Prevalence and incidence of heart failure among community in China during a three-year follow-up

BACKGROUND Epidemiological surveys on heart failure(HF)in Chinese community are relatively lacking.This study aimed to estimate the prevalence and incidence of HF among community residents in southern China.METHODS Baseline data of this prospective study was collected from 2015 to 2017 among 12,013 permanent residents aged≥35years in Guangzhou,China.The same survey process was carried out for individuals aged≥65 years after a three-year follow-up.RESULTS The overall prevalence of HF in community residents aged≥35 years was 1.06%.Male had significantly higher risk of HF prevalence[odds ratio(OR)=1.50,P=0.027].The gender-adjusted risk of HF was 1.48 times higher per 10 years aging.HF prevalence was statistically associated with atrial fibrillation,valvular heart disease,hypertension and chronic obstructive pulmonary disease after adjusting for age and gender(OR=8.30,5.17,1.11,2.28,respectively;all Pline atrial fibrillation,valvular heart disease,and diabetes mellitus were risk factors for HF incidence for individuals aged≥65 years adjusting for age and gender(OR=5.05,3.99,2.11,respectively;all P<0.05).Besides,residents with new-onset atrial fibrillation and myocardial infarction were at significantly higher risk of progression to HF(OR=14.41,8.54,respectively;all P<0.05).CONCLUSIONS Both pre-existing and new-onset cardiovascular diseases were associated with HF incidence in southern China.Management of related cardiovascular diseases may be helpful to reduce the incidence of HF.

Numerical simulation of Marangoni effect induced by species transfer across iron droplet–molten slag interface

A mathematical model accounting for unsteady mass transfer across interface of a stationary iron droplet immersed into molten slag was established through the volume of fluid coupled with level set method.The Marangoni effect induced by mass transfer was reproduced successfully,and the hydrodynamic instability phenomena at the interface,such as the Marangoni convection flow,the evolution of the interfacial tension during the mass transfer,and the influence of Marangoni effect on the mass transfer rate,were revealed.The results show that the Marangoni convection flow develops quickly and behaves as an ordered structure in the forms of four pairs of the convection cell at the edge of the droplet once the oxygen transfer across the interface starts.The average convection flow velocity along the interface is high,even more than 0.025 m/s,depending on the droplet diameter,which facilitates the mass transfer.The Marangoni convection flow of the large droplet develops more easily than that of the small droplet,and the larger the droplet diameter is,the higher the convection flow velocity and the mass transfer rate are.Moreover,it is shown that the droplet diameter influences the impacting region of the Marangoni convection flow and its duration period.

As a ceramic material,AlN has very good thermophysical and mechanical properties.In addition,AlN is an effective refining agent for Mg alloys because it has a lattice constant similar to that of Mg.Therefore,AlN is an ideal reinforcement for magnesium matrix composites(MMCs),and is attracting increasing attention.This review addresses the development of preparation technologies for AlN-reinforced Mg matrix composites.The mainstream preparation techniques include stir casting,melt infiltration,powder metallurgy,and in-situ methods.In addition,the advantages and disadvantages of these techniques are analyzed in depth,and it is pointed out that the next direction for the preparation of high-performance AlN-reinforced MMCs is less aluminization and multiple technologies integration.

Quasi-Optimized Overlapping Schwarz Waveform Relaxation Algorithm for PDEs with Time-Delay

Schwarzwaveformrelaxation(SWR)algorithmhas been investigated deeply and widely for regular time dependent problems.But for time delay problems,complete analysis of the algorithm is rare.In this paper,by using the reaction diffusion equations with a constant discrete delay as the underlying model problem,we investigate the convergence behavior of the overlapping SWR algorithm with Robin transmission condition.The key point of using this transmission condition is to determine a free parameter as better as possible and it is shown that the best choice of the parameter is determined by the solution of a min-max problem,which is more complex than the one arising for regular problems without delay.We propose new notion to solve the min-max problem and obtain a quasi-optimized choice of the parameter,which is shown efficient to accelerate the convergence of the SWR algorithm.Numerical results are provided to validate the theoretical conclusions.

Left atrial enlargement and non-paroxysmal atrial fibrillation as risk factors for left atrial thrombus/spontaneous Echo contrast in patients with atrial fibrillation and low CHA2DS2-VASc score

Objective To determine the risk factors for thromboembolism in lower risk patients with non-valvular atrial fibrillation(AF)and low CHA2DS2-VASc scores,which remain undefined.Methods We retrospectively analyzed the baseline clinical characteristics,routine laboratory parameters,and echocardiographic measurements of 705 patients(71.1%male;mean age:52.10±9.64 years)with low CHA2DS2-VASc score(0 or 1;1 point for female sex)out of 1346 consecutive patients with non-valvular AF who underwent transesophageal echocardiography(TEE)at Guangdong Cardiovascular Institute between January 2013 and December 2015.Results Patients with left atrial thrombus(LAT)or spontaneous echo contrast(SEC)on TEE(24/705,4%)showed a higher incidence rate of vascular disease(54.2%vs.32.9%,P=0.045)and non-paroxysmal AF(79.2%vs.29.4%,P<0.001),larger left atrial diameter(43.08±4.59 vs.36.02±5.53 mm,P<0.001),and lower left ventricular ejection fraction(58.23±8.82%vs.64.15±7.14%,P<0.001)than those without.Multivariate logistic regression analysis identified left atrial diameter[odds ratio(OR)=1.171,95%confidence interval(CI):1.084–1.265,P<0.001]and non-paroxysmal AF(OR=3.766,95%CI:1.282–11.061,P=0.016)as independent risk factors for LAT/SEC.In ROC curve analysis,a left atrial dimeter cutoff of 37.5 mm yielded 95.0%sensitivity and 62.7%specificity(AUC:0.847,P<0.0001,95%CI:0.793–0.914).Conclusion In patients with non-valvular AF with low CHA2DS2-VASc score,the presence of LAT or SEC was associated with left atrial enlargement,which had moderate predictive value,and non-paroxysmal AF.

Nano-SiC_P particles distribution and mechanical properties of Al-matrix composites prepared by stir casting and ultrasonic treatment

Nano-ceramic particles are generally difficult to add into molten metal because of poor wettability. Nano-SiC_Particles reinforced A356 aluminum alloy composites were prepared by a new complex process, i.e., a molten-metal process combined with high energy ball milling and ultrasonic vibration methods. The nano particles were β-SiC_P with an average diameter of 40 nm, and pre-oxidized at about 850 °C to form an oxide layer with a thickness of approximately 3 nm. The mm-sized composite granules containing nano-SiC_P were fi rstly produced by milling the mixture of oxidized nano-SiC_P and pure Al powders, and then were remelted in the matrix-metal melt with mechanical stirring and treated by ultrasonic vibration to prepare the composite. SEM analysis results show that the nano-SiC_P articles are distributed uniformly in the matrix and no serious agglomeration is observed. The tensile strength and elongation of the composite with 2 wt.% nano-SiC_P in as-cast state are 226 MPa and 5.5%, improved by 20% and 44%, respectively, compared with the A356 alloy.

Distribution pattern of acoustic and streaming field during multi-source ultrasonic melt treatment process

The ultrasonic melt treatment(UMT)is widely used in the fields of casting and metallurgy.However,there are certain drawbacks associated with the conventional process of single-source ultrasonic(SSU)treatment,such as the fast attenuation of energy and limited range of effectiveness.In this study,the propagation models of SSU and four-source ultrasonic(FSU)in Al melt were respectively established,and the distribution patterns of acoustic and streaming field during the ultrasonic treatment process were investigated by numerical simulation and physical experiments.The simulated results show that the effective cavitation zone is mainly located in a small spherical region surrounding the end of ultrasonic horn during the SSU treatment process.When the FSU is applied,the effective cavitation zone is obviously expanded in the melt.It increases at first and then decreases with increasing the vibration-source spacing(Lv)from 30 mm to 100 mm.Especially,when the Lv is 80 mm,the area of effective cavitation zone reaches the largest,indicating the best effect of cavitation.Moreover,the acoustic streaming level and flow pattern in the melt also change with the increase of Lv.When the Lv is 80 mm,both the average flow rate and maximum flow rate of the melt reach the highest,and the flow structure is more stable and uniform,with the typical morphological characteristics of angular vortex,thus significantly expanding the range of acoustic streaming.The accuracy of the simulation results was verified by physical experiments of glycerol aqueous solution and tracer particles.

Effects of heat treatment on microstructure evolution and mechanical properties of Mg-6Zn-1.4Y-0.6Zr alloy

To investigate the effects of solution temperature and the decomposition of I-phase on the microstructure, phase composition and mechanical properties of as-cast Mg-6Zn-1.4Y-0.6Zr alloy, solution treatment at 440 oC, 460 oC and 480 oC and further aging treatment were conducted on the alloy. The results indicate that the net-like intermetallic compounds(mainly I-phase) dissolve into the α-Mg matrix gradually with the increase of solution temperature from 440 oC to 480 oC. Besides, the I-phase decomposes completely at 480 oC, with the formation of fine W-phase(thermal stable phase) and Mg_7Zn_3 phase. In addition, a great number of fine and dispersive Mg-Zn binary phases precipitate in the α-Mg matrix during the aging treatment. Due to the increase of solute atoms and the precipitation of strengthening phases, such as W-phase and Mg-Zn phases, the optimal strength is obtained after solution treatment at 460 oC for 8 h and aged at 200 oC for 16 h. The yield strength(YS), ultimate tensile strength(UTS) and elongation are 208 MPa, 257 MPa and 3.8%, respectively. Compared with the as-cast alloy, the increments of YS and UTS are 117% and 58%, respectively, while the decrement of elongation is 46%.

nfluence of immune inflammation on androgen receptor expression in benign prostatic hyperplasia tissue

This study was designed to investigate the association between immune inflammation and androgen receptor （AR） expression in benign prostatic hyperplasia （BPH）. We retrospectively analyzed 105 prostatectomy specimens. An immune inflammation score for each specimen was defined by combining three immunohistochemical markers （CD4, CD8 and CD20）. The immunohistochemical markers were CD4 and CD8 for T lymphocytes, CD20 for B lymphocytes and AR antibody for the AR in BPH samples. Rates of CD4, CD8, CD20 and AR expression in BPH were 20 （19.0%）, 21 （20.0%）, 101 （96.2%） and 48 （45.7%）, respectively. Total prostate volume （TPV） was higher in the immune inflammation group than in the non-immune inflammation group （62.7 ml vs. 49.2 ml, t=-2.482, P〈0.05）. Patients in the immune inflammation group had a higher serum prostate-specific antigen （PSA） than those in the non-inflammation group （7.5 ng m1-1 vs. 5.4 ng m1-1, t=-2.771, P〈0.05）. Specifically, the immune inflammation group showed a higher rate of AR expression than the non-inflammation group （56.1% vs. 28.2%, χ2=7.665, P〈0.05）. Our study revealed a strong association between immune inflammation and TPV, serum PSA and AR expression in BPH tissue. Prostate hyperplasia caused by an immune inflammatory process may contribute to BPH progression over time. Therefore, the inflammatory response involved in BPH may be a prime therapeutic target.
