Malignant disease,including mucinous carcinomas of the colorectum,may complicate long-standing Crohn’s *** 18-year-old male with extensive small and large bowel involvement with Crohn’s disease developed recurrent peri-rectal fistulous disease that persisted for more than a decade despite pharmacological and surgical therapy as well as later therapy with biological agents. Eventually,an extensive and difficult-to-detect mucinous carcinoma developed in the fistulous *** fistula cancer is rarely described in Crohn’s disease,use of immunosuppressant and biological agents may play an initiating or exacerbating role in its development or *** potent biological agents are frequently used, often to avoid surgical treatment,clinicians should have an increasingly high index of suspicion for this potential complication,especially if fistulous drainage persists and remains refractory to medical therapy.

Background:Porto-sinusoidal vascular disease(PSVd)and portal vein thrombosis(PVT)are causes of portal hypertension characterized respectively by an intrahepatic and a pre-hepatic obstacle to the flow in the portal *** PVT may be a consequence of PSVd,in PVT patients at presentation,a pre-existing PSVd should be *** these patients the identification of an underlying PSVd would have relevant implication regarding follow-up and therapeutic management,but it could be *** this setting ultrasonography may be valuable in differential *** aim of the study was to use ultrasonography to identify parameters to discriminate between PSVd and“pure”PVT and then to suspect PVT secondary to a pre-existing ***:Fifty-three patients with histologically proven PSVd and forty-eight patients affected by chronic PVT were enrolled and submitted to abdominal ultrasonography with elastography by acoustic radiation force impulse(ARFI).Results:ARFI was higher and superior mesenteric vein(SMV)diameter was wider in PSVd patients than in PVT ***,a prognostic score was obtained as linear combinations of the two parameters with a good discrimination capacity between PSVd and PVT(the area under the curve=0.780;95%confidence interval:0.690-0.869).Conclusions:A score based on ARFI and SMV diameter may be useful to suspect an underlying PSVd in patients with PVT and to identify a subgroup of patients to be submitted to liver biopsy.

In this paper,a compact mathematical model having an elegant structure,together with a generic control framework,are proposed for generic power systems dominated by power converters that are interconnected through a passive transmission and distribution(T&d)grid,by adopting the port-Hamiltonian(pH)systems theory and the fundamental circuit *** models of generic T&d lines are developed and then the model of a generic T&d grid is *** the proposed control framework,the controlled converters are proven to be passive and Input-to-State Stable(ISS).The compact mathematical model is scalable and can be applied to power systems with multiple power electronic converters with generic passive controllers,passive local loads,and different types of passive T&d lines connected in a meshed configuration without self-loops,so it is very ***,the resulting power system is proven to be ISS as *** analysis is carried out without assumptions on constant frequency/voltage,constant loads,and/or lossless networks,except the need of passivity for all parts involved,and without using the Clarke/Park transformations or the graph *** simplify the presentation,three-phase balanced systems are adopted but the results can be easily adapted for single-phase or unbalanced three-phase systems.

BACKGROUNd:There are currently no effective drugs to mitigate the ischemia/reperfusion injury caused by fluid resuscitation after hemorrhagic shock(HS).The aim of this study was to explore the potential of the histone deacetylase 6(HdAC6)-specific inhibitor tubastatin A(TubA)to suppress nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3)inflammasome activation in macrophages under hypoxia/reoxygenation(H/R)***:The viability of RAW264.7 cells subjected to H/R after treatment with different concentrations of TubA was assessed using a cell-counting kit-8(CCK8)***,2.5μmol/L TubA was used with RAW264.7 cells under H/R ***264.7 cells were divided into three groups,namely the control,H/R,and TubA *** levels of reactive oxygen species(ROS)in the cells were detected using fluorescence *** protein expression of HdAC6,heat shock protein 90(Hsp90),inducible nitric oxide synthase(iNOS),NLRP3,gasdermin-d(GSdMd),Caspase-1,GSdMd-N,and Caspase-1 p20 was detected by western *** levels of interleukin-1β(IL-1β)and IL-18 in the supernatants were detected using enzyme-linked immunosorbent assay(ELISA).RESULTS:HdAC6,Hsp90,and iNOS expression levels were significantly higher(Pduced in the TubA group(Pd higher NLRP3,GSdMd,Caspase-1,GSdMd-N,and Caspase-1 p20 expression levels than the control group(Pd significantly lower expression levels than the H/R group(Pd IL-18 levels in the supernatants were significantly higher in the H/R group compared to the control group(Pd to the H/R group(Pd the expression of HdAC6,Hsp90,and iNOS in macrophages
