Objective: To study the effects of continuous morphine infusion on arterial bl ood pressure in ventilated neonates. Design: Blinded randomised placebo controll ed trial. setting: Level III neonatal intensive care unit in two centres. patients: A total of 144 ventilated neonates. Inclusion criteria were postnatal age s, ventilation hours, and indwelling arterial line. Exclusion criteria were severe asphyxia, severe intraventricular haemorrhage, major congenital anomalies, neu romuscular blockers. Intervention: Arterial blood pressure was measured before t he start and during the first 48 hours of masked infusion of drug (morphine/plac ebo; 100 μg/kg+10 μg/kg/h). Outcome measures: Arterial blood pressure and blo od pressure variability. Results: There were no significant differences in overa ll mean arterial blood pressure between the morphine group (median (interquartil e range) 36 mm hg (6) and the placebo group (38 mm hg (6)) (p=0.11). Although si gnificantly more morphine treated patients (70%) showed hypotension than the pl acebo group (47%) (p=0.004), the use of volume expanders and vasopressor drugs was not significantly different (morphine group, 44%; placebo group, 48%; p=0. 87), indicating the limited clinical significance of this side effect. Blood pre ssure variability was not influenced by routine morphine analgesia (p=0.81) or a dditional morphine (p=0.80). patients with and without intraventricular haemorrh age showed no differences in blood pressure (Mann-Whitney U test 1953; p=0.14) or incidence of hypotension (χ2 test 1.16; df 1; p=0.28). Conclusions: Overall arterial blood pressure, use of inotropes, and blood pressure variability were n ot influenced by morphine infusion. Therefore the clinical impact of hypotension as a side effect of low dosemorphine treatment in neonates is negligible.

The prevalence and prevention of nasopharyngeal carcinoma in China

Nasopharyngeal carcinoma (NpC) has remarkable epidemiological features, including regional, racial, and familial aggregations. The aim of this review is to describe the epidemiological characteristics of NpC and to propose possible causes for the high incidence patterns in southern China. since the etiology of NpC is not completely understood, approaches to primary prevention of NpC remain under consideration. This situation highlights the need to conduct secondary prevention, including improving rates of early detection, early diagnosis, and early treatment in NpC patients. since the 1970's, high-risk populations in southern China have been screened extensively for early detection of NpC using anti-Epstein-Barr virus (EBV) serum biomarkers. This review summarizes several large screening studies that have been conducted in the high-incidence areas of China. screening markers, high-risk age range for screening, time intervals for blood re-examination, and the effectiveness of these screening studies will be discussed. Conduction of prospective randomized controlled screening trials in southern China can be expected to maximize the cost-effectiveness of early NpC detection screening.

Early microrecanalization of vas deferens following biodegradable graft implantation in bilaterally vasectomized rats

We evaluated a biodegradable graft for reconstruction of rat vasa deferentia with long obstructed or missing segments. A total of 47 sprague-Dawley rats underwent bilateral vasectomy and were divided into groups according to length of the vas deferens affected （0.5, 1, 1.5 cm）. After 8 weeks, poly-（D,L-lactide）（pDLA） grafts were used to reconnect the vas deferens, Grafts and adjoining vasa deferentia were excised 8 and 12 weeks later and evaluated microscopically. At 8 weeks, microscopic changes included a robust inflammatory response around the grafts. All grafts were still intact but in the early stages of degradation. No microtubules, indicative of vas deferens recanalization, were identified. One specimen showed evidence of healing and neovascularization at the interface zone between the vas deferens and the graft. At 12 weeks, grafts were further degraded but still present. Microscopic evaluation showed decreased inflammation. seven specimens showed neovascularization at the interface zone; two of these showed distinct epithelialized vas deferens microcanals at the graft edges. One specimen showed a microcanal spanning the entire 0.5-cm graft. A time period of 8 weeks is not ample enough for vas deferens regeneration in the setting of a biodegradable pDLA graft; however, early evidence of re-growth was seen at 12 weeks. A longer healing time should permit further biodegradation of the graft, as well as re-growth and possible eventual reconnection of the vas deferens, allowing passage of sperm. These findings suggest a potential role for biodegradable grafts in the reconstruction ofvas deferens with long obstructed segments.

Neurovascular interactions are crucial for the normal development of the central nervous system. To study suchinteractions in primary cultures, we developed a procedure to simultaneously isolate neural progenitor and endothelialcell fractions from embryonic mouse brains. Depending on the culture conditions endothelial cells were found to favormaintenance of the neuroprogenitor phenotype through the production of soluble factors, or to promote neuronal differ-entiation of neural progenitors through direct contact. These apparently opposing effects could reflect differential cellularinteractions needed for the proper development of the brain.

An Assessment of Thermal Comfort in Multi storey Office Buildings in Ghana

Amidst the recent development in the usage of curtain walls for office buildings, high utilization of energy and poor thermal comfort issues have become paramount. This paper assesses thermal comfort in multi storey (naturally and mechanically ventilated) office buildings in Accra, the Capital city of Ghana using Fanger’s predicted Mean Votes (pMV) and predicted percentage of Dissatisfied persons (ppD) model. The model relates to the American society of heating, Refrigerating and Air-Conditioning Engineers (AshRAE) standard 55 (which gives an acceptable temperature range of 23℃ - 26℃) and the International standards Organization (IsO) 7730. Indoor environmental parameters (temperature and relative humidity) of 4 multi storey office buildings were recorded over a 10 month period. The environmental parameters were analyzed using pMVcalc_v2 software which resulted in the generation of pMV-ppD values. The findings reveal high pMV-ppD values in the Naturally Ventilated Building (NVB) whiles the Mechanically Ventilated Buildings (MVB) fall within the comfort zone. Meanwhile, the Actual Mean Votes (AMV) by the occupants suggest all four buildings are relatively comfortable with the mechanically ventilated offices being more comfortable. Additionally, it is recommended that buildings are orientated with their longer sides facing north-south, with enough shading in order to improve the thermal comfort conditions of work spaces.

Attosecond magnetization dynamics in non-magnetic materials driven by intense femtosecond lasers

Irradiating solids with ultrashort laser pulses is known to initiate femtosecond timescale magnetization ***,sub-femtosecond spin dynamics have not yet been observed or ***,we explore ultrafast light-driven spin dynamics in a highly nonresonant strong-field *** state-of-the-art ab initio calculations,we predict that a nonmagnetic material can transiently transform into a magnetic one via dynamical extremely nonlinear spin-flipping processes,which occur on attosecond timescales and are mediated by cascaded multi-photon and spin–orbit *** are nonperturbative nonresonant analogs to the inverse Faraday effect,allowing the magnetization to evolve in very high harmonics of the laser frequency(*** up to the 42nd,oscillating at~100 attoseconds),and providing control over the speed of magnetization by tuning the laser power and ***,we show that even for linearly polarized driving,where one does not intuitively expect the onset of an induced magnetization,the magnetization transiently oscillates as the system interacts with *** response is enabled by transverse light-driven currents in the solid,and typically occurs on timescales of~500 attoseconds(with the slower femtosecond response suppressed).An experimental setup capable of measuring these dynamics through pump–probe transient absorption spectroscopy is *** results pave the way for attosecond regimes of manipulation of magnetism.

目的在全球范围内,癌症患者的非正式照顾者在其照顾角色方面遇到的挑战显著影响了他们的生活质量。此研究旨在分析癌症照护如何影响非正式照顾者的生活质量,以及非正式照顾者的癌症照护角色转换应对策略。方法按照提高质性研究系统评价报告透明度的声明(Enhancing transparency in reporting the synthesis of qualitative research,ENTREQ),检索发表于2009年5月1日至2019年12月31日的文献。检索万方数据库、中国知网、CINAhL、pubMed、Cochrane图书馆、psycARTICLEs和psycINFO数据库和灰色文献。采用2018版质性研究关键质量评价清单(Critical Appraisal skills programme,CAsp)进行了质量评价和主题整合的方法进行数据整用。结果共检索出8945篇文章,6篇文章符合纳入标准。分析关于癌症患者非正式照顾者的生活质量,呈现出一个主题“照顾的挑战”。分析言非正式照护者癌症照护角色转换应对策略,呈现出两个主题:“自我调整”和“寻求正式和非正式支持”。结论癌症照护显著影响非正式照护者的生活质量。为了应对照护过程中出现的困难、平衡照护与自己生活之间的关系,非正式照护者发展出了多样化的应对策略。同时,对非正式照顾者的专业支持和政策支持仍然不足,需要从专业健康照护者和政策制定者两个角度进行完善和提升。
