Dental?hemorrhagic?complications,?including?postoperative?bleeding?and?traumatic?hemorrhage?as?emergency?cases,?often?occur?in?patients?undergoing?oral?anticoagulant?therapy?such?as?warfarin?therapy.?Recent?research?recommends?that?warfarin?dosage?should?be?assessed?every?12?weeks.?Therefore,?most?physicians?generally?accept?international?normalized?ratio?(INR)?monitoring?at?longer?intervals.?However,?cases?are?encountered?in?which?the?INR?prolongation?is?observed?despite?of?invariable?dosage?of?warfarin.?In?this?report,?we?present?2?cases?of?patients?with?dental?hemorrhagic?complications?undergoing?oral?anticoagulant?therapy?who?exhibited?excessive?INR?prolongation.?These?patients?exhibited?decreased?appetite?and?hypoalbuminemia. We?speculate?that?long-term?appetite?loss?resulted?in?the?increase?in?the?serum?concentration?of?free?warfarin and?vitamin?K deficiency. Our?study?indicates that we?should?notice malnourishment?when?we?treat patients?who?have?undergone?warfarin?therapy with dental?surgical procedures.?It?is?recommended?that measurement?of?INR just before?a?dental?surgical?treatment.

Objective: This multicenter clinical study was to assess the clinical usability of an oral moisture-checking device in detecting the dry mouth patients and evaluating the optimal measurement site. Materials and Methods: The study group comprised 250 patients with dry mouth and 241 healthy volunteer subjects at 13 medical centers. This device was used to measure the moisture degrees of the lingual mucosa and the buccal mucosa. Subjective oral dryness, objective oral dryness, and saliva flow rates were also compared between the two groups. For statistical analysis, receiver-operating characteristic analysis was performed to calculate the area under the curve (AUC). Results: The moisture degree of the lingual mucosa was significantly lower in the dry mouth group (27.2 ± 4.9) than that in the healthy group (29.5 ± 3.1, AUC = 0.653). When a lingual mucosa moisture degree of 31.0 or higher was defined as normal, less than 27.0 as dry mouth, and 27.0 to less than 31.0 as borderline zone of dry mouth, both the sensitivity and the specificity for the diagnosis of dry mouth were close to 80%. Conclusion: These results suggest that the oral moisture-checking device is a usable screening device for dry mouth.

Distributed erosion models, which simulate the physical processes of water flow and soil erosion, are effective for predicting soil erosion in forested catchments. Although subsurface flow through multiple pathways is dominant for runoff generation in forested headwater catchments, the process-based erosion model, Geo-spatial interface for Water Erosion Prediction Project(Geo WEPP), does not have an adequate subsurface component for the simulation of hillslope water flow. In the current study, the quick flow related to soil erosion during rainfall events is separated from the total hydrograph using base flow separation for Geo WEPP to use direct runoff to predict sediment discharge from a granitic-forested catchment in Japan. Water runoff and sediment discharge also are simulated using the conventional method that reproduces the total hydrograph, and the accuracy of the calculation and estimated erodibility parameters are evaluated. The proposed procedure reproduced the quick-flow runoff during rainfall and the cumulative sediment discharge from the catchment. Simultaneously, the proposed procedure overestimates water runoff during extreme weather events, such as typhoons. The estimated erodibility parameters for the hillslope soil were comparable to those of the surface soil in Japan using both the proposed procedure and the conventional one. Conversely, the channel erodibility obtained from the proposed procedure matched the observed sediment characteristics in the granitic forested catchment. The proposed procedure is a promising method to predict sediment discharge from forested catchments based on the geological features of the catchment.

Phosphatidylinositol polyphosphates (PIPx) are related with tyrosine kinase activation, cell proliferation and carcinogenesis. In order to investigate the action mechanism of PIPx, it is desirable to synthesize affinity column of PI-3,4-P-2, which is expected to be able to isolate the binding proteins of PI-3,4-P-2. Thyramine reacted with CH-Sepharose 4B giving column 13. The p-amino group of 3'-(1',2'-distearoyl-glyceryl)-1-(2-p-aminobenzyl)-3,4-di-O-phosphoryl-myo-inosityl phosphate (12) was diazotized, then diazo-coupled with column 13 to give PI-3,4-P-2 affinity column 14. This PI-3,4-P-2 affinity column is an effective tool to pick up binding proteins of PI-3,4-P-2.

AIM： TO introduce a new method： small endoscopic sphincterotomy （ES） combined with endoscopic papillary large balloon dilation （SES ＋ EPLBD） to treat patients with large biliary stones.METHODS： Retrieval of large biliary stones was performed in 88 patients. Mean stone size was 14 ± 3 mm and mean number of stones was 2.5 ± 3.5. Firstly, ES with a small incision was performed. Next, endoscopic papillary dilation was performed with a large balloon to slowly match the size of the bile duct. Stones were then retrieved from the biliary duct with a balloon and a basket.RESULTS： Stone retrieval was successful in all cases except one cystic duct stone case without the need to crush large stones. Mean procedure time was 30 ± 5 min. Dilating the papillary orifice with a large balloon made it possible to remove large stones smoothly without crushing them. After dilation with the large balloon, there were some instances of oozing, but no perforations. One instance of post-procedural pancreatitis （1%） occurred. CONCLUSION： SES ＋ EPLBD was effective for the retrieval of large biliary stones without the use of mechanical lithotripsy.

The El Nino-Southern Oscillation(ENSO)has great impacts on the Indian Ocean sea surface temperature(SST).In fact,two major modes of the Indian Ocean SST namely the Indian Ocean Basin(IOB)and the Indian Ocean Dipole(IOD)modes,exerting strong influences on the Indian Ocean rim countries,are both influenced by the *** on a combined linear regression method,this study quantifies the ENSO impacts on the IOB and the IOD during ENSO concurrent,developing,and decaying *** removing the ENSO impacts,the spring peak of the IOB disappears along with significant decrease in number of events,while the number of events is only slightly reduced and the autumn peak remains for the *** isolating the ENSO impacts during each stage,this study reveals that the leading impacts of ENSO contribute to the IOD development,while the delayed impacts facilitate the IOD phase switch and prompt the IOB ***,the decadal variations of ENSO impacts are various during each stage and over different *** imply that merely removing the concurrent ENSO impacts would not be sufficient to investigate intrinsic climate variability of the Indian Ocean,and the present method may be useful to study climate variabilities independent of ENSO.

Pancreatic neuroendocrine tumors(p NETs) are particularly rare. The various forms of PNETs, such as cystic degeneration, make differentiation from other similar pancreatic lesions difficult. We can detect small lesions by endoscopic ultrasound(EUS) and obtain preoperative pathological diagnosis by EUS-guided fine needle aspiration(FNA). We describe, here, an interesting case of p NET in a 42-year-old woman with no family history. Computed tomography and magnetic resonance imaging revealed an 18 mm × 17 mm cystic lesion with a nodule in the pancreatic tail. Two microtumors about 7 mm in diameter in the pancreatic body detected only by EUS, cystic rim and nodules all showed similar enhancement on contrast-harmonic EUS. Preoperative EUS-FNA of the microtumor was performed, diagnosing multiple p NETs. Macroscopic examination of the resected pancreatic body and tail showed that the cystic lesion had morphologically changed to a 13-mm main nodule, and 11 new microtumors(diameter 1-3 mm). Microscopically, all microtumors represented p NETs. From the findings of a broken peripheral rim on the main lesion with fibrosis, rupture of the cystic p NET was suspected. Postoperatively, pituitary adenoma and parathyroid adenoma were detected. The final diagnosis was multiple grade 1 p NETs with multiple endocrine neoplasia type 1. To the best of our knowledge, no case of spontaneous rupture of a cystic p NET has previously been reported in the English literature. Therefore, this case of very rare p NET with various morphological changes is reported.

We studied the effects of reductive annealing on insulating polycrystalline thin films of anatase Nb- doped TiO2 （TNO）. The insulating TNO films were intentionally fabricated by annealing conductive TNO films in oxygen ambient at 400 ℃. Reduced free carrier absorption in the insulating TNO films indicated carrier com- pensation due to excess oxygen. With H2-annealing, both carrier density and Hall mobility recovered to the level of conducting TNO, demonstrating that the excess oxygen can be efficiently removed by the annealing process without introducing additional scattering centers.
