为调查饱和珊瑚砂液化相关特性与发展规律,针对饱和Chibishi珊瑚砂,开展一系列不排水循环三轴试验,研究相对密度D r和循环应力比CSR对饱和Chibishi珊瑚砂的超孔隙水压力Δu、轴向应变εa及动强度特性的影响,并分析不同地区珊瑚砂抗液化强度的差异性。结果表明,饱和Chibishi珊瑚砂的Δu发展模式根据不同的CSR水平可以分为前期均匀增长型、前期突增型和后期突增型;此外,采用一种新的孔压模型对前期均匀增长型、前期突增型的孔压比r u进行表征,Chibishi珊瑚砂在相同的D r条件下,双幅应变εDA达到5%的循环次数N随着CSR水平的增加而逐渐减小;各珊瑚砂在同一里氏震级对应的等效循环振动次数下,抗液化强度CRR的增长模式存在显著差异。研究结果可丰富对珊瑚砂液化特征的认知,同时对近岸和沿海工程的抗震设计提供参考依据。

Patients with inflammatory bowel disease(IBD)are more likely to have concurrent immune-mediated inflammatory diseases(IMIDs)than those without IBD.IMIDs have been observed to alter the phenotype and outcomes of IBD in recent studies.Several studies have found that IBD patients with concurrent IMIDs may have more extensive or severe disease phenotypes,and are considered to be at increased risk of requiring biologics and IBD-related surgeries,suggesting that having multiple IMIDs is a poor prognostic factor for IBD.Furthermore,IBD patients with primary sclerosing cholangitis and Takayasu arteritis are reported to have unique endoscopic phenotypes,suggesting concurrent IMIDs can influence IBD phenotype with specific intestinal inflammatory distributions.In this review,we discuss the pathogenesis,disease phenotypes,and clinical outcomes in IBD patients with concomitant IMIDs.

Magnifying endoscopy is a useful technique to differentiate neoplasia from non-neoplastic lesions. Data regarding the clinical utility of magnifying endoscopy for neoplasia in patients with inflammatory bowel disease(IBD) has been emerging.While Kudo’s pit pattern types Ⅲ-Ⅴ are findings suggestive of neoplasia in non-IBD patients, these pit patterns are predictive of IBD-associated neoplasia as well.However, active chronic inflammatory processes, particularly regenerative changes, can mimic neoplastic pit patterns and may affect a meticulous evaluation of pit pattern diagnosis in patients with IBD. The clinical evidence regarding the utility of magnifying endoscopy with narrow band imaging or endocytoscopy has also been evolving in regard to the diagnosis of IBD-associated neoplasia. These advanced endoscopic techniques are promising for multiple reasons;not only for making an accurate diagnosis of neoplasia, but also in determining if endoscopic resection is appropriate for such lesions in patients with IBD. In this review, we discuss the diagnostic accuracy and limitations of magnifying endoscopy in assessing IBD-associated neoplasia and examine the feasibility and outcomes of endoscopic resection for these lesions.

BACKGROUND Portal vein thrombosis(PVT)after liver resection is rare but can lead to lifethreatening liver failure.This prospective study evaluated patients using contrastenhanced computed tomography(E-CT)on the first day after liver resection for early PVT detection and management.AIM To evaluate patients by E-CT on the first day after liver resection for early PVT detection and immediate management.METHODS Patients who underwent liver resection for primary liver cancer from January 2015 were enrolled.E-CT was performed on the first day after surgery in patients undergoing anatomical resection,multiple resections,or with postoperative bile leakage in the high-risk group for PVT.When PVT was detected,anticoagulant therapy including heparin,warfarin,and edoxaban was administered.E-CT was performed monthly until PVT resolved.RESULTS The overall incidence of PVT was 1.57%(8/508).E-CT was performed on the first day after surgery in 235 consecutive high-risk patients(165 anatomical resections,74 multiple resections,and 28 bile leakages),with a PVT incidence of 3.4%(8/235).Symptomatic PVT was not observed in the excluded cohort.Multivariate analyses revealed that sectionectomy was the only independent predictor of PVT[odds ratio(OR)=12.20;95%confidence interval(CI):2.22-115.97;P=0.003].PVT was found in the umbilical portion of 75.0%(6/8)of patients,and sectionectomy on the left side showed the highest risk of PVT(OR=14.10;95%CI:3.17-62.71;P<0.0001).CONCLUSION Sectionectomy on the left side should be chosen with caution as it showed the highest risk of PVT.E-CT followed by anticoagulant therapy was effective in managing early-phase PVT for 2 mo without adverse events.

Inflammatory bowel disease(IBD)is often diagnosed during the peak reprodu-ctive years of young women.Women with active IBD around conception are at a significantly increased risk of disease relapse during pregnancy,which is associated with poor pregnancy and neonatal outcomes.Given these substantial risks,it is prudent that disease remission should ideally be achieved before conception.Unfortunately,some patients may experience a disease flare-up even if they are in a state of remission before pregnancy.Patients must continue their IBD medications to reduce the risk of disease flare and subsequent poor outcomes during the gestational and postpartum periods.When treating IBD flare-ups during pregnancy,the management is quite similar to the therapeutic approach for non-pregnant patients with IBD,including 5-aminosalicylate,steroids,calcineurin inhibitors(CNIs),and biologic therapies.While the data regarding the safety of CNIs in pregnant women with IBD is limited,the findings in our recent meta-analysis suggest that CNIs may be safer to use in those with IBD than in solid organ transplant recipients.There are several types of biologics and small-molecule therapies currently approved for IBD,and physicians should thoroughly understand their clinical benefits and safety profiles when utilizing these treatments in the context of pregnancy.This review highlights recent studies,including our systematic review and meta-analysis,and discusses the clinical advantages and safety considerations of biologics and small molecules for pregnant women with IBD.

BACKGROUND Patients with mesenteric ischemia frequently suffer from bowel necrosis even after revascularization.Hydrogen gas has showed promising effects for ischemiareperfusion injury by reducing reactive oxygen species in various animal and clinical studies.We examined intestinal tissue injury by ischemia and reperfusion under continuous initiation of 3%hydrogen gas.AIM To clarify the treatment effects and target cells of hydrogen gas for mesenteric ischemia.METHODS Three rat groups underwent 60-min mesenteric artery occlusion(ischemia),60-min reperfusion following 60-min occlusion(reperfusion),or ischemiareperfusion with the same duration under continuous 3%hydrogen gas inhalation(hydrogen).The distal ileum was harvested.Immunofluorescence staining with caspase-3 and leucine-rich repeat-containing G-protein-coupled 5(LGR5),a specific marker of intestinal stem cell,was conducted to evaluate the injury location and cell types protected by hydrogen.mRNA expressions of LGR5,olfactomedin 4(OLFM4),hairy and enhancer of split 1,Jagged 2,and Neurogenic locus notch homolog protein 1 were measured by quantitative polymerase chain reaction.Tissue oxidative stress was analyzed with immunostaining for 8-hydroxy-2'-deoxyguanosine(8-OHdG).Systemic oxidative stress was evaluated by plasma 8-OHdG.RESULTS Ischemia damaged the epithelial layer at the tip of the villi,whereas reperfusion induced extensive apoptosis of the cells at the crypt base,which were identified as intestinal stem cells with double immunofluorescence stain.Hydrogen mitigated such apoptosis at the crypt base,and the LGR5 expression of the tissues was higher in the hydrogen group than in the reperfusion group.OLFM4 was also relatively higher in the hydrogen group,whereas other measured RNAs were comparable between the groups.8-OHdG concentration was high in the reperfusion group,which was reduced by hydrogen,particularly at the crypt base.Serum 8-OHdG concentrations were relatively higher in both reperfusion and hydrogen groups without significance.CONCLUSION

When the graft volume is too small to satisfy the recipient’s metabolic demand, the recipient may thus experience small-for-size syndrome (SFSS). Because the occurrence of SFSS is determined by not only the liver graft volume but also a combination of multiple negative factors, the definitions of small-for-size graft (SFSG) and SFSS are different in each institute and at each time. In the clinical setting, surgical inflow modulation and maximizing the graft outflow are keys to overcoming SFSS. Accordingly, relatively smaller-sized grafts can be used with surgical modification and pharmacological manipulation targeting portal circulation and liver graft quality. Therefore, the focus of the SFSG issue is now shifting from how to obtain a larger graft from the living donor to how to manage the use of a smaller graft to save the recipient, considering donor safety to be a priority.
