated partly by inhibiting phosphorylation of MAPKs and IκB-α. These results indicate that spices and vegetables in a complex diet still possess strong anti-inflammatory activities which warrant confirming such activities to ameliorate the pathogenesis of inflammatory-associated chronic diseases in vivo.

泰国旅游业目前已经超过传统的纺织、汽车零部件和电子零部件等劳动力密集型产业,成为国民经济总量当中占比最大的部门。因此保障旅游产业的健康发展对于泰国国民经济的长期可持续发展具有重大的意义。目前泰国的游客来源主要是东亚和东欧等国,虽然政府高度重视给世界各国的游客提供各种便利条件,但是目前,在东南亚区域范围内,泰国旅游产业的竞争力显得并不突出,大量的中小旅游企业无序经营、劳动力素质差以及旅游产业的法律保障和监管措施不足等问题表现得较为突出。基于泰国旅游产业比较优势指数的测算、游客来源和政府产业政策等角度,分析了泰国旅游产业的发展现状。由于目前泰国旅游企业的融资条件并不完善,中小旅游企业难以获得国内金融机构的支持。在泰国,外商对泰国旅游产业的投资受到约束。通过国际金融市场进行股权投资难以有效的将外商的管理经验引入到泰国旅游产业当中。除此之外,过度依赖自身良好的自然环境和人文景观也带来了劳动力质量难以提升的问题。长时间的政局动荡以及政府的纵容直接导致了泰国旅游产业色情泛滥缺少监管的问题。为了解决泰国旅游产业面临的问题,泰国政府应采取措施鼓励银行加大对中小旅游企业的信贷支持,通过开发不为游客熟知的新景点以及能够满足游客个性化需求的特色产品,实现泰国旅游业竞争力的提升。泰国还需要通过促进银行业对中小旅游企业的信贷支持、建立针对中小旅游企业的信贷担保体系、鼓励中小旅游企业上市融资等手段,加强金融业对旅游业的支持。

Acute Diarrhea Etiology in Young Children and Adults in the Republic of Maldives—A Point Prevalence Study

Introduction: Despite its recent status of middle-income country in WHO’s South-East Asia Region, diarrhea remains an important yet unresearched public health issue in the Republic of Maldives. Methodology: We conducted a one-month cross-sectional study in children and adults with acute diarrhea at three regional hospitals in Maldives in August-September 2007 to investigate the point-prevalence of diarrhea etiologic agents. Enteric Bacteria was identified by a standard microbiology technique and isolates were submitted for antimicrobial susceptibility testing. Rotavirus, astrovirus and adenovirus were detected by enzyme-linked immunosorbent assays (ELISA). Real-time reverse-transcription polymerase chain reaction (RT-PCR) was used to test for norovirus. Results: We enrolled 73 children and 57 adults with acute diarrhea. The most common pathogens detected in children were norovirus (43%) and rotavirus (18%). Vibrio parahaemolyticus (18%) and rotavirus (17%) were the most common pathogens found in adults. Multiple and mixed infections were common. All noroviruses were identified as genogroup II/type 4(GII/4). The genotype distributions of rotaviruses were G2P[4] (48%), G12P[6] (37%), G2P[6] (5%), G9P[8] (5%), and non-typeable G2 (5%). Conclusions: This study provides preliminary data on the importance of norovirus and rotavirus as diarrhea etiologic agents in Maldives. A systematic prospective diarrhea surveillance documenting disease burden, etiology, seasonal variation, as well as risk factors should be conducted for the development of public health interventions to reduce diarrhea morbidity and mortality in Maldives.

AIM:To study the anesthetic management of patients undergoing small bowel enteroscopy in the World Gastroenterology Organization(WGO) Endoscopy Training Center in ***:Patients who underwent small bowel enteroscopy during the period of March 2005 to March 2011 in Siriraj Gastrointestinal Endoscopy Center were retrospectively *** patients' characteristics,pre-anesthetic problems,anesthetic techniques,anesthetic agents,anesthetic time,type and route of procedure and anesthesia-related complications were ***:One hundred and forty-four patients underwent this procedure during the study *** mean age of the patients was 57.6 ± 17.2 years,andmost were American Society of Anesthesiologists(ASA) class Ⅱ(53.2%).Indications for this procedure were gastrointestinal bleeding(59.7%),chronic diarrhea(14.3%),protein losing enteropathy(2.6%) and others(23.4%).Hematologic disease,hypertension,heart disease and electrolyte imbalance were the most common pre-anesthetic *** anesthesia with endotracheal tube was the anesthetic technique mainly employed(50.6%).The main anesthetic agents administered were fentanyl,propofol and *** mean anesthetic time was 94.0 ± 50.5 *** balloon and oral(antegrade) intubation was the most common type and route of *** anesthesia-related complication rate was relatively *** overall and cardiovascular-related complication rates including hypotension in the older patient group(aged ≥ 60 years old) were significantly higher than those in the younger ***:During anesthetic management for small bowel enteroscopy,special techniques and drugs are not routinely ***,for safety reasons anesthetic personnel need to optimize the patient's condition.

Early detection of cardiac involvement in thalassemia: From bench to bedside perspective

Myocardial siderosis is known as the major cause of death in thalassemia major(TM) patients since it can lead to iron overload *** this condition can be prevented if timely effective intensive chelation is given to patients,the mortality rate of iron overload cardiomyopathy still remains high due to late detection of this *** direct and indirect methods of iron assessment,including serum ferritin level,echocardiogram,non-transferrin-bound iron,cardiac magnetic resonance T2*,heart rate variability,and liver biopsy and myocardial biopsy,have been proposed for early detection of cardiac iron overload in TM ***,controversial evidence and limitations of their use in clinical practice *** this review article,all of these iron assessment methods that have been proposed or used to directly or indirectly determine the cardiac iron status in TM reported from both basic and clinical studies are comprehensively summarized and *** there has been growing evidence in the past decades that cardiac magnetic resonance imaging as well as cardiac autonomic status known as the heart rate variability can provide early detection of cardiac involvement in TM patients,these two methods are also presented and *** existing controversy regarding the assessment of cardiac involvement in thalassemia is also discussed.
