Efficacy and safety of telaprevir- and simeprevir-based triple therapies for older patients with chronic hepatitis C

AIMTo evaluate and compare the efficacy and safety of telaprevir (TVR)-and simeprevir (SMV)-based triple therapies in elderly patients, specifically patients aged 66 years or older. METHODSThe present study enrolled 112 and 76 Japanese patients with chronic hepatitis C virus genotype 1b infection who were treated with a 12-wk TVR-based or SMV-based triple therapy, respectively, followed by a dual therapy that included pegylated interferon α and ribavirin (RBV) for 12 wk. The patients were categorized into two groups according to age as follows: A younger group of patients aged ≤ 65 years old and an older group of patients aged > 65 years old. Among the patients treated with TVR-based triple therapy, 34 patients were included in the older group. The median ages were 56 years (range: 28-65 years) in the younger group and 69 years (range: 66-81 years) in the older group. Among the patients treated with SMV-based triple therapy, 39 patients were included in the older group. The median ages were 59 years (range: 36-65 years) in the younger group and 71 years (range: 66-86 years) in the older group. The clinical, biochemical and virological data were analyzed before and during treatment. RESULTSAmong the patients treated with the TVR-based triple therapy, no significant difference in the sustained virological response (SVR) was found between the younger (80.8%) and older (88.2%) groups. The SVR rates for patients with the interleukin 28B (IL28B) (rs8099917) TG/GG-genotypes (73.9% and 60.0% in the younger and older groups, respectively) were significantly lower than for patients with the IL28B TT-genotype (86.3% and 92.9%, respectively). The cumulative exposure to RBV for the entire 24-wk treatment period (as a percentage of the target dose) was significantly higher in the younger group than in the older group (91.7% vs 66.7%, respectively, P vs 81.9%, respectively). Regression of hepatocellular carcinoma during vitamin K administration

An 85-year-old man with HCV infection and diabetes mellitus was diagnosed as having hepatocellular carcinoma （HCC, 13 cm in diameter） based on high serum alpha-fetoprotein （AFP）, AFP-L3, and des-y-carboxy prothrombin levels as well as typical enhancement pattern on contrast-enhanced CT. The patient did not receive any interventional treatments because of advanced age and the advanced stage of HCC. He chose to take vitamin K, which was reported to suppress the growth of HCC in vitro. Three months after starting vitamin K, all three tumor markers were normalized and HCC was markedly regressed, showing no enhancement in the early arterial phase on CT. Here we present the report describing the regression of HCC during the administration of vitamin K.

Rain Attenuation in the Microwave-to-Terahertz Waveband

In recent years, there has been increased interest in the terahertz waveband for application to ultra-high-speed wireless communications and remote sensing systems. However, atmospheric propagation at these wavelengths has a significant effect on the operational stability of systems using the terahertz waveband, so elucidating the effects of rain on propagation is a topic of high interest. We demonstrate various methods for calculating attenuation due to rain and evaluate these methods through comparison with calculated and experimental values. We find that in the 90 - 225 GHz microwave band, values calculated according to Mie scattering theory using the Best and P-S sleet raindrop size distributions best agree with experimental values. At 313 and 355 GHz terahertz-waveband frequencies, values calculated according to Mie scattering theory using the Weibull distribution and a prediction model following ITU-R recommendations best agree with experimental values. We furthermore find that attenuation due to rain increases in proportion to frequency for microwave-band frequencies below approximately 50 GHz, but that there is a peak at around 100 GHz, above which the degree of attenuation remains steady or decreases. Rain-induced attenuation increases in proportion to the rainfall intensity.

A coarse-grained particle （CGP） method has been proposed by R.E. Rudd and J.Q. Broughton [1] for a crystalline of atoms at finite temperature, to describe the propagation of long-wavelength waves with low computational costs. Thought the method has attractive features such as its natural incorporation of atomic phonons and its potential suitableness to connection to an atomistic simulation method, the original formulation limits its application to simple small system. In this paper we propose fast coarse-grained particle （FCGP） method, which is CGP method modified to calculate the stiffness matrix faster and to simulate larger system. The FCGP method is hybridized concurrently with the molecular dynamics method for atoms, in which the FCGP method gives the dynamic boundary conditions to the molecular dynamics method.

Hepatocellular carcinoma(HCC) is one of the most common cancers and the third highest cause of cancerassociated mortality worldwide. The treatment of HCC is complicated by its variable biological behavior and the frequent coexistence of chronic liver disease, particularly cirrhosis. To date, multiple treatment modalities have been developed according to the stage of the tumor and the hepatic functional reserve, including transarterial treatments such as transarterial chemoembolization, transarterial oily chemoembolization(TOCE), and hepatic arterial infusion chemotherapy(HAIC). We conducted a phase I and II study of the combination therapy with double platinum agents, miriplatin and cisplatin, and confirmed its safety and efficacy. Here, we describe two cases of unresectable HCC who were successfully treated by miriplatin-TOCE/cisplatin-HAIC combination therapy, resulting in complete responses with no significant adverse events. This report will provide that the combination therapy can be the therapeutic option for HCC patients in the advanced stage.

Using data from seven ionosonde stations, Haikou and Chongqing, Guangzhou, Haikou, Chongqing, Beijing, Lanzhou, Changchun and Manzhouli stations, we have developed an empirical model by using Support Vector Machine, to predict the ionospheric foF2 parameter in storm-time. In this paper, we investigate whether foF2 during disturbed geomagnetic conditions at a single station can be well predicted one hour ahead by using some inputs to an SVM network, such as the latest foF2 observations, hourly quiet- time foF2 (foF2 QT ), time, and the hourly time-weighted accumulation series derived from the geomagnetic index ap(τ).It indicated that the model described here can capture the low latitude storm time F2 layer variability at most times.

This paper proposes mechanisms for tracing objects (consumer products) without disclosing identities of their owners, while enabling exact owner detections when required. The mechanisms enhance reliability of product-recycling systems while forcing anonymous entities to execute appropriate procedures for maintaining and discarding their owning products. Together with remote monitoring systems, it becomes possible to maintain conditions of products so that safe and efficient uses of them are ensured while protecting privacies of their owners.
