Anaemia and thrombocytopenia are haematological disorders that can be detected in many human immunodeficiency virus(HIV)-positive patients during the development of HIV infection. The progressive decline of erythrocytes and platelets plays an important role both in HIV disease progression and in the clinical and therapeutic management of HIV-positive patients. HIV-dependent impairment of the megakaryocyte and erythrocyte lineages is multifactorial and particularly affects survival, proliferation and differentiation of bone marrow(BM) CD34+ haematopoietic progenitor cells, the activity of BM stromal cells and the regulation of cytokine networks. In this review, we analyse the ma-jor HIV-related mechanisms that are involved in the genesis and development of the anaemia and thrombocytopenia observed in HIV positive patients.

Obesity and sarcopenia combination, appropriately defined as sarcopenic obesity (sO), due to disproportionally reduced/low lean body mass compared to excess fat mass, may lead to disability. Aims: The aim of our study was to investigate the relationship among sarcopenic obesity, physical performance, disability, and quality of life in a rehabilitation setting. Methods: Participants were recruited among obese patients (BMI > 30 kg/m2) admitted to the rehabilitation facility at the Department of Experimental Medicine, Medical Physiopatology, food science and Endocrinology section during a 1-year period. A multidimensional evaluation was performed through bioelectrical impedance analysis and anthropometry, handgrip strength test, short Physical Performance Battery (sPPB), 6-minute walk test (6MWT) and blood chemistry parameters. Psychological status (sCL-90 questionnaire), quality of life, and comorbidity (Charlson comorbidity index score) were also evaluated. Obesity was diagnosed as increased fat mass by 35% in women and by 25% in men. sarcopenia was defined if lean body mass (LBM) was f the subject’s ideal LBM. Results: 79 patients (48 women and 31 men;mean age: 60.1 ± 11.5 years, and 58.6 ± 10.8 years, respectively) were enrolled. Results showed a high prevalence of sO (54.4%) in our samples of obese subjects. sarcopenia was present not only among older obese adults but also among younger obese subjects, and was related to reduced functional performance, to inflammatory status and to worse psychological status and quality of life.

strong>Objective:strong> To evaluate the role of invasive imaging in the identification and pre-surgical localization of endoductal breast lesions. strong>Methods:strong> We retrospectively evaluated cytological outcomes, non-invasive/invasive breast imaging obtained between January 2016 and December 2019 in women with pathological nipple discharge (PND). We analysed sensitivity, specificity, positive predictive value and negative predictive value. We also evaluated the advantages of a pre-surgical radiological study using an endoductal contrast medium (with 3D-technique, in young women with dense breasts). strong>Results:strong> A total of 286 women with PND underwent cytological examination, mammography and/or breast ultrasound. When the cytological outcome was reported as “negative” (66.78%) in agreement with negative noninvasive imaging, patients were sent to follow up. Patients with cytological outcomes defined as “bloody with papillary clusters” (29.37%) “bloody not associated to cytological modifications” (2.44%), or “atypical/suspected” for malignant (1.39%) underwent an invasive procedure. sensitivity, specificity, positive predictive value and negative predictive value were, respectively: 92.63%, 100%, 100% and 96.46% for cytological examination;64.28%, 96.95%, 60% and 97.44% for mammography;41.11%, 97.44%, 88.09% and 78.27% for ultrasound;93.68%, 100%, 100% and 96.95% for invasive procedures. Post-surgical histological outcomes confirmed the diagnosis. strong>Conclusion:strong> In absence of a standard diagnostic algorithm, we recommend invasive procedures to identify intraductal breast lesions and for preoperative planning. Digital imaging and new technologies such as 3D-tomosynthesis lead to a renaissance of breast invasive imaging;they are confirmed to be an essential diagnostic modality for preoperative planning, to define localization and extension of multiple coexisting endoductal lesions.

In Brazil, the prominent climate-induced disasters are floods and mass movements, with the latter being the most lethal. The spate of major landslide events, especially those in 2011, catalyzed the creation of CEMADEN (National Center for Monitoring and Early Warning of Natural Disasters). This article introduces one of CEMADEN’s pivotal systems for early landslide warnings and traces its developmental timeline. The highlighted sNAKE system epitomizes advancements in digital monitoring, forecasting, and alert mechanisms. By leveraging precipitation data from pluviometers in observed municipalities, the system bolsters early warnings related to potential mass movements, like planar slides and debris flows. Its deployment in CEMADEN’s situation Room attests to its suitability for overseeing high-risk municipalities, attributed primarily to its robustness and precision.

It is widely accepted that quantitative reference materials (RM), are indispensable tools for verification of the precision and accuracy of analytical measurements. The RM can be used by food microbiology laboratories, as part of their quality assurance programmes, to achieve their quality control. In Brazil, Anvisa RDC No. 12/01 specifies the enumeration of coliforms as one of the parameters for evaluating cheese quality. The aim of this study was to produce a quantitative RM for proficiency testing (PT) for use in the testing of enumeration of coliforms in cheese matrixes. A sample of an ultra-filtered cheese with a coliforms count of f viable aerobes of 1.2 × 103 CfU/g was used as the matrix to produce the RM. The ultra-filtered cheese matrix was distributed in flasks, contaminated with a specific concentration of an Escherichia coli strain and submitted to freeze-drying. sucrose was used as the cryo-protector. The RM produced was considered sufficiently homogeneous and stable at ≤ ?70?C during the entire study period (348 days). The material was also considered sufficiently stable at 4?C for six days, but instable at 30?C and 35?C for the same period. At ?20?C the RM was sufficiently stable for 161 days. It was concluded that the material showed all the necessary requirements for a quality RM to be used as PT items and could be transported to the laboratories taking part in a PT at up to 4?C for up to 6 days, since the results indicated maintenance of the cell concentrations during this period. This is the first study to describe a methodology for producing RM containing coliforms in a cheese matrix.

BACKGROUND Neoadjuvant treatment(NT)with chemotherapy(Ch)is a standard option for resectable stage III(N2)*** studies have suggested benefits with the addition of radiotherapy(RT)to NT *** International Association for the study of Lung Cancer(IAsLC)published recommendations for the pathological response(PHR)of NsCLC resection specimens after *** To contribute to the IAsLC recommendations showing our results of PHR to NT Ch vs NT chemoradiotherapy(ChRT).METHODs We analyzed 67 consecutive patients with resectable stage III NsCLC with positive mediastinal nodes treated with surgery after NT Ch or NT ChRT between 2013 and *** NT,all patients were evaluated for radiological response(RR)according to Response Evaluation Criteria in solid Tumours criteria and evaluated for surgery by a specialized group of thoracic *** histological samples were examined by the same two *** was evaluated by the percentage of viable cells in the tumor and the resected lymph *** forty patients underwent NT ChRT and 27 NT ***-six(83.6%)patients underwent surgery(35 ChRT and 21 Ch).The median time from ChRT to surgery was 6 wk(3-19)and 8 wk(3-21)for Ch *** observed significant differences in RR,with disease progression in 2.5%and 14.8%of patients with ChRT and Ch,respectively,and partial response in 62.5%ChRT vs 29.6%Ch(P=0.025).In PHR we observed≤10%viable cells in the tumor in 19(54.4%)and 2 cases(9.5%),and in the resected lymph nodes(RLN)30(85.7%)and 7(33.3%)in ChRT and Ch,respectively(P=0.001).Downstaging was greater in the ChRT compared to the Ch group(80%vs 33.3%;P=0.002).In the univariate analysis,NT ChRT had a significant impact on partial RR[odds ratio(OR)12.5;95%confidence interval(CI):1.21-128.61;P=0.034],a decreased risk of persistence of cancer cells in the tumor and RLN and an 87.5%increased probability for achieving downstaging(OR 8;95%CI:2.34-27.32;P=0.001).CONCLUsION We found significant benefits in RR and PHR
