AIM:To analyze the efficacy and safety of a combination therapy of pegylated interferon(PEG-IFN) α-2b plus ribavirin(RBV) in older Japanese patients(65 years or older) infected with hepatitis C virus(HCV).METHODS:This multicenter study included 938 patients with HCV genotype 1 who received 1.5 μg/kg per week PEG-IFN α-2b plus RBV 600-1000 mg/d for 48 wk and 313 HCV genotype 2 patients who received this treatment for 24 ***:At 24 wk after the end of combination therapy,the overall sustained virological response(SVR) for genotypes 1 and 2 were 40.7% and 79.6%,*** SVR rate decreased signif icantly with age in each genotype,and was markedly reduced in genotype 1(Pronic hepatitis C patients 65 years or older who completed treatment with at least the minimum acceptable treatment dosage.

We experienced a case of acute urinary retention after an elective cesarean section. In this case, an epidural catheter was inserted for the postoperative pain relief, and we had to rule out anesthetic complications including epidural hematoma. After careful investigation, sacral herpes zoster was found to be responsible for urinary retention. Early administration of antiviral agent was started and the outcome was good. As various factors may lead to postpartum urinary disorders, we should be careful not to miss serious complications.

A 66-year-old man was diagnosed with a gastric submucosal tumor. Endoscopic ultrasound(EUS) revealed an iso/hypoechoic mass in the third layer. No malignant cells were detected in a histological examination. Yearly follow-up endoscopy and EUS showed the slow growth of the tumor. Endoscopic submucosal dissection(ESD) was performed and a glistening tumor was resected. The lesion showed a multinodular plexiform growth pattern consisting of spindle cells with an abundant fibromyxoid stroma that was rich in small vessels. The tumor was diagnosed as plexiform fibromyxoma(PF) by immunohistochemistry. Although difficulties are associated with reaching a diagnosis preoperatively, chronological changes on EUS may contribute to the diagnosis of PF. ESD may also be useful in the diagnosis and treatment of PF.

A real-time pricing system of electricity is a system that charges different electricity prices for different hours of the day and for different days, and is effective for reducing the peak and flattening the load curve. In this paper, using a Markov decision process (MDP), we propose a modeling method and an optimal control method for real-time pricing systems. First, the outline of real-time pricing systems is explained. Next, a model of a set of customers is derived as a multi-agent MDP. Furthermore, the optimal control problem is formulated, and is reduced to a quadratic programming problem. Finally, a numerical simulation is presented.

大环内酯类抗生素(MA)可显著改善弥漫性泛细支气管炎(DPB)的预后.同时对慢性副鼻窦炎、绿脓杆菌感染等均产生显著疗效,提示着MA除抗菌作用以外还具有抗炎作用,这种抗炎作用不仅对宿主,对致病菌本身也发挥着重要作用.近年来逐渐认识到MA对慢性阻塞性肺疾病(COPD)、流感病毒感染等常见病也发挥了显著疗效,减缓了疾病的进展和抑制了病情的加重.本文将总结25年MA疗法的应用,并介绍其对常见病的应用进展.

A New Generator for Tsunami Wave Generation

A middle size experiental wave generator has been implemented is the Ujikawa Open Laboratory, Disaster Prevention Research Institute, Kyoto University. The generator is composed of a pistontype wave maker, a head strage water tank and a current generator to mainly reproduce long waves like tsunami and storm surge. The paper desribes several experimental series to predict the applicability of the generator to model tests. The three operating sysemes are capable to be controlled in one operating sysytem and start time is contorolled separately according with the target tsunami and storm surge profiles. A sharp tsunami profile is reproduced in adjusting the start timing of piston type wave maker and opening gates of head storage tunk. Any type of tsunami waves are reproduced in the generator and it becomes a storong tool to predict the effective of＂resiliency＂ of hardwares.

AIM： To determine the efficacy of long-term lamivudine treatment of a large number of Japanese patients with chronic hepatitis B. METHODS： In this retrospective, multi-center trial, 318 Japanese patients with chronic hepatitis B received 100 mg of lamivudine daily for up to 36 （median 21） mo. Virological response was a decline to a serum HBV DNA level less than 3.7 log copies/mL. Virological breakthrough was defined as the reappearance of a serum HBV DNA level to more than 10-fold the minimum during treatment. RESULTS： Lamivudine produced virological response in 86.8% of the 318 patients at 6 mo, in 80.2% of 252 patients at 12 mo, in 69.2% of 133 patients at 24 mo, and in 53.6% of 28 patients at 36 mo. Forward stepwise logistic regression analysis showed an HBV DNA level less than 6.8 log copies/mL （P〈 0.0001）, HBeAg negativity （P〈 0.0001）, a platelet count of 100×10^9/L or more （P= 0.0162） at baseline, and a decline of the HBV DNA level of more than 3.2 log copies/mL as compared with the baseline level at 3 mo after the start of treatment （P= 0.0003） to be significantly associated with virological response. Among patients with a virological response, virological breakthrough was seen in 5.3% of 19 patients who responded virologically at 1 mo, in 20.7% of 203 patients at 3 mo, in 27.5% of 51 patients at 6 mo, in 33.3% of 12 patients at 9 mo, and in 100% of 3 patients at ≥15 mo. A virological breakthrough was found significantly more often in patients with delayed virological response. CONCLUSION： Lamivudine treatment could suppress serum HBV DNA in most of the tested Japanese patients. Long-term efficacy might be seen in patients without HBeAg at baseline, in the absence of cirrhosis, and in patients with a decline in HBV DNA level soon after the start of treatment.

With the increasing trend of development and industrialization in Malaysia, air pollution has become an inevitable part of the process, resulting from increased vehicular activities and industrial processes. Toll operators are potentially exposed to high levels of air pollutants from working in proximity to traffic pollution sources, thereby increasing their risk of health defects associated with air pollution exposure. This study assessed the levels of Nitrogen dioxide (NO2) toll operators are exposed to, considering the influences traffic density and meteorological factors. This is intended to serve as an indicator of the cumulative pollution emanating from the combustion process of vehicles at toll plazas. Using Passive diffusion samplers saturated with Triethanolamine (TEA), the weekly indoor and outdoor NO2 concentrations at tollbooth were measured at Sungai Besi (SB), Kajang (KJ) and Putra-Makhota (PM) toll Plazas, to capture the varying traffic densities of 138,000, 65,000 and 24,100 vehicles/day respectively. The results showed that NO2 concentrations increase with traffic densities, and indoor NO2 concentrations correlated highly with outdoor NO2 concentrations (R2: SB = 0.767, KJ = 0.689 and PM = 0.877). The indoor/outdoor NO2 ratio varied from 0.7 to 1.2 for all toll booths, suggesting pollution control in-efficiency, caused by technical and behavioural factors. Also, meteorological factors had no significant effect on nitrogen dioxide concentrations, contrary to previous studies, which is likely due to the far distance between tool plazas and meteorological stations. Furthermore, the NO2 concentrations reported in this study were higher in comparison to weekly standards adopted in Germany (0.032 ppm) and previous literature. Therefore, toll operators are potentially exposed to high levels of pollution, and we advise that toll operators to wear pollution protection gears to reduce the risk of exposure, and the ventilation systems and mitigation measure (a
