Background: Cesarean section (CS) has increased steadily over the last decade, with an estimated one-third of women delivering by cesarean section worldwide. Objective: Our study aimed to investigate the demographic and associated factors influencing vaginal birth after one cesarean (VBAC-1) success focusing on variables like pre-pregnancy BMI, diabetes, hypertension, education, and smoking. Study Design and Methods: In this retrospective study, we analyzed 285 cases (81 unsuccessful VBAC-1, 204 successful VBAC-1) from San Juan City Hospital (Puerto rico) between January 1, 2019, and December 31, 2020. We used odds ratios and model selection comparison to assess the impact of variables on successful VBAC-1, using a significance threshold of 95% CI. Model selection assessed binomial model combinations using a generalized linear approach to identify key risk factors. results: Unsuccessful VBAC-1 (a repeat cesarean), was associated with diabetes (Or: 0.376, p = 0.086), hypertension (Or: 0.23, p = 0.006), and university-educated women (Or: 1.372, p = 0.711). High school-educated women had an Or of 3.966 (p = 0.105), while overweight women were 0.481 times more likely to have unsuccessful VBAC-1 (p = 0.041). Significant associations were not found with obesity (Or: 0.574, p = 0.122), underweight/normal (Or: 1.01, p = 0.810), or smoking (Or: 1.227, p = 0.990). Conclusion: results revealed women with higher education levels, hypertension, or diabetes are less likely to have a successful VBAC-1. Understanding the complex interactions affecting these outcomes is aimed at establishing guidelines for healthcare professionals to conduct systematic risk/benefit assessments. This study lays a foundation for evidence-based practices and policies, offering initial insights into VBAC-1 success factors in Puerto rico.

Background: Intussusception is defined as a telescoping of a proximal gastrointestinal segment with its mesentery to a distal one, only 5% occur in adults and in colon the probability that it is caused by a malignant disease is up to 65%. Only 1% occurs in a retrograde manner, the rest occur in an anterograde manner. Aim: Describe the clinical presentation of an intussusception in the adult patient as well as its most frequent causes and possible complications that influence decision making for a definitive treatment. Case Presentation: A 66-year-old woman diagnosed with colon adenocarcinoma who underwent elective transverse colectomy and colonic anastomosis with an incidental finding of a transverse colon tumor in a retrograde intussusception was studied. Conclusion: In any adult patient with an intussusception especially in colon a neoplasia should be suspected and the affected segment should be resected without being reduced due to the risk of perforation and tumor dissemination.

Expert perspectives on GIS use in Spanish geographic education

The emergence of new technological tools for territorial analysis,such as cloud-based GIS,or GIS viewers,compels us to closely examine its usefulness in geographic education. Specifically,it raises the professors and teachers’ questions: what to teach? how to teach? and what will students learn? This research responds to these three questions by interviewing Spanish experts who know how to use these tools to teach geography at university and secondary school levels. The Delphi technique was applied to achieve this,which established consensus and led to the following results from experts’ own experience: (1) that GIS help to understand territory in an experiential and non-memoristic way and (2) they enable the development of spatial thinking. This paper concludes that changes in methodologies and integrate technical knowledge into teacher training are needed.

Genetic transformation has been an effective technology for improving the agronomic traits of ***,it is highly reliant on the use of embryonic callus(EC)and shows a serious genotype *** this study,we performed genomic sequencing for 80 core maize germplasms and constructed a high-density genomic variation map using our newly developed pipeline(MQ2Gpipe).Based on the induction rate of EC(rEC),these inbred lines were categorized into three *** low-rEC germplasms displayed more abundant genetic diversity than the high-rEC *** integrating a genome-wide selective signature screen and region-based association analysis,we revealed 95.23 Mb of selective regions and 43 rEC-associated *** variants had phenotypic variance explained values ranging between 21.46 and 49.46%.In total,103 candidate genes were identified within the linkage disequilibrium regions of these rEC-associated *** genes mainly participate in regulation of the cell cycle,regulation of cytokinesis,and other functions,among which MYB15 and EMB2745 were located within the previously reported QTl for EC *** leaf area-associated variants with l.rge effects were closely linked to several.rEC-related loci,implying a potential synergistic selection of rEC and leaf size during modern maize breeding.
