BACKGROUND Large-cell neuroendocrine carcinoma(NEC)is an uncommon type of tumor that can occur in the *** aggressive cancer requires definitive ***,we describe the clinical characteristics and treatment of a postmenopausal woman with large cell NEC of the *** SUMMARY A 55-year-old Asian female presented with a 1-year history of postmenopausal vaginal *** ultrasound revealed a thickened endometrium(30.2 mm)and a hypervascular *** tomography revealed that the tumor had invaded more than half of the myometrium and spread to the pelvic lymph *** tumor marker,carcinoembryonic antigen,was elevated(3.65 ng/mL).Endocervical biopsy revealed high-grade endometrial *** underwent radical hysterectomy,bilateral salpingo-oophorectomy,omentectomy,and bilateral pelvic and para-aortic lymph node *** examination revealed mixed neuroendocrine and endometrioid adenocarcinoma,pT2N0M0,grade 3,and International Federation of Gynecology and Obstetrics stage *** showed moderate estrogen and progesterone receptor expressions(20%and 1%,respectively),focal CD56 expression(NEC marker),positive staining for vimentin,p53(wild type),and ki67(90%),and loss of expression of PMS2(Lynch syndrome marker).The patient received five cycles of cisplatin and etoposide after *** recurrence was noted after 5 *** We report the characteristics and successful management of a rare case of large cell endometrial NEC concomitant with Lynch syndrome.

Background:To test clinically relevant factors associated with quantitative artifact-free deep capillary plexus(DCP)metrics in patients with diabetes mellitus(DM).Methods:563 eligible eyes(221 with no diabetic retinopathy[DR],135 with mild DR,130 with moderate DR,and 77 with severe DR)from 334 subjects underwent optical coherence tomography-angiography(OCT-A)with a swept-source OCT(Triton DRI-OCT,Topcon,Inc.,Tokyo,Japan).Strict criteria were applied to exclude from analysis those DCP images with artifacts and of poor quality,including projection artifacts,motion artifacts,blurriness,signal loss,B-scan segmentation error,or low-quality score.A customized MATLAB program was then used to quantify DCP morphology from the artifact-free DCP images by calculating three metrics:foveal avascular zone(FAZ),vessel density(VD),and fractal dimension(FD).Results:166(29.5%)eyes were excluded after quality control,leaving in the analysis 397 eyes(170 with no DR,101 with mild DR,90 with moderate DR,36 with severe DR)from 250 *** the multiple regression models,larger FAZ area was associated with more severe DR(β=0.687;p=0.037),shorter axial length(AL)(β=−0.171;p=0.003),thinner subfoveal choroid thickness(β=−0.122;p=0.031),and lower body mass index(BMI)(β=−0.090;p=0.047).Lower VD was associated with more severe DR(β=−0.842;p=0.001),shorter AL(β=0.107;p=0.039),and poorer visual acuity(VA)(β=−0.133;p=0.021).Lower FD was associated with more severe DR(β=−0.891;pngful interpretations of DCP changes in DM patients.

We investigated the association of time to prostate-specific antigen nadir （TTPN） and logarithm of prostate-specific antigen velocity after progression Log（PSAVAP） in metastatic prostate cancer with prior primary androgen deprivation therapy （ADT）. All metastatic prostate cancer patients treated with primary ADT from 2000 to 2009 were reviewed. Patients who developed disease progression were included in the subsequent analyses. Patients were categorized into three groups according to their TTPN： TTPN of 〈3 months, 3-17 months, and 〉17 months. We compared the Log（PSAVAP） between the different TTPN groups using mann-Whitney U-test and Kruskal-Wallis test. Further multiple linear regression analyses on Log（PSAVAP） were performed to adjust for other potential confounding factors. Among 419 patients who were treated with primary ADT, 306 patients developed disease progression with a median follow-up of 28 months, Longer TTPN was associated with lower Log（PSAVAP）（P = 0.008） within all subgroup analyses （TTPN of 〈3 vs 3-17 months, P = 0.020; TTPN of 3-17 vs 〉17 months, P = 0.009; and TTPN of 〈3 vs 〉17 months, P = 0.001）. Upon multiple linear regression analyses, baseline PSA （regression coefficient 0.001, P = 0.045）, PSA nadir （regression coefficient 0.002, P = 0.040）, and TTPN （regression coefficient -0.030, P = 0.001） were the three factors that were significantly associated with Log（PSAVAP）. In conclusion, a longer TTPN was associated with lower Log（PSAVAP） in metastatic prostate cancer patients following primary ADT. TTPN cut-offs at 3 months and 17 months appeared to have prognostic significance in predicting Log（PSAVAP）. TTPN may serve as a good prognostic indicator in deciding the treatment strategy in patients with disease progression.

Introduction and Objective: There is increase recognition of the role of various parameters measured by non-contrast computerized tomogram (NCCT) in predicting treatment outcomes of urinary stone by shockwave lithotripsy (SWL). In our previous analysis, a clinical algorithm, based on stone volume (SV), mean stone density (MSD) and skin-to-stone distance (SSD), was formulated. (J Urol 2007;177 supp: A1316) Therefore, we would like to use our subsequent patients to validate the usefulness of this algorithm. Methods: After our first part of study, a further prospective collection of data from patients diagnosed to have solitary upper ureteral stone by NCCT and received primary SWL was performed. The primary outcome was the stone free rate at 3-month after one session of SWL by Sonolith 4000+. Pre-treatment NCCT were reviewed by a single radiologist with SV, MSD and SSD measured. These patient parameters were then applied into our algorithm to assess its validity in predicting treatment outcome. Results: 37 patients (24 males and 13 females) were recruited. The overall stone free rate was 45.9%. By applying the algorithms, for patients with either SV ≤ 0.2cc or MSD ≤600 or SDep ≤ 85mm, the stone free rate was 52.2% (95% CI 31.7%, 72.7%). For those patients without any favourable factors, the stone free rate was 35.7% (95% CI 10.6%, 60.8%). Although the difference was not statistically significant, a trend of difference can still be observed in this small sample. Conclusions: Clinical algorithm based on the measurement of stone volume, mean stone density and skin to stone distance from NCCT can help to predict the treatment outcome of SWL in upper ureteral stone.

It is largely unknown whether lower urinary tract symptoms(LUTS)or acute retention of urine(AROU)is linked to shorter life expectancy in *** conducted a multicenter,retrospective database analysis of patients undergoing transurethral resection of prostate(TURP)to study their *** Cox regression analysis and Kaplan-Meier analysis with stratification to age and indication of TURP were *** further performed an age-and sex-matched survival analysis with the general population using data from the Census and Statistics Department of the Hong Kong Special Administrative Region(Hong Kong,China).From January 2002 to December 2012,3496 patients undergoing TURP were included in our study,with 1764 patients in the LUTS group and 1732 patients in the AROU *** age,ischemic heart disease,cerebrovascular accident,and AROU were risk factors of *** aged80 *** to the general population,younger patients in the LUTS group appeared to have better survival(nger patients in the AROU group had worse survival(80 years,survival was similar with the general population in both the LUTS and AROU *** conclusion,AROU at young age was associated with mortality,while early detection and management of LUTS may improve survival.

An evaluation of compliance and performance following the introduction of the Inter-Facility Transport Triage Guideline

BACKGROUND：In Hong Kong, the reorganization of healthcare frame work for better utilization of resources has led to an increase in the frequency of inter-facility transport in recent years. An Inter-Facility Transport Triage Guideline （IFTTG） was introduced and evaluated on its compliance and performance. This study aimed to evaluate the compliance to the IFTTG and performance of inter-facility transport after the IFTTG was implemented. METHODS： The patients who required emergency IFT with accompanying personnel in two consecutive periods （pre-implementation period： January 1,2006 to April 30, 2006; post-implementation period： May 1,2006 to August 31,2006） were included. The compliance to pre-transport triage and transport team configuration was evaluated by a review panel with eight performance indicators identified. The performance indicators were compared individually and as an overall score. RESULTS：Sixty-eight （26 in the pre-implementation period and 42 in the post-implementation period） IFTs were reviewed. There was demonstrable improvement on compliance to the IFTTG. The appropriateness of pre-transport triage increased from 34.6% to 54.8% whereas appropriateness of transport team configuration rose significantly from 73.1% to 92.9% （P〈0.05）. Staff performance on individual IFT performance indicator was satisfactory in both periods and the means of overall score on performance indicators were 7.12 and 7.29 respectively. The most improved performance indicator was the appropriateness of transport team configuration. CONCLUSIONS： The compliance and performance with the newly implemented IFTTG were satisfactory. However, staff comment and satisfaction with the use of the new guideline should be collected so as to achieve continuous quality improvement.
