Risk Stratification in Obstetrics: An Integrated Approach to Maternal Health

Introduction: Prenatal care is crucial for maternal and fetal health, highlighting the importance of obstetric risk stratification. This enables personalized care, avoiding unnecessary interventions and reducing costs. Conditions such as first trimester bleeding, spontaneous abortion, gestational trophoblastic disease, and ectopic pregnancy require specific approaches. Early identification of these complications is vital, especially in urgent and emergency obstetric cases, which demand immediate hospital attention. Objective: Comprehensive review of first trimester pathologies and gestational hypertension, focusing on obstetric risks and personalized prenatal management. Methods: Literature review on bleeding and gestational hypertension. Results: The study provides a detailed analysis of pathologies associated with first trimester uterine bleeding and hypertensive disorders, focusing on obstetric risk stratification. The main causes of maternal and fetal mortality are hypertensive disorders, hemorrhages, infections, childbirth complications, and abortion. Spontaneous abortion is common, with different classifications, ranging from threatened to missed abortion. Infected abortion is a severe complication. Brazil has restrictive abortion laws, but many unsafe abortions occur, resulting in high public health costs. Pharmacological treatment with misoprostol is a safe option. Other pathologies include gestational trophoblastic disease, ectopic pregnancy, and placenta previa. Preeclampsia and eclampsia are severe conditions requiring immediate treatment to avoid complications. Conclusion: Early identification and management of obstetric risk factors, such as uterine bleeding, are essential for positive maternal and fetal outcomes. A multidisciplinary approach is fundamental.

Prevalence of Latent Tuberculosis Infection [LTBI] in Prison Officers: A Systematic Review and Meta-Analysis

This study aimed to perform a systematic review and meta-analysis to determine the LTBI prevalence in prison officers worldwide. A systematic search was performed in PubMed, WoS, Embase, and BVS, including all articles related to LTBI prevalence and risk factors. After critical evaluation and qualitative synthesis of the identified articles, a meta-analysis was used. Five studies carried out between 2012 and 2022 were included, with a total sample size of 1718 prison officers. The overall LTBI prevalence was 50% [95% confidence interval [CI]: 48% - 52%;n = 816], with high heterogeneity between studies. Smoking [OR = 1.76;CI 95% = 1.26 - 2.46] and males [OR = 2.08;CI 95% = 1.31 - 3.31] were positively related to a higher LTBI prevalence among prison officers. Thus, preventive measures and the rapid and accurate diagnosis of new cases should be emphasized to ensure tuberculosis control, especially among risk groups such as prison officers.

Objective: The aim of the present study was to investigate the psychic repercussions of sexual assault among female students in Parakou in 2023. Methods: Descriptive cross-sectional study conducted from December 2022 to July 2023 among all students at the University of Parakou. The sampling technique was stratified non-proportional at two levels. The dependent variable was self-reported sexual assault. The psychopathological repercussions linked to sexual assault and explored in these students were: post-traumatic stress disorder assessed with the Post Traumatic Stress Disorder Checklist-5 (PCL-5) scale;and current depression screened with the Patient Health Questionnaire-9 (PHQ-9) scale. Results: A total of 376 female students from the University of Parakou meeting the inclusion criteria were surveyed. The average age of the subjects surveyed was 20.56 ± 2.46 years, with extremes of 16 and 32 years. A predominance of subjects under 25 was observed. The prevalence of sexual assault among female students at the University of Parakou was 10.63%. The types of aggression suffered were mostly rape (18 people;45%) and sexual touching (16 people;40%), and the aggressors were mainly friends/boyfriends (13;32.5%), family members (12;30.0%) and people from the house/neighborhood (9;22.5%). Psychological repercussions included depression in 72.5% (29) of cases, with mild depression in 30% (12), moderate depression in 37.5% (15) and severe depression in 5% (2). Anxiety disorders were present in 27.50% (11) of victims. The risk factors associated with sexual assault in these students were gravidity (p = 0.014;OR = 15.32), unhappy life events (p = 0.01;OR = 17.24), and age of first sexual intercourse (p = 0.016;OR = 4.608). Conclusion: One in ten female students in Parakou has been sexually assaulted. D2 dissection in laparoscopic and open gastrectomy for gastric cancer

AIM: To evaluate the radicalness and safety of laparoscopic D2 dissection for gastric cancer. METHODS: Clinicopathological data from 209 patients with gastric cancer, who underwent radical gastrectomy with D2 dissection between January 2007 and February 2011, were analyzed retrospectively. Among these patients, 131 patients underwent laparoscopyassisted gastrectomy (LAg) and 78 underwent open gastrectomy (Og). The parameters analyzed included operative time, blood loss, blood transfusion, morbidity, mortality, the number of harvested lymph nodes (HLNs), and pathological ***: There were no significant differences in sex, age, types of radical resection [radical proximal gastrectomy (Pg + D2), radical distal gastrectomy (Dg + D2) and radical total gastrectomy (Tg + D2)], and stages between the LAg and Og groups (P > 0.05). Among the two groups, 127 cases (96.9%) and 76 cases (97.4%) had 15 or more HLNs, respectively. The average number of HLNs was 26.1 ± 11.4 in the LAg group and 24.2 ± 9.3 in the Og group (P = 0.233). In the same type of radical resection, there were no signifi cant differences in the number of HLNs between the two groups (Pg + D2: 21.7 ± 7.5 vs 22.4 ± 9.3; Dg + D2: 25.7 ± 11.0 vs 22.3 ± 7.9; Tg + D2: 30.9 ± 13.4 vs 29.3 ± 10.4; P > 0.05 for all comparisons). Tumor free margins were obtained in all cases. Compared with Og group, the LAg group had signifi cantly less blood loss, but a longer operation time (P g group was 9.9%, which was not signifi cantly different from the Og group (7.7%) (P = 0.587). The mortality was zero in both groups. CONCLUSION: Laparoscopic D2 dissection is equivalent to Og in the number of HLNs, regardless of tumor location. Thus, this procedure can achieve the same radicalness as Og.

从40余件四山刻石全部被视作郑道昭亲笔所为的书迹,到只有30余件,乃至于10余件被认为是郑道昭笔迹的论争之中走出来,笔者以为其实并没有一件作品是郑道昭亲笔书写的,这种现象可称作“无迹论”。那么,没有真迹的主持者郑道昭是否还具有“书圣”的意义呢?笔者认为恰恰相反,通过书刊行为,其将一座座山峦改变本来的意义(图1~图4),反而凸显出郑道昭真实的作用。这是可以对应现当代艺术之中的“大地艺术”“观念艺术”的思想层面创作判断。同时,这又是别于西方艺术纯粹的视觉张力的中国式的艺术方式。在此意义上,郑道昭创造了一种书法的新样式、新方法,也作出了对于书法史的真实变革。就此而言,对他的“北方书圣”判断才是恰当的。

目的:比较腘动脉与膝关节后囊间隙(IPACK)阻滞与关节周围局部浸润镇痛(lia)在膝关节矫形术后镇痛中的效果。方法:招募60例膝关节矫形术患者,随机分为IPACK阻滞组(I组)和lia组(L组)。I组行IPACK阻滞,L组行lia。记录疼痛评分、满意度评分、主动屈膝最大角度、步行距离、镇痛补救率和不良事件发生情况。结果:与L组比较,8h(T2)时点I组静态疼痛评分降低,差异有统计学意义(Z=-2.171,Plia更适合于膝关节矫形术后镇痛。

Electrospun CuFe_2O_4 nanotubes as anodes for high-performance lithium-ion batteries

Herein,we report on the synthesis and lithium storage properties of electrospun one-dimensional（1D） CuFe2O4 nanomaterials.1D CuFe2O4nanotubes and nanorods were fabricated by a single spinneret electrospinning method followed by thermal decomposition for removal of polymers from the precursor *** as-prepared CuFe2O4 nanotubes with wall thickness of 50 nm presented diameters of 150 nm and lengths up to several *** was found that phase separation between the electrospun composite materials occured during the electrospinning process,while the as-spun precursor nanofibers composed of polyacrylonitrile（PAN）,polyvinylpyrrolidone（PVP） and metal salts might possess a core-shell structure（PAN as the core and PVP/metal salts composite as the shell） and then transformed to a hollow structure after ***,as a demonstration of the functional properties of the 1D ***2O4 nanotubes and nanorods were investigated as anodes for lithium ion batteries（LIBs）.It was demonstrated that CuFe2O4 nanotubes not only delivered a high reversible capacity of 816 mAh·g-1 at a current density of 200 mA·g-1over 50 cycles,but also showed superior rate capability with respect to counterpart ***,the enhanced electrochemical performance can be attributed to its high specific surface areas as well as the unique hollow structure.

Vedolizumab for inflammatory bowel disease:from randomized controlled trials to real-life evidence

The biologic antitumor necrosis factor alpha(anti-TNFα) agents have revolutionised the treatment of inflammatorybowel disease(IBD). However,some patients experience primary nonresponse,loss of response,or intolerance. Therefore,introducing a newer class of therapy with a mechanism of action that acts on different inflammatory pathways involved in IBD pathogenesis is appealing. Vedolizumab is a fully humanised monoclonal antibody that selectively targets α4β7 integrin. Based on the results of the pivotal clinical gEMINI trials,vedolizumab was approved for the treatment of adult patients with moderately to severely active ulcerative colitis(UC) and Crohn's disease(CD) refractory or intolerant to either conventional therapy or TNFα inhibitors. This review describes the efficacy,safety,and tolerability of vedolizumab reported in both randomized,controlled,clinical trials and from real-world experience in patients with UC and CD in order to identify its place in treatment algorithms for IBD.
