We propose a software reliability growth model with testing-effort based on a continuous-state space stochastic process, such as a lognormal process, and conduct its goodness-of-fit evaluation. We also discuss a parameter estimation method of our model. Then, we derive several software reliability assessment measures by the probability distribution of its solution process, and compare our model with existing continuous-state space software reliability growth models in terms of the mean square error and the Akaike’s information criterion by using actual fault count data.

Testing-time when a change of a stochastic characteristic of the software failure-occurrence time or software failure-occurrence time-interval is observed is called change-point. It is said that effect of the change-point on the software reliability growth process influences on accuracy for software reliability assessment based on a software reliability growth model (SRGM). We propose an SRGM with the effect of the change-point based on a bivariate SRGM, in which the software reliability growth process is assumed to depend on the testing-time and testing-effort factors simultaneously, for accurate software reliability assessment. And we discuss an optimal software release problem for deriving optimal testing-effort expenditures based on our model. Further, we show numerical examples of software reliability assessment based on our bivariate SRGM and estimation of optimal testing-effort expenditures by using actual data.

Dual-purpose systems for both normal and disaster situations are necessary for providing continuous services from normal situations to disaster situations. We have been developing the dual-purposed systems based on the assurance network design principle. The assurance network design principle makes the dual-purpose systems work stably in both normal and disaster situations. This paper proposes a connectivity-dependent data propagation scheme, in which each terminal transfers data adaptively by wireless multi-hop data transfer or store-and-forward data transfer depending on whether the terminal has connections to its neighboring terminals. To verify the resilience against disconnection among neighboring terminals, we show field experimental results on data propagation time. Also we propose the dual-purpose system, in which there are two types of graphical user interface (GUI) for both situations. Whenever each terminal receives a special packet in disaster situations, the GUI automatically switches from one type for normal situations to another type for disaster situations. We have unified these two types of GUI so that users can understand how to use them even when GUI is automatically switched. To validate feasibility of the dual-purpose normal and disaster situations system, we show experimental results on dissemination of assessment information and automatical switching of GUIs.

AIM: To compare the clinical efficacy of the secondgeneration H2 RA lafutidine with that of lansoprazole in Japanese patients with mild gastroesophageal reflux disease(GERD). METHODS: Patients with symptoms of GERD and a diagnosis of grade A reflux esophagitis(according to the Los Angeles classification) were randomized to receive lafutidine(10 mg, twice daily) or lansoprazole(30 mg, once daily) for an initial 8 wk, followed by maintenance treatment comprising half-doses of the assigned drug for 24 wk. The primary endpoint was the frequency and severity of heartburn during initial and maintenance treatment. The secondary endpoints were the sum score of questions 2 and 3 in the Gastrointestinal Symptom Rating Scale(GSRS), and the satisfaction ***: Between April 2012 and March 2013, a total of 53 patients were enrolled, of whom 24 and 29 received lafutidine and lansoprazole, respectively. After 8 wk, the frequency and severity of heartburn was significantly reduced in both groups. However, lafutidine was significantly inferior to lansoprazole with regard to the severity of heartburn during initial and maintenance treatment(P = 0.016). The sum score of questions 2 and 3 in the GSRS, and satisfaction scores were also significantly worse in the lafutidine group than the lansoprazole group(P = 0.0068 and P = 0.0048, respectively).CONCLUSION: The clinical efficacy of lafutidine was inferior to that of lansoprazole, even in Japanese patients with mild GERD.

Biochemically curative surgery for gastrinoma in multiple endocrine neoplasia type 1 patients

AIM： To search for the optimal surgery for gastrinoma and duodenopancreatic neuroendocrine tumors in patients with multiple endocrine neoplasia type 1. METHODS： Sixteen patients with genetically confirmed multiple endocrine neoplasia type 1 （MEN 1） and Zollinger-Ellison syndrome （ZES） underwent resection of both gastrinomas and duodenopancreatic neuroendocrine tumors （NETs） between 1991 and 2009. For localization of gastrinoma, selective arterial secretagogue injection test （SASI test） with secretin or calcium solution was performed as well as somatostatin receptor scintigraphy （SRS） and other imaging methods such as computed tomography （CT） or magnetic resonance imaging （MRI）. The modus of surgery for gastrinoma has been changed over time, searching for the optimal surgery： pancreaticoduodenectomy （PD） was first performed guided by localization with the SAST test, then local resection of duodenal gastrinomas with dissection of regional lymph nodes （LR）, and recently pancreas-preserving total duodenectomy （PPTD） has been performed for multiple duodenal gastrinomas. RESULTS： Among various types of preoperative localizing methods for gastrinoma, the SASI test was the most useful method. Imaging methods such as SRS or CT made it essentially impossible to differentiate functioning gastrinoma among various kinds of NETs. However, recent imaging methods including SRS or CT were useful for detecting both distant metastases and ectopic NETs; therefore they are indispensable for staging of NETs. Biochemical cure of gastrinoma was achieved in 14 of 16 patients （87.5%）; that is, 100% in 3 patients who underwent PD, 100% in 6 patients who underwent LR （although in 2 patients （33.3%） second LR was performed for recurrence of duodenal gastri- noma）, and 71.4% in 7 patients who underwent PPTD. AIM:To investigate the causal relationship between hypoadiponectinemia and colorectal carcinogenesis in in vivo experimental model,and to determine the con-tribution of adiponectin defi ciency to colorectal cancer development and ***:We examined the influence of adiponectin defi ciency on colorectal carcinogenesis induced by the administration of azoxymethane(AOM)(7.5 mg/kg,in-traperitoneal injection once a week for 8 wk),by using adiponectin-knockout(KO) ***:At 53 wk after the fi rst AOM treatment,KOmice developed larger and histologically more progres-sive colorectal tumors with greater frequency com-pared with wild-type(WT) mice,although the tumor incidence was not different between WT and KO *** mice showed increased cell proliferation of colorec-tal tumor cells,which correlated with the expression levels of cyclooxygenase-2(COX-2) in the colorectal *** addition,KO mice showed higher incidence and frequency of liver tumors after AOM *** percent of WT mice developed liver tumors,and these WT mice had only a single *** contrast,50% of KO mice developed liver tumors,and 58% of these KO mice had multiple ***:Adiponectin deficiency enhances colorectal carcinogenesis and liver tumor formation induced by AOM in *** study strongly suggests that hypoadiponectinemia could be involved in the pathogenesis for colorectal cancer and liver tumor in human subjects.

HIV encephalopathy presenting as schizophrenia-like delusions during highly active antiretroviral therapy (HAART) case report

After HAART (highly active antiretroviral therapy), which is a powerful antiviral drug regimen, was introduced into HIV therapy for AIDS patients, AIDS mortality decreased dramatically. The incidence of encephalopathy among AIDS patients was reported to be 10% to 40% in the past, but the introduction of HAART resulted in a decrease in HIV encephalopathy and an increase in mild CNS-related deficits, which affect the quality of life of AIDS patients. We report the case of an AIDS patient who developed schizophrenia-like delusions associated with encephalopathy. Administration of an antipsychotic drug and hypnotics was effective in relieving the symptoms of patients with insomnia, depressed mood, and delusions. Further information on the neuronal mechanism underlying the mental symptoms observed in our patient will be necessary to understand the process of pathogenesis of the encephalopathy and to devise an adequate clinical strategy for treatment.

Bipolar I disorder and expressed emotion of families: A cohort study in Japan

The relationships between expressed emotion (EE) of the families and the course of bipolar disorder have been examined only in a limited number of cohort studies. No study has yet been reported from Asia. The subjects were 12 patients that had been diagnosed with bipolar I disorder according to DSM-IV and their 12 key family members. The families of the patients were interviewed using the Camberwell Family Interview (CFI) within 2 weeks of the admission of the patients, and their EE were evaluated. The patients were then followed up for 9 months after their discharge from the hospital. The patients were divided into a high-EE group and a low-EE group using the cut-off based on the number of critical comments (CC) and emotional overinvolvement (EOI), and the 9-month relapse risk was compared. When the subjects with 3 or more CC or an EOI score of 3 or more were regarded as the high-EE group, and the others as the low-EE group, the 9-month relapse risk was 100% (3/3) for the high EE group and 0% (0/9) for the low EE group. (Fisher’s exact test p = 0.005) EE based on the CFI appear to be correlated with relapse in bipolar I disorder in Japan.
