Highlighting the importance of early diagnosis in progressive multi-organ involvement of IgG4-related disease: A case report and review of literature

IgG4-related disease(IgG4-RD)is an increasingly recognized pathological entity that tends to involve multiple organs with an elevated level of serum IgG4,which is easily misdiagnosed owing to sharing common clinical features with a variety of other ***,we report an interesting IgG4-RD case of a woman with progressive multi-organ involvement for over 19 years,started with swollen eyelids,dry eye and mouth,and polydipsia and *** diagnosis revealed diffuse enlargement of the parotid glands,enlargement of the head of the pancreas,pulmonary infection and interstitial *** tests showed a remarkable elevation of the serum IgG4,and cytological analysis further revealed a large amount of lymphoplasmacytic infiltration into the focal lobule,and IgG 4-positive cell infiltration in bladder ***,the patient responded well to steroid therapy,and thus,she was diagnosed as IgG 4-RD *** report highlights the importance of an early diagnosis in this autoimmune disease and suggests that patients with a clinically unclear cause of inflammation,swelling and refractory glands,rhinitis,pancreatitis,hypophysitis,and/or interstitial pneumonia should be considered for IgG *** plasma IgG4 level and lymphoplasmacytic infiltration may be useful indexes for screening,and a low dose of steroid maintaining therapy may offer benefits for patients with IgG4-RD.

Early liver retransplantation versus late liver retransplantation： analysis of a single-center experience

Background Orthotopic liver retransplantation （re-OLT） is the only effective therapy for irreversible failure of a liver graft. Early and late graft failure gives way to two different clinical conditions that should be discussed separately. This study was designed to compare early and late re-OLT for patients with poor graft function after primary transplantation at our center and sum up our clinical experience in re-OLT. Methods The clinical data of 31 re-OLTs at our center from January 2004 to February 2007 were analyzed retrospectively, consisting of the first group with 14 cases of early re-OLT and the second group with 17 cases of late re-OLT.Results Biliary tract complications were the main indications for early re-OLT （57.1%） and late re-OLT （52.9%）. Other common indications were vascular complications in early re-OLT and recurrence of primary diseases in late re-OLT. No significant differences were found between the groups with regard to the volume of bleeding during operation, cold ischemia time, operative duration, and perioperative mortality; except for the model of end-stage liver disease （MELD） score. Outcome was fatal for 7 patients in early re-OLT and 9 patients in late re-OLT. Two deaths were due to multiple organ failure with 3 deaths due to severe sepsis-related disease in early re-OLT, and 4 deaths were due to severe sepsis-related disease with 3 deaths due to recurrence of hepatocellular carcinoma （HCC） in late re-OLT. One and 2-year actuarial survival rates after re-OLT were 55.2% and 36.9%, respectively, for patients in early re-OLT, and 65.1% and 52% respectively, for patients in late re-OLT. No significant differences were found regarding survival rates between the two groups. Conclusions Similar clinical results can be achieved in early and late re-OLT. To the Editor:Interstitial lung disease(ILD)is a common extra-articular manifestation in patients with connective tissue diseases(CTDs),including rheumatoid arthritis(RA),which contributes significantly to the disease burden and excess mortality.[1]It has been well-recognized that RA-associated ILD(RA-ILD)patients have a threefold higher risk of death than RA patients without ILD.[2]However,among approximately 30%of ILD patients,the specific diagnosis cannot be made from clinical findings and high-resolution computed tomography features,resulting in diagnostic and management uncertainty.

A stable one-point quadrature rule for three-dimensional numerical manifold method

We present a numerically stable one-point quadrature rule for the stiffness matrix and mass matrix of the three-dimensional numerical manifold method(3D NMM).The rule simplifies the integration over irregularly shaped manifold elements and overcomes locking issues,and it does not cause spurious modes in modal *** essential idea is to transfer the integral over a manifold element to a few moments to the element center,thereby deriving a one-point integration rule by the moments and making modifications to avoid locking *** the stiffness matrix,after the virtual work is decomposed into moments,higher-order moments are modified to overcome locking issues in nearly incompressible and bending-dominated *** the mass matrix,the consistent and lumped types are derived by *** particular,the lumped type has the clear advantage of *** proposed method is naturally suitable for 3D NMM meshes automatically generated from a regular *** tests justify the accuracy improvements and the stability of the proposed procedure.

Efficient Finite Difference WENO Scheme for Hyperbolic Systems withNon-conservativeProducts

Olefin polymerization is one of the most im portant chemical reactions in *** work presents a strategy that emphasizes the synergistic meta/poro-steric hindrance of N-aryl groups and electronic effects in newly synthesized neutral salicylaldiminato nickel *** nickel(Ⅱ)catalysts exhibit exceptional thermostability,ranging from 30℃to 130℃,demonstrating enhanced catalytic activities and broadly regulated polyethylene molecular weights(3-341 kg·mol^(-1))and controlled polymer branch density(2-102 brs/1000C).The preferred catalyst Ni3 with concerted steric and electronic effects enables the production of solid-state semi-crystalline polyethylene materials at temperatures below 90℃.Notably,Ni3 exhibits an impressive tolerance of 110℃and can withstand even the challenging polymerization temperature of 130℃,leading to the production of polyethylene wax and ***,functionalized polyethylene is produced.

Prox1 Suppresses Proliferation and Drug Resistance of Retinoblastoma Cells via Targeting Notch1

Experimental study and numerical simulation of the impact of under-sleeper pads on the dynamic and static mechanical behavior of heavy-haul railway ballast track

Laying the under-sleeper pad(USP)is one of the effective measures commonly used to delay ballast degradation and reduce maintenance *** explore the impact of application of the USP on the dynamic and static mechanical behavior of the ballast track in the heavy-haul railway system,numerical simulation models of the ballast bed with USP and without USP are presented in this paper by using the discrete element method(DEM)-multi-flexible body dynamic(MFBD)coupling analysis *** ballast bed support stiffness test and dynamic displacement tests were carried out on the actual operation of a heavy-haul railway line to verify the validity of the *** results show that using the USP results in a 43.01%reduction in the ballast bed support stiffness and achieves a more uniform distribution of track loads on the *** effectively reduces the load borne by the sleeper directly under the wheel load,with a 7.89%reduction in the pressure on the ***,the laying of the USP changes the lateral resistance sharing ratio of the ballast bed,significantly reducing the stress level of the ballast bed under train loads,with an average stress reduction of 42.19 *** also reduces the plastic displacement of ballast particles and lowers the peak value of rotational angular velocity by about 50%to 70%,which is conducive to slowing down ballast bed settlement deformation and reducing maintenance *** summary,laying the USP has a potential value in enhancing the stability and extending the lifespan of the ballast bed in heavy-haul railway systems.

Analysis of mechanical strengths of extreme line casing joint considering geometric, material, and contact nonlinearities

To address the challenges associated with difficult casing running,limited annular space,and poor cementing quality in the completion of ultra-deep wells,the extreme line casing offers an effective solution over conventional ***,due to its smaller size,the joint strength of extreme line casing is reduced,which may cause failure when running in the *** address this issue,this study focuses on the CST-ZTΦ139.7 mm×7.72 mm extreme line casing and employs the elastic-plastic mechanics to establish a comprehensive analysis of the casing joint,taking into account the influence of geometric and material nonlinearities.A finite element model is developed to analyze the forces and deformations of the extreme line casing joint under axial tension and external collapse *** model investigates the stress distribution of each thread tooth subjected to various tensile forces and external ***,the tensile strength and crushing strength of the extreme line casing joint are determined through both analytical and experimental *** findings reveal that,under axial tensile load,the bearing surface of each thread tooth experiences uneven stress,with relatively high equivalent stress at the root of each thread *** end thread teeth are valuable spots for *** is observed that the critical fracture axial load of thread decreases linearly with the increase of thread tooth *** external pressure,the circumferential stress is highest at the small end of the external thread,leading to yield *** tensile strength of the joint obtained from the finite element model exhibits a relative error of less than 7%compared to the analytical and experimental values,proving the reliability of the finite element *** tensile strength of the joint is 3091.9 k ***,in terms of anti-collapse capability,the joints demonstrate higher resistance to collapse compared to the casing body,which is consistent with the test resul
