Community Pharmacist-Based Collaborative Disease Management Program for Patients with Poorly Controlled Diabetes

Objective： Patients with poorly controlled diabetes have more medical complications and are more difficult to manage. The objective of the present study was to evaluate the clinical outcomes of successful implementation of an employer initiated community pharmacist-based disease management program for diabetic patients with poorly controlled diabetes. Methods： Employees with poorly controlled diabetes （glycosylated hemoglobin （A1 C） level 〉 7.5%） were identified fi＇om a large diabetes disease management program, in a rural setting in Texas, US. A longitudinal retrospective study was conducted, analyzing clinical indicators in the diabetes patients following the community pharmacist-based disease management program. The program involved a comprehensive drug therapy assessment and individualized disease management education. Primary outcome measured in the present study was A1C levels, assessed at the baseline visit and at the end of the intervention. Results： A total of 64 patients with poorly controlled diabetes were identified. Significant improvement in mean clinical outcome scores was achieved for A1C levels （p = 0.0011）. At the end of the 1 year longitudinal intervention, targeted body mass index and A1C goals were attained by 35.9% （p 〈 0.001） and 15.6% patients, respectively. The 10 patients reaching goal levels post intervention were in the group that had baseline A1C of 7.5 to 9%. However, patients with 〉 9% A1C levels at baseline had a significant reduction （mean 2.1, p 〈 0.001） post intervention. Conclusion： The community pharmacist-based diabetes disease management program improved A1C levels of patients with poorly controlled diabetes.

Fusion of spherical-octupole pairs of colliding nuclei for compact and elongated configurations

The deformation and associated optimum/uniquely fixed orientations play an important role in the syn-thesis of compound nuclei via cold and hot fusion reactions,respectively,at the lowest and highest barrier *** choice of optimum orientation(0_(op))for the'cold or elongated'and hot or cormpact'fusion configurations of quadrupole(β_(2))deformed nuclei depends only on the+/-signs ofβ_(2)-deformation[***.G:***.31,631-644(2005)].In our recent study[***.C 101,051601(R)2020],we proposed a new sct of Oopt(iferent from the values reported for quadrupole deformed nuclei)after the inclusion of octupole deformation(up to B3)*** the respective 0op1 of B3-deformed nuclei for cold and hot optimum orientations,we analyzed the im-pact of the soft-and rigid-pear shapes of octupole deformed nuclei on the fusion barrier characterstics(barrier height Vg and barrier position RB).This analysis is applied to approximately 200 spherical-plus B3 deformed nucle-ar partners,that is,^(16)O,^(18)Cat octupole deformed *** with the compact configuration,the elongated fu-sion configuration has a relatively larger impact on the fusion barrier and cross sections ow ing to the inclusion of de-formations up to *** agreement with available experimental data for the^(16)O+^(150)Sm reaction(β_(22)-0.205,β_(32)=0.055)also improves when the optimum orientation degree of freedom is fixed in view of octupole *** reinforces the fact that nuclear structure effects play an important role in the nuclear fusion ***,octu-pole deformed nuclei can be used for the synthesis of heavy and superheavy nuclei.

Sensitivity of Labile Soil Organic Carbon Pools to Long-Term Fertilizer, Straw and Manure Management in Rice-Wheat System

Labile soil organic carbon （SOC） pools, estimated through chemical fractionation techniques, are considered sensitive indicators of management-induced changes in quality and composition of soil organic matter. Although the impacts of organic manure and crop residue applications on C sequestration in rice-wheat system are fairly well documented, their influence on labile SOC pools is relatively less k.own. Impacts of organic manure, rice straw, and inorganic fertilizer nitrogen （N） applications on soil total organic carbon （TOC） and SOC pools including water-extractable organic C （WEOC）, hot water-soluble organic C （HWOC）, potassium permanganate- oxidizable organic C （k.nO4-C）, microbial biomass C （MBC）, mineralizable organic C （Cmin）, and the oxidizable fractions of decreasing oxidizability （easily-oxidizable, oxidizable, and weak.y-oxidizable） were investigated in an ll-year field experiment under rice-wheat system. The field experiment included treatments of different combinations of farmyard manure, rice straw, and fertilizer N application rates, with C inputs estimated to be in the range from 23 to 127 Mg ha-1. After 11 years of experiment, WEOC, HWOC, and k.nO4-C were 0.32%-0.50%, 2.2%-3.3%, and 15.0%-20.6% of TOC, respectively. The easily-oxidizable, oxidizable, and weak.y-oxidizable fractions were 43%-57%0, 22%-27%, and 10%-19% of TOC, respectively. The applications of farmyard manure and rice straw improved WEOC, HWOC, k.nO4-C, easily-oxidizable fraction, Cmin, and MBC, though the rates of change varied considerably from -14% to 145% and -1170 to 83% of TOC, respectively. At the C input levels between 29 and 78 Mg C ha-1 during the ll-year period, the greatest increase was observed in WEOC and the minimum in k.nO4-C. Water-extractable organic C exhibited a relatively greater sensitivity to management than TOC, suggesting that it may be used as a sensitive indicator of management-induced changes in soil organic matter under rice-wheat system. Vesicular nanocarrier based treatment of sk.n fungal infections: Potential and emerging trends in nanoscale pharmacotherapy

Occurrence of sk.n fungal infections is increasing nowadays and their presence is more prominent in patients suffering from immunocompromised diseases lik. AIDS. Sk.n fungal infections are a major cause of visits by patients to dermatology clinics. Although, a large number of antifungal agents are available for treatment of sk.n fungal infections, but, their toxic profile and physicochemical characteristics reduce therapeutic outcome. When these antifungal agents are delivered topically using conventional formulations lik. creams and gels, they may cause various side effects lik. redness, burning, and swelling at the site of application. Therefore, various vesicular formulations(phospholipid based or non phospholipid based) have been explored by pharmaceutical scientists to treat sk.n fungal infections topically. Vesicular formulation explored for the purpose are liposomes, ethosomes, transfersomes, transethosomes, niosomes, spanlastics, oleic acid vesicles, and *** formulations show various advantages lik. bioavailability enhancement of bioactives,high sk.n permeation power, no side effects at application site, dosing frequency reduction,and sustained drug release. Therefore, in the present article, we have discussed about the utility of various vesicular nanocarrier systems to treat sk.n fungal infections.

Novel targeting approaches and signaling pathways of colorectal cancer: An insight

Colorectal cancer(CRC) is the third most common cancer of mortality in the world. Chemotherapy based treatment leads to innumerable side effects as it delivers the anticancer drug to both normal cells besides cancer cells. Sonic Hedgehog(SHH), Wnt wingless-type mouse mammary tumor virus/β-catenin, transforming growth factor-β/SMAD, epidermal growth factor receptor and Notch are the main signaling pathways involved in the progression of CRC. Targeted therapies necessitate information regarding the particular aberrant pathways. Advancements in gene therapies have resulted in the recognition of novel therapeutic targets related with these signal-transduction cascades. CRC is a stepwise process where mutations occur over the time and activation of oncogenes and deactivation of tissue suppressor genes tak.s place. Genetic changes which are responsible for the induction of carcinogenesis include loss of heterozygosity in tumor suppressor genes such as adenomatous polyposis coli, mutation or deletion of genes lik. p53 and k.ras. Therefore, many gene-therapy approaches lik. gene correction, virusdirected enzyme-prodrug therapy, immunogenetic manipulation and virotherapy are currently being explored. Development of novel strategies for the safe and effective delivery of drugs to the cancerous site is the need of the hour. This editorial accentuates different novel strategies with emphasis on gene therapy and immunotherapy for the management of CRC.

Effect of recombinant and native buffalo OVGP1 on sperm functions and in vitro embryo development: a comparative study

Back.round： An oviduct-specific glycoprotein, OVGP1, is synthesized and secreted by non-ciliated epithelial cells of the mammalian oviduct which provides an essential milieu for reproductive functions. The present study reports the effects of recombinant buffalo OVGP1 that lack. post-translational modifications, and native Buffalo OVGP1 isolated from oviductal tissue, on frozen-thawed sperm functions and in vitro embryo ***： The proportion of viable sperms was greater（P 〈 0.05） in the recombinant OVGP1-treated group compared to the native OVGP1-treated group at 2 h, 3 h, and 4 h of incubation. The proportion of motile sperms at3 h and 4 h of incubation; and membrane-intact sperms at 4 h was greater（P 〈 0.05） in the native OVGP1-treated group compared to the control and recombinant OVGP1-treated groups. The proportion of capacitated and acrosome-reacted sperms was greater（P 〈 0.05） in the native OVGP1-treated group compared to the recombinant OVGP1 group at 4 h. The rates of cleavage of embryos and their development to the blastocyst stage were greater（P 〈 0.05） in the presence of either native or recombinant OVGP1 in comparison to control at 10 μg/mL concentration as compared to 5 or 20 μg/***： The study suggests that both native and recombinant OVGP1 impart a positive effect on various sperm features and in vitro embryo development. However, native OVGP1 was found to have a more pronounced effect in comparison to recombinant non-glycosylated OVGP1 on various sperm functions except viability. Hence,our current findings infer that glycosylation of OVGP1 might be essential in sustaining the sperm functions but not the in vitro embryo development.

The Spontang Ophiolite complex represents the most complete ophiolite sequence amongst the South Ladak. ophiolites and comprises mantle rock.(depleted harzburgites,dunites and minor lherzolites)as well as crustal rock.(basalt,isotropic gabbros,layered gabbros etc.).In the present study,detailed geochemistry(whole rock.as well as mineral chemistry)and Sr-Nd isotopic analyses of thirty-six ultramaficmafic samples have been attempted to constraint the evolution and petrogenetic history of the Tethyan oceanic ***,trace-element and REE patterns of the peridotites and their minerals indicate that the lherzolites experienced lower degrees of partial melting resembling abyssal peridotites(at higher temperatures,TREE=$1216℃)than the harzburgites(6%–8%versus 15%–17%).Elevated eNd(t)and variable^(87) Sr/^(86) Sr(t)ratios along with REE patterns suggest that the Spontang mafic rock. display N-MORB affinity with negligible participation of oceanic sediments in their genesis are originated from a depleted upper mantle with little contribution from subduction-related ***-type Neotethyan oceanic crust is associated with the earliest phase of subduction(of older Jurassic age)through which a younger intra-oceanic island arc(Spong arc)subsequently *** REE display typical U-shaped patterns,suggesting that these rock. have been metasomatized by LREE-enriched *** the other side,mafic rock. are characterized by heterogeneous(Nb/La)PMand(Hf/Sm)PMand relatively homogeneous eNd(t),indicating interaction of subduction-related melts with the upper mantle during the initiation of subduction,in Early Cretaceous times.

AIM: To study the mechanism of antimigraine activity of Tanacetum parthenium ( Feverfew), its extracts and parthenolide, a component of Feverfew, by observing their effect on 5-HT storage and release, and stimulation of 5-HT2B and 5-HT2A receptors. Also to standardize a dosage form of Feverfew with respect to its parthenolide content. METHODS: Isometric responses to 5-HT and an indirect acting serotonergic, d-fenfluramine, were obtained on rat fundus and ileum. In one set of experiments the effect of dichloromethane extract of Feverfew and parthenolide was observed on the above. The extract was then thermally degraded upto 10 %, 23 %, and 33 % with respect to its parthenolide content by k.eping at 60℃ and 75 % relative humidity and the experiments were repeated. In another set of experiments rats were fed with 20 mg/k.
