Long-term results after revisions of failed primary vertical banded gastroplasty

AIM: To compare the results after revision of primary vertical banded gastroplasty(Re-VBG) and conversion to sleeve gastrectomy(cS G) or gastric bypass(cR YGB).METHOdS: In this retrospective single-center study, all patients with a failed VBG who underwent revisional surgery were included. Medical charts were reviewed and additional postal questionnaires were sent to update follow-up. Weight loss, postoperative complications and long-term outcome were assessed. RESULTS: A total 152 patients were included in this study, of which 21 underwent Re-VBG, 16 underwent c SG and 115 patients underwent c RYGB. Sixteen patients necessitated a second revisional procedure. No patients were lost-to-follow-up. Two patients deceased during the follow-up period, 23 patients did not return the questionnaire. Main reasons for revision were dysphagia/vomiting, weight regain and insufficient weight loss. Excess weight loss(%EWL) after Re-VBG, cS G and cR YGB was, respectively, 45%, 57% and 72%. Eighteen patients(11.8%) reported postoperative complications and 27% reported long-term complaints. CONCLUSION: In terms of additional weight loss, postoperative complaints and reintervention rate, Rouxen-Y gastric bypass seems feasible as a revision for a failed VBG.

The Global Wheat Head detection(GWHd)dataset was created in 2020 and has assembled 193,634 labelled wheat heads from 4700 RGB images acquired from various acquisition platforms and 7 countries/*** an associated competition hosted in Kaggle,GWHd_2020 has successfully attracted attention from both the computer vision and agricultural science *** this first experience,a few avenues for improvements have been identified regarding data size,head diversity,and label *** address these issues,the 2020 dataset has been reexamined,relabeled,and complemented by adding 1722 images from 5 additional countries,allowing for 81,553 additional wheat *** now release in 2021 a new version of the Global Wheat Head detection dataset,which is bigger,more diverse,and less noisy than the GWHd_2020 version.

Nutrition is an important aspect of public health because it is linked to many significant diseases and health problems. This work is part of the promotion of traditional foods from the democratic Republic of Congo in general and in the province of Bandundu, in particular by assessing the selenium content and the antioxidant capacity of wild edible mushrooms. Methanolic extracts from selected mushrooms were characterized for their chemical fingerprint by TLC and their in vitro antioxidant activities using ABTS, dPPH assays. Phytochemical screening revealed the presence of alkaloids, free amines, steroids and terpenes in all species. However, Auricularia delicata and Pleurotus tuberregium contain flavonoids. All extracts displayed a good radical-scavenging activity at the concentration range 1 - 300 μg/mL in the following order: Auricularia delicata > Lentinus cf cladopus > Pleurotus tuberregium > Marasmius buzungolo > Schizophillum commune. Studied mushrooms showed the interest quantity of selenium and L. cf cladodus had the highest concentration. Moderate selenium content of these wild mushrooms associated with their important antioxidant activities could provide health benefits to Bandundu’s populations in protecting against oxidative damage under different conditions including konzo.

背景：超薄切面三维快速自旋回波(3-dimentional fast spin-echo。3-d FSE)T2加权成像(T2-weighted imaging,T2WI)是一种新的磁共振成像(MRI)技术，我们用其来评估眼内及眶内恶性肿瘤。并与常规MRI技术比较，以观察其有效性。方法：回顾性分析就诊于加州大学旧金山分校眼部肿瘤科的连续26例患者的影像学资料，分析者为眼部肿瘤科医师和神经影像学医师。将所有患者的3-d FSE T2加权成像(共27份)与常规MRI的图像相比较。并尽可能联系患者的其他检查结果。包括：计算机断层扫描、A超、B超、超声生物显微镜、临床检查、组织病理学检查。结果：与标准MRI资料相比。27例中17例(63％)患者的3-d FSE T2加权成像序列可以全面提高影像学检查发现的准确度；11例(41％)患者因提高了图像分辨率而导致放射影像学下的附加病变的发现；对6例(22％)患者提高了放射影像学诊断的信心。非视网膜母细胞瘤病变的5例患者中有3例因3-d FSE T2加权序列提高了分辨率。从而改变了疾病的治疗方案。使用3-d FSE或者常规成像技术均可以发现所有的活动性视网膜母细胞瘤病变。但对于非活动性的病变。常规成像技术并非都能发现。结论：与常规MRI技术相比。3-d FSE T2加权序列提供了对眼内和眶内结构的出众的分辨率。在评估眼内肿瘤和神经鞘复合体时尤其有用。这一新技术也提高了对眼内和眶内恶性肿瘤患者的诊断和治疗水平。

Global regulations and many regional and national controls restrict the use of substances that exhibit the potential for environmental persistence and long-range ***,many current-use pesticides(CUPs)continue to be newly discovered in remote regions,including the *** present review serves as an update,summarizing newly available information for CUPs in the Arctic environment and biota published from 2010 to *** 2010,at least seven new CUPs have been measured in Arctic media:2-methyl-4-chlorophenoxyacetic acid(MCPA),metribuzin,pendimethalin,phosalone,quizalofop-ethyl,tefluthrin and *** the large number of pesticides in current use,the number measured in the Arctic is very limited,however,modelling studies have identified additional CUPs as potential Arctic contaminants that have yet to be investigated in the *** to their recent detection,reports of CUPs in the Arctic are limited,but *** have been reported in a wide range of abiotic Arctic matrices,including air,snow,ice,freshwater and seawater,indicating their capacity for long-range atmospheric transport,however,concentrations are generally low in comparison to legacy pesticides and other persistent organic pollutants(POPs).Recent food-web studies indicate CUPs can enter Arctic terrestrial and marine food chains,however,in contrast to POPs,the highest concentrations of many CUPs were found in lower trophic-level organisms,and the lowest concentrations detected in animals at the highest trophic levels(i.e,ringed seals,polar bear,caribou,and wolves)indicating significant trophic *** detection of CUPs in the remote Arctic ecosystem reinforces the need for continued monitoring of both known and potential Arctic pollutants to prevent impacts on human and environmental health as the global arsenal of pesticides used in agriculture continuously changes.

AIM:To analyze whether high-intensity focused ultrasound(HIFU) ablation is an effective bridging therapy for patients with hepatocellular carcinoma(HCC).METHOdS:From January 2007 to december 2010,49 consecutive HCC patients were listed for liver transplantation(UCSF criteria).The median waiting time for transplantation was 9.5 ***-nine patients received transarterial chemoembolization(TACE) as a bringing therapy and 16 patients received no treatment before *** patients received HIFU ablation as a bridging *** five patients with the same tumor staging(within the UCSF criteria) who received HIFU ablation but not on the transplant list were included for *** were comparable in terms of Child-Pugh and model for end-stage liver disease scores,tumor size and number,and cause of ***:The HIFU group and TACE group showed no difference in terms of tumor size and tumor *** patient in the HIFU group and no patient in the TACE group had gross *** median hospital stay was 1 d(range,1-21 d) in the TACE group and two days(range,1-9 d) in the HIFU group(P d complication *** the HIFU group,nine patients(90%) had complete response and one patient(10%) had partial response to the *** the TACE group,only one patient(3%) had response to the treatment while 14 patients(48%) had stable disease and 14 patients(48%) had progressive disease(P = 0.00).Seven patients in the TACE group and no patient in the HIFU group dropped out from the transplant waiting list(P = 0.559).CONCLUSION:HIFU ablation is safe and effective in the treatment of HCC for patients with advanced *** may reduce the drop-out rate of liver transplant candidate.
