Primary percutaneous coronary intervention is the preferred reperfusion strategy for patients presenting with ST-segment elevation myocardial infarction(STEMI). First generation drug-eluting stents(DES),(sirolimus drug-eluting stents and paclitaxel drug-eluting stents), reduce the risk of restenosis and target vessel revascularization compared to bare metal stents. However, stent thrombosis emerged as a major safety concern with first generation DES. In response to these safety issues, second generation DES were developed with different drugs, improved stent platforms and more biocompatible durable or bioabsorbable polymeric coating. This article presents an overview of safety and efficacy of the first and second generation DES in STEMI.

The vascular diseases including aneurysm, occlusion, and thromboses in the mesenteric lesions could cause severe symptoms and appropriate diagnosis and treatment are essential for managing patients. With the development and improvement of imaging modalities, diagnostic frequency of these vascular diseases in abdominal lesions is increasing even with the small changes in the vasculatures. Among various vascular diseases, fibromuscular dysplasia(FMD) and segmental arterial mediolysis(SAM) are noninflammatory, nonatherosclerotic arterial diseases which need to be diagnosed urgently because these diseases could affect various organs and be lethal if the appropriate management is not provided. However, because FMD and SAM are rare, the cause, prevalence, clinical characteristics including the symptoms, findings in the imaging studies, pathological findings, management, and prognoses have not been systematically summarized. Therefore, there have been neither standard diagnostic criteria nor therapeutic methodologies established, to date. To systematically summarize the information and to compare these disease entities, we have summarized the characteristics of FMD and SAM in the gastroenterological regions by reviewing the cases reported thus far. The information summarized will be helpful for physicians treating these patients in an emergency care unit and for the differential diagnosis of other diseases showing severe abdominal pain.

Flavonoids are a group of natural compounds in plants with versatile health benefits for humans. Grapes are a dietary source of flavonoids and the flavonoid components in grape berries can depend on the grape species and cultivar. In this experiment, proanthocyanidins, flavonols, and anthocyanins were analyzed in Vitis coignetiae and V. ficifolia var. ganebu, wild grapes native to Japan, and compared with those in V. labruscana cv. Muscat Bailey A, to evaluate the potential of the wild grapes as a grape resource. Proanthocyanidin contents in seeds were lower in the two wild grapes than in Muscat Bailey A. However, the skin of V. ficifolia var. ganebu was the richest source of proanthocyanidins. Flavonol levels in the skins of the two wild grapes were lower than that in the skin of Muscat Bailey A. Colorimetry determined that the total anthocyanin content in the skin of V. ficifolia var. ganebu was 6 times and 7 times higher, respectively, than those of V. coignetiae and Muscat Bailey A. Although monoglucoside anthocyanin levels analyzed by high-pressure liquid chromatography (HPLC) were in the order Muscat Bailey A > V. ficifolia var. ganebu > V. coignetiae, most of the diglucoside and acylated monoglucoside and diglucoside anthocyanin levels identified by HPLC-mass spectrometry were highest in V. ficifolia var. ganebu. These data suggest that V. ficifolia var. ganebu might be a novel source of flavonoids and superior to V. coignetiae as a source of flavonoids.

Effects of anti-hypertensive drugs on esophageal body contraction

AIM:To clarify the effects of anti-hypertensive drugs on esophageal contraction and determine their possi-ble relationship with gastro-esophageal reflux ***:Thirteen healthy male volunteers were enrolled. Esophageal body peristaltic contractions and lower esophageal sphincter (LES) pressure were measured using high resolution manometry. All subjects were randomly examined on four separate occasions following administrations of nifedipine,losartan,and atenolol,as well as without any drug ***:Peristaltic contractions by the esophageal body were separated into three segments by two troughs. The peak peristaltic pressures in the mid and lower segments of the esophageal body under atenolol administration were signifi cantly higher than those without medication in a supine position. On the other hand,peristaltic pressures under nifedipine administration were lower than those observed without drug ad-ministration. Losartan did not change esophageal body peristalsis. Atenolol elevated LES pressure and slowed peristaltic wave transition,while the effects of nifedip-ine were the opposite. CONCLUSION:Among the anti-hypertensive drugs tested,atenolol enhanced esophageal motor activity,which was in contrast to nifedipine.

Expression of AID, P53, and Mlh1 proteins in endoscopically resected differentiated-type early gastric cancer

AIM: To analyze the expression of the tumor-related proteins in differentiated-type early gastric carcinoma (DEGC) samples. METHODS: Tumor specimens were obtained from 102 patients (75 males and 27 females) who had received an endoscopic tumor resection at Tottori University Hospital between 2007 and 2009. Ninety-one cancer samples corresponded to noninvasive or intramucosal carcinoma according to the Vienna classification system, and 11 samples were submucosal invasive carcinomas. All of the EGCs were histologically differentiated carcinomas. All patients were classified as having Helicobacter pylori (H. pylori) infections by endoscopic atrophic changes or by testing seropositive for H. pylori IgG. All of the samples were histopathologically classified as either tubular or papillary adenocarcinoma according to their structure. The immunohistochemical staining was performed in a blinded manner with respect to the clinical information. Two independent observers evaluated protein expression. All data were statistically analyzed then. RESULTS: The rates of aberrant activation-induced cytidine deaminase (AID) expression and P53 overexpression were both 34.3% in DEGCs. The expression of Mlh1 was lost in 18.6% of DEGCs. Aberrant AID expression was not significantly associated with P53 overexpression in DEGCs. However, AID expression was associated with the severity of mononuclear cell activity in the non-cancerous mucosa adjacent to the tumor (P = 0.064). The rate of P53 expression was significantly greater in flat or depressed tumors than in elevated tumors. The frequency of Mlh1 loss was significantly increased in distal tumors, elevated gross-type tumors, papillary histological-type tumors, and tumors with a severe degree of endoscopic atrophic gastritis (P < 0.05). CONCLUSION: Aberrant AID expression, P53 overexpression, and the loss of Mlh1 were all associated with clinicopathological features and gastric mucosal alterations in DEGCs. We report the design concept and performance of a compact, lightweight and economical imaging polarimeter, the Triple Range Imager and POLarimeter(TRIPOL), capable of simultaneous optical imagery and polarimetry. TRIPOL splits the beam in wavelengths from 400 to 830 nm into g′-, r′-and i′-bands with two dichroic mirrors, and measures polarization with an achromatic half-waveplate and a wire grid polarizer. The simultaneity makes TRIPOL a useful tool for small telescopes for the photometry and polarimetry of time variable and wavelength dependent phenomena. TRIPOL is designed for a Cassegrain telescope with an aperture of^1 m. This paper presents the engineering considerations of TRIPOL and compares the expected with observed performance. Using the Lulin 1-m telescope and 100 seconds of integration, the limiting magnitudes are g′~19.0 mag, r′~18.5 mag and i′~18.0 mag with a signal-to-noise ratio of 10, in agreement with design expectation. The instrumental polarization is measured to be^0.3% in the three bands. Two applications, one to the star-forming cloud IC 5146 and the other to the young variable GM Cep, are presented as demonstrations.

低压静电场处理对鱿鱼保鲜效果的影响

为探究低压静电场(low voltage electrostatic field,LVEF)处理对鱿鱼贮藏品质的影响,采用pH值、蒸煮损失率、挥发性盐基氮(total volatile basicnitrogen,TVB⁃N)和硫代巴比妥酸(thiobarbituric acid,TBA)等评价指标,以无电场处理的样品为对照组,研究比较低压静电场贮藏条件下的鱿鱼品质变化情况。结果表明,低压静电场处理能够改善鱿鱼的贮藏品质。随着贮藏时间的延长,LVEF组的样品pH值增加缓慢;贮藏期间对照组的蒸煮损失率、TVB⁃N值始终低于LVEF组,且当贮藏至15 d时LVEF组的TBA值远低于对照组的1.24 mg/kg;低压静电场对微生物繁殖有明显的抑制作用,贮藏15 d内LVEF组菌落总数均低于6 lg(CFU/g);在硬度、弹性、感官评价和蒸煮损失率上LVEF组均优于对照组。此外,十二烷基硫酸钠⁃聚丙烯酰胺凝胶电泳分析显示LVEF组条带未发生明显变化,故低压静电场处理能更好地保持鱿鱼品质。
