In this paper we aim to analyse temporal variation of CD4 cell counts for HIV-infected individuals under antiretroviral therapy by using statistical methods. This is achieved by resorting to recursive binary regression tree approach [1]?[2]. This approach has made it possible to highlight the existence of several segments of the population of interest described by the interactions between the predictive covariates of the response to the treatment regimen.

background: Gabon is endemic for hepatitis b, but they have still not reached the WHO goal for Hepatitis b vaccination coverage. We aimed to determine the rate of completeness and timeliness of hepatitis b vaccination among children under 15 years of age in Libreville. Methods: Cross-sectional study conducted over 9 months, including children randomly selected from vaccination centres, nurseries and kindergartens. We collected data on the child, socio-economic characteristics of the families, and dates of administration of hepatitis b vaccines. We determined the compliance of the vaccination dates according to the vaccination start pattern. Results: We included 453 children, of whom 236 were girls, for a sex ratio of 0.92. A total of 87% (95% CI [83.9% - 90.1%]) of children had started and completed their 3 doses of vaccine before the 12th month of age. 149 subjects had received their 3 doses on time according to the original vaccination schedule, i.e. 32.9% (95% CI [28.6% - 37.2%]). Factors significantly associated with completeness were vaccination in a public centre OR = 114 [47.2 - 347] p st dose OR = 3 [1.6 - 5.5] p < 0.001. Age at first dose was the predictor of timeliness aOR = 1.3 (95% CI [1.2 - 1.5] p 0.001). Conclusion: The respect for the vaccination deadlines is insufficient in our context, solutions exist to improve this situation.

Dear Editor, In the eukaryotic cell, the nuclear envelope separates genomic DNA from the rest of the cell, but ensures a molecu- lar communication between the nucleus and the cytoplasm through the nuclear pore complexes （NPCs）. Nuclear trans- port of macromolecules is an energy-dependent process, exemplified by the classical nuclear import pathway. In the cytoplasm, proteins containing classical nuclear localization signals （cNLSs） are recognized by a hetero-dimeric transport receptor （importin-α （Impα）：importin-β （Impl3） complex）. Impa is employed as an adaptor protein that specifically rec-ognizes the classical nuclear localization signal （cNLS） from the cargo protein （Marfori et al., 2011）. Translocation of the trimeric cargo：transport receptor import complex through the NPCs is mediated by transient interactions between Impβ and nucleoporins in the NPC. The directionality of the trans- port is imparted by the asymmetric distribution, between the cytoplasm and the nucleus, of the distinct nucleotide-bound （GTP/GDP） states of the small GTPase Ran （Tran et al., 2007）.

The mass of 40 Ti has been determined by using the isobaric multiplet mass equation method. The experimental data of the 40Tiβ-decay were used to determine the level of the isospin analogue state of^( 40)Sc. The ground-state mass excess and the QEc value for 40 Ti were determined to be -9060 ± 12 keV and 11 466 ± 13keV, respectively.

Hepatocellular carcinoma (HCC) is the commonest primary malignant cancer of the liver in the world. Given that the burden of chronic liver disease is expected to rise owing to increasing rates of alcoholism, hepatitis b and C prevalence and obesity-related fatty liver disease, it is expected that the incidence of HCC will also increase in the foreseeable future. This article summarizes the international epidemiology, the risk factors and the pathogenesis of HCC, including the roles of viral hepatitis, toxins, such as alcohol and aflatoxin, and insulin resistance.

The outcome of hepatocellular carcinoma(HCC) patients significantly differs between western and eastern population centers. Our group previously developed and validated the Chinese University Prognostic Index(CUPI) for the prognostication of HCC among the Asian HCC patient population. In the current study, we aimed to validate the CUPI using an international cohort of patients with HCC and to compare the CUPI to two widely used staging systems, the barcelona Clinic Liver Cancer(bCLC) classification and the Cancer of the Liver Italian Program(CLIP). To accomplish this goal, two cohorts of patients were enrolled in the United Kingdom(UK; n = 567; 2006-2011) and Hong Kong(HK; n = 517; 2007-2012). The baseline clinical data were recorded. The performances of the CUPI, bCLC, and CLIP were compared in terms of a concordance index(C-index) and were evaluated in subgroups of patients according to treatment intent. The results revealed that the median follow-up durations of the UK and HK cohorts were 27.9 and 29.8 months, respectively. The median overall survival of the UK and HK cohorts were 22.9 and 8.6 months, respectively. The CUPI stratified the patients in both cohorts into three risk subgroups corresponding to distinct outcomes. The median overall survival of the CUPI low-, intermediate-, and high-risk subgroups were 3.15, 1.24, and 0.29 years, respectively, in the UK cohort and were 2.07, 0.32, and 0.10 years, respectively, in the HK cohort. For the patients who underwent curative treatment, the prognostic performance did not differ between the three staging systems, and all were suboptimal. For those who underwent palliative treatment, the CUPI displayed the highest C-index, indicating that this staging system was the most informative for both cohorts. In conclusion, the CUPI is applicable to both western and eastern HCC patient populations.
