Etiological Factors of Infective Endocarditis in Children Aged 1 Month to 15 Years Hospitalised in the Paediatric Department of CHU Gabriel Touréof Bamako (Mali)

Introduction: In emerging countries where studies are rarer, the epidemiological profile of infective endocarditis resembles that of developed countries at the beginning of the antibiotic era. There is little data available in Mali on this subject, motivating this study, the aim of which was to study the etiological factors of infective endocarditis in children aged between 1 month and 15 years hospitalised in the paediatric department of the CHU Gabriel Touré. Patients and Methods: We conducted a cross-sectional and descriptive study, which took place from August 2018 to August 2019 (13 months), in children aged between 1 month and 15 years hospitalised in the paediatric department of the Gabriel Touré University Hospital with an ultrasound- confirmed cardiac disease and a temperature above 38°C. Results: The hospitalization rate was 0.4%. The average age was 11 months, the sex ratio was 1.35. Respiratory distress (63.6%) and fever (30.3%) were the most common reasons for referral. Congenital heart disease (87.9%) and dyspnoea on feeding (24.2%) were the most common medical histories. IVC was the most frequent congenital heart disease (36.4%). The mean temperature was 39.1°C. Blood cultures were taken in 24 patients (72.7%) and were positive in 27.3%. Staphylococcus aureus was found in 55.6% of cases. Trans-thoracic ultrasound found vegetations in 03 cases. According to the modified DUKE criteria, the diagnosis of endocarditis was retained in 27.30%. The evolution was marked by a high case fatality rate (45.45%). Conclusion: Endocarditis is an uncommon disease in children with a pulmonary portal of entry and Staphylococcus aureus as the main germ. Its mortality remains high.

It is well known that hierarchies of mathematical programming formulatlons with different numbers of variables and constraints have a considerable impact regarding the quality of solutions obtained once these formulations are fed to a commercial solver. In addition, even if dimensions are kept the same, changes in formulations may largely influence solvability and quality of results. This becomes evident especially if redundant constraints are used. We propose a related framework for information collection based on these constraints. We exemplify by means of a well-known combinatorial optimization problem from the knapsack problem family, i.e., the multidimensional multiple-choice knapsack problem （MMKP）. This incorporates a relationship of the MMKP to some generalized set partitioning problems. Moreover, we investigate an application in maritime shipping and logistics by means of the dynamic berth allocation problem （DBAP）, where optimal solutions are reached from the root node within the solver.

Pulmonary Complications in Children with Sickle Cell Disease Followed at the Pediatric Department of Gabriel Toure University Hospital

Objective: To study the pulmonary complications of major sickle cell syndromes in children aged 6 months to 15 years followed at the Department of Pediatrics of Gabriel Toure University Hospital. Materials and methods: This was a retrospective study from January 1st, 2017 to February 28, 2018 and a prospective study from March 1, 2018 to December 31, 2019, concerning sickle cell disease children followed at the Gabriel Touré University Hospital. All children with sickle cell disease confirmed by hemoglobin electrophoresis, aged 6 months to 15 years with a pulmonary complication and admitted to pediatrics were included. Were not included: 1) Infants and children with sickle cell disease not presenting a pulmonary complication coming only to their routine follow-up. 2) Sickle cell patients over 16 years of age and those whose parents do not consent. 3) Those who have not done electrophoresis. Results: During the study period, we were able to include 45 sickle cell children with a pulmonary complication. The frequency of pulmonary complications was 13.76%, involving homozygotes in 91.11% of cases. The age group 6 - 10 years predominated with 46.66% and the sex ratio was 0.45. Vaso-occlusive crisis was the most frequent reason for consultation with 35.56%. Respiratory distress (80%), fever (66.67%), crepitus rales (64.44%) and chest pain (60%) were the most frequent clinical signs. The main pulmonary complication was acute chest syndrome with 86.67%. Treatment was based on hydration (91.91%), analgesics (91.91%) and antibiotics (73.33%). Lethality was 4.44%. Conclusion: Pulmonary complications of sickle cell disease are serious and constitute the main cause of mortality in our context.

Electrochromic materials are of great interest for their potential in eyewear protection and data storage devices, as they change colors in response to electrochemical switching. While many of the systems currently used are based on inorganic materials, organic materials such as triazenes have emerged as viable alternatives due to their unique properties, including optical properties. Triazenes are a class of organic compounds with three consecutive nitrogen atoms in an acyclic arrangement, and they have been used for a variety of applications in medicinal and synthetic chemistry. However, the effects of solvents on the UV-visible absorption spectrum of triazenes have not been fully investigated. The neutral molecules of 3,3-diisopropyl-1-phenyltriazene and 1-(4-chlorophenyl)-3-cyclopentyltriazene in acetonitrile, the UV-visible spectra corresponded respectively to HOMO → LUMO transitions with a large maximum absorption at 299.74 nm (4.1364 eV) and 299.57 nm (4.1387 eV) and the most intense oscillator strength (f = 0.6988) and (f = 0.7372). These results suggest that the electronic transitions of the compounds are highly influenced by the nature of the substituents on the triazene unit, as well as the solvent used in the experiment. The redox couple 0.92 and -0.44 V/Ag/AgCl is attributed to the phenyl group. Compound III showed an oxidation and reduction peak respectively -0.27 and -0.8 V/Ag/AgCl attributed to the phenyl molecule. The study concluded that all three compounds were electroactive and exhibited reversible characteristics with oxidizing/reducing couples. This study aims to contribute to research on the optical properties of triazenes compounds and the application of quantum chemical calculation methods for understanding their molecular structures. By investigating the solute-solvent interactions occurring in the solvation shell of the solutes, we aim to gain insights into the effects of solvents on the UV-visible absorption spectrum of triazenes. A detailed vibrational Raman-lR spectroscopic and diffractional analyses have been performed on basalts from two locations from Tenerife Island：（1） the Arenas Negras volcano which belongs to the historical eruption not showing visible alteration and （2） Pillow Lavas zone from Anaga Massif which shows a clearly fluid-rock interaction caused by submarine alteration. These places have been extensively studied due to its similarity with the surface of Mars. The analysis is based on the mineral detection of selected samples by a Micro-Raman study of the materials. The complementary techniques have confirmed the mineralogy detected by the Raman measurement. The results show a volcanic environment behavior with primary phases like olivine, pyroxene, and feldsparJplagioclase. Moreover, the presence of accessory minerals or secondary mineralization like phosphate, iron oxides, zeolite or carbonates shows the alteration processes on each outcrop. The variation in the crystallinity and amorphous phases is related to fluid-rock interaction caused by hydrothermal episodes and external weathering processes, which shows several analogies with the ancient volcanic activity from Mars.

Background Many organizations have issued recommendations to limit elective surgery during the coronavirus disease 2019(COVID-19)pandemic.We surveyed providers of children’s surgical care working in low-income and middle-income countries(LMICs)to understand their perspectives on surgical management in the wake of the COVID-19 pandemic and how they were subsequently modifying their surgical care practices.Methods A survey of children’s surgery providers in LMICs was performed.Respondents reported how their perioperative practice had changed in response to COVID-19.They were also presented with 26 specific procedures and asked which of these procedures they were allowed to perform and which they felt they should be allowed to perform.Changes in surgical practice reported by respondents were analyzed thematically.Results A total of 132 responses were obtained from 120 unique institutions across 30 LMICs.117/120 institutions(97.5%)had issued formal guidance on delaying or limiting elective children’s surgical procedures.Facilities in LICs were less likely to have issued guidance on elective surgery compared with middle-income facilities(82%in LICs vs 99%in lower middle-income countries and 100%in upper middle-income countries,p=0.036).Although 122(97%)providers believed cases should be limited during a global pandemic,there was no procedure where more than 61%of providers agreed cases should be delayed or canceled.Conclusions There is little consensus on which procedures should be limited or delayed among LMIC providers.Expansion of testing capacity and local,context-specific guidelines may be a better strategy than international consensus,given the disparities in availability of preoperative testing and the lack of consensus towards which procedures should be delayed.

Identification of Planktonic Genera/Species and Determination of the Physicochemical Factors Favoring the Growth of These Species in Cape 7 (Aftissat) Zone, South of Morocco

The diversity of oceanic phytoplankton communities depends in part on environmental, physical, chemical and biological factors where they are. The study that we conducted in cape 7 (Aftissat) zone, south of Morocco, located 232 km south of Laayoune city and 62 km south of Boujdour city, aims at the identification of planktonic genera/species and determination of the physicochemical factors favoring the growth of these species. The study was carried out on 427 samples, distributed, therefore, as follows, 57.2% (n = 245) of the samples during the year 2016 and 42.5% (n = 182) in the year 2017. In addition, 32.8% and 27.6% of the samples were taken, respectively, in winter and autumn, 24.4% of the samples in summer and 15.2% of all samples taken in the spring. We have identified 30 phytoplankton species, of which 70% (n = 21) are diatoms, 26.67% are dinoflagellates and one species is silicoflagellates. Moreover, the most abundant species in the diatoms are Navicula sp. (69), Licmophora (47), Nitzschia sp. (35), Pseudo nitzschia spp. (31) and in the class Dinoflagellates, Scrippsiella spp. (24), Protoperidinium spp. (11). The physicochemical parameters show very significant associations with density, so they favor a very high abundance species, especially those belonging to the diatom class. Some toxic species have also been identified, but with very low frequencies, below the norm. The cape d’Afissat has a very important role in the economic life of the region, so we must increase efforts to preserve it and if it is possible to improve the quality of water.

Glycine supplementation reduces the severity of chemotherapy-induced oral mucositis in hamsters

Objective: Oral mucositis (OM) is a devastating toxicity associated with cytotoxic cancer therapy. The OM pathogenesis and the complex interactions occur in response to tissue insult. Application of this evolving model has aided in the development of mechanistically based therapies for the prevention and treatment of mucositis. The present study was to assess the effects of glycine supplementation on chemotherapy-induced oral mucositis. Methods: In a hamster cheek pouch model of chemotherapy-induced oral mucositis, one group of 20 animals received systemic glycine supplementation for 7 days, while another similar control group did not. Clinical mucositis severity and neutrophil infiltrate (on histology) were assessed by blinded examiners. Free radical production was measured as malondialdehyde (MDA) levels. Results: As compared to control animals, glycine-treated animals demonstrated a highly significant reduction in clinical severity of oral mucositis, neutrophil infiltrate, and MDA levels (p < 0.001 for all). Conclusions: Glycine supplementation reduces the severity of chemotherapy-induced oral mucositis in an animal model. This effect is at least partly mediated through inhibition of the inflammatory response and reduced production of damaging free radicals.

Background: Fetal and neonatal hepatic arteriovenous fistulas are rare and associated with a high mortality rate; they can be prenatally detected by ultrasonography. Management of these malformations can be a challenge for pediatric surgeons. Methods: Two patients with a prenatal diagnosis of intrahepatic arteriovenous shunts were treated at our institution in the last 2 years. A hepatic complex arteriovenous malformation fed respectively by prominent branches of the hepatic artery and of the celiac trunk rising from dilated suprarenal aortae and draining into suprahepatic veins was detected. In the first case, an embolization was performed; in the second, the surgical resection of the vascular malformation was the treatment of choice. Results: The first patient died after embolization and before surgery for hemodynamic complications. The second patient, at a follow-up of 16 months, is alive and doing well. Conclusion: Hepatic resection is the treatment of choice for localized intrahepatic arteriovenous malformation. Theoretically, embolization could be curative or reduce the size of a malformation, making consequent hepatic resection feasible. Results do not support this theory because of the high rate of complications recorded that brought in every case, ours included, to the death of the child.
