Ventriculo-atrial shunt with occlusion of the internal jugular vein: operative experience and surgical technique

Background Ventricular shunts are one of the most frequent techniques used for the management of hydrocephalus. The ventriculoperitoneal shunt (VPS) is the most commonly performed procedure, and the ventriculoatrial shunt (VAS) is the second option in most medical centers. The main objective of this study is to introduce and describe a surgical approach for VAS outlining our experience and comparing it with traditional shunting *** In this retrospective cohort comparison study, we included patients with hydrocephalus treated with a surgical procedure between January 2010 and February 2021 at a single academic institution. We categorized the procedures into two groups: patients with VPS and conventional VAS grouped together into the conventional technique (CT) group, and the second group was patients with whom we performed VAS with complete internal jugular vein occlusion (IJVOT). We compared the surgical time, postoperative complications, and occurrence of shunt failure among the groups by performing univariate analysis using the Fisher exact *** Out of the 106 patients included in the analysis, IJVOT was performed in 66 patients, and CT in 40 patients. The median surgical time was 60 min (IQR 60-90) for IJVOT versus 100 min (IQR 60-120) for CT (p < 0.01). In the follow-up a month after the procedure, 83.3% of patients with IJVOT and 62.5% of patients with CT did not require shunt removal or shunt revision (p < 0.01). Shunt revision rates were 12.5% and 1.5% for CT while 1.5% and 2.5% for IJVOT at 1 and 6 months after the *** Our findings demonstrate that VAS with IJVOT is a safe method that exhibited shorter surgical times and outcomes comparable to CT. However, since the present study represents the first cohort evaluating IJVOT, it is imperative to conduct larger prospective studies, along with clinical trials, to fully explore and establish efficacy, long-term outcomes, and an in-depth comparison among shunting techniques

Recent advances in bird sperm morphometric analysis and its role in male gamete characterization and reproduction technologies

Postcopulatory sexual selection through sperm competition may be an important evolutionary force affecting many reproductive traits, including sperm morphometrics. Environmental factors such as pollutants, pesticides, and climate change may affect different sperm traits, and thus reproduction, in sensitive bird species. Many sperm-handling processes used in assisted reproductive techniques may also affect the size of sperm cells. The accurately measured dimensions of sperm cell structures （especially the head） can thus be used as indicators of environmental influences, in improving our understanding of reproductive and evolutionary strategies, and for optimizing assisted reproductive techniques （e.g., sperm cryopreservation） for use with birds. Computer-assisted sperm morphometry analysis （CASA-Morph） provides an accurate and reliable method for assessing sperm morphometry, reducing the problem of subjectivity associated with human visual assessment. Computerized systems have been standardized for use with semen from different mammalian species. Avian spermatozoa, however, are filiform, limiting their analysis with such systems, which were developed to examine the approximately spherical heads of mammalian sperm cells. To help overcome this, the standardization of staining techniques to be used in compute^-assessed light microscopical methods is a priority. The present review discusses these points and describes the sperm morphometric characteristics of several wild and domestic bird species.

Deposition of the H2A.Z histone variant by the SWR1 complex （SWRI-C） in regulatory regions of specific loci modulates transcription. Characterization of mutations in Arabidopsis thaliana homologs of yeast SWRI-C has revealed a role for H2A.Z exchange in a variety of developmental processes. Nevertheless, the exact composition of plant SWRI-C and how it is recruited to target genes remains to be established. Here we show that SWC4, the Arabidopsis homolog of yeast SANT domain protein Swc4/Eaf2, is a DNA-binding protein that interacts with SWR1-C subunits. We demonstrate that the swc4-1 knockout mutant is embryo- lethal, while SWC4 RNAi knockdown lines display pleiotropic phenotypic alterations in vegetative and repro- ductive traits, including acceleration of flowering time, indicating that SWC4 controls post-embryonic processes. Transcriptomic analyses and genome-wide profiling of H2A.Z indicate that SWC4 represses tran- scription of a number of genes, including the floral integrator FT and key transcription factors, mainly by modulating H2A.Z deposition. Interestingly, SWC4 silencing does not affect H2A.Z deposition at the FLC locus nor expression of this gene, a master regulator of flowering previously shown to be controlled by SWR1-C. Importantly, we find that SWC4 recognizes specific AT-rich DNA elements in the chromatin regions of target genes and that SWC4 silencing impairs SWRI-C binding at FT. Collectively, our data suggest that SWC4 regulates plant growth and development by aiding SWR1-C recruitment and modulating H2A.Z deposition.

When Information Does Not Translate into Knowledge. Ebola and Hemorrhagic Fevers Knowledge among Primary Care Physicians and Nurses

After the first secondarily-transmitted ebola case in Spain, a wave of divergent opinions flooded mass and sanitary media. Very few of these opinions, however, came from experts on epidemiology or hemorrhagic fevers. This observational study aimed to assess the specific knowledge of Primary Care physicians and nurses about ebola and hemorrhagic fevers by means of analyzing the results obtained from a 5-item self-reported questionnaire dealing on hemorrhagic fevers basic knowledge. Validity and reliability of questionnaire were confirmed by a pilot study. The participants were 138 family doctors and nurses from the 64 public Primary Care centers sited in the North Metropolitan Area of Barcelona (1,400,000 inhab;Catalonia, Spain) taking part in training-the-trainers ebola workshops. Overall, there were 117 (84.8%) respondents out from 138 workshop participants;of them were physicians 61 (51.2%). The main age was 46.7 (8.8) years;stating previous specific knowledge on hemorrhagic fevers 39 (33.3%). On the whole, up to 92 (78.6%) of respondents shown a poor knowledge. Previous specific formation was significantly and independently associated with having proper knowledge (p < 0.001);OR = 8.6 (CI 95%: 3.199 - 23.623). In summary, confusion that accompanied the single secondary-transmitted ebola case in Spain could probably be explained by the existence of a serious gap on hemorrhagic fevers knowledge. More accurate, scientific and formally-presented information should be provided to Primary Care physicians and nurses.

Hepatitis C virus(HCV)infection affects about 170 million people worldwide and it is a major cause of liver cirrhosis and hepatocellular *** is a hepatotropic non-cytopathic virus able to persist in a great percentage of infected hosts due to its ability to escape from the immune *** damage and disease progression during HCV infection are driven by both viral and host ***,adaptive immune response carries out an essential task in controllingnon-cytopathic viruses because of its ability to recognize infected cells and to destroy them by cytopathic mechanisms and to eliminate the virus by non-cytolytic *** is able to impair this response by several means such as developing escape mutations in neutralizing antibodies and in T cell receptor viral epitope recognition sites and inducing HCV-specific cytotoxic T cell anergy and *** impair HCV-specific T cell reactivity,HCV affects effector T cell regulation by modulating T helper and Treg response and by impairing the balance between positive and negative co-stimulatory molecules and between pro-and antiapoptotic *** this review,the role of adaptive immune response in controlling HCV infection and the HCV mechanisms to evade this response are reviewed.

The creeping verminous dermatitis or cutaneous larva migrans is a parasitosis caused by percutaneous penetration and migration of larval nematode parasites characterized by producing one or more serpiginous erythematous,indurated,raised and pruritic *** most common cause of cutaneous larva migrans is the Ancytostoma braziliensc located in warm climate *** the present study,authors reported a case of cutaneous larva migrans with a characteristic clinical picture:erythematous-papnlar and vesicular lesion and serpiginous path,with progressive,and pruritic growth and it shown that a living area with immigration,tropical weather conditions and poverty may lead to this common infection.

Plant-based bioproduction of insect sex pheromones has been proposed as an innovative strategy to increase the sustainability of pest control in ***,we describe the engineering of transgenic plants producing(Z)-11-hexadecenol(Z11-16OH)and(Z)-11-hexadecenyl acetate(Z11-16OAc),two main volatile components in many Lepidoptera sex pheromone *** assembled multigene DNA constructs encoding the pheromone biosynthetic pathway and stably transformed them into Nicotiana benthamiana *** constructs contained the Amyelois transitella AtrΔ11 desaturase gene,the Helicoverpa armigera fatty acyl reductase HarFAR gene,and the Euonymus alatus diacylglycerol acetyltransferase EaDAct gene in different *** the pheromone-producing plants showed dwarf phenotypes,the severity of which correlated with pheromone *** but one of the recovered lines produced high levels of Z11-16OH,but very low levels of Z11-16OAc,probably as a result of recurrent truncations at the level of the EaDAct *** one plant line(SxPv1.2)was recovered that harboured an intact pheromone pathway and which produced moderate levels of Z11-16OAc(11.8μg g^(-1) FW)and high levels of Z11-16OH(111.4μg g^(-1)).Z11-16OAc production was accompanied in SxPv1.2 by a partial recovery of the dwarf ***1.2 was used to estimate the rates of volatile pheromone release,which resulted in 8.48 ng g-1 FW per day for Z11-16OH and 9.44 ng g^(-1 )FW per day for *** results suggest that pheromone release acts as a limiting factor in pheromone biodispenser strategies and establish a roadmap for biotechnological improvements.

Thermal Comparison of an Organic Rankine Cycle Activated by a Low-Temperature Geothermal Heat Source

This study compares the results from the thermodynamic analysis at an Organic Rankine Cycle power plant,for first and second law efficiency,operating with 14 different working *** plant—located in the town of Los Negritos,in the municipality of Villamar,in the State of Michoacán,Mexico—uses heat from a low-temperature geothermal source with the purpose of identifying the working fluids that are best suited for the operating conditions at *** v.8.0 software was used to collect the thermodynamic *** all the thermodynamic parameters that were analyzed,the fluid that performed the best had an overall energy efficiency of 5.87%,an overall exergy efficiency of 43.07%,and a net output power of 13.04 kWe.
