Effect of pH,lactate,electrolyte,and strong ion difference variability on prediction of intensive care unit mortality:A retrospective study

Objective:To investigate the effect of the variability of Nas,Cls,Ks,lactate values and sodium effect(NaEffect),chloride effect(ClEffect),non-lactate strong ion difference(SIDnl)values calculated according to Stewart’s approach on predicting intensive care unit(ICU)***:The study was conducted on 1539 patients,*** Na(Nas),serum Cl(Cls),serum K(Ks),serum Ca(Cas),serum Mg(Mgs),lactate,pH values and SIDnl,NaEffect,ClEffect,APACHEⅡ(first,last),and SOFA(first,last)scores were *** ABL 800(Denmark)was used for blood gas *** variability of each parameter was *** effect of variability of each parameter on 30-day ICU mortality was ***:The variability of lactate(P<0.001,OR=0.580,95%CI=0.505-0.652),pH(P=0.001,OR=0.004,95%CI=0.000-0.104),NaEffect(P<0.001,OR=0.550,95%CI=0.378-0.592),Ks(P<0.001,OR=0.385,95%CI=0.244-0.565)values were protective factors of ICU mortality and Cls value was a risk factor(P=0.004,OR=1.095,95%CI=1.024-1.164).Variability of ClEffect,SIDnl values did not affect ICU ***:The variability of electrolytes is ***,effects,and lactate variability can guide treatment and fluid applications in ICU.

Objective:To investigate the effect of the first prone position on arterial blood gas analysis and respiratory parameters of acute respiratory distress syndrome(ARDS)patients with and without ***:This study was conducted retrospectively with 22 COVID-ARDS and 22 non-COVID ARDS patients,who were placed in a prone position for at least 16 hours on the first day at the intensive care unit admission,and arterial blood gas analysis was taken in the pre-prone,prone and post-prone ***:PaO2 were significantly increased in the pre-prone *** comparison in both groups,but the increase in the PaO2/FiO2 ratio was not *** comparing the pre-prone ***-prone PaO2/FiO2 ratios,there was a significant difference only in the non-COVID ARDS ***:The improved oxygenation provided by prone positioning is more permanent with the“post-prone effect”in non-COVID ARDS *** can be attributed to the differences in the pathogenesis of the two ARDS types.

Code blue applications as an indicator of clinical quality

Objective:To investigate the code blue application at a training and research hospital in ***:The code blue declaration forms and the hospital database with 238 complete records between January 2016 and July 2017 were *** form involved individual characteristics,the reason for issuing the code blue call,the unit and block where the code was given,time and location related properties such as working time and arrival duration,properties regarding the intervention process such as its type,duration or *** 24-hour and 30-day long survival data of the patients to whom cardiopulmonary resuscitation was implemented were obtained from the hospital database,or from their *** influencing factors of arrival duration were ***:The median duration of arrival was 2.14(2.00-3.02)*** blue applications were performed more frequently in Departments of General Surgery,Internal Medicine,Orthopaedics,and Cardiology *** of the code blue calls were due to cardiac arrest;the other half was due to shortness of breath or respiratory distress,syncope,and respiratory ***-out-of four code blue calls were treated with orotracheal intubation and cardiopulmonary resuscitation,or only orotracheal intubation or only medical treatment;one-fourth of the calls were not ***,72.36%of the code blue calls patients were intervened;69.35%of them were made both orotracheal intubation and cardiopulmonary resuscitation,and 20.5%of them were made only orotracheal intubation and 10.55%of them made only medical *** was found that giving the code blue day or night had no effect on the time to reach the area where the code was ***,it was found that giving the code blue within daytime or night shift had no effect on the time to reach the area where the code was given.(P>0.05).The survival rates were 39.1%within the first 24 h and 18.1%within the first 30 ***:Applications of code blue should be anal
