Therapeutic efficacy of the Qing Dai in patients with intractable ulcerative colitis

Ulcerative colitis (UC) is a chronic inflammatory bowel disease that may become intractable when treated with conventional medications such as aminosalicylates, corticosteroids, and azathioprine. The herbal medicine Qing Dai has traditionally been used in Chinese medicine to treat UC patients, but there is a lack of published data on the efficacy of Qing Dai in UC treatment. We report several cases of patients with intractable UC who take Qing Dai in a retrospective observational study. Furthermore, we explore the mechanisms of action of Qing Dai. Nine patients with active UC who received conventional medications but wished to receive Qing Dai as an alternative medication were included in our analysis. The UC severity level was determined based on the clinical activity index (CAI). Additionally, 5 of the 9 patients were endoscopically evaluated according to the Matts grading system. Each patient received 2 g/d of Qing Dai orally and continued taking other medications for UC as prescribed. Electron spin resonance was applied to explore the mechanisms of action of Qing Dai. After 4 mo of treatment with Qing Dai, the CAI score decreased from 8.3 ± 2.4 to 2.4 ± 3.4 (mean ± SD; P < 0.001). Similarly, the endoscopic Matts grade decreased from 3.4 ± 0.5 to 2.2 ± 0.8 (P = 0.02). Six of 7 patients who were on prednisolone upon enrollment in the study were able to discontinue this corticosteroid. Electron spin resonance revealed that Qing Dai possesses strong hydroxyl radical scavenging activity. Qing Dai showed significant clinical and endoscopic efficacy in patients who failed to respond to conventional medications. Scavenging of hydroxyl radicals appears to be a potential mechanism through which Qing Dai acts, but the significance of the scavenging ability of Qing Dai with respect to the anti-inflammatory effect in UC patients warrants further investigation.

Influence of Temperature, Moisture, and Light Conditions on the Germination of Seeds of <i>Erythronium japonicum</i>Decne

Erythronium japonicum is a representative species of spring ephemeral, forest floor plants in substitution forests in Japan. Seed germination tests were conducted to observe the effects of temperature, light, and moisture content on the germination of E. japonicum. E. japonicum seeds did not germinate at constant temperatures of 5°C, 10°C, 15°C, 20°C, or 25°C. Seeds germinated at 5°C and 10°C after storage at 20°C for 1 or 2 months. Seed germinability was affected by desiccation to only 40% moisture content. Seeds germinated well in both light and dark conditions. Conditions under the leaf litter layer showed unique characteristics of temperature, light, and moisture content. The temperature was around 20°C under the leaf litter layer, even though it was 30°C above. The moisture content under the leaf litter layer was higher than that above. The leaf litter blocked out the sunlight. The temperature of 20°C necessary for E. japonicum germination occurred below, but not above, the leaf litter layer. The avoidance of decreasing in germinability from reduced seed moisture content in a short time is consistent with the high moisture conditions under the leaf litter layer. The leaf litter layer plays an important role in providing optimal conditions of temperature, light, and moisture content for the germination of E. japonicum seeds.

Radiotherapy Intensified by Increase of Fraction Size Using Three-Dimensional Conformal Radiotherapy in Bladder Preservation Therapy

Purpose: The impact of radiotherapy on bladder cancers heightened by increase of fraction size using three-dimensional conformal radiotherapy (3DCRT) was verified with biologically effective dose (BED, Gy10). Method: Twenty-five patients who were considered unsuitable for total cystectomy underwent 3DCRT between 2009 and 2014. When a gross tumor volume (GTV) did not neighbor guts and the patient was cooperative to radiotherapy (radical), the GTV was treated with a fraction size of ≥3.0 Gy, while a clinical target volume (CTV) of a whole bladder and lymph nodes at small pelvis were treated with a conventional fraction size of 2.0 Gy. When a GTV neighbored guts (palliative), only a CTV was treated with an increased fraction size of 2.5 Gy. Results: Compared to conventional two-dimensional radiotherapy, BED increased to 62.3 - 77.0 Gy10 (radical) or to 56.1 - 68.0 Gy10 (palliative) but the treatment period shortened to 4 - 5 weeks. Acute adverse events (≤3 months), determined by common terminology criteria for adverse events, were Grade 2 at most, but late adverse events were not observed. Only one patient (6%) lost the bladder. Conclusions: Patients who receive bladder preservation therapy appear good candidates to intensify impact of radiotherapy with 3DCRT.
