Preoperative albumin-bilirubin score and liver resection percentage determine postoperative liver regeneration after partial hepatectomy

BACKGROUND The success of liver resection relies on the ability of the remnant liver to *** of the knowledge regarding the pathophysiological basis of liver regeneration comes from rodent studies,and data on humans are ***,there is limited knowledge about the preoperative factors that influence postoperative *** To quantify postoperative remnant liver volume by the latest volumetric software and investigate perioperative factors that affect posthepatectomy liver *** A total of 268 patients who received partial hepatectomy were *** were grouped into right hepatectomy/trisegmentectomy(RH/Tri),left hepa-tectomy(LH),segmentectomy(Seg),and subsegmentectomy/nonanatomical hepatectomy(Sub/Non)*** regeneration index(RI)and late rege-neration rate were defined as(postoperative liver volume)/[total functional liver volume(TFLV)]×100 and(RI at 6-months-RI at 3-months)/RI at 6-months,*** lower 25th percentile of RI and the higher 25th percentile of late regeneration rate in each group were defined as“low regeneration”and“delayed regeneration”.“Restoration to the original size”was defined as regeneration of the liver volume by more than 90%of the TFLV at 12 months *** The numbers of patients in the RH/Tri,LH,Seg,and Sub/Non groups were 41,53,99 and 75,*** RI plateaued at 3 months in the LH,Seg,and Sub/Non groups,whereas the RI increased until 12 months in the RH/Tri *** to our multivariate analysis,the preoperative albumin-bilirubin(ALBI)score was an independent factor for low regeneration at 3 months[odds ratio(OR)95%CI=2.80(1.17-6.69),P=0.02;per 1.0 up]and 12 months[OR=2.27(1.01-5.09),P=0.04;per 1.0 up].Multivariate analysis revealed that only liver resection percentage[OR=1.03(1.00-1.05),P=0.04]was associated with delayed ***,multivariate analysis demonstrated that the preoperative ALBI score[OR=2.63(1.00-1.05),P=0.02;per 1.

Relationships between gait properties on soft surfaces, physical function, and fall risk for the elderly

The plantar aspect change caused by contact with soft surfaces creates and unstable gait and increases the risk of falling, especially in the elderly. This study aimed to analyze gait property by three-dimensional motion analysis on soft and normal surfaces and to clarify the relationship with physical function and fall risk. Twenty-four older people aged 65-88 years old and living independently without any assistive device (7 men, 17 women) performed 5 m of walking with own maximal speed on normal and soft surface walkways. The soft surface walkway used was a low rebound urethane foam mattress. The three-dimensional kinematic gait analysis by sixteen anatomic points was used to evaluate gait property on both walkways. The gait property on soft surfaces tended to be swinging up and down in each joint and to largely lean left and right as compared with the normal surface. Moreover, it tended to decrease in a step length and to increase in a step width. All gait parameters on soft surfaces correlated significantly with functional reach. On the other hand, that on normal surface correlated significantly with leg strength. Gait properties on soft surfaces which changes in plantar aspect during foot contact differs from those on normal surfaces. Walking on soft surfaces may cause an unanticipated inverted pendulum sway supporting a foot contact point because of the disturbance by a sagging walkway;in short, requiring more effort to keep a body balanced. In conclusion, gait on the soft surfaces requires balance ability (functional reach) rather than leg strength.

Lignan Glycosides and Phenolic Compound Glycosides from the Branches of <i>Tabebuia chrysotricha</i>

From the branches of Tabebuia chrysotricha (Bignoniaceae), two lignan glycosides (1, 2) and two phenolic compound glycosides (3, 4) were isolated, along with 15 known compounds (5 - 19). The structures of the new compounds were elucidated on the basis of spectroscopic evidence and those of the known compounds were identified by comparison of spectroscopic data with those reported in the literature. The DPPH radical-scavenging activity was assayed for the compounds isolated, the aryltetralin-type lignans showing moderate activity.

Semiconductor Optical Amplifier (SOA)-Fiber Ring Laser and Its Application to Stress Sensing

We have developed a novel optical fiber ring laser using a semiconductor optical amplifier (SOA) as the gain medium, and taking advantage of polarization anisotropy of its gain. The frequency difference of the bi-directional laser is controlled by birefringence which is introduced in the ring laser cavity. The beat frequency generated by combining two counter-propagating oscillations is proportional to the birefringence, the fiber ring laser of the present study is, therefore, applicable to the fiber sensor. The sensing signal is obtained in a frequency domain with the material which causes the retardation change by a physical phenomenon to be measured. For the application to stress sensing, the present laser was investigated with a photoelastic material.

Producing single microbubbles with controlled size using microfiber

Microbubble-mediated pore formation in sonoporation includes a combination of complex processes such as collision or coalescence of translating or collapsing microbubbles with vascular cells. Although understanding the pore formation mechanisms may improve drug delivery efficiency, their details are still poorly understood. In the present study, we describe an experimental model that produces single air bubbles with controllable size. A carbon microfiber in liquids is illuminated by an infrared laser to produce individual bubbles having size comparable to that of the microfiber. The microbubbles can be physically isolated from the fiber for placing at arbitrary positions in the liquids. The lifetime of the bubbles is several tens of minutes depending on the intensity of the laser used. The preparation of the controllable air bubbles may be useful in future investigations of ultrasound-mediated microbubble–cell interactions.

AIM:To analyze the efficacy and safety of a combination therapy of pegylated interferon(PEG-IFN) α-2b plus ribavirin(RBV) in older Japanese patients(65 years or older) infected with hepatitis C virus(HCV).METHODS:This multicenter study included 938 patients with HCV genotype 1 who received 1.5 μg/kg per week PEG-IFN α-2b plus RBV 600-1000 mg/d for 48 wk and 313 HCV genotype 2 patients who received this treatment for 24 ***:At 24 wk after the end of combination therapy,the overall sustained virological response(SVR) for genotypes 1 and 2 were 40.7% and 79.6%,*** SVR rate decreased signif icantly with age in each genotype,and was markedly reduced in genotype 1(P<0.001).Moreover,the SVR was significantly higher in patients with genotype 1 who were less than 65 years(47.3% of 685) than in those 65 years or older(22.9% of 253)(P<0.001) and was higher in patients with genotype 2 who were less than 65 years(82.9% of 252) than in those 65 years or older(65.6% of 61)(P=0.004).When patients received a dosage at least 80% or more of the target dosage of PEG-IFN α-2b and 60% or more of the target dosage of RBV,the SVR rate significantly increased to 66.5% in patients less than 65 years and to 45.2% in those 65 years or older(P<0.001).Adverse effects resulted in treatment discontinuation more often in patients with genotype 1(14.4%) than in patients with genotype 2(7.3%),especially by patients 65 years or older(24.1%).CONCLUSION:PEG-IFN α-2b plus RBV treatment was effective in chronic hepatitis C patients 65 years or older who completed treatment with at least the minimum acceptable treatment dosage.

Present Status of the Nd:YAG Thomson Scattering System Development for Time Evolution Measurement of Plasma Profile on Heliotron J

A new high repetition rate Nd：YAG Thomson scattering system is developed for the Heliotron J helical device. A main purpose of installing the new system is the temporal evolution measurement of a plasma profile for improved confinement physics such as the edge transport barrier （H-mode） or the internal transport barrier of the helical plasma. The system has 25 spatial points with -10 mm resolution. Two high repetition Nd：YAG lasers （〉 550 m J@ 50 Hz） realize the measurement of the time evolution of the plasma profile with ~10 ms time intervals. Scattered light is collected by a large concave mirror （D----800 mm, f/2.25） with a solid angle of -100 mstr and transferred to interference filter polychromators by optical fiber bundles in a staircase form. The signal is amplified by newly designed fast preamplifiers with DC and AC output, which reduces the low frequency background noise. The signals are digitized with a multi-event QDC, fast gated integrators. The data acquisition is performed by a VME-based system operated by the CINOS.

Development of a Heavy Ion Bearn Probe for Measuring Electrostatic Potential Profile and Its Fluctuation in LHD

A heavy ion beam probe （HIBP） using a 3-MV tandem accelerator has been installed on large helical device （LHD）. Electrostatic potential in core plasma can be measured under the toroidal magnetic field strength of up to 3 T. By using the HIBP, the transition of potential profiles from electron-root to ion^root is observed in core plasmas during ramp-up of the electron density. Potential fluctuations are also measured electron cyclotron current drive （ECCD）. Two kind of characteristic fluctuations are observed. One is a reversed-shear-induced Alfv^n eigenmode （RSAE）, whose frequency varies during the evolution of the rotational transform profile, and the other is with a constant geodeisc acoustic mode （GAM） frequency.

Predictors of Discontinuation of Antipsychotic Therapy in Patients with Acute Schizophrenia: A 1-Year Observational Study with More Than 1000 Patients

Discontinuation of antipsychotic therapy has been a significant clinical issue among patients with schizophrenia, since the patients who discontinued antipsychotic treatment showed worse clinical and functional outcomes, and higher risks of relapse of schizophrenia symptoms and hospitalization. We conducted a post-hoc analysis of a post-marketing research with a 12-month follow-up period to identify the predictors for discontinuation of antipsychotic monotherapy in Japan. This is a prospective, naturalistic multicenter observational study, designed to evaluate the discontinuation rates of olanzapine monotherapy and non-olanzapine antipsychotic monotherapy in Japanese adult patients with acute schizophrenia. Patients were treatment-naive, or had switched from other antipsychotics or from poly-pharmacotherapy to oral antipsychotic monotherapy. We analyzed the correlation of discontinuation of antipsychotic monotherapy with baseline characteristics of patients. A total of 1089 patients (578 patients treated with olanzapine and 511 with non-olanzapine antipsychotics) were eligible for analysis. By the end of the 12-month study period, 614 patients (56.4%) discontinued antipsychotic therapy. Multivariate logistic regression analyses indicated significantly lower discontinuation rates in all patients treated with antipsychotics: older age (Odds ratio [OR], 0.871;95% confidence interval [CI], 0.797 to 0.953;p = 0.003), outpatient status (OR, 0.508;95% CI, 0.383 to 0.675;p < 0.001), prior use of antipsychotics (OR, 0.693;95% CI, 0.516 to 0.930;p = 0.015), and olanzapine group showed lower discontinuation rate than that of non-olanzapine group (OR, 1.416;95% CI, 1.086 to 1.846;p = 0.010). The present study indicated that the outpatient status, older age, and prior use of antipsychotics have better adherence to antipsychotic treatment. In addition to these factors, use of anti-parkinson agents showed lower discontinuation rates in the olanzapine monotherapy group.

Differences in Baseline Characteristics of Patients Treated with Olanzapine or Other Antipsychotics in Japanese Patients with Acute Schizophrenia: A 1-Year Observational Study under Routine Clinical Practice in Japan

Objective: Baseline characteristics of acute schizophrenia patients were analyzed to identify differences in the baseline characteristics of patients treated with olanzapine monotherapy compared with those treated with other antipsychotic monotherapies. Methods: This prospective, naturalistic observational study was designed to evaluate discontinuation rates of olanzapine and non-olanzapine antipsychotic monotherapy in Japanese adult patients with acute schizophrenia. Results: A total of 1089 patients were assessed: 578 patients were treated with olanzapine, 487 with non-olanzapine atypical antipsychotics, and 24 with typical antipsychotics. The mean Clinical Global Impression-Severity (CGI-S) Schizophrenia, Brief Psychiatric Rating Scale (BPRS) total, and BPRS positive scores were higher in patients treated with olanzapine compared with most of the non-olanzapine treated patients. The majority of patients with a CGI-S Schizophrenia score of 7 (29/41 patients) as well as patients with a BPRS total score of 90 or higher (14/18 patients) were treated with olanzapine. On the other hand, physicians tended to prescribe antipsychotics other than olanzapine for patients with heavier body weight or diabetes mellitus. Conclusion: The present study demonstrated that olanzapine was more likely to be prescribed to patients with more severe schizophrenia symptoms. However, further studies are warranted to reach a definite conclusion.
