Prevention of esophageal strictures after endoscopic submucosal dissection

Endoscopic mucosal resection (EMR) and endoscopic submucosal dissection (ESD) have recently been accepted as less invasive methods for treating patients with early esophageal cancers such as squamous cell carcinoma and dysplasia of Barrett’s esophagus. However, the large defects in the esophageal mucosa often cause severe esophageal strictures, which dramatically reduce the patient’s quality of life. Although preventive endoscopic balloon dilatation can reduce dysphagia and the frequency of dilatation, other approaches are necessary to prevent esophageal strictures after ESD. This review describes several strategies for preventing esophageal strictures after ESD, with a particular focus on anti-inflammatory and tissue engineering approaches. The local injection of triamcinolone acetonide and other systemic steroid therapies are frequently used to prevent esophageal strictures after ESD. Tissue engineering approaches for preventing esophageal strictures have recently been applied in basic research studies. Scaffolds with temporary stents have been applied in five cases, and this technique has been shown to be safe and is anticipated to prevent esophageal strictures. Fabricated autologous oral mucosal epithelial cell sheets to cover the defective mucosa similarly to how commercially available skin products fabricated from epidermal cells are used for skin defects or in cases of intractable ulcers. Fabricated autologous oral-mucosal-epithelial cell sheets have already been shown to be safe.

A Pilot Survey of Odontomas for Size Constancy Using Cone-Beam Computed Tomography: Effect of Age, Sex, Lesion Location, and Histological Type: A Case Series

Objectives: Odontoma is the most common type of odontogenic tumors. Many studies have analyzed the statistical associations between odontoma location and patient age and sex, according to the histological tumor type. However, few studies have assessed odontoma morphological characteristics using cone-beam computed tomography (CBCT). We aimed to evaluate the association between odontoma location and size. Methods: We performed CBCT on patients with odontomas (19 patients;10 women, 9 men;average age, 12.6 [range, 6 - 34] years) according to the pathology type at a university hospital between April 2008 and February 2017. The locations of the lesions were noted, and their sizes were measured on CBCT images. Buccolingual, mesiodistal, and vertical diameters of the lesions were recorded on the same slice with the greatest diameters on axial, coronal, and sagittal CBCT images. Results: Altogether, 9 (47.4%) and 10 (52.6%) odontomas were located in the mandible and maxilla, respectively. There was no significant difference in the mesiodistal and vertical diameters on the CBCT image between the mandibular and maxillary groups when the odontoma size was compared with location (p Conclusions: These data suggest that the sizes of odontomas in the maxilla are affected by bone expansion in the buccolingual direction, but they may be invariable in most settings. Three-dimensional assessment across the age groups suggests a lack of variation in size.

To investigated characteristics, diagnosis, bowel-cleansing preparation, sedation, and colonoscope length and diameter in Japanese pediatric patients receiving total colonoscopy. METHODSThe present study evaluated consecutive patients aged ≤ 15 years who had undergone their first colonoscopy in Kurume University between January 2007 and February 2015. Data were retrospectively analyzed. We identified 110 pediatric patients who had undergone colonoscopy that had reached the cecum, allowing the observation of the total colon. RESULTSHematochezia, abdominal pain, and diarrhea were the most common symptoms. For bowel-cleansing preparation, pediatric patients aged ≤ 12 years were treated with magnesium citrate, and patients aged 13-15 years were treated with polyethylene glycol 4000. For sedation, thiamylal with pentazocine, which has an analgesic effect, was used in patients aged ≤ 6 years, and midazolam with pentazocine was used in patients aged ≥ 7 years. Regarding the choice of endoscope, short and thin endoscopes were selected for younger patients, particularly patients aged ≤ 3 years. Positive diagnoses were made in 78 patients (70.9%). Inflammatory bowel disease (n = 49, 44.5%), including ulcerative colitis (n = 37, 33.6%) and Crohn’s disease (n = 12, 10.9%), was the most common diagnosis. CONCLUSIONColonoscopy offers a high diagnostic capability for pediatric patients with gastrointestinal symptoms. The selection of appropriate management the performance of colonoscopy is important in pediatric patients.

Numerical Study of the Lift Force, Velocities and Pressure Distribution of a Single Air Bubble and Two Interacting Air Bubbles Rising in Quiescent Liquid

The study of multiphase flow consisting of liquid and air bubbles has been attracting the interest of many researchers. Numerical methods for such a system are, however, facing difficulty in numerical accuracy and a heavy computational load. In this paper, we made corrections to the modified force-coupling method in our previous papers and applied it to the numerical studies of a single air bubble rising near a vertical wall and two interacting air bubbles rising in line in quiescent liquid. Corrections were made to the effective ranges of the force-coupling method. The calculation results showed that the lift force acting on an air bubble obtained by the experimental data was more accurately reproduced than those by our previous method. We accurately calculated the time evolution of the velocities of interacting two air bubbles rising in line obtained in the previous experiments and resolved the physical mechanism of the relative movement of two bubbles. We also found the present method is much quicker and needs much smaller memory capacity than other methods, such as the volume of fluid method.

Optical Measurements of Shock Waves in Critical Nozzles at Low Reynolds Numbers

Two-dimensional critical nozzle flows at low Reynolds numbers are visualized by the rainbow schlieren deflectometry. Experiments have been performed in a region of overexpanded nozzle flow. The variation of the shock structure against the back pressure ratio can be clearly visible with color gradation. Static pressure rises due to the shock-induced flow separation are compared with the previous theories. The unsteady characteristics of overexpanded critical nozzle flows at low Reynolds numbers are quantitatively and qualitatively visualized using laser schlieren and Mach-Zehnder interferometer systems combined with a high-speed digital camera. It was found that an oscillating normal shock wave appears inside the nozzle, and that the shock wave has a specified dominant frequency. Also the time-history of the oscillating shock wave is obtained from both the systems and compared with each other.

Self-Reported Adherence after Overnight Switching from Immediate- to Extended-Release Pramipexole in Parkinson’s Disease

Background: Drug adherence decreased in patients with Parkinson’ s disease (PD) because of taking many different types of drugs. We evaluated drug adherence after switching from immediate-release (IR) to once-daily extended-release (ER) pramipexole (PPX) in PD. Methods: This study included 35 PD patients (20 men, 15 women);10 were taking oral PPX alone, and 25 were also using another anti-PD drug. PPX-IR was switched overnight to PPX-ER without gradual tapering. One month after switching, improvement in timing adherence and reduction in medication burden were evaluated by a questionnaire using a visual analog scale (VAS) (0: No change;10: Better). Motor function was assessed using part III of the Unified Parkinson’s Disease Rating Scale (UPDRS). Results: The VAS score for improvement in timing adherence was 8.1 ± 0.5 (mean ± standard error), and that for reduction in medication burden was 7.3 ± 0.6. There was a significant negative correlation (ρ = -0.43, p = 0.01) between the VAS score and number of types of medications. The UPDRS part III score improved significantly after switching

Comparative Assay of Hepatitis B and C Virus Infection Markers by Different Assay Kits^1

In order to compare sensitivity of EIA and RIA assay kits for hepatitis B and C virus (HBV and HCV, respectively) infection markers, 100 serum samples in total were collected form 50 adult women each in urban and rural areas in northeast China. The number of positive cases to the three infection markers on HBV (i.e., HBsAg +, anti HBs +, and anti HBc +) and the one on HCV (anti HCV +) were examined in two laboratories, i.e., in Laboratory A with EIA kits produced in China and in Laboratory B with RIA kits. HCV infection positivity (anti HCV +) was examined by EIA kits in both laboratories, but from different sources in and outside of China, respectively. The assay in Laboratory A gave 2 HBsAg + cases out of the 100 cases examined, whereas there were 9 positive cases in Laboratory B. In contrast, 19 cases were positive to anti HCV when examined in Laboratory A, and there were 3 cases in Laboratory B. Thus, the kits used in Laboratory A gave fewer HBsAg + and more anti HCV + cases than the kits used in Laboratory B. The prevalence of anti HBs + or anti HBc + and cases did not differ when assayed in the two laboratories with EIA and RIA kits, respectively. The agreement of positive and negative findings between the two sets of testing were 93%, 93%, 93%, 86% and 82% for HBsAg, anti HBs, anti HBc, HBV (i.e., either positive to anyone of the three markers or negative to all three markers), and anti HCV, respectively. The implication of the observation on epidemiology on HBV and HCV infection prevalence was discussed.

Application of the Modified Force-Coupling Method of Tracing the Trajectories of Spherical Bubbles with Solid-Like and Slip Surfaces

The force-coupling method (FCM) developed by Maxey and Patel (2001) was modified and applied to trace the trajectories of spherical bubbles with solid-like and slip surfaces. Careful comparison was made to the experimental results of Takemura et al. (2000, 2002a, 2002b). First, the result obtained by use of the original version of the FCM was compared to the experimental results. It was found that the original FCM was not feasible for tracing spherical bubble trajectories. Then, a correction was made to the FCM calculation of the bubble velocity by renormalization in terms of the bubble Reynolds number, which could very well trace the trajectory of the bubble with a solid-like, no-slip surface, but not that of a bubble with a slip surface. Finally, a substantial correction was made to the monopole term of the FCM, which could trace the trajectory of a bubble with a solid-like or slip surface very well even for the Reynolds number up to 20.

Formation Processes of Zinc Excimer Thin Films Due to Ion-Recombination Processes

In materials science, the number of d-electrons of transition metals is an essentially important factor controlling characteristics of alloys and compounds. In this paper, we show an example to control the number of d-electrons (holes) by using inner-core electron excitation of zinc atoms. An important feature of our research is that we can make a long lifetime excited electronic state of zinc (3d8), and the life-time of excited zinc is more than 307 days. At first, the experimental apparatus and boundary conditions of the ion-recombination processes were explained. From results of XPS, excited zinc films showed satellites peaks what caused by the final state of 3d8 and the charge transfer final state of 3d10L2. Excited states of zinc were formatted at the surface of substrate caused by ion-recombination process between Zn+ and Zn-. The excited zinc diffused from substrate surface to the surface of the excited zinc thin film. Intensity of excited zinc is proportional to the intensity of electron on the substrate.

Background:The end-stage liver disease causes a metabolic dysfunction whose most prominent clinical feature is the loss of skeletal muscle mass(SMM).In living-donor liver transplantation(LDLT),liver graft regeneration(GR)represents a crucial process to normalize the portal hypertension and to meet the metabolic demand of the *** data are available on the correlation between pre-LDLT low SMM and ***:Retrospective study on a cohort of 106 LDLT patients receiving an extended left liver lobe *** skeletal muscle index(SMI)at L3 level was used for muscle mass measurement,and the recommended cut-off values of the Japanese Society of Hepatology guidelines were used as criteria for defining low *** was evaluated as rate of volume increase at 1 month post-LT[graft regeneration rate(GRR)].Results:The median GRR at 1 month post-LT was 91%(IQR,65-128%)and a significant correlation with graft volume-to-recipient standard liver volume ratio(GV/SLV)(rho-0.467,P<0.001),graft-to-recipient weight ratio(GRWR)(rho-0.414,P<0.001),donor age(rho-0.306,P=0.001),1 month post-LT cholinesterase serum levels(rho 0.397,P=0.002)and pre-LT low muscularity[absent *** GRR 97.5%(73.1-130%)vs.83.5%(45.2-110.9%),P=0.041]was *** in male recipients,but not in women,it was shown a direct correlation with pre-LT SMI(rho 0.352,P=0.020)and inverse correlation with 1 month post-LT SMI variation(rho-0.301,P=0.049).A low GRR was identified as an independent prognostic factor for recipient overall survival(HR 6.045,P<0.001).Conclusions:Additionally to the hemodynamic factors of portal circulation and the quality of the graft,the metabolic status of the recipients has a significant role in the GR process.A pre-LT low SMM is associated with impaired GRR and this negative impact is more evident in male recipients.
