We simultaneously observed both the fast proton generation and terahertz(THz) radiation in the laser pulse interaction with a thin-foil target. The maximum proton energy of ～2.3 Me V and an intense THz radiation were observed at the pulse duration of～30 fs. We also measured the proton beam and UV harmonics from a thin-foil target by changing the laser pulse duration. In the case of the ～500 fs, peaks of UV harmonics up to fourth order appeared. This unique combination of the multiple beams will provide useful applications such as pump-probe experiments.

Adult case of an omphalomesenteric cyst resected by laparoscopic-assisted surgery

This report describes an extremely rare adult case of an omphalomesenteric cyst resected by laparoscopicassisted surgery. A 29-years-old Japanese man was referred and admitted to Kyushu University Hospital because of an abdominal mass and an elevated serum CEA （carcinoembryonic antigen） level （21.3 ng/mL） in August 2001. Abdominal CT and US demonstrated a cystic mass with septum and calcification. Laparoscopy showed a large mass to be attached to his abdominal wall, measuring 110 mm × 70 mm× 50 mm and filled with mucus. The mass was resected by laparoscopicassisted surgery. The histological findings of its wall showed fibromuscular tissue, adipose tissue, calcification, and an intestinal structure. It was finally diagnosed to be an omphalomesenteric cyst.

Phase II Study of Carboplatin and Pemetrexed Followed by Gefitinib for Patients with Advanced Non-Small Cell Lung Cancer Harboring Sensitive EGFR Mutation

We conducted a phase II study of combination chemotherapy with carboplatin (Cb) and pemetrexed (Pem) followed by gefitinib (Gef) to determine the effects and toxicities in patients with non-small cell lung cancer (NSCLC) harboring sensitive EGFR mutation. Eligible patients received four courses of Cb at a dose corresponding to a target area under the curve equal to 6 mg/mL·min and 500 mg/m2 Pem on day 1 every three to four weeks followed by sequential Gef 250 mg once a day until tumor progression. Sixteen of registered 28 patients responded to Cb and Pem combination. Twenty-seven patients received sequential Gef and 8 non-responders to Cb and Pem achieved PR. The overall response rate was 85.7%. Among the major toxicities, grade 3 SGPT elevation, nausea and thrombosis were observed in 3, 3 and 1 patients, respectively, who received Cb and Pem, and grade 3 SGPT elevation and dry skin were observed in 5 and 1 patients, respectively, who received Gef. There was no febrile neutropenia and no treatment-related death. The median progression-free survival time was 19.1 months. Among 21 patients who were followed up for more than 2 years, 14 survived during that time. Cb and Pem followed by Gef maintenance are recommended for further evaluation for patients with metastatic NSCLC harboring sensitive EGFR mutation.

目的心脏结节病（CS）可以通过***／CT、心血管磁共振（CMR）和123I-BMIPP／201T1C1双核素心肌SPECT显像来诊断。该研究旨在说明这3种方法之间的关系。方法和结果16例确诊为CS的患者[男性11例，年龄（55±13）岁]在类固醇治疗前进行了18F-FDGPET／CT、CMR和双核素门控SPECT显像。以上显像时左心室心肌均被分为17段，PET／CT显像时计算各段的最大标准摄取值（SUVmax）；双核素SPECT显像时计算放射性缺损评分（0-4；0为放射性分布正常，4为放射性缺损），包括123I—BMIPP显像的缺损评分（BMDS）、201T1C1显像的缺损评分（TLDS）和两者间的不匹配评分（MS）；CMR显像时计算延时钆增强（LGE）评分（0为无延时增强；1为部分增强；2为均匀增强）。17个节段总计BMDS、TLDS和MS分别为18．6±12．6、12．9±10．9和5．7±3．1。各段的BMDS和TLDS评分随着LGE评分增加而显著增加（P〈0．001）。与LGE评分为0的节段相比，LGE评分为1的节段其MS评分显著增加（P〈0．001），但是LGE评分为1和2的节段其MS评分无明显差异。就LGE评分为1的节段而言，其SUVmax与LGE评分为0的节段相比显著增加（P〈O．025），但是与LGE评分为2的节段相比无明显差异。结论CS患者具有较高SUVmax的区域主要位于有延时增强效应的节段，提示有活动性心肌炎性反应，这些患者123I-BMIPP和和201T1C1显像有明显的不匹配。
