Long-term prognosis and its associated predictive factors in patients with eosinophilic gastroenteritis

BACKGROUND Eosinophilic gastroenteritis(EGE)is a chronic recurrent disease with abnormal eosinophilic infiltration in the gastrointestinal tract.Glucocorticoids remain the most common treatment method.However,disease relapse and glucocorticoid dependence remain notable problems.To date,few studies have illuminated the prognosis of EGE and risk factors for disease relapse.AIM To describe the clinical characteristics of EGE and possible predictive factors for disease relapse based on long-term follow-up.METHODS This was a retrospective cohort study of 55 patients diagnosed with EGE admitted to one medical center between 2013 and 2022.Clinical records were collected and analyzed.Kaplan-Meier curves and log-rank tests were conducted to reveal the risk factors for long-term relapse-free survival(RFS).RESULTS EGE showed a median onset age of 38 years and a slight female predominance(56.4%).The main clinical symptoms were abdominal pain(89.1%),diarrhea(61.8%),nausea(52.7%),distension(49.1%)and vomiting(47.3%).Forty-three(78.2%)patients received glucocorticoid treatment,and compared with patients without glucocorticoid treatments,they were more likely to have elevated serum immunoglobin E(IgE)(86.8%vs 50.0%,P=0.022)and descending duodenal involvement(62.8%vs 27.3%,P=0.046)at diagnosis.With a median follow-up of 67 mo,all patients survived,and 56.4%had at least one relapse.Six variables at baseline might have been associated with the overall RFS rate,including age at diagnosis24 kg/m^(2)(HR 0.3922,95%CI:0.1916-0.8027,P=0.014),disease duration from symptom onset to diagnosis>3.5 mo(HR 2.4725,95%CI:1.220-5.0110,P=0.011),vomiting(HR 3.1259,95%CI:1.5246-6.4093,P=0.001),total serum IgE>300 KU/L at diagnosis(HR 0.2773,95%CI:0.1204-0.6384,P=0.022)and glucocorticoid treatment(HR 6.1434,95%CI:2.8446-13.2676,P=0.003).CONCLUSION In patients with EGE,younger onset age,longer disease course,vomiting and glucocorticoid treatment were

Pd-catalyzed intermolecular Si–O formation via Si–C activation

The development of efficient Si–O bond formation reaction with 100% atom-economy, excellent functional group tolerance, and broad scope under mild conditions is highly desired due to the prevalence of silanol, silyl ether, and their derivatives in synthetic chemistry and materials science. Here, we have realized the Pd-catalyzed Si–O formation reaction via a Si–C activation approach with 100% atom-economy by employing silacyclobutanes(SCBs) and various hydroxy-containing substrates, including water,alcohols, phenols, and silanols. This protocol features a broad substrate scope, remarkable functional compatibility and mild conditions, providing a series of silanols, silyl ethers in high efficiency. Notably, this protocol could also be used for selective protection of hydroxy functionalities, and for the access of a class of novel polymers containing Si–O main chain. Preliminary mechanistic studies unveiled that this reaction underwent a Pd-catalyzed concerted ring-opening mechanism.

Construction of a bivalent vaccine candidate against HAdV4/HAdV7 based on capsid-display strategy via Red-homologous recombination and counter-selection methodology

Human adenoviruses(HAdvs)are major respiratory pathogens.Specifically,human adenovirus type 4(HAdV4)and human adenovirus type 7(HAdV7)are known for causing fever and pneumonia,with docu-mented cases of fatalities among the population.In recent years,HAdV4/HAdv7 has been implicated in caus-ing substantial outbreaks,leading to increased morbidity in multiple countries.Most HAdV4 and HAdV7 infections have been reported in North America,Asia,Europe,Africa,South America,Oceania,and the Middle East.Most fatalities occurred in North America(the United States)and Asia(China and Singapore).Engineered recombinant adenoviruses have played a crucial role as vaccine vectors.In this study,we con-structed a recombinant adenovirus,Ad4ITRmut-Ad7E3,and evaluated it in vitro and in vivo.We observed that the replication rate of Ad4ITRmut-Ad7E3 was lower than that of the RI-67 strain,indicating that the mutation of inverted terminal repeats(ITRs)weakened the replication ability of HAdV4.Immunization of BALB/c mice with the bivalent Ad4ITRmut-Ad7E3 vaccine strain,administered by intraperitoneal injection and oral gavage,resulted in the elicitation of neutralizing antibodies targeting HAdV4 and HAdv7.This finding not only pro-vides a novel method and technique for the efficient construction of a polyvalent recombinant adenovirus vac-cine candidate against HAdV4 and HAdv7 but also against other prevalent adenovirus serotypes such as HAdV3,HAdV11,HAdV14,and HAdv55,from various regions.

More common RNAemia in the early stage of severe SARS-CoV-2 BF.7.14 infections in pediatric patients

The risk factors of severe infections in children during the severe acute respiratory syndrome coronavirus 2(SARS‐CoV‐2)outbreak in Beijing remain elusive.SARS‐CoV‐2‐positive children admitted to the intensive care unit(ICU)with collected plasma specimens were enrolled and screened for common pathogens using capillary electrophoresis‐based multiplex PCR from December 12,2022,to January 24,2023.The SARS‐CoV‐2 subvariants were identified using next‐generation sequencing.Plasma was positive for two(positive;P),one(suspicious;S),or no(negative;N)SARS‐CoV‐2 genes were classified as plasmatic RNA‐positive(RNAemia;P+S)or without RNAemia(N).Clinical and laboratory data of the enrolled cases were then collected and analyzed.The 34 enrolled children included 26 males and 24 younger than three years.All were negative for other respiratory pathogens.BF.7.14(18/29)was the predominant subvariant.Viral loads in respiratory specimens,hours from symptom onset to the first respiratory specimen collection(time‐variable),with comorbidities and BF.7.14 and BA.5.2 distributions were significantly different in P vs.N and RNAemia vs.without RNAemia group.Among most cases,the T lymphocyte ratios decreased,while the cytokine level and the B lymphocyte ratio increased.The time variables were 2.22±2.05 and 4.00±2.49 days in BF.7.14 and BA.5.2 infections,respectively.In conclusion,SARS‐CoV‐2 was more likely to cause severe infections among males aged≤3 years old with comorbidities during the SARS‐CoV‐2 outbreak in Beijing,while RNAemia is more common in children at the early stage of severe BF.7.14 infections,and most had high cytokine levels and B‐cell activation.

Differential depression of the glacier equilibrium-line altitudes in the Yarlung Zangbo Downstream Basin in the Last Glacial Maximum compared to the pre-industrial era

Glaciers in the Yarlung Zangbo Downstream Basin(YZDB) are sensitive to global climate change. The equilibrium-line altitude(ELA) is a key indicator of glacial development.Current models for simulating the meteorological ELA underestimate the extent of glacial advance during the Last Glacial Maximum(LGM) in the YZDB and cannot explain the large-scale glacial extension compared with the Yarlung Zangbo Midstream Basin(YZMB). In this study, the distribution of ELA in the LGM is reconstructed using high-resolution 80-km ECHAM5 simulations and empirical relationships between temperature and precipitation.Changes in ELA between the LGM and pre-industrial era(PI) are compared. Our simulated ELA closely fits the published field data. In the YZDB, simulated LGM ELAs range from ca.3500 m to over 4900 m, representing declines of ca. 300–950 m. The ECHAM5 simulations better reflect the complex topographic features than most coarse-resolution climate models,and the ELA distribution is controlled by the spatial arrangement of river valley systems and mountain ranges and their impact on precipitation. Compared with the PI era, most of the monsoon precipitation in the LGM was concentrated in the YZDB, which is the main driver of glacial extension and the differential response of the downstream and midstream basins.

Quantifying the impact of earthquakes and geological factors on spatial heterogeneity of debris-flow prone areas:A case study in the Hengduan Mountains

Understanding the spatial heterogeneity of debris-flow-prone areas holds significant implications for regional risk management, particularly in seismically active regions with geological faults. Despite the significance of this knowledge, a comprehensive quantification of the influence of regional topographical and geological factors on the spatial heterogeneity of debris-flow-prone areas has been lacking. This study selected the Hengduan Mountains, an earthquake-prone region characterized by diverse surface conditions and complex landforms, as a representative study area. An improved units zoning and objective factors identification methodology was employed in earthquake and fault analysis to assess the impact of seismic activity and geological factors on spatial heterogeneity of debrisflow prone areas. Results showed that the application of GIS technology with hydrodynamic intensity and geographical units analysis can effectively analyze debris-flow prone areas. Meanwhile, earthquake and fault zones obviously increase the density of debrisflow prone catchments and make them unevenly distributed. The number of debris-flow prone areas shows a nonlinear variation with the gradual increase of geomorphic factor value. Specifically, the area with 1000 m-2500 m elevation difference, 25°-30° average slope, and 0.13-0.15 land use index is the most favorable conditions for debris-flow occurrence;The average annual rainfall from 600 to 1150 mm and landslides gradient from 16° to 35° are the main causal factors to trigger debris flow. Our study sheds light on the quantification of spatial heterogeneity in debris flow-prone areas in earthquake-prone regions, which can offer crucial support for post-debris flow risk management strategies.

We investigate high-pressure phase diagrams of Pr–N compounds by proposing five stable structures(PnmaPr N,Ⅰ4/mmm-PrN_(2),C2/m-PrN_(3),P■-PrN_(4),and R3-PrN_(8))and two metastable structures(P■-PrN_(6)and P■-PrN_(10)).The P■-PrN_(6)with the N14-ring layer and R3-PrN_(8)with the N18-ring layer can be quenched to ambient conditions.For the P■-PrN_(10),the N_(22)-ring layer structure transfers into infinite chains with the pressure quenched to ambient pressure.Remarkably,a novel polynitrogen h R8-N designed by the excision of Pr atoms from R3-PrN_(8)is obtained and can be quenched to ambient conditions.The N-rich structures of P■-PrN_(6),R3-PrN_(8),c-PrN_(10)and the solid pure nitrogen structure exhibit outstanding properties of energy density and explosive performance.

Active and passive compliant force control of ultrasonic surface rolling process on a curved surface

Ultrasonic surface rolling process(USRP)is one of the effective mechanical surface enhancement techniques.During the USRP,unstable static force will easily do harm to the surface quality.In order to achieve a higher surface quality on the part with a curved surface,an active and passive compliant USRP system has been developed.The compliant USRP tool can produce the natural obedience deformation along the part surface.Force control based on the fuzzy Proportional-integral-derivative(PID)method is then designed to maintain the static force during the USRP.Experiments have been performed on a real aero-engine blade with curved surface.It is proved that the deigned active and passive compliant USRP system can significantly reduce the force variation from 42.2 N to 4.2 N,and achieve a uniform surface quality after processing.

Joint computation offloading and resource allocation in vehicular edge computing networks

Vehicular Edge Computing(VEC)is a promising technique to accommodate the computation-intensive and delaysensitive tasks through offloading the tasks to the RoadSide-Unit(RSU)equipped with edge computing servers or neighboring vehicles.Nevertheless,the limited computation resources of edge computing servers and the mobility of vehicles make the offloading policy design very challenging.In this context,through considering the potential transmission gains brought by the mobility of vehicles,we propose an efficient computation offloading and resource allocation scheme in VEC networks with two kinds of offloading modes,i.e.,Vehicle to Vehicle(V2V)and Vehicle to RSU(V2R).We define a new cost function for vehicular users by incorporating the vehicles’offloading delay,energy consumption,and expenses with a differentiated pricing strategy,as well as the transmission gain.An optimization problem is formulated to minimize the average cost of all the task vehicles under the latency and computation capacity constraints.A distributed iterative algorithm is proposed by decoupling the problem into two subproblems for the offloading mode selection and the resource allocation.Matching theorybased and Lagrangian-based algorithms are proposed to solve the two subproblems,respectively.Simulation results show the proposed algorithm achieves low complexity and significantly improves the system performance compared with three benchmark schemes.
