Tubulointerstitial nephritis complicated by primary Sj?gren’s syndrome in a patient under treatment for type 2 diabetes mellitus was diagnosed in the early stage of the disease by renal biopsy. The symptoms of primary Sj?gren’s syndrome, such as thirst and polydipsia, were masked by the characteristic symptoms of type 2 diabetes mellitus. An association between sicca symptoms and diabetes mellitus (types 1 and 2) has been previously reported. Hence, it is possible that there are common underlying immunological mechanisms between primary Sj?gren’s syndrome and diabetes mellitus of both types. Intervention with steroids in a timely manner appears to have prevented or slowed the progression of re- nal impairment.

Vibrio vulnificus (V. vulnificus) infection is a rare disease in Japan but the leading cause of death related to raw seafood consumption. We hereby reported a successfully treated case of V. vulnificus septicemia, severe necrotizing fasciitis, disseminated intravascular coagulation and multiple organ failure after raw perch consumption with underlying alcoholic liver cirrhosis and diabetes mellitus. It is the first report of a case of V. vulnificusinfection caused by eating raw perch, whereas V. vulnificus infection should be suspected in all of middle-aged to elderly men with underlying immunosuppressive diseases, who have recent consumption of raw seafood or contact with seawater, especially in the summer. The levels of HbA1c and glycoalbumin were not high in the present case, however, obvious hyperglycemia was found even after the infection had completely healed. On reviewing 166 case of V. vulnificus infection in Japan including ours, the complication of diabetes mellitus, one of immunocompromised condition, was found only in 11%, although it had been reported that individuals strongly suspected of having diabetes were 17.2% among the Japanese male population aged from 40 to 74 years. Because diabetes mellitus might be underdiagnosed in the previous reports, intensive examinations are considered to be necessary in order to correctly diagnose diabetes mellitus in patients with severe V. vulnificus infection.

Reduced endogenous insulin secretion in diabetic patients with low-titer positive antibodies against GAD

Aim: To investigate the clinical characteristics of diabetic patients with a low-titer positive for the anti-glutamic acid decarboxylase 65 antibody (GAD antibody). Methods: The subjects were 420 diabetic inpatients. The endogenous insulin secretion was estimated on the basis of the C-peptide immunoreactivity from a 24 h urine collection (uCPR). Clinical variables were compared between patients negative for the GAD antibody (GAD antibody titer < 1.5 U/mL), a low-titer positive GAD antibody (1.5 U/mL ≤ GAD antibody titer < 10 U/mL) and a high-titer positive GAD antibody (10 U/mL ≤ GAD antibody titer). Results: The low and high-titer positive GAD antibodies were found in 25 and 10 patients, respectively. The uCPR was significantly lower in both the patients with a low (37 ± 33 ug/24h) and high-titer (39 ± 27 ug/24h) positive GAD antibodies than in those negative for GAD antibodies (71 ± 52 ug/24h). The uCPR level was significantly lower in the low-titer positive GAD antibody group (29 ± 22 ug/24h) than in the negative group (67 ± 55 ug/24h) among the patients not taking insulin secretagogues. The difference disappeared in the subjects taking insulin secreagogues. In the stepwise multiple regression analysis, a low-titer positive GAD antibody was independently associated with the uCPR level. Conclusions: Endogenous insulin secretion is reduced in diabetic patients positive for GAD antibodies, even if the titer is low. Earlier initiation of insulin therapy might therefore protect the pancreatic β-cell function in diabetic patients with a low-titer positive GAD antibody.

Aims: The associations between the number of antihypertensive agents being taken by type 2 diabetic patients and 1) the grade of atherosclerosis according to non-invasive surrogate markers for atherosclerosis and 2) the other risk factor for atherosclerosis, were cross-sectionally investigated. Methods: The association between the blood pressure control and the clinical characteristics was evaluated in 1359 patients with type 2 diabetes mellitus. Results: The number of antihypertensive agents was 1.5 ± 1.4 (2.0 ± 1.2 among the 990 patients with hypertension). The proportion of patients taking no antihypertensive agents was 29%, 22% were taking one, 29% were taking two and 21% of the patients were taking three or more antihypertensive agents. The value of the ankle-brachial pressure index significantly decreased with the increase in the number of antihypertensive agents even if the blood pressure was corrected to the target value. The values of brachial-ankle pulse wave velocity and carotid intima-media thickness were also increased. The prevalence of risk factors for atherosclerosis, such as obesity, hyperlipidemia, chronic kidney disease, hyperuricemia and anemia was significantly elevated with the number of agents. Conclusions: The number of antihypertensive agents simply reflects the grade and risk of atherosclerosis in patients with type 2 diabetes mellitus.

Tree diversity effects on forest productivity increase through time because of spatial partitioning

Background: Experimental manipulations of tree diversity have often found overyielding in mixed-species plantations. While most experiments are still in the early stages of stand development, the impacts of tree diversity are expected to accumulate over time. Here, I present findings from a 31-year-old tree diversity experiment(as of2018) in ***: I find that the net diversity effect on stand biomass increased linearly through time. The species mixture achieved 64% greater biomass than the average monoculture biomass 31 years after planting. The complementarity effect was positive and increased exponentially with time. The selection effect was negative and decreased exponentially with time. In the early stages(≤ 3 years), the positive complementarity effect was explained by enhanced growths of early-and mid-successional species in the mixture. Later on(≥ 15 years), it was explained by their increased survival rates owing to vertical spatial partitioning — i.e. alleviation of self-thinning via canopy stratification. The negative selection effect resulted from suppressed growths of late-successional species in the bottom ***: The experiment provides pioneering evidence that the positive impacts of diversity-driven spatial partitioning on forest biomass can accumulate over multiple decades. The results indicate that forest biomass production and carbon sequestration can be enhanced by multispecies afforestation strategies.

Experimental study on the free surface deformation in the oxide melt

Experiments on the thermocapillary convection of high temperature Bi12SiO20 melts have been carried out in the in situ observation system. The steady flow pattern comprises of the main trunk and branches, which is deemed as the surface deformation. The oscillatory thermocapillary convection is characterized by the oscillatory main trunk and traveling branches. The transition of the melt from steady spatial to oscillatory behavior under the temperature differences 120, 60 and 10 K has been considered. The free surface deformation is observed to transform with the change of the applied temperature difference, which manifests that the thermocapillary convection is sensitive to the temperature difference. Moreover, taking the temperature distribution into account, it is noted that the deformation is formed in the colder area of the melt. The oscillatory frequency of the main trunk, which is also sensitive to the applied temperature difference, increases with the rise of temperature.

Chain reaction bankruptcy is regarded as common phenomenon and its effect is to be taken into account when credit risk portfolio is analyzed. But consideration and modeling of its effect leave much room for improvement. That is mainly because method for grasping relations among companies with limited data is underdeveloped. In this article, chance discovery method is applied to estimate industrial relations that are to include companies＇ relations that transmit chain reaction of bankruptcy. Time order method and directed KeyGraph are newly introduced to distinguish and express the time order among defaults that is essential information for the analysis of chain reaction bankruptcy. The steps for the data analysis are introduced and result of example analysis with default data in Kyushu, Japan, 2005 is presented. The structure estimated by the new method is compared with the structure of actual account receivable holders of bankrupted companies for evaluation.

This paper presents three-dimensional finite element （FE） analyses of an all-frame model of a three-story reinforced concrete （RC） building damaged in the 1999 Taiwan Chi-Chi Earthquake. Non-structural brick walls of the building acted as a seismic resistant element although their contributions were neglected in the design. Hence, the entire structure of a typical frame was modeled and static and dynamic nonlinear analyses were conducted to evaluate the contributions of the brick walls. However, the results of the analyses were considerably overestimated due to coarse mesh discretizations, which were unavoidable due to limited computer resources. This study corrects the overestimations by modifying （1） the tensile strengths and （2） shear stiffness reduction factors of concrete and brick. The results indicate that brick walls improve frame strength although shear failures are caused in columns shortened by spandrel walls. Then, the effectiveness of three types of seismic retrofits is evaluated. The maximum drift of the first floor is reduced by 89.3%, 94.8%, and 27.5% by Steel-confined, FuI1-RC, and Full-brick models, respectively. Finally, feasibility analyses of models with soils were conducted. The analyses indicated that the soils elongate the natural period of building models although no significant differences were observed.

The Gaps Between Institutional and Practical Disaster Risk Management Measures on Coastal Flood Risks in South Korea’s Coastal Communities

Global warming and climate change significantly increase the frequency of coastal floods caused by sea level rise(SLR) as a permanent factor and hydrometeorological hazards as tentative factors. The combined risks will affect coastal *** Korea is gradually facing SLR risks, mainly in its southern coastal regions;however, disaster risk reduction(DRR)in coastal regions remains fragmented. This study aimed to investigate the status of DRR for coastal communities in South Korea by looking at government practices and testimonies from residents. This study reviewed DRR-related regulations and documents and collected data from interviews with local government officials, field visits, and informal conversations with residents in six coastal communities. The findings show that the coastal communities were less resilient to coastal floods than to other hazards, such as typhoons and heavy rains, and the potential consequences could be expanded due to demographic challenges, fragmented institutional systems, and low risk awareness. Moreover, this study emphasized the necessity of an integrated approach to reducing the impact of coastal floods caused by both SLR and other factors. It also highlighted the importance of coastal community engagement in local DRR activities through increasing risk awareness and adapting to environmental change based on appropriate risk information disclosure by the government. The impacts of coastal floods triggered by SLR and other hazard factors can be reduced by aligning practical regulatory measures with adaptive strategies and enhancing the disaster resilience of coastal communities.
