Evidence-based consensus suggests that physical activity and regular exercise training can reduce modifiable risk factors as well as rate of mortality and morbidity in patients with chronic diseases,such as cardiovascular disease(CVD),diabetes,obesity and ***,long-term exercise training and drastic increase in vigorous physical activity may also cause acute cardiovascular events(*** myocardial infarction)and deleterious cardiac remodeling,particularly when exercise is performed by unfit or susceptible *** is a reversed J-shaped hormesis-like curve between the duration and intensity of exercise and level of CVD ***,it is important for an early detection of cardiac injuries in professional and amateur *** this context,this article focuses on the use of biomarker testing,an indispensable component in the current clinical practices especially in Cardiology and *** attempt to justify the importance of using circulating biomarkers in routine practices of Sports Medicine for an objective assessment of CVD events following *** attentions are dedicated to three established or emerging cardiac biomarkers(*** troponins,natriuretic peptides,hypoxanthine)for myocardial tissue hypoxia/ischemia events,muscle stress,and the consequent cellular necrotic *** on these focused analyses,we propose use of circulating biomarker testing in both laboratory and point-of-care settings with an increasingly broader involvement or participation of team physicians,trainers,coaches,primary care doctors,as well as educated athlete *** diagnostic approach may improve the quality of medical surveillance and preventive measures on exercise-related CVD risks/outcomes.

BACKGROUND Laparotomy remains one of the commonest emergency surgical *** prognostic evaluation would aid in selecting the high-risk patients for an aggressive treatment. Awareness about risks could potentially contribute to the quality of perioperative care and optimum utilization of resources. Portsmouth modification of Physiological and operative severity for the enumeration of mortality and morbidity(P-POSSUM) and the acute physiology and chronic health evaluation Ⅱ(APACHE-Ⅱ) have been the most widely used scoring systems for emergency laparotomies. It is always better to have a single scoring system to predict outcomes and audit healthcare *** To compare the ability of APACHE-Ⅱ and P-POSSUM to predict postoperative morbidity and mortality in patients undergoing emergency *** All patients undergoing emergency laparotomy at the Tata Main Hospital,Jamshedpur between December 2013 and November 2014 were included in the study. In this observational study, P-POSSUM and APACHE-Ⅱ scoring were done, and the outcome analysis evaluated with mortality being the primary *** For P-POSSUM, at a cut off value of 63 to predict mortality using receiver operating characteristics curve analysis, the area under the curve was 0.989; and for APACHE-Ⅱ, at the cut off value of 24, the area under the curve was *** Because the ability of APACHE-Ⅱ to predict mortality was similar to P-POSSUM and APACHE-Ⅱ does not need scoring for intra-operative findings and histopathology reports, APACHE-Ⅱ can be used pre-operatively to assess the risk in patients undergoing emergency laparotomy. However, for audit purposes,either of the two scoring systems can be used.

Context:Polycystic ovary syndrome(PCOS) is a common endocrine disorder occurring in 5%-20% reproductive age *** is considered as the first-line infertility treatment in women with PCOS for decades,but its impact on sex steroids hormone changes in early conceived women was not raised ***:To assess the impact of Clomiphene and acupuncture on steroids status in PCOS ***/Setting/Participants:A secondary analysis of a randomized controlled trial(PCOSAct) in women 20-40 years,with PCOS defined by modified Rotterdam ***:Clomiphene or acupuncture or their combination(Combined) for 16 *** Outcome Measure(s):within-group changes and between group comparison in steroids including total testosterone,free testosterone,progesterone,estradiol,sex hormone binding globulin,luteinizing hormone and follicle stimulating hormone from baseline to last ***:Clomiphene was associated with a significant increase in circulating ovary hormones progesterone,total testosterone,estradiol,and SHBG in all subjects and conceived subjects compared with placebo ***-conceived subjects with true acupuncture plus CC treatment showed significantly higher increase in progesterone(P=0.02) and estradiol(P=0.05)than with control acupuncture plus *** in women with early conception were twice higher with CC than those with placebo *** were no obvious difference between active acupuncture and control acupuncture on steroids ***:Clomiphene shows ovary stimulation property in steroids,with interaction to acupuncture for corpus *** hyperandrogenism accompanying clomiphene induction for infertility in maternal PCOS may contribute to family cluster in PCOS and long-term health in their offspring.
