Cystic lymphangiomas are rare benign *** frequently occurring in children and involving the neck or axilla,these tumors are much less common in adults and very rarely involve the *** known congenital and acquired(traumatic)etiologies result in failure of the lymphatic channels and consequent proliferation of lymphatic *** case report describes a very rare case of a giant mesenteric cystic lymphangioma in an adult male with no clear etiology and successful resolution by standard radical resection.A previously healthy 44-year-old male presented with a 6-wk history of progressive upper abdominal pain,vomiting,anorexia and unintentional weight loss accompanied by rapid abdominal distension.A palpable mass was detected upon physical examination of the distended abdomen and abdominal computed tomography scan showed a giant multilobulated cystic process,measuring 40 cm in *** laparotomy revealed an enormous cystic mass containing 6 L of serous *** process appeared to originate from the lesser omentum and the lesser curvature of the *** resection of the tumor was performed along with a partial gastrectomy to address potential invasion into the adjacent *** analysis confirmed the diagnosis of a multicystic *** postoperative recovery was uneventful and the patient was discharged after 6 *** 3-mo follow-up,the patient was in good health with no signs of recurrence.

Peritoneal carcinomatosis (PC) is one manifestation of metastatic colorectal cancer (CRC). Tumor growth on intestinal surfaces and associated fluid accumulation eventually result in bowel obstruction and incapacitating levels of ascites, which profoundly affect the quality of life for affected patients. PC appears resistant to traditional 5-fluorouracil-based chemotherapy, and surgery was formerly reserved for palliative purposes only. In the absence of effective treatment, the histori-cal prognosis for these patients was extremely poor, with an invariably fatal outcome. These poor outcomes likely explain why PC secondary to CRC has received little attention from oncologic researchers. Thus, data are lacking regarding incidence, clinical disease course, and accurate treatment evaluation for patients with PC. Recently, population-based studies have revealed that PC occurs relatively frequently among patients withCRC. Risk factors for developing PC have been identi-fied:right-sided tumor, advanced T-stage, advanced N-stage, poor differentiation grade, and younger age at diagnosis. During the past decade, both chemother-apeutical and surgical treatments have achieved prom-ising results in these patients. A chance for long-term survival or even cure may now be offered to selected patients by combining radical surgical resection with intraperitoneal instillation of heated chemotherapy. This combined procedure has become known as hy-perthermic intraperitoneal chemotherapy. This edito-rial outlines recent advancements in the medical and surgical treatment of PC and reviews the most recent information on incidence and prognosis of this disease. Given recent progress, treatment should now be considered in every patient presenting with PC.

Method of cells-based multiscale modeling of elastic properties of filament wound C/C-SiC including free Si and matrix porosity

Three different multiscale models, based on the method of cells(generalized and high fidelity) micromechanics models were developed and used to predict the elastic properties of C/C-SiC composites. In particular, the following multiscale modeling strategies were employed: Concurrent modeling of all phases using the generalized method of cells, synergistic(two-way coupling in space) multiscale modeling with the generalized method of cells, and hierarchical(one-way coupling in space) multiscale modeling with the high fidelity generalized method of cells. The three models are validated against data from a hierarchical multiscale finite element model in the literature for a repeating unit cell of C/***, the multiscale models are used in conjunction with classical lamination theory to predict the stiffness of C/C-SiC plates manufactured via a wet filament winding and liquid silicon infiltration process recently developed by the German Aerospace Institute. Finally, un-reacted Si(or free Si) and porosity in the C matrix are included in the multiscale model, and the effect of these new phases on the stiffness and local stresses are considered.

An algorithm which allows the generation of representative volume elements(RVEs) for complex woven and warp-interlaced fiber-reinforced composite topologies while avoiding unphysical tow intersections is presented. This is achieved by extending an existing RVE generation strategy in two significant ways:(1) the local cross section shape of the tow is adjusted depending on the local tow curvature in a way that preserves the cross sectional area of the tow, and(2) the elementary crimp interval is separated into a planar and a transition region. The modifications facilitate the generation of a wide range of elaborate textile topologies without tow intersections, which are present without the proposed modifications unless complex tow to tow contact models are introduced. The mechanical properties of plain weaves were predicted based on the topology generated with the proposed algorithm as well as based on RVEs which were constructed based on actual micrographs, i.e. a "digital twin" of the actual microstructure. A comparison of the predicted mechanical properties based on finite element and Multiscale Generalized Method of Cells techniques shows close agreement. However, some differences exist with respect to experimentally determined material parameters due to the finite dimensions of the specimens. Lastly,mechanical properties of multilayered weaves are predicted with the finite element method. The considered material systems are carbon fiber in epoxy matrix as well as C/C-SiC. However, the procedure is applicable to a wide range of material systems.

We describe an impressive and rare case of surgical emphysema after minimally invasive rectal surgery. This case reports on a patient who developed mas-sive retroperitoneal, intraperitoneal and subcutaneous emphysema directly following a transanal endoscopic microsurgery(TEM) procedure for a rectal intramuco-sal carcinoma. Free intra-abdominal air after gastro-intestinal surgery can be a sign of a bowel perforation or anastomotic leakage. This is a serious complication often requiring immediate surgery. In our patient an abdominal computed tomography-scan with rectal con-trast showed no signs of a rectal perforation. Therefore this emphysema was caused by the insufflation of CO2 gas in the rectum during the TEM-procedure. Conserva-tive treatment resulted in an uneventful recovery. With the increasing usage of TEM for rectal lesions we ex-pect this complication to occur more often. After ruling out a full thickness rectal wall perforation in patients with surgical emphysema following TEM, conservative treatment is the treatment of choice.

AIM: To compare the results after revision of primary vertical banded gastroplasty(Re-VBG) and conversion to sleeve gastrectomy(cS G) or gastric bypass(cR YGB).METHODS: In this retrospective single-center study, all patients with a failed VBG who underwent revisional surgery were included. Medical charts were reviewed and additional postal questionnaires were sent to update follow-up. Weight loss, postoperative complications and long-term outcome were assessed. RESULTS: A total 152 patients were included in this study, of which 21 underwent Re-VBG, 16 underwent c SG and 115 patients underwent c RYGB. Sixteen patients necessitated a second revisional procedure. No patients were lost-to-follow-up. Two patients deceased during the follow-up period, 23 patients did not return the questionnaire. Main reasons for revision were dysphagia/vomiting, weight regain and insufficient weight loss. Excess weight loss(%EWL) after Re-VBG, cS G and cR YGB was, respectively, 45%, 57% and 72%. Eighteen patients(11.8%) reported postoperative complications and 27% reported long-term complaints. CONCLUSION: In terms of additional weight loss, postoperative complaints and reintervention rate, Rouxen-Y gastric bypass seems feasible as a revision for a failed VBG.
