Prevalence of left ventricular dyssynchrony in patients with heart failure assessed by a novel programmer-cardioGRAF

Objectives Left ventricular systolic dyssynchrony is the most important determinant of response to cardiac resynchronization therapy （CRT）, playing a vital role to predict improvement of systolic function or LV reverse remodeling. CardioGRAF is a novel programmer based on the ECG gated single photon emission computed tomography （G-SPECT） imaging to detect LV systolic and diastolic dyssynchrony simultaneously. This study was to investigate the prevalence of systolic and diastolic left ventricular （LV） dyssynchrony in patients with heart failure. Methods We retrospectively studied 69 patients with heart disease, including 31 patients who had symptoms of heart failure （NYHA class Ⅱ-Ⅲ）, and 38 patients who had no symptoms of heart failure. （NYHA class Ⅰ）. G- SPECT data were analyzed by cardiaGRAF, and measurements included the time to end systole （TES）, the time to peak ejection （TPE）, the time to peak filling （TPF）, TES＋TPF and maximal difference （MD） of each parameters were obtained, using the 95th percentile of the control group as a cutoffof 150 ms for MD-TES, 139 ms for MD-TPE, 345 ms for MD-TPF and 315 ms for MD-TES＋TPF. Results The prevalence of LV systolic dyssynchrony was significantly higher in heart failure patients with reduced LV ejection fraction （LVEF）〈45% （72% for MD-TES; 64% for MD-TPE） compared with heart failure patients with preserved LVEF=45% （14% for both MD-TES and MD-TPE; P=0.002, P=0.005, respectively）; The prevalence of MD-TES〈150 ms was higher in NYHA class Ⅲ patients （64%） compared with NYHA class Ilpatients （27%, P=0.049）. However, the prevalence of the LV diastolic dyssynchrony were high but not difference between NYHA class III（47% for both MD-TPF and MD-TES＋TPF） and class Ⅲ（63% for MD-TPF; 69% for MD-TES＋TPF; P=NS） patients as well as between patients with preserved LVEF （43% for both MD-TPF and MD-TES＋TPF） and patients with reduced LVEF（64% for MD-TPF; 72% for MD-TES＋TPF; P=NS）. Expression of AID, P53, and Mlh1 proteins in endoscopically resected differentiated-type early gastric cancer

AIM: To analyze the expression of the tumor-related proteins in differentiated-type early gastric carcinoma (DEGC) samples. METHODS: Tumor specimens were obtained from 102 patients (75 males and 27 females) who had received an endoscopic tumor resection at Tottori University Hospital between 2007 and 2009. Ninety-one cancer samples corresponded to noninvasive or intramucosal carcinoma according to the Vienna classification system, and 11 samples were submucosal invasive carcinomas. All of the EGCs were histologically differentiated carcinomas. All patients were classified as having Helicobacter pylori (H. pylori) infections by endoscopic atrophic changes or by testing seropositive for H. pylori IgG. All of the samples were histopathologically classified as either tubular or papillary adenocarcinoma according to their structure. The immunohistochemical staining was performed in a blinded manner with respect to the clinical information. Two independent observers evaluated protein expression. All data were statistically analyzed then. RESULTS: The rates of aberrant activation-induced cytidine deaminase (AID) expression and P53 overexpression were both 34.3% in DEGCs. The expression of Mlh1 was lost in 18.6% of DEGCs. Aberrant AID expression was not significantly associated with P53 overexpression in DEGCs. However, AID expression was associated with the severity of mononuclear cell activity in the non-cancerous mucosa adjacent to the tumor (P = 0.064). The rate of P53 expression was significantly greater in flat or depressed tumors than in elevated tumors. The frequency of Mlh1 loss was significantly increased in distal tumors, elevated gross-type tumors, papillary histological-type tumors, and tumors with a severe degree of endoscopic atrophic gastritis (P < 0.05). CONCLUSION: Aberrant AID expression, P53 overexpression, and the loss of Mlh1 were all associated with clinicopathological features and gastric mucosal alterations in DEGCs. ＜正＞Moving aspects of structures are taken up. In our daily structural design the structures are assumed to be immovable, and most of structural calculations are carried out on the basis of static principles. There are cases, however, where large movements are actually experienced by our structures due to different reasons. Many of them are due to excessive loading and unexpected instability, often leading to collapse of the structures. Some other cases are related to vibration where resonance of structures with external agencies such as earthquakes and wind is discussed. Self-excited oscillation sometimes produces catastrophic and spectacular motion of structures. Since the task of structural design is to realize safe and strong structures, we should be aware of such motions and the means to prevent or control them. Controlled motions can be obtained by adopting isolators to cope with the effects of earthquakes. Structures can be designed to be assembled on the ground and then hoisted to the position. In erection of such structures a big movement is observed as in Pantadome System. Finally those structures whose intended functions are movements are described with examples of rocking stones and a flying carp.

Formation of Gel-Like Shear-Induced Structure with Dosing of Dilute Surfactant Solution and Its Effect on Turbulent Channel Flow

A shear-induced structure (SIS) is formed under appropriate concentration and shear conditions in a surfactant micellar solution. In this study, we performed experiments of surfactant solution dosing in a fully developed two-dimensional turbulent channel flow from a sintered metallic wire mesh plate attached to a side wall. We investigated the behavior of the solution under the elongation during its passing through the wire mesh and under the strong shear due to the channel flow. It was confirmed that the dosed solution containing a laser dye was visualized by a laser sheet, and the accumulated gel from the wire mesh formed a layer and developed with time. Consequently, on dosing the dilute surfactant solution from the wire mesh, a gel-like SIS layer was formed, which majorly covered the wire mesh plate. The gel-like SIS layer on the wire mesh plate acted as a sticky solid and restricted the flow in the channel. This layer continued to grow while dosing, owing to which the pressure drop of the channel flow significantly increased. The gel-like SIS layer grew rapidly even in the turbulent flow and reached the equilibrium thickness. After the termination of the dosing, the gel layer collapsed gradually. In addition, the thickness of the gel-like SIS layer (indicating the strength indirectly) strongly depended on the surfactant concentration and the elongation rate in the wire mesh.

Effective Medical Creation (EMC) <br/>—A New Approach to Improvement of Patient Management in the Standpoint of Hospital Room Environment

Effective Medical Creation (EMC) is a kind of campaign to implement a team healthcare that can provide the most efficient and effective intensive care units (ICUs) by improving hospital room environment for not only patients and their family but also healthcare providers. This campaign was based on the concept of the “Art of Medicine”, which provides everyone a comfortable ICU environment of meditation and feeling with the five senses of sight, hearing, touch, smell, and taste. EMC can provide a healthcare environment that is not restricted by existing and traditional verse rules or style and aims to always provide better healthcare by working on the five senses. Provision of facilities for aromatherapy massage at an ornamental hospital room and landscape through a false window has been the ongoing activity of the EMC at Nara Medical University. These seemed effective in reducing a patient’s stress response in the ICU. However, the effect of EMC on the outcomes of critically ill patients has not been determined. EMC might be one of the promising measures to create environments that positively affect both patients and healthcare providers. In this review article, the concept of EMC and practice of EMC at Nara Medical University Hospital are presented.

Investigation of Inspiratory Circuit Condensation due to Room Temperature in a Heated Humidifier during Mechanical Ventilation

Heating and humidification control during mechanical ventilation is important. Water condensation can occur due to insufficient humidification. Condensation in the breathing circuit can cause various adverse events. In this study, we investigated whether the occurrence of tubing condensation in the breathing circuit could be affected by room temperature using a simulated respiratory circuit with a humidifier. The room temperature was set at 23°C, 25°C, and 27°C, and mechanical ventilation was performed for 8 h. The inspired gas was appropriately heated and humidified according to the manufacturer’s instructions. The weight of the circuit was measured every 2 h at room temperature to estimate the amount of condensation. During the mechanical ventilation, condensation continued to increase at a room temperature of 23°C, but only for the first 2 h at 25°C, and did not occur at 27°C. The room temperature of the indoor environment was one of the factors generating water condensation in the breathing circuit with a humidifier even when a heater wire in the breathing circuit was appropriately operated.

Study on the Shear Effect on Dye Patches Diffused in Wall-Bounded Shear Flow

This paper focuses on the high intensity filaments (dye patches) embedded in dye plumes in a wall-bounded shear flow, to investigate the shear effect on the dye patch distribution. Motivated by the widely concerned inverse estimation of the source location, we try extracting useful information to know the source location from down-stream dye patches. Accordingly, we changed the dye injection location at different distances from the wall and made observations at different downstream (diffusion) distances from the source. The orientation angle and roundness of dye patches were concerned to examine the shear effect and dye patch characteristics. To capture the dye plume images, a planar laser induced fluorescence (PLIF) technique was used. The orientation and roundness of each dye patch were calculated by least-square fitting. The statistics of both the orientation angle and the roundness were compared with those in homogeneous turbulent cases to reveal the shear effect. Different from uniformly-orientated dye patches in the homogeneous flow, larger occurrence probabilities with positive orientation angles of dye patches are observed in wall-bounded shear flow, in particular, when the injection location is near the wall. As with information extraction for the inverse estimation of source location, it is found that the orientation distribution of dye patches is independent of the diffusion distance, but related with the injection location from the wall. While for the homogeneous flow cases, a strong dependence on the diffusion distance is observed in the orientation distribution profiles. As for the roundness, similar aspects are found regarding the dependencies on the injection location in shear flow and on diffusion distance in homogeneous flow.

Effect of False Windows on Light Exposure and Sleep Quality in Hospitalized Patients

Purpose: We aimed to investigate the effects of installing false windows next to hospital beds without windows on the amount of light received by patients and their sleep quality. Methods: The study included patients admitted to the Department of Neurology at our hospital between September 2020 and August 2021. An Actigraph device was fitted to patients’ wrist and their beds to measure the amount of light received and sleep quality. Patients were divided into three groups: bed with a window, aisle bed with a false window, and aisle bed without a window. Mean sleep efficiency (%), mean steps (per day), and the amount of light (lux) received by the patients and beds were measured. Results: Valid data were obtained for 48 participants (median age, 66.5 years). There were 23 patients in beds with a window, 13 patients in aisle beds without a false window, and 12 in aisle beds with a false window. No statistically significant differences were found in terms of mean sleep efficiency, number of steps taken, and the amount of light received by the patients (P > 0.05);however, difference in the mean amount of light received by the beds at the location of the bed was statistically significant (P Conclusion: The amount of light that the patient receives is not necessarily affected by the location of the bed or the presence of a false window.
