First Use of Bedaquiline in democratic Republic of Congo: Two Case Series of Pre Extensively drug Resistant Tuberculosis

In this manuscript the authors have studied the first two patients who were successfully treated with the treatment regimen containing Bedaquiline as second-line drug. The patients were diagnosed with pre-extensively drug-resistant tuberculosis (preXdR TB) whose prognosis was fatal in democratic Republic of Congo (dRC). Bedaquiline is arguably one of the molecules of the future in the management of ultra-resistant tuberculosis. However, a larger cohort study may help to establish its effectiveness. Case report: Patients 1, 29 years old, with a history of multidrug-resistant TB (MdR-TB) one year previously. He showed signs of TB impregnation again 6 months after the last treatment. Bascilloscopy was positive again. The pre-extensively tuberculosis (pre-XdR TB) diagnosis was made by the Hain test (GenoType®MTBdRsl, Hain Lifescience). Patient 2, brother of the first patient, with a history of MdR TB a year before. He had low back pain with right parietal dorso swelling four months after the last treatment. The x-ray of the column showed L4-L5 disc disease. Parietal ultrasound showed a parietal abscess to the right of thoracic vertebrae with fistulization. Surgical biopsy and pus culture confirmed the diagnosis of Pre-XdR Extrapulmonary TB. The treatment regimen was the same for both patients: 6 months with Amikacin (Am) Bedaquiline (Bdq) Prothionamide (Pto) Paraamino Salicylic acid (PAS) Linezolid (Lzd) Cycloserine (Cs) Pyrazinamide (Z) and 14 months with PAS Lzd Cs Z. The side effects were minor. Bacteriological controls (smears and cultures) after 20 months of treatment are negative to date.

Introduction: In the North-Benin, there are three agents causing pediatric purulent meningitis outside the neonatal period. These are: Streptococcus pneumoniae, Neisseria meningitidis and Haemophilus influenzae type b. The aim of this research work was to investigate bacteria serotypes that caused childhood purulent meningitis in the pediatric unit of the Borgou à Regional University Teaching Hospital (CHUd-Borgou) located in Parakou (North-Benin). Patients and Methods: Through a prospective and descriptive study centered on children aged 0 to 5 years old suspected of meningitis and hospitalized, the cerebrospinal fluid (CSF) samples of those children were analyzed at the WHO reference laboratory in Banjul for serotyping by real time polymerase chain reaction (RT/PCR). Results: Among the 1396 children hospitalized during that period, 366 were suspected of meningitis and had benefitted from lumbar puncture. Among those 366 suspected cases, 51 cases of purulent meningitis were confirmed after CSF cytobacteriological and biochemical test at the CHUd-Borgou laboratory. Among 51 CSF samples in which purulent meningitis was confirmed, 44 were sent to Banjul. In addition, 310 CSF samples from non-confirmed cases of meningitis were also sent to Banjul. In the whole set of samples sent for real time PCR, 151 cases of Streptococcus pneumoniae (42.7%) were found, 5 cases of Neisseria meningitidis (1.4%) and 1 case of Haemophilus influenzae (0.3%) were also encountered. As regards Streptococcus pneumonia, the serotypes encountered were: 1, 3, 4, 5, 7F, 8, 9V, 9V/9A, 9N/9L, 14, 18C, 19A, 23F, 33F as well as non typed and non typable serotypes. As for Neisseria meningitidis, only serogroup A was found in it. For Haemophilus influenzae, only serotype b was identified. Conclusion: Four non vaccine serotypes (8, 9V/9A, 9N/9L and 33F), non typed and non typable serotypes which are not covered by 13-valent pneumococcal conjugate vaccine (PCV 13) were identified. Introduction: The purpose of cardiac rehabilitation is to improve the effects of aging and maintain a good quality of life for older individuals. This study aims to assess how cardiac rehabilitation affects the autonomy and quality of life of older adults. Patients and Method: This was a longitudinal, descriptive and comparative before-and-after cardiovascular rehabilitation study conducted over a two-year period from January 2019 to december 2021. This study was conducted at four cardiac rehabilitation units in dakar: the Aristide Le dantec Hospital (HALd), National dalal Jamm Hospital, and the General Idrissa Pouye Hospital (HOGIP). We compared the degree of autonomy, dependence and quality of life of subjects aged over 65 before and after the cardiac rehabilitation program using the Katz index, the Lowton index and the SF12 quality-of-life questionnaire. Results: Over a two-year period, a total of 345 patients had benefited from a complete cardiovascular rehabilitation program in the four cardiovascular rehabilitation units in dakar, and 86 patients, or 24.92% of the population, were at least 65 years old. The patients were predominantly male (sex ratio M/F = 4.73). The mean age was 70.35 ± 4.55 years for men and 69.27 ± 4.59 years for women. The main pathology motivating cardiac rehabilitation was ischemic heart disease, which was found in 73 patients (84.88%). Initial assessment revealed exertional dyspnea in 35 patients (40.69%), followed by residual exertional angina in 21 patients (4.41%). Mean functional capacity increased from 5.81 ± 2.38 Mets in pre-cardiac rehabilitation to 8.68 ± 2.28 Mets in post-cardiac rehabilitation (p distance covered in the 6-minute walk test increased from 330.42 ± 170.50 m in pre-cardiac rehabilitation to 524 ± 98.54 m in post-cardiac rehabilitation (p = 0.119). Background: Opposite clear corneal incisions (OCCIs) have been reported to red uce pre-existing astigmatism (PEA) during cataract surgery. Our goal was to eva luate the effect of OCCIs on correcting PEA in cataract surgery. Methods: Nonran domized prospective study. Thirty-four patients with PEA of greater than 1.5 di opters (d) underwent clear cornea phacoemulsification cataract extraction with 3 .2-mm OCCIs (OCCI group). The control group consisted of 23 successive patients with PEA d who underwent cataract extraction without OCCI. Best-correcte d visual acuity, keratometry and refraction were recorded for all patients pre- operatively and post-operatively. Results: Using keratometric findings, mean as tigmatism correction was 1.3 d (±0.9 Sd; decreased from 2.6 d preoperatively to 1.4 d post-operatively) in the OCCI group but only 0.4 d in the control group (P d greater change for OCCI patients (1.8 d vs 1.0 d, P=0.002). Using the Ho lladay method for calculating surgically induced refractive change (SIRC), the O CCI group showed a higher value of SIRC (-1.6 d vs-0.97 d), but this was not s tatistically significant. The OCCI patients showed a greater and significant cha nge in refraction spherical equivalent than the controls. No complications relat ed to OCCI or cataract surgery occurred during the follow-up period. Conclusion s: Opposite clear cornea incision seems to be a simple, predictable, safe and ef fective procedure in reducing pre-existing corneal astigmatism in cataract surg ery. It has an enhanced effect in correcting astigmatism compared to a single cl ear cornea incision when using keratometric findings value but not when using re fractive data. Future studies are needed to document the long-term effect of OC CI and to evaluate the correlation between incisions of different size and astig matism correction.

Mortality is a well-established patient-important outcome in critical care *** contrast,morbidity is less uniformly reported(given the myriad of critical care illnesses and complications of each)but may have a common end-impact on a patient’s functional capacity and health-related quality-of-life(HRQoL).Survival with a poor quality-of-life may not be acceptable depending on individual patient values and ***,as mortality decreases within critical care,it becomes increasingly important to measure intensive care unit(ICU)survivor *** measurements with a preference-based scoring algorithm can be converted into health utilities on a scale anchored at 0(representing death)and 1(representing full health).They can be combined with survival to calculate quality-adjusted life-years(QALY),which are one of the most widely used methods of combining morbidity and mortality into a composite *** QALYs have been use for health-technology assessment decision-making,an emerging and novel role would be to inform clinical decision-making for patients,families and healthcare providers about what expected HRQoL may be during and after ICU *** care randomized control trials(RCTs)have not routinely measured or reported HRQoL(until more recently),likely due to incapacity of some patients to participate in patient-reported outcome *** differences in HRQoL measurement tools can lead to non-comparable *** this end,we propose the validation of a gold-standard HRQoL tool in critical care,specifically the *** combined health-utility and mortality(disaggregated)and QALYs(aggregated)can be reported,with disaggregation allowing for determination of which components are the main drivers of the QALY *** use of HRQoL,health-utility,and QALYs in critical care RCTs has the potential to:(1)Increase the likelihood of finding important effects if they exist;(2)improve research efficiency;and(3)help inform optimal m

Introduction: Hepatitis B is a significant public health problem. Infected children Objective: We aimed to assess newborns’ management and outcome from reactive HBs Ag mothers at the El Rapha Polyclinic in Libreville, Gabon. Methodology: Prospective longitudinal study, carried out over four years, including all neonates born from reactive HBsAg mothers admitted in the delivery room. The parameters studied were maternal age, gestation, marital status, pregnancy follow-up, hepatitis B vaccination status, knowledge of hepatitis B, anti-Hbc antibodies’ levels, HBeAg. For the newborn: term, birth weight, management at birth, and hepatitis B serological status at 12 months. Results: We included 45 newborns. Mothers’ mean age was 30.9 ± seven years. In 32.6% of cases, antenatal consultations were ≤4, and 9.3% were vaccinated for hepatitis B. Mothers had chronic hepatitis in 74.4% of cases. All neonates were vaccinated for hepatitis B at birth;68.9% received hepatitis B immunoglobulin. At 12 months, 77.8% had HBs antibody positive, and 6.7% were HBsAg positive. Conclusion: Hepatitis B prevalence is significant among parturients. It is essential to set up a national strategy for managing the mother-child couple in a context of positive HBsAg.
