To investigated characteristics, diagnosis, bowel-cleansing preparation, sedation, and colonoscope length and diameter in Japanese pediatric patients receiving total colonoscopy. METHODSThe present study evaluated consecutive patients aged ≤ 15 years who had undergone their first colonoscopy in Kurume University between January 2007 and February 2015. Data were retrospectively analyzed. We identified 110 pediatric patients who had undergone colonoscopy that had reached the cecum, allowing the observation of the total colon. RESULTSHematochezia, abdominal pain, and diarrhea were the most common symptoms. For bowel-cleansing preparation, pediatric patients aged ≤ 12 years were treated with magnesium citrate, and patients aged 13-15 years were treated with polyethylene glycol 4000. For sedation, thiamylal with pentazocine, which has an analgesic effect, was used in patients aged ≤ 6 years, and midazolam with pentazocine was used in patients aged ≥ 7 years. Regarding the choice of endoscope, short and thin endoscopes were selected for younger patients, particularly patients aged ≤ 3 years. Positive diagnoses were made in 78 patients (70.9%). Inflammatory bowel disease (n = 49, 44.5%), including ulcerative colitis (n = 37, 33.6%) and Crohn’s disease (n = 12, 10.9%), was the most common diagnosis. CONCLUSIONColonoscopy offers a high diagnostic capability for pediatric patients with gastrointestinal symptoms. The selection of appropriate management the performance of colonoscopy is important in pediatric patients.

Prediction of colorectal tumor grade and invasion depth through narrow-band imaging scoring

AIM To determine the usefulness of assigning narrow-band imaging(NBI) scores for predicting tumor grade and invasion depth in colorectal *** A total of 161 colorectal lesions were analyzed from138 patients who underwent endoscopic or surgical resection after conventional colonoscopy and magnifying endoscopy with NBI. The relationships between the surface and vascular patterns of the lesions, as visualized with NBI, and the tumor grade and depth of submucosa(SM) invasion were determined *** were assigned to distinct features of the surface microstructures of tubular and papillary-type *** a multivariate analysis, a model was developed for predicting the tumor grade and depth of invasion based on NBI-finding *** NBI findings that correlated with a high tumor grade were associated with the "regular/irregular"(P < 0.0001)surface patterns and the "avascular area" pattern(P =0.0600). The vascular patterns of "disrupted vessels"(P = 0.0714) and "thick vessels"(P = 0.0133) but none of the surface patterns were associated with a depth of invasion of ≥ 1000 μm. In our model, a total NBIfinding score ≥ 1 was indicative of a high tumor grade(sensitivity: 0.97; specificity: 0.24), and a total NBIfinding score ≥ 9(sensitivity: 0.56; specificity: 1.0) was predictive of a SM invasion depth ≥ 1000 μm. Scores less than these cutoff values signified adenomas and a SM invasion depth < 1000 μm, respectively. Inflammatory bowel disease(IBD), including Crohn's disease and ulcerative colitis, is a chronic intestinal inflammation of unknown etiology. The diagnosis of IBD is based on endoscopic, radiologic and histopathologic criteria. Recently, the search for a noninvasive marker that could augment or replace part of this diagnostic process has become a focus of IBD research. In this review, antibody markers, including microbial antibodies, autoantibodies and peptide antibodies, will be described, focusing on their common features. At present, no single marker with qualities that are satisfactory for the diagnosis and treatment of IBD has been identified, although panels of some antibodies are being evaluated with keen interest. The discovery of novel IBD-specific and sensitive markers is anticipated. Such markers could minimize the use of endoscopic and radiologic examinations and could enable clinicians to implement individualized treatment plans designed to improve the long-term prognosis of patients with IBD.

Improving Ambient Wind Environments of a Cross-flow Wind Turbine near a Structure by using an Inlet Guide Structure and a Flow Deflector

A cross-flow wind turbine near a structure was tested for the performance. The results showed that the performance of a cross-flow wind turbine near a structure was up to 30% higher than the one without a structure. In addition, we tried to get higher performance of a cross-flow wind turbine by using an Inlet Guide Structure and a Flow Deflector. An Inlet Guide Structure was placed on the edge of a structure and a Flow Deflector was set near a cross-flow wind turbine and can improve ambient wind environments of the wind turbine, the maximum power coefficients were about 15 to 40% higher and the tip speed ratio range showing the high power coefficient was wide and the positive gradients were steep apparently.

Effects of Aspect Ratio in a Transonic Shock Tube Airfoil Flow

In the present study, the flow visualizations were performed around the NACA 0012 models which differ in aspect ratios. We discussed the effects of the aspect ratio in the test models. Additionally the unsteady, two-dimensional, compressible Euler equations were solved for the NACA 0012 airfoil. Experiments were performed utilizing the conventional gas driven shock tube as the intermittent transonic wind tunnel. The aspect ratios of the models are about 0.86 and 1.5, respectively. The Mach numbers M 2 are about 0.84. The Reynolds numbers of the present experimental conditions were constant that Re based on chord length is about 4.0×10 5 . The results are as follows: in different aspect ratios, the difference of the shock wave location is confirmed though the Mach number and Reynolds number are same. It indicates the different correction Mach number by the effects of the side wall boundary layer though the nominal Mach number measured the same value. Also, on the difference of shock wave location for the effects of the aspect ratio, the tend of CFD shows the qualitative agreement with the result of an experiment.

The development of iterative learning control combined with disturbance-observer-based(DOB)control for the digital low-level radio frequency(LLRF)system of the International Linear Collider project is *** objective of this study is to compensate for both repetitive(or predictable)and unpredictable disturbances in a radio frequency system(e.g.,beam loading,Lorentz force detuning,and microphonics).The DOB control approach was verified using the LLRF system at the Superconducting Test Facility(STF)at KEK in the absence of a *** method comprising DOB control combined with an iterative learning control algorithm was then demonstrated in a cavity-simulator-based test bench,where a simulated beam was *** results showed that the performance of the LLRF system was improved,as expected by this combined control *** plan to further generalize this approach to LLRF systems at the STF and the future International Linear Collider project.

A Study on High Subsonic Airfoil Flows in Relatively High Reynolds Number by Using OpenFOAM

In the present study,numerical calculations of the flow-field around the airfoil model are performed by using the OpenFOAM in high subsonic *** airfoil model is NACA *** maximum thickness is 10 % of the chord *** SonicFOAM and the RhoCentralFOAM are selected as the solver in high subsonic *** grid point is 158,000 and the Mach numbers are 0.277 and 0.569 *** CFD data are compared with the experimental data performed by the cryogenic wind tunnel in the *** results are as *** numerical results of the pressure coefficient distribution on the model surface calculated by the SonicFOAM solver showed good agreement with the experimental data measured by the cryogenic wind *** the data calculated by the SonicFOAM have the capability for the quantitative comparison of the experimental data at low angle of attack.

Background diseases and the number of previous intravitreal aflibercept injections on immediate intraocular pressure increase and vitreous reflux rate in phakic eyes

●AIM:To evaluate the effect of background diseases and number of previous intravitreal aflibercept injections(IVAIs)on immediate intraocular pressure(IOP)increase and vitreous reflux(VR)rate and to evaluate the correlation of both age and axial length with immediate IOP increase and VR rate.●METHODS:This study included 105 patients with cystoid macular edema secondary to retinal vein occlusion,35 patients with diabetic macular edema,69 patients with neovascular age-related macular degeneration(nAMD),and 12 patients with myopic choroidal neovascularization,which underwent first-time *** correlation of immediate IOP increase and VR rates with the four background diseases was ***,the correlation of age with immediate IOP increase and VR rate as well as correlation of axial length with immediate IOP increase and VR rate were ***,54 patients with nAMD were treated with IVAI>10 times(multiple IVAIs).Moreover,the correlation of immediate IOP increase and VR rates with first-time and multiple IVAIs in nAMD was determined.●RESULTS:The immediate IOP increase(P=0.16)and VR rates(P=0.50)were almost similar among the four background *** immediate postinjection IOP and age,VR rate and age,immediate postinjection IOP and axial length,or VR rate and axial length were not correlated in the four background *** immediate IOP increase(P=0.66)and VR rates(P=0.28)did not significantly differ between first-time and multiple IVAIs in nAMD.●CONCLUSION:Background diseases and number of previous IVAIs have no effect on immediate IOP increase and VR ***,age and axial length have no correlation on immediate IOP increase and VR rate.
