Early-stage gastrointestinal cancer is often treated by endoscopic submucosal dissection(ESD)using a flexible *** with conventional percutaneous surgery,ESD is much less invasive and provides a high quality of life for the patient because it does not require a skin incision,and the organ is ***,the operator must be highly skilled because ESD requires using a flexible endoscope with energy devices,which have limited degrees of *** facilitate easier manipulation of these flexible devices,we developed a surgical robot comprising a flexible endoscope and two articulating *** robotic system is based on a conventional flexible endoscope,and an extrapolated motor unit moves the endoscope in all its degrees of *** instruments are thin enough to allow insertion of two instruments into the endoscope channel,and each instrument has a bending section that allows for up–down,right–left,and forward–backward *** this study,we performed an ex vivo feasibility evaluation using the proposed robotic system for ESD in a porcine *** procedure was successfully performed by five novice operators without *** findings demonstrated the feasibility of the proposed robotic system and,furthermore,suggest that even operators with limited experience can use this system to perform ESD.

The elastic properties of KF-substituted perovskite lead titanate(PbTiO_(3))were investigated by dielectric measurements and Brillouin light *** ferroelectric phase transition occurred at substantially lower temperature due to KF substitution,which was attributed to the modification of the covalency in Pb–O and Ti–O *** longitudinal acoustic(LA)mode of KF-substituted PbTiO_(3) showed a frequency softening in the paraelectric phase,which was accompanied by increasing acoustic *** indicated that polarization fluctuations responsible for the acoustic anomalies were enhanced by KF substitution.

Different characteristics of the structure of atmospheric boundary layer between dry and rainy periods over the northern Tibetan Plateau

In this paper, based on in-situ observational data of the Coordinated Enhanced Observing Period （CEOP） Asia-Australia monsoon Project （CAmP） on the Tibetan＇Plateau （CAmP-Tibet）, structure of the Atmospheric Boundary Layer （ABL） was preliminarily studied during the dry and rainy seasons. The main results show：（a） Diurnal variation of the ABL is obvious over the northern Tibetan Plateau area. The height of the ABL is different with the season change, which ranges from 2,211 m to 4,430 m during the pre-monsoon season and from 1,006 m to 2,212 m during the monsoon season. The ABL height is higher during the dry period than during the rainyigeriod. （b） The humidity is lower during the dry period than during the rainy period, and there are reverse humidity during both periods. （c） Horizontal wind direction is mostly west during the dry period, east under the height of 2,500 m and west above the height of 2,500 m during the rainy period. The wind speed is low during both the rainy and dry periods in the lower ABL layer. The wind speed is stronger within the upper ABL during the dry period than dtn-ing the rainy period.

Practical Application of a Tongue-Operated Joystick Device with Force Feedback mechanism

The human tongue has superior movement and tactile sensations. For individuals with severe disabilities, a tongue operated interface device can be used to operate life-support equipment, such as powered wheelchairs and robotic manipulators. A joystick-type device can directly translate various tongue motions to external equipment behavior. In addition, the user can interactively communicate with the equipment by tactile feedback. This helps the user to control the equipment safely and skillfully. Considering these factors, in a previous study [1], we developed a novel tongue-operated joystick device with reaction force feedback mechanism. We described the design process including the analysis of human tongue movement and tactile sensations and showed fundamental performances of reaction force feedback with the prototype device. In this study, we discuss the shape of the operational part that is used by the tongue. Two types of operational tools are prepared and their operability and perception of reaction force feedback are compared. Furthermore, we confirm the effectiveness of reaction force feedback to operate the joystick device safely and skillful controlling a mobile robot in an unknown environment.

Long-Term Environmental Accounting on Civil Infrastructures in Developing Countries

This paper proposes the environmental burden and benefit assessment method, which contributes to the evaluation of public works, by considering a quantitative environmental impact. The method developed is applied for the Bangkok subway construction project, so that the environmental impact following the Bangkok subway construction work is evaluated on the basis of environmental accounting. As a result, it was possible to quantify the burden and benefits to the environment in the life cycle of the Bangkok subway. In addition by converting the burden and benefits of the Bangkok subway construction project into monetary terms and introducing the Economic Internal Rate of Return （EIRR）, it was possible to evaluate the subway construction project from an economic point of view.

Up-Regulation of Local TGF-β Contributes to a Decrease in Renal Tubular Na⁺-K⁺ ATPase and Hyperkalemia in a mouse model of Crush Syndrome

Hyperkalemia is one of the most important risk factors in patients suffering from crush syndrome with rhabdomyolysis. Glycerol-injected animals have been used as an experimental model of rhabdomyolysis-induced acute kidney injury (AKI), but little information is available for the onset and molecular mechanism of hyperkalemia. In our murine model, plasma potassium levels increased after a single injection of 50%-glycerol solution (10 ml/kg, i.m.) during the progression of muscular and renal injuries. Renal tubular Na+-K+-ATPase functions as ion-exchange pomp for potassium clearance from blood into renal tubular epithelial cells. Renal histochemistry revealed an apparent decrease in the tubular Na+-K+-ATPase expression, especially at 24 hours post-glycerol challenge in our AKI model. In contrast to the loss in active Na+-K+-ATPase, there was a significant increase in the renal levels of transforming growth factor-β (TGF-β) that is known to suppress Na+-K+-ATPase production in vitro. When anti-TGF-β antibody was administered in mice after the glycerol challenge, the suppression of renal Na+-K+-ATPase activity was partially restored. As a result, hyperkalemia was improved in the TGF-β-neutralized AKI mice, associated with a significant decrease in plasma potassium concentration. Taken together, we predict that endogenous TGF-β is a key regulator for inhibiting Na+-K+-ATPase production and, in part, enhancing hyperkalemia during progression of rhabdomyolysis-induced AKI. This is, to our knowledge, the first report to determine a critical role of endogenous TGF-β in renal potassium metabolism during crush syndrome.

Numerical Simulation on Inclusion and Bubble Entrapment in Solidified Shell in model Experiment and in mold of Continuous Caster With DC magnetic Field

A model experiment investigating entrapment of inclusions and bubbles on the solidified shell was performed using molten steel,and the conditions for inclusion and bubble entrapment and mechanism of entrapment were studied. The results were applied to the flow behavior in the casting mold of a continuous *** the solid-liquid interface, entrapment of inclusions is greatly reduced by the existence of a low velocity flow,e.g.,0.05m/*** above-mentioned interfacial flow velocity dependency of inclusion entrapment is considered to be largely influenced by changes in the thickness of the concentration boundary layer,which depend on the interfacial flow ***,bubbles and inclusions which enter the concentration boundary layer are drawn to the solid-liquid interface by a suction force which is several orders larger than the Saffman's *** addition to the above-mentioned suction force,the so-called cleaning effect is determined by fluid-dynamic forces such as drag force,*** act on particles,and furthermore,by resident time of particles at the solid-liquid interface,which depends on the solidification *** a FC mold with a 2-stage electromagnetic brake,flotation of bubbles entrained in the jet flow from the nozzle is accelerated with the large DC magnetic *** is attributed to the braking effect of the DC field on the nozzle jet and the upward flow by the buoyancy of the *** a result,the interfacial flow velocity can be normalized by increasing the strength of the magnetic field,and entrapment of large bubbles and inclusions can be reduced.

AIm: To examine whether the sedative effects assessed by psychomotor tests would depend on the cytochrome P450 (CYP ) 2C19 genotypes after an infusion regimen of diazepam commonly used forgastrointestinal endoscopy in Japan. mETHODS: Fifteen healthy Japanese volunteers consisting of three different CYP2C19 genotype groups underwent a critical ? icker fusion test, an eye movement analysis and a postural sway test as a test for physical sedative effects, and a visual analog scale (VAS) symptom assessment method as a test for mental sedative effects during the 336 h period after the intravenous infusion of diazepam (5 mg). RESULTS: The physical sedative effects assessed by the critical flicker test continued for 1 h (t values of 5 min, 30 min and 60 min later: 4.35, 5.00 and 3.19, respectively) and those by the moving radial area of a postural sway test continued for 3 h (t values of 5 h, 30 h, 60 min and 3 h later: -4.05, -3.42, -2.17 and -2.58, respectively), which changed significantly compared with the baseline level before infusion (P mental sedative effects by the VAS method improved within 1 h. The CYP2C19 genotype-dependent differences in the postinfusion sedative effects were not observed in any of the four psychomotor function tests. CONCLUSION: With the psychomotor tests, the objective sedative effects of diazepam continued for 1 h to 3 h irrespective of CYP2C19 genotype status and the subjective sedative symptoms improved within 1 h. Up to 3 h of clinical care appears to be required after the infusion of diazepam, although patients feel subjectively improved.

Colonoscopy techniques combining or replacing air insufflation with water infusion are becoming *** were originally designed to reduce colonic spasms,facilitate cecal intubation,and lower patient discomfort and the need for *** maneuvers straighten the rectosigmoid colon and enable the colonoscope to be inserted deeply without causing looping of the ***-immersion colonoscopy minimizes colonic distension and improves visibility by introducing a small amount of *** addition,since pain during colonoscopy indicates risk of bowel perforation and sedation masks this important warning,this method has the potential to be the favored insertion technique because it promotes patient safety without ***,this water-immersion method has not only been used for colonoscope insertion,but has also been applied to therapy for sigmoid volvulus,removal of lesions,lower gastrointestinal bleeding,and therapeutic diagnosis of abnormal bowel morphology and irritable bowel *** a larger sample size and prospective head-to-head-designed studies will be needed,this review focuses on the usefulness of waterimmersion colonoscopy for diagnostic and therapeutic applications.
