The surgical treatment of localized breast cancer has become progressively less aggressive over the *** management of the axillary lymph nodes has been modified by the introduction of sentinel lymph node biopsy. Axillary dissection can be avoided in patients with sentinel lymph node negative biopsies. Based on randomized trials data, it has been proposed that no lymph node dissection should be carried out even in certain patients with sentinel lymph node positive biopsies. This commentary discusses the basis of such recommendations and cautions against a general omission of lymph node dissection in breast cancer patients with positive sentinel lymph node biopsies. Instead, an individualized approach based on axillary tumor burden and biology of the cancer should be considered.

The multistage triaxial compression test was mainly utilized in this research to investigate the degradation in the strength of swelling shale under different wetting *** samples were tested in four different conditions:(1)under their natural moisture content denoted as intact,and after soaking in,(2)water,(3)polymer solution,and(4)bentonite solution,*** experimental results showed that a considerable decrease in the shale’s strength was noted after soaking in water and the lubricant fluids(LFs).The polymer solution caused the least decrease in the strength compared to other *** envelopes of the tested shale were constructed for intact and soaked *** envelopes showed fair agreement between the single stage and the multistage triaxial compression test results,indicating suitability of the multistage triaxial compression test for the tested swelling shale.

Early Paleozoic accretionary orogens dominated the Western Gondwana margin and were characterized by nearly continuous subduction associated with crustal extension and back-arc basin *** southwestern margin is represented by Famatinian and Pampean basement realms exposed in South America,both related to the protracted Paleozoic evolution of the Terra Australis Orogen,whereas the northwestern margin is mainly recorded in Cadomian domains of Europe and adjacent ***,no clear relationships between these regions were so far *** on a compilation and reevaluation of geological,paleomagnetic,petrological,geochronological and isotopic evidence,this contribution focuses on crustal-scale tectonic and geodynamic processes occurring in Western Gondwana accretionary orogens,aiming at disentangling their common Early Paleozoic *** show that accretionary orogens were dominated by high-temperature/lowpressure metamorphism and relatively high geothermal gradients,resulting from the development of extended/hyperextended margins and bulk transtensional *** this sense,retreating-mode accretionary orogens characterized the Early Paleozoic Gondwana margin,though short-lived pulses of compression/transpression also *** existence of retreating subduction zones favoured mantle-derived magmatism and mixing with relatively young(meta)sedimentary sources in a thin continental *** reworking of previous forearc sequences due to trenchward arc migration thus took place through assimilation and anatexis in the arc/back-arc ***,retreating-mode accretionary orogens were the locus of Early Paleozoic crustal growth in Western Gondwana,intimately associated with major flare-up events,such as those related to the Cadomian and Famatian *** roll back,probably resulting from decreasing convergence rates and plate velocities after Gondwana assembly,was a key factor for orogen-scale geodynamic ***

The objective was to measure the visual acuity (VA) of children with the diagnosis of hydrocephalus with or without peritoneal-ventricular shunt (PVS). A total of 55 children were included in the study (34 Female), with an age range of 0 to 291 weeks. The VA was measured by the sweep visual evoked potential technique. Of those with a PVS, in 31 the ventricular valve was inserted before 15 days after the diagnosis whereas in 14 the ventricular valve was inserted after 15 days. The sweep VEP was performed in all children, 95 exams (94%) were abnormal and only 6 were normal. There was a statistical difference in the VA between children with a PVS inserted before 15 days of the diagnosis and children with a PVS after 15 days (p = 0.038) or those without a shunt (p = 0.031). Children with no complications of the PVS had a better VA as compared to those with shunt complications (p 699). We conclude that the delayed insertion of the PVS may compromise the visual development of these children.

The Tour de Ski(TDS:6-9 sprint and distance races across 9-11 days)represents the most intense competition series of the cross-country(XC)ski season and is characterized by accumulated stress from consecutive days of high-intensity(~85%-160% VO_(2max))racing,travel,cold temperatures and low to moderate altitude(500-1500 m above sea level).Here,nutritional strategies play a key supportive role for optimized health,recovery and *** narrative review aims to provide an evidence-based discussion on the energetic demands of the TDS and recommendations for nutritional strate-gies to optimize health and performance of XC skiers during and following the *** highlight several challenges that may arise during the TDS,including the following:poor energy availability(EA)due to decreased appetite or a pressure to maintain a low body weight,suboptimal carbohydrate availability due to a failure to replenish muscle glycogen stores across consecutive-day racing and increased risk of illness due to a combination of factors,including high-intensity racing,poor nutrition,sleep,travel and *** encourage XC skiers to maintain optimal overall EA across the~1.5-week period,ensure high daily carbohydrate availability,as well as the use of strategies to maintain a healthy immune *** addition,we include practical guidelines on the management of nutrition support prior to and during the *** recognize that many nutritional questions remain unanswered both in the context of elite XC ski racing and specifically for extreme demands like the TDS that should be addressed in future investigations.

A thrombin-like serine protease, jararassin-I, was isolated from the venom of Bothrops jararaca. The protein was obtained in high yield and purity by a single chromatographic step using the affinity resin Benzamidine-Sepharose CL-6B. SDS-PAGE and dynamic light scattering analyses indicated that the molecular mass of the enzyme was about 30 kD. The enzyme possessed fibrinogenolytic and coagulant activities. The jararassin-I degraded the Bβ chain of fibrinogen while theAα chain and γ chain were unchanged. Proteases inhibitors, PMSF and benzamidine inhibited the coagulant activity. These results showed jararassin- I is a serine protease similar to coagulating thrombin-like snake venom proteases, but it specifically cleaves Bβ chain of bovine fibrinogen. Single crystals of enzyme were obtained (0.2 mm×0.2 mm×0.2 mm) and used for X-ray diffraction experiments.

AIM： To examine the in vivo phenotype associated with hepatic metastatic lymph node 64 （MLN64） overexpression.METHODS： Recombinant-adenovirus-mediated MLN64 gene transfer was used to overexpress MLN64 in the livers of C57BL/6 mice. We measured the effects of MLN64 overexpression on hepatic cholesterol content, bile flow, biliary lipid secretion and apoptosis markers. For in vitro studies cultured CHO cells with transient MLN64 overexpression were utilized and apoptosis by TUNEL assay was measured.RESULTS： Livers from ***64-infected mice exhibited early onset of liver damage and apoptosis. This response correlated with increases in liver cholesterol content and bilian/bile acid concentration, and impaired bile flow. We investigated whether liver MLN64 expression could be modulated in a murine model of hepatic injury. We found increased hepatic MLN64 mRNA and protein levels in mice with chenodeoxycholic acid-induced liver damage. In addition, cultured CliO cells with transient MLN64 overexpression showed increased apoptosis.CONCLUSION： In summary, hepatic MLN64 overexpression induced damage and apoptosis in murine livers and altered cholesterol metabolism. Further studies are required to elucidate the relevance of these findings under physiologic and disease conditions.
