Influence of hepatitis viruses on clinico-pathological profiles and long-term outcome in patients undergoing surgery for hepatocellular carcinoma

BACKGROUND: The global risk of hepatocellular carcinoma(HCC) is largely due to hepatitis B virus (HBV) and hepatitis C virus (HCV) infections. In recent years, however, an increased prevalence of non-viral HCC has been noted. The clinical impact of the presence/absence of viral infections in HCC remains controversial. The present study aimed to assess the effect of hepatitis viruses on demographics, clinical and pathological features and long-term outcome in a large cohort of Romanian patients who underwent surgery for HCC. METHODS: The study included 404 patients with HCC who had undergone resection, transplantation or radiofrequency ablation at a single institution between 2001 and 2010. The patients were divided into four groups: 85 patients with hepatitis B virus infection (HBV group), 164 patients with hepatitis C virus infection (HCV group), 39 patients with hepatitis B and C virus co-infection (HBCV group), and 116 patients without viral infection (non-BC group). RESULTS: The patients of both HBV (56.0±11.3 years) and HBCV groups (56.0±9.9 years) were significantly younger than those of the HCV (61.0±8.5 years, P=0.001) and non-BC groups(61.0±13.0 years, P=0.002). Interestingly, the prevalence of liver cirrhosis was significantly lower in the non-BC group (47%)than in any other subsets (72%-90%, P<0.002). Furthermore, the non-BC patients were more advanced according to the Barcelona Clinic Liver Cancer stages than the patients of the HCV or HBCV groups (P<0.020); accordingly, they were more frequently assessed beyond the Milan criteria than any other groups (P=0.001). No significant differences in the disease-free or overall survival rates were observed among these groups. CONCLUSIONS: Patients with non-viral HCC are diagnosed at advanced ages and stages, a situation plausibly due to the poor effectiveness of cancer surveillance in community practice. The presence of viral infections does not appear to impair the longterm prognosis after surgical treatm

Pre-compacted elements （disks, tutus） of bentonite/sand mixture are candidate materials for sealing plugs of radioactive waste disposal. Choice of this material is mainly based on its swelling capacity allowing all gaps in the system to be sealed, and on its low permeability. When emplaced in the gallery, these elements will start to absorb water from the host rock and swell. Thereby, a swelling pressure will develop in the radial direction against the host rock and in the axial direction against the support structure. In this work, the swelling pressure of a small scale compacted disk of bentonite and sand was experimentally studied in both radial and axial directions. Different swelling kinetics were identified for different dry densities and along different directions. As a rule, the swelling pressure starts increasing quickly, reaches a peak value, decreases a little and finally stabilises. For some dry densities, higher peaks were observed in the radial direction than in the axial direction. The presence of peaks is related to the microstructure change and to the collapse of macro- pores. In parallel to the mechanical tests, microstructure investigation at the sample scale was conducted using microfocus X-ray computed tomography （BCT）. Image observation showed a denser structure in the centre and a looser one in the border, which was also confirmed by image analysis. This structure heterogeneity in the radial direction and the occurrence of macro-pores close to the radial boundary of the sample can explain the large peaks observed in the radial swelling pressure evolution. Another interesting result is the higher anisotropy found at lower bentonite dry densities, which was also analysed by means ofμCT observation of a sample at low bentonite dry density after the end of test. It was found that the macro-pores, especially those between sand grains, were not filled by swelled bentonite, which preserved the anisotropic microstructure caused by uniaxial compression

Drug-induced liver injury in inflammatory bowel disease: 1-year prospective observational study

AIM To analyze 1-year liver injury burden in inflammatory bowel disease(IBD) patients. METHODS During a 6-mo inclusion period, consecutive IBD cases having a control visit at IBD center were included. Basic demographics, IBD phenotype and IBD treatment were recorded on entry. Aminotransferase(AT) activities of ALT, AST, ALP and gamma-glutamyl transpeptidase(GGT) were measured at baseline, 3 mo prior to study entry and prospectively every 3 mo for 1 year. Liver injury patterns were predefined as: Grade 1 in ALT 1-3 × upper limit of normal(ULN), grade 2 in ALT > 3 × ULN, hepatocellular injury in ALT > 2 × ULN, cholestatic injury in simultaneous GGT and ALP elevation > *** injury was reported when AT elevations were found on > 1 measurement. Risk factors for the patterns of liver injury were identified among demographic parameters, disease phenotype and IBD treatment in univariate and multivariate analysis. Finally, implications for the change in IBD management were evaluated in cases with persisting hepatocellular or cholestatic *** Two hundred and fifty-one patients were included having 917 ALT and 895 ALP and GGT measurements. Over one year, grade 1 injury was found in 66(26.3%), grade 2 in 5(2%) and hepatocellular injury in 16 patients(6.4%). Persisting hepatocellular injury was found in 4 cases. Cholestasis appeared in 11 cases(4.4%) and persisted throughout the entire study period in 1 case. In multivariate analysis, hepatocellular injury was associated with BMI(OR = 1.13, 1.02-1.26), liver steatosis(OR = 10.61, 2.22-50.7), IBD duration(1.07, 1.00-1.15) and solo infliximab(OR = 4.57, 1.33-15.7). Cholestatic liver injury was associated with prior intestinal resection(OR = 32.7, 3.18-335), higher CRP(OR = 1.04, 1.00-1.08) and solo azathioprine(OR = 10.27, 1.46-72.3). In one case with transient hepatocellular injury azathioprine dose was decreased. In 4 cases with persisting hepatocellular injury, fatty liver or alcohol were most likely causes

《测量不确定度表示指南》(GUM)这个技术规范已被广泛认可,且其测量值关联一个不确定度值的建议也被广泛采纳。然而,这个规范遵循一种固有的概率方法,其应用并不总是可行的,且因为一些技术和经济原因,在可行的情况下,其应用也并不是简单直接的。总结了一种更一般化的不确定度评定和表示的随机模糊变量RFVs方法,系统评述了其关键技术与难点,通过实例表明,RFVs方法在非线性测量函数中传递不确定度具有简单高效的特点,最后对该领域的研究扩展提出了两点建议,给出了使用RFVs扩展贝叶斯定理的初步探讨结果。RFVs方法基于数学可能性理论,从GUM及其基本概念和定义出发对GUM方法进行了扩展,具有明显的优势,该方法的广泛应用也证明了其远大的发展前景。

Limited Bandwidths and Correlation Ambiguities: Do They Co-Exist in Galileo Receivers

Galileo is the Global Navigation Satellite System that Europe is building and it is planned to be operational in the next 3-5 years. Several Galileo signals use split-spectrum modulations, such as Composite Binary Offset Carrier (CBOC) modulation, which create correlation ambiguities when processed with large or infinite front-end bandwidths (i.e., in wideband receivers). The correlation ambiguities refer to the notches in the correlation shape (i.e., in the envelope of the correlation between incoming signal and reference modulated code) which happen within +/– 1 chip from the main peak. These correlation ambiguities affect adversely the detection probabilities in the code acquisition process and are usually dealt with by using some form of unambiguous processing (e.g., BPSK-like techniques, sideband processing, etc.). In some applications, such as mass-market applications, a narrowband Galileo receiver (i.e., with considerable front-end bandwidth limitation) is likely to be employed. The question addressed in this paper, which has not been answered before, is whether or not this bandwidth limitation can cope inherently with the ambiguities of the correlation function, to which extent, and which the best design options are in the acquisition process (e.g., in terms of time-bin step and ambiguity mitigation mechanisms).

BACKGROUND Chemotherapy and radiotherapy followed by durvalumab is currently the standard treatment for locally advanced node-positive non-small-cell lung cancer(NSCLC).We describe the case of a patient with locally advanced node-positive NSCLC(LA-NSCLC)treated in a phase II prospective protocol with chemotherapy,accelerated hypofractionated radiotherapy(AHRT)and surgery in the preimmunotherapy *** SUMMARY A 69-year-old male,ex-smoker(20 PY),with a Karnofsky performance status of 90,was diagnosed with locally advanced squamous cell lung *** was staged by total body computed tomography(CT)scanning,and integrated 18Ffluorodeoxyglucose positron emission tomography/CT scan[cT4 cN3 cM0,stage IIIC according to TNM(tumor-node-metastasis)8th edition]and received AHRT between chemotherapy cycles,in accordance with the study protocol(EudractCT registration 2008-006525-14).At the end of the study the patient underwent surgery,which was not part of the protocol,and showed a complete pathological *** This case report confirms that AHRT can be used successfully to treat primary LA-NSCLC with bilateral mediastinal lymph node *** case is of particular interest because of the pathological response after AHRT and the lack of surgical *** hypothesize that this radiotherapeutic approach,with its proven efficacy,could be delivered as a short course reducing treatment costs,increasing patient compliance and reducing *** are currently investigating the possibility of combining hypofractionation,chemotherapy and immunotherapy for patients with LA-NSCLC.

Successful double fertilization and subsequent seed development in flowering plants requires the delivery of two sperm cells, transported by a pollen tube, into the embryo sac of an ovule. The embryo sac cells tightly control synergid cell death, and as a result the polyspermy block. Arabinogalactan proteins are highly glycosylated proteins thought to be involved in several steps of the reproductive process. We show that JAGGER, Arabinogalactan Protein 4, is an important molecule necessary to prevent the growth of multiple pollen tubes into one embryo sac in Arabidopsis thaliana. In jagger, an AGP4 knockout mutant, the pistils show impaired pollen tube blockage as a consequence of the survival of the persistent synergid. JAGGER seems to be involved in the signaling pathway that leads to a blockage of pollen tube attraction. Our results shed light on the mechanism responsible for preventing polyspermy in Arabidopsis and for safe- guarding successful fertilization of all ovules in one pistil, ensuring seed set and the next generation.
