Limb length discrepancy after total knee arthroplasty: A systematic review and meta-analysis

BACKGROUND Limb length discrepancy(LLD)after total knee arthroplasty(TKA)has been considered as one of the reasons for the unsatisfactory ***,there is no consensus about the extent of LLD that can be considered as clinically *** To evaluate the incidence of radiographic LLD and its impact on functional outcome following *** All randomized-controlled trial and observational studies on LLD in TKA,published till 22nd June 2020,were systematically searched and *** primary outcome was“limb lengthening or LLD after TKA”.The secondary outcomes included“assessment of LLD in varus/valgus deformity”and“impact of LLD on the functional outcome”.RESULTS Of 45 retrieved studies,qualitative and quantitative assessment of data was performed from eight studies and six studies,*** studies(n=1551)reported the average limb lengthening of 5.98 *** LLD after TKA was ranging from 0.4±10 mm to 15.3±2.88 *** incidence of postoperative radiographic LLD was reported in 44%to 83.3%of *** was no difference in the preoperative and postoperative LLD(MD-1.23;95%CI:-3.72,1.27;P=0.34).Pooled data of two studies(n=219)revealed significant limb lengthening in valgus deformity than varus(MD-2.69;95%CI:-5.11,0.27;P=0.03).The pooled data of three studies(n=611)showed significantly worse functional outcome in patients with LLD of≥10 mm compared to<10 mm(standard MD 0.58;95%CI:0.06,1.10;P=0.03).CONCLUSION Limb lengthening after TKA is common,and it is significantly more in valgus than varus *** LLD(≥10 mm)is associated with suboptimal functional outcome.

The study by Zhao et al identifies the factors leading to the failure of unicompartmental knee arthroplasty(UKA)in their *** factors include substandard suturing of the wound,the presence of osteophytes and intra-articular loose bodies causing impingement,premature loosening of the tibial component,choosing unsuitable patients for the procedure,dislocation of the movable insert,and damage to the anterior cruciate ligament and medial collateral *** findings suggest that employing the correct surgical techniques and indications is essential for successful outcomes in the UKA.

Is dark energy necessary for the sustainability of traversable wormholes?

Dark energy is typically the principal component needed for the traversability of wormholes(WH),as it provides the negative gravity effect required to keep the throat ***,can this be achieved without dark energy?It turns out that if we couple the trace of energy-momentum with the standard Einstein-Hilbert Lagrangian and utilize a specific equation of state(EoS),dark energy may be *** Casimir stress energy is known to result in the violation of the null energy condition(NEC)on the energy momentum *** phenomenon makes such an EoS an ideal candidate for generating traversable WH *** laboratory proven phenomenon provides a natural mechanism to sustain an open WH throat without relying on dark ***,we generate two classes of WH solutions using this in energy-momentum trace-coupling *** the specific choice of the Casimir EoS relating the energy-momentum tensor components[Kar and Sahdev:***.D 52,2030(1995)]and different choices of redshift functions,we determine the WH geometry *** obtained WH solutions violate the NECs,and all qualitative constraints demanded for physically realizable WHs are *** is demonstrated via graphical plots for a suitably chosen range of coupling parameter ***,our study investigates the repulsive effect of gravity,revealing that its presence leads to a negative deflection angle for photons traveling along null ***,we observe a consistent pattern of negative values for the deflection angle across all values of in the three scenarios considered,thus indicating the clear manifestation of the repulsive gravity *** of this is possible without invoking the existence of dark energy.

The 18 kDa translocator protein(TSPO)located on the outer mitochondrial membrane regulates several key cellular processes including mitochondrial homeostasis,cholesterol transport,apoptosis,cell proliferation,and maintenance of mitochondrial health(Rupprecht et al.,2022,2023).TSPO is expressed in both peripheral organs and the central nervous system,with a more pronounced expression in tissues that produce *** main reason why TSPO has garnered so much attention is because it plays a key role in neurosteroidogenesis by transferring cholesterol from the outer to the inner mitochondrial membrane,which is the rate-limiting step in neurosteroid synthesis.A cholesterol-recognizing amino acid consensus domain has been identified in the cytosolic C terminus of the TSPO protein by both in vitro and site-directed mutagenesis experiments(Li et al.,2001).However,the role of TSPO in the process of neurosteroid synthesis has been challenged by several studies,particularly TSPO knockout models,which suggest that TSPO removal does not affect the phenotype or the system’s viability(Tu et al.,2014).However,ligands targeting TSPO have been shown to enhance levels of neurosteroids which suggests that neurosteroidogenesis is one of the major functional roles mediated by the TSPO protein.

This study explores the f(R,T)gravity theory,which introduces a coupling between matter and curvature,through the simplest linear functional form f(R,T)=R+2β*** derive the modified Einstein field equations and conservation equations for this theory and then apply this framework to study the structural properties of quark stars(QSs)composed of interacting quark matter,considering perturbative QCD corrections and color *** solving the modified Tolman-Oppenheimer-Volkoff equations,we investigate the mass-radius relation,stability criteria,and energy conditions of *** results indicate that the f(R,T)gravity significantly influences the properties of QSs,leading to deviations from General *** analysis is consistent with recent observational data,suggesting that the modified gravity framework could provide viable models for the study of compact stars.

Anisotropic Bianchi Type-I Magnetized String Cosmological Models with Decaying Vacuum Energy Density A(t)

The present study deals with a spatially homogeneous and anisotropic Bianehi-I cosmological models representing massive strings with magnetic field and decaying vacuum energy density A. The energy-momentum tensor, as formulated by Letelier （1983）, has been used to construct massive string cosmological models for which we assume the expansion scalar in the models is proportional to one of the components of shear tensor. The Einstein＇s field equations have been solved by applying a variation law for generalized Hubble＇s parameter in Bianchi-I space-time. The variation law for Hubble＇s parameter generates two types of solutions for the average scale factor, one is of power-law type and other is of the exponential form. Using these two forms, Einstein＇s field equations are solved separately that correspond to expanding singular and non-singular models of the universe respectively. We have made a comparative study of accelerating and decelerating models in the presence of string scenario. The study reveals that massive strings dominate in the decelerating universe whereas strings dominate in the accelerating universe. The strings eventually disappear from the universe for sufficiently large times, which is in agreement with current astronomical observations. The cosmological constant A is found to be a positive decreasing function of time which is corroborated by results from recent supernovae Ia observations. The physical and geometric properties of the models have been also discussed in detail.

Thick Domain Walls in Lyra Geometry with Bulk Viscosity

In this paper, we evaluate the general solutions for plane-symmetric thick domain walls in Lyra geometry in presence of bulk viscous fluid. Expressions for the energy density and pressure of domain walls are derived in both cases of uniform and time varying displacement field β. Some physical consequences of the models are also given. Finally, the geodesic equations and acceleration of the test particle are discussed.
