Atrial fibrillation and coronary artery disease:An integrative review focusing on therapeutic implications of this relationship

The incidence of both atrial fibrillation(AF)and coronary artery disease(CAD)increases with advancing age.They share common risk factors and very often coexist.Evidence points to an intricate relationship between atrial tissue excitability and neuronal remodeling with ischemia at the microcirculatory level.In this review,we delineated this complex relationship,identified a common theme between the two,and discussed how the knowledge of this relationship translates into a positive and meaningful impact in patient management.Recent research indicates a high prevalence of CAD among AF patients undergoing coronary angiography.Further,the incidence of AF is much higher in those suffering from CAD compared to age-matched adults without CAD underlying this reciprocal relationship.CAD adversely affects AF by promoting progression via re-entry and increasing excitability of atrial tissue as a result of ischemia and electrical inhomogeneity.AF in turn accelerates atherosclerosis via endothelial dysfunctional and inflammation and together with enhanced thrombogenicity and hypercoagulability contribute to micro and macrothrombi throughout cardiovascular system.In a nutshell,the two form a vicious cycle wherein one disease promotes the other.Most AF recommendations focuses on rate/rhythm control and prevention of thromboembolism.Very few studies have discussed the importance of unmasking coexistent CAD and how the treatment of underlying ischemia will impact the burden of AF in these patients.Inflammation and endothelial dysfunction remain central to both disease processes and form a handsome therapeutic target in the management of the two diseases.The relationship between AF and CAD is complex and much more than mere coincidence.The two diseases share common risk factor and pathophysiology.Hence,it is impractical to treat them in isolation.Accordingly,we share the implications of managing underlying ischemia and inflammation to positively impact and improve quality of life among AF patients.

Neglected scrub typhus:An updated review with a focus on omics technologies

Scrub typhus is a neglected disease and one of the most serious health problems in the Asia-Pacific region.The disease is caused by an obligate intracellular bacteria Orientia tsutsugamushi,which is transmitted by chigger bites or larval mite bites.Scrub typhus is a threat to billions of people worldwide causing different health complications and acute encephalitis in infants and growing children.The disease causes multiple organ failure and mortality rates may reach up to 70%due to a lack of appropriate healthcare.Currently available genome and proteome databases,and bioinformatics methods are valuable tools to develop novel therapeutics to curb the pathogen.This review discusses the state-of-the-art of information about Orientia tsutsugamushi-mediated scrub typhus and delineates the role of omics technologies to develop drugs against the pathogen.The role of proteome-wide in silico approaches for the identification of therapeutic targets is also highlighted.

Curvelet Transform Based on Edge Preserving Filter for Retinal Blood Vessel Segmentation

Segmentation of vessel in retinal fundus images is a primary step for the clinical identification for specific eye diseases.Effective diagnosis of vascular pathologies from angiographic images is thus a vital aspect and generally depends on segmentation of vascular structure.Although various approaches for retinal vessel segmentation are extensively utilized,however,the responses are lower at vessel’s edges.The curvelet transform signifies edges better than wavelets,and hence convenient for multiscale edge enhancement.The bilateral filter is a nonlinear filter that is capable of providing effective smoothing while preserving strong edges.Fast bilateral filter is an advanced version of bilateral filter that regulates the contrast while preserving the edges.Therefore,in this paper a fusion algorithm is recommended by fusing fast bilateral filter that can effectively preserve the edge details and curvelet transform that has better capability to detect the edge direction feature and better investigation and tracking of significant characteristics of the image.Afterwards C mean thresholding is used for the extraction of vessel.The recommended fusion approach is assessed on DRIVE dataset.Experimental results illustrate that the fusion algorithm preserved the advantages of the both and provides better result.The results demonstrate that the recommended method outperforms the traditional approaches.

Emerging sub-synchronous interactions(SSI)in wind-integrated power systems have added intense attention after numerous incidents in the US and China due to the involvement of series compensated transmission lines and power electronics devices.SSI phenomenon occurs when two power system elements exchange energy below the synchro-nous frequency.SSI phenomenon related to wind power plants is one of the most significant challenges to main-taining stability,while SSI phenomenon in practical wind farms,which has been observed recently,has not yet been described on the source of conventional SSI literature.This paper first explains the traditional development of SSI and its classification as given by the IEEE,and then it proposes a classification of SSI according to the current research status,reviews several mitigation techniques and challenges,and discusses analysis techniques for SSI.The paper also describes the effect of the active damping controllers,control scheme parameters,degree of series compensation,and various techniques used in wind power plants(WPPs).In particular,a supplementary damping controller with converter controllers in Doubly Fed Induction Generator based WPPs is briefly pronounced.This paper provides a real-istic viewpoint and a potential outlook for the readers to properly deal with SSI and its mitigation techniques,which can help power engineers for the planning,economical operation,and future expansion of sustainable development.

Revisiting the role of mesenchymal stem cells in tuberculosis and other infectious diseases

Mesenchymal stem cells(MsCs)play diverse roles ranging from regeneration and wound healing to immune signaling.Recent investigations have indicated the crucial role of these multipotent stem cells in regulating various aspects of the immune system.MsCs express unique signaling molecules and secrete various soluble factors that play critical roles in modulating and shaping immune responses,and in some other cases,MSCs can also exert direct antimicrobial effects,thereby helping with the eradication of invading organisms.Recently,it has been demonstrated that MSCs are recruited at the periphery of the granuloma containing Mycobacterium tuberculosis and exert"Janus"-like functions by harboring pathogens and mediating host protective immune responses.This leads to the establishment of a dynamic balance between the host and the pathogen.MsCs function through various immunomodulatory factors such as nitric oxide(NO),IDO,and immunosuppressive cytokines.Recently,our group has shown that M.tb uses MSCs as a niche to evade host protective immune surveillance mechanisms and establish dormancy.MSCs also express a large number of ABC efflux pumps;therefore,dormant M.tb residing in MSCs are exposed to a suboptimal dose of drugs.Therefore,it is highly likely that drug resistance is coupled with dormancy and originates within MSCs.In this review,we discussed various immunomodulatory properties of MsCs,their interactions with important immune cells,and soluble factors.We also discussed the possible roles of MsCs in the outcome of multiple infections and in shaping the immune system,which may provide insight into therapeutic approaches using these cells in different infection models.

Solution for generalized fuzzy fractional Kortewege-de Varies equation using a robust fuzzy double parametric approach

The nonlinear Kortewege-de Varies(KdV)equation is a functional description for modelling ion-acoustic waves in plasma,long internal waves in a density-stratified ocean,shallow-water waves and acoustic waves on a crystal lattice.This paper focuses on developing and analysing a resilient double parametric analytical approach for the nonlinear fuzzy fractional KdV equation(FFKdVE)under gH-differentiability of Caputo fractional order,namely the q-Homotopy analysis method with the Shehu transform(q-HASTM).A triangular fuzzy number describes the Caputo fractional derivative of orderα,0<α≤1,for modelling problem.The fuzzy velocity profiles with crisp and fuzzy conditions at different spatial positions are in-vestigated using a robust double parametric form-based q-HASTM with its convergence analysis.The ob-tained results are compared with existing works in the literature to confirm the efficacy and effectiveness of the method.

Recent advancements in perovskites’ application as a solar energy harvester have been astonishing. The power conversion efficiency(PCE) of perovskite solar cells(PSCs) is currently reaching parity(>25 percent), an accomplishment attained over past decades. PSCs are seen as perovskites sandwiched between an electron transporting material(ETM) and a hole transporting material(HTM). As a primary component of PSCs, HTM has been shown to have a considerable effect on solar energy harvesting, carrier extraction and transport, crystallization of perovskite, stability, and price. In PSCs, it is still necessary to use a HTM.While perovskites are capable of conducting holes, they are present in trace amounts, necessitating the use of an HTM layer for efficient charge extraction. In this review, we provide an understanding of the significant forms of HTM accessible(inorganic, polymeric and small molecule-based HTMs), to motivate further research and development of such materials. The identification of additional criteria suggests a significant challenge to high stability and affordability in PSC.

Magnesium is one of the largely available elements in the earth’s crust. It has a low structural density with high specific strength. This unique material property has forced an increase in the use of magnesium and its alloys in various applications pertaining to industrial sector,automobiles, aerospace and biomedical. Since magnesium is a highly reactive metal, it is prone to higher rate of corrosion as compared to its counterparts. Thus, it is essential to analyze the corrosion behavior of magnesium and its alloys in its applications. An appropriate process is to be followed in the design and development of magnesium alloys which overcome the limitations of magnesium and enhance the desired material properties in accordance to their applications. This review paper summarizes the importance of magnesium and its material properties. The influence of various alloying elements on the mechanical properties of magnesium is reviewed. The broad classification of Mg alloys and their behavioral trends are detailed. The corrosion behavior of magnesium and the influence of corrosion products on the material characteristics of magnesium, in aqueous medium, are discussed. The manufacturing techniques of magnesium alloys along with the secondary techniques are also covered. The various applications and the limitations of magnesium in these applications are covered. A complete section is dedicated towards detailing the recent trends of magnesium(Mg) alloys, i.e., the biodegradable nature and applications of Mg alloys. The influence of biocorrosion on Mg alloys and techniques to overcome it have been deliberated. This paper provides a thorough review on recent developments of magnesium with respect to engineering applications.

Studying the efficacy of isolation as a control strategy and elimination of tuberculosis in India:A mathematical model

India has the highest burden of both tuberculosis(TB)and multidrug-resistant TB(MDRTB)based on the WHO Global TB Report 2019.Although the available data suggest that the total TB incidence has declined,the absolute number of new cases is still increasing.The number of reported TB cases in India in 2018 was 2.2 million,which was 1.5 million in 2009.About 47%increment in TB case notification in India within a decade shows a persistent public health problem.India contributes about 22%of the World's TB burden.Indian National Strategic Plan 2017e2025,sets out the government plans to eliminate TB by 2025.However,the milestone seems unrealistic to achieve the TB eradication goal by 2025.We developed a five-dimensional mathematical model to understand the TB dynamics in India and investigate the possibility of the earliest TB eradication time frame.The model stratifies the entire TB class into three different classes as drug-sensitive(DS),MDR,and isolated classes.The effective reproduction number,equilibrium points,and stability analysis of the model were carried out.This model predicts the total estimated cases of DS-TB and MDR-TB from 2018 to 2035 through numerical simulation and suggests that TB may be eliminated by 2035 in India if the treatment success rate could be achieved to 95%,by contact tracing and isolating at least 50%of MDR-TB.

Cloud-based SDN(Software Defined Network)integration offers new kinds of agility,flexibility,automation,and speed in the network.Enterprises and Cloud providers both leverage the benefits as networks can be configured and optimized based on the application requirement.The integration of cloud and SDN paradigms has played an indispensable role in improving ubiquitous health care services.It has improved the real-time monitoring of patients by medical practitioners.Patients’data get stored at the central server on the cloud from where it is available to medical practitioners in no time.The centralisation of data on the server makes it more vulnerable to malicious attacks and causes a major threat to patients’privacy.In recent days,several schemes have been proposed to ensure the safety of patients’data.But most of the techniques still lack the practical implementation and safety of data.In this paper,a secure multi-factor authentication protocol using a hash function has been proposed.BAN(Body Area Network)logic has been used to formally analyse the proposed scheme and ensure that no unauthenticated user can steal sensitivepatient information.Security Protocol Animator(SPAN)–Automated Validation of Internet Security Protocols and Applications(AVISPA)tool has been used for simulation.The results prove that the proposed scheme ensures secure access to the database in terms of spoofing and identification.Performance comparisons of the proposed scheme with other related historical schemes regarding time complexity,computation cost which accounts to only 423 ms in proposed,and security parameters such as identification and spoofing prove its efficiency.
