Survival in men older than 75 years with low- and intermediate-grade prostate cancer managed with watchful waiting with active surveillance

Objective:Recent studies have reported the underuse of active surveillance or watchful wait-ing for low-risk prostate cancer in the United *** study examined prostate cancer-specific and all-cause death in elderly patients older than 75 years with low-risk tumors managed with active treatment versus watchful waiting with active surveillance(WWAS).methods:We performed survival analysis in a cohort of 18,599 men with low-risk tumors(early and localized tumors)who were 75 years or older at the time of prostate cancer diagnosis in the linked Surveillance,Epidemiology,and End Results(SEER)-medicare database(from 1992 to 1998)and who were followed up through December *** was defined as having an-nual screening for prostate-specific antigen and/or digital rectal examination during the follow-up *** risks of prostate cancer-specific and all-cause death were compared by Cox regression *** propensity score matching technique was used to address potential selection ***:In patients with well-differentiated(Gleason score 2-4)and localized disease,those managed with WWAS without delayed treatment had higher risk of all-cause death(hazard ratio 1.20,95%confidence interval 1.13-1.28)but a substantially lower risk of prostate cancer-specific death(hazard ratio 0.62,confidence interval 0.51-0.75)than patients undergoing active *** managed with WWAS with delayed treatment had comparable mortality ***-tivity analyses based on propensity score matching yielded similar ***:In men older than 75 years with well-differentiated and localized prostate cancer,WWAS without delayed treatment had a lower risk of prostate cancer-specific death and compa-rable all-cause death as compared with active *** patients in whom treatment was delayed had comparable mortality *** results support WWAS as an initial management option for older men with well-differentiated and localized prostate cancer.

Objective:Determine uptake of prostate-specific antigen(PSA)testing in medicare benefi-ciaries according to previous receipt of PSA ***:A 5%random sample of men aged 67 years or older without a previous diagnosis of prostate cancer was identified through 2009-2012 medicare *** measured the annualized frequency of PSA screening among men due for PSA testing,stratified by PSA testing use in the previous 2 years,and clustered by ordering ***:Throughout the study period,PSA testing use was consistently higher for men with previous screening than for men without previous *** men without previous screening,there was a decline in testing that was most pronounced in *** with 2009,the cor-responding odds ratios were 0.98[95%confidence interval(CI)(0.96-1.00)]in 2010,0.94[95%CI(0.92-0.95)]in 2011,and 0.66[95%CI(0.65-0.68)]in *** contrast,for men with previous screening,PSA testing frequency was stable from 2009 to 2011,and declined to a lesser extent in 2012[odds ratio 0.80,95%CI(0.79-0.81)].Conclusion:Receipt of PSA testing is highly dependent on whether an individual was tested in the recent *** previously unscreened men,the largest decrease occurred in 2012,which may reflect in part the publication of US Preventive Services Task Force guidelines,but there was much less impact among men already being screened.

A highly selective catalyst of Co/La_(4)Ga_(2)O_(9) for CO_(2) hydrogenation to ethanol

A new catalyst of Co/La_(4)Ga_(2)O_(9) for CO_(2) hydrogenation to produce ethanol was prepared by reducing LaCo^(0).5 Ga0.5 O3,which showed excellent selectivity to ethanol(%35 C-mol%)at mild reaction conditions(270°C,3.5 mPa,3000 m L g-1 h-1).The catalysts were characterized by N_(2) adsorption/desorption,XRD,XAFS,CO and CO_(2)-TPD,H2 chemisorption,XPS and TEm *** interaction between Co nanoparticles(NPs)and La2+4 Ga_(2)O_(9) oxide resulted in Co^(0)-Coon the surface of Co *** was proposed that La_(4)Ga_(2)O_(9) could catalyze reverse water gas shift reaction(r-WGS),which converted CO_(2) to ***,the CO migrated to Co^(0)-Co^(2+)on Co NPs,where it was hydrogenated to form ethanol like higher alcohols synthesis from *** results suggest that by controlling the oxidation state of cobalt,and combined with a kind of active site for activating CO_(2) to form CO,a catalyst with excellent selectivity to ethanol could be obtained for CO_(2) hydrogenation,which means that the complex reaction may be proceed with high selectivity using only one active metal component.

本文以历史发展为线索,认为行政责任伦理内在价值向度转换的历史轨迹经历了威尔逊的"无道德行政"、马克斯.韦伯的个体先在的责任伦理、罗尔斯的社会道义的公共责任伦理、后罗尔斯的公共伦理与个人伦理相统一的责任伦理的变化。同时,本文认为,行政责任伦理的研究内容包括责任伦理理念、责任伦理要素、制度(规则)伦理三个方面,行政责任伦理的特征表现为:应然与实然相统一、非自律与自律性相统一、公共伦理与个体伦理相统一、普遍性与特殊性相统一。

Production of Biodiesel from Sludge Palm Oil by Esterification Process

Sludge palm oil （SPO） is an attractive feedstock and a significant raw material for biodiesel production. The use of SPO can lower the cost ofbiodiesel production significantly. In this study biodiesel fuel was produced from SPO by esterification process using P-toluenesulfonic acid （PTSA） as acid catalyst in different dosages in presence of methanol to convert free fatty acid （FFA） to fatty acid methyl ester （FAmE）. Batch esterification process of SPO was carried out to study the influence of PTSA dosage （0.25-10% wt/wt）, molar ratio of methanol to SPO （6：1-20：1）, temperature （40-80 ℃）, reaction time （30-120 min）. The effects of those parameters on the yield of crude biodiesel and conversion of FFA to FAmE were monitored. The optimum condition for batch esterification process was 0.75% wt/wt, 10：1 molar ratio, 60 ℃ temperature and 60 minutes reaction time.

为研究衡盾泥在带压开舱时的闭气保压效果,通过室内试验及理论计算的方法对衡盾泥的泥膜气密性进行分析。结果表明:1)3 d龄期的衡盾泥泥膜中自由水所占比例约为10%;2)在0.292 mPa压力下,改良黏土浆体泥水质量比为1∶2、塑化剂掺入质量比为1/20、成膜龄期为3 d的衡盾泥泥膜,仅1.7%的孔隙可能出现渗气;3)衡盾泥泥膜渗气系数为10^(-8)~10^(-7)cm^3/s,换算到直径为6 m的隧道开挖断面上,泥膜的渗气量为2~3 m^3/h,盾构开舱时配置的空压机完全满足补气条件。
