Liver transplantation is a conventional treatment for terminal stage liver diseases. However, several complications still hinder the survival rate. Intestinal barrier destruction is widely observed among patients receiving liver transplant and suffering from ischemia-reperfusion or rejection injuries because of the relationship between the intestine and the fiver, both in anatomy and function. Importantly, the resulting alteration of gut microbiota aggravates graft dysfunctions during the process. This article reviews the research progress for gut microbial alterations and liver transplantation. Especially, this work also evaluates research on the management of gut microbial alteration and the prediction of possible injuries utilizing microbial alteration during liver transplantation. In addition, we propose possible directions for research on gut microbial alteration during liver transplantation and offer a hypothesis on the utilization of microbial alteration in liver transplantation. The aim is not only to predict perioperative injuries but also to function as a method of treatment or even inhibit the rejection of liver transplantation.

Nonlinear robust adaptive control for bidirectional stabilization system of all-electric tank with unknown actuator backlash compensation and disturbance estimation

Since backlash nonlinearity is inevitably existing in actuators for bidirectional stabilization system of allelectric tank,it behaves more drastically in high maneuvering environments.In this work,the accurate tracking control for bidirectional stabilization system of moving all-electric tank with actuator backlash and unmodeled disturbance is solved.By utilizing the smooth adaptive backlash inverse model,a nonlinear robust adaptive feedback control scheme is presented.The unknown parameters and unmodelled disturbance are addressed separately through the derived parametric adaptive function and the continuous nonlinear robust term.Because the unknown backlash parameters are updated via adaptive function and the backlash effect can be suppressed successfully by inverse operation,which ensures the system stability.Meanwhile,the system disturbance in the high maneuverable environment can be estimated with the constructed adaptive law online improving the engineering practicality.Finally,Lyapunov-based analysis proves that the developed controller can ensure the tracking error asymptotically converges to zero even with unmodeled disturbance and unknown actuator backlash.Contrast co-simulations and experiments illustrate the advantages of the proposed approach.

Liver transplantation(LT)is a highly curative therapy for patients with hepatocellular carcinoma(HCC).However,due to the shortage of donor livers and rapid progression of HCC,a majority of patients are dropped out from the waitlist.Recently,immunotherapy has shown great promise in the treatment of advanced HCC.However,the use of immunotherapy is limited in LT mainly due to the potentially increasing risk of graft rejection.One of the main challenges for researchers is the protection of donor graft from an immunotherapy-boosted immune response mounted by the host.Besides,the safety,availability,and costs of immunotherapy are other challenges that need to be addressed.Here,we reviewed the literature involving patients who received immunotherapy prior to transplant to avoid waitlist dropouts and following transplantation to prevent the progression of tumor recurrence and metastasis.Statistically,the incidence of rejection was 25.0%pre-transplant and 18.5%post-transplant.Based on the review of these clinical studies,we can conclude that conducting clinical trials on the safety and efficacy of currently available immunotherapy drugs and identifying novel immunotherapy targets through extensive research may be promising for patients who do not meet the selection criteria for LT and who experience post-transplant recurrence.To date,the clinical experience on the use of immunotherapy before or after LT comes from individual case studies.Although some of the reported results are promising,they are not sufficient to support the standardized use of immunotherapy in clinical practice.

Background:The long noncoding RNA HOTTIP has recently been described as a biomarker of poor prognosis in patients with hepatocellular carcinoma(HCC).Methods:In the present study,we evaluated the clinical significance of HOTTIP expression in predicting the rate of tumor recurrence in HCC patients after liver transplantation(LT).We examined the expression pattern and single nucleotide polymorphism(SNP)genotype of HOTTIP in HCC samples from 155 patients underwent LT,and its correlation with clinical parameters and patient prognosis was analyzed.HOTTIP was suppressed using siRNA to explore the role HOTTIP plays in tumor progression.Results:The expression level of HOTTIP in cancer tissues was higher than in adjacent noncancerous tissues.Multivariate analyses revealed that HOTTIP expression was an independent prognostic factor for tumor recurrence and lower overall survival times in HCC patients after LT.Patients who beyond the Milan criteria and exhibit decreased HOTTIP expression experienced longer recurrence-free survival and overall survival.HOTTIP rs2071265 is associated with an earlier recurrence in HCC patients.Moreover,the suppression of HOTTIP in liver cancer cell lines reduced cell invasion rates and increased chemosensitivity.Conclusions:In summary,the high expression level of HOTTIP in HCC could serve as a candidate biomarker for predicting poor prognosis in HCC patients underwent liver transplant therapy.Furthermore,HOTTIP might be a potential therapeutic target.

中亚与中国毗邻,在中国对外关系中具有重要地位。尽管中亚地区只有五个国家,但该地区与中国外交布局中的大国外交、周边外交、与发展中国家外交、多边外交均息息相关。巩固和提高中国中亚外交的成效、加大对中亚问题研究的投入,是新时期我国需着力开展的工作。在当前错综复杂的国际和地区形势下,中亚地区的地缘政治地位进一步凸显。近年来,我们看到诸多域外大国明显加大了对中亚地区的关注和投入力度,各种“C5+1”机制的出现和升级就是明证。而中亚国家在复杂的内外形势下也遭遇了不少困难,仅在2022年,就先后有三个中亚国家出现了程度不同的国内骚乱。这些形势的发展,使中国的中亚外交面临一系列机遇和挑战。得益于中国对中亚国家的尊重和双方基于互利共赢理念所开展的友好合作,中国三十多年来对中亚的外交取得了显著成效,双方的政治互信达到了新的高度,这为下一阶段双方关系的提质升级奠定了良好基础。与此同时,时代和形势的变化,以及中亚国家自身发展需要的变化,也对双方关系的深化提出了许多新的要求,这为区域国别学的创设提供了契机。应该抓紧机遇,推进中亚研究的深入和细化;同时在政策层面创新思路、完善举措,以更周全的思维、精心思考的行动,保障中国中亚外交行稳致远,实现双方的协同发展。

Objective:Sex-specific differences are observed in various liver diseases,but the influence of sex on the outcomes of hepatocellular carcinoma(HCC)after liver transplantation(LT)remains to be determined.This study is the first Chinese nationwide investigation of the role of sex in post-LT outcomes in patients with HCC.Methods:Data for recipients with HCC registered in the China Liver Transplant Registry between January 2015 and December 2020 were analyzed.The associations between donor,recipient,or donor-recipient transplant patterns by sex and the post-LT outcomes were studied with propensity score matching(PSM).The survival associated with different sex-based donor-recipient transplant patterns was further studied.Results:Among 3,769 patients enrolled in this study,the 1-,3-,and 5-year overall survival(OS)rates of patients with HCC after LT were 96.1%,86.4%,and 78.5%,respectively,in female recipients,and 95.8%,79.0%,and 70.7%,respectively,in male recipients after PSM(P=0.009).However,the OS was comparable between recipients with female donors and male donors.Multivariate analysis indicated that male recipient sex was a risk factor for post-LT survival(HR=1.381,P=0.046).Among the donor-recipient transplant patterns,the male-male donor-recipient transplant pattern was associated with the poorest post-LT survival(P<0.05).Conclusions:Our findings highlighted that the post-LT outcomes of female recipients were significantly superior to those of male recipients,and the male-male donor-recipient transplant pattern was associated with the poorest post-LT survival.Livers from male donors may provide the most benefit to female recipients.Our results indicate that sex should be considered as a critical factor in organ allocation.

In recent years, the Eurasian Economic Community has developed rapidly, 1 and it has entered a substantive cooperative period. On January 1, 2010, Russia, Belarus and Kazakhstan decided to launch the Customs Union within the framework of Eurasian Economic Community, and in July the three countries adopted unified customs tariffs. They removed the customs borders between each other beginning from July 2011. Then on January 1, 2012, the three countries launched their Common Economic Space. At the same time, the supranational Eurasian Economic Commission for the promotion of the integration process of the three countries also came into effect. On December 24, 2013, the Supreme Council of the Eurasian Economic Commission meetin~ was held in Moscow.

新时代健康中国战略的根本目标是实现全民健康,而政府配置公共医疗卫生资源的方式对实现健康中国战略目标具有重大影响。本文基于两部门世代交叠模型,研究政府预防性支出占比和医疗保险报销比例对个人健康投资、居民健康和福利的经济影响及其作用机制,并通过数值模拟分析医疗卫生政策调整对卫生总费用、个人卫生支出、社会总消费及医保基金财政依赖程度的定量影响。研究发现,适度提高政府预防性支出占比不仅改善居民健康并提升福利,还能降低卫生总费用占GDP比重和医保基金财政依赖程度,增加社会总消费。在现有预算体制下,提高医疗保险报销比例能够降低个人自付医疗负担,但会对个人健康投资产生两种相反的激励效应,且负向激励占主导,导致个人健康投资水平随报销比例上升而下降。上述发现对持续优化政府卫生支出结构、有效实施健康中国战略提供了定量政策参考。
