Nanoparticles are considered to be a powerful approach for the delivery of poorly watersoluble drugs. One of the main challenges is developing an appropriate method for preparation of drug nanoparticles. As a simple, rapid and scalable method, the flash nanoprecipitation(FNP) has been widely used to fabricate these drug nanoparticles, including pure drug nanocrystals, polymeric micelles,polymeric nanoparticles, solid lipid nanoparticles, and polyelectrolyte complexes. This review introduces the application of FNP to produce poorly water-soluble drug nanoparticles by controllable mixing devices, such as confined impinging jets mixer(CIJM), multi-inlet vortex mixer(MIVM) and many other microfluidic mixer systems. The formation mechanisms and processes of drug nanoparticles by FNP are described in detail. Then, the controlling of supersaturation level and mixing rate during the FNP process to tailor the ultrafine drug nanoparticles as well as the influence of drugs, solvent, anti-solvent, stabilizers and temperature on the fabrication are discussed. The ultrafine and uniform nanoparticles of poorly watersoluble drug nanoparticles prepared by CIJM, MIVM and microfluidic mixer systems are reviewed briefly. We believe that the application of microfluidic mixing devices in laboratory with continuous process control and good reproducibility will be benefit for industrial formulation scale-up.

Climate change, social unrest and dynastic transition in ancient China

The evident connection between human evolu- tion and climatic changes has been concurred by scientists. Although many people are trying to forecast the impacts of climatic changes on our future society, there are not any studies to quantitatively scrutinize the interrelation between climatic changes and social developments by using historical data. In line with this knowledge gap, this study adopted a scientific approach to compare the paleoclimatic records with the historical data of wars, social unrests, and dynastic transitions in China spanned from the late Tang to Qing Dy- nasties. Results showed that war frequency in cold phases was much higher than that in mild phases. Besides, 70%— 80% of war peaks and most of the dynastic transitions and nationwide social unrests in China took place in cold phases. This phenomenon could be attributed to the diminishing thermal energy input in cold phases resulting in the fall of land-productivity and hence, the deficiency of livelihood re- sources across society. Accompanied with certain social cir- cumstances, this kind of ecological stress was transformed into wars and social unrests, followed by dynastic transitions in most of the cases. By closer examination, it was even found that war frequency was negatively correlated with tempera- ture anomaly series. As land carrying capacities vary from one climatic zone to another, the magnitude of war-tempera- ture association also differed among different geographic regions. It is suggested that climatic change was one of the most important factors in determining the dynastic cycle and alternation of war and peace in ancient China.

AIM To investigate the usefulness of aspartate aminotransferase to platelet ratio index(APRI) in predicting hepatocellular carcinoma(HCC) risk in primary biliary cholangitis(PBC).METHODS We identified PBC patients between 2000 and 2015 by searching the electronic medical database of a tertiary center. The hazard ratio(HR) of HCC with different risk factors was determined by Cox proportional hazards model. RESULTS One hundred and forty-four PBC patients were recru-ited. Patients were diagnosed at a median age of 57.8 years [interquartile range(IQR): 48.7-71.5 years), and 41(28.5%) patients had cirrhosis at baseline. The median follow-up duration was 6.9 years(range: 1.0-26.3 years). Twelve patients developed HCC, with an incidence rate of 10.6 cases per 1000 patient-years. The overall 5-, 10-and 15-year cumulative incidences of HCC were 2.3% 95%CI: 0%-4.8%), 8.4%(95%CI: 1.8%-14.5%) and 21.6%(6.8%-34.1%), respectively. Older age(HR = 1.07), cirrhosis(HR = 4.38) and APRI at 1 year after treatment(APRI-r1) > 0.54(HR = 3.94) were independent factors for HCC development. APRI-r1, when combined with treatment response, further stratified HCC risk(log rank P < 0.05). The area under receiver operating curve of APRI-r1 in predicting HCC was 0.77(95%CI: 0.64-0.88).CONCLUSION APRI-r1 can be used to predict the development of HCC in PBC patients. Combination of APRI-r1 with treatment response can further stratify the HCC risk.

AIM To investigate the epidemiology and natural history of Wilson's disease in the *** Data were retrieved via electronic search of hospital medical registry of the Hong Kong Hospital Authority,which covers all the public healthcare services. We identified cases of Wilson's disease between 2000 and 2016 by the International Classification of Diseases(ICD)-9 code. We analyzed the incidence rate,prevalence and adverse outcomes of Wilson's *** We identified 211 patients(male cases 104; female cases 107; median age 27.2 years,IQR: 17.1-38.6 years; duration of follow-up 8.0 years,IQR: 5.0-14.0 years). The average annual incidence rate was 1.44 per million person-years while the prevalence was 17.93 per million. Between 2000 and 2016,there was a decrease in the annual incidence rate from 1.65 to 1.23 per million person-years(P = 0.010),whereas there was an increase in the annual prevalence from 7.80 to 25.20 per million(P < 0.001). Among the 176 cases with hepatic involvement,38(21.6%) had cirrhosis,three(1.7%) developed hepatocellular carcinoma,24(13.6%) underwent liver transplantations,and 26(14.8%) died. Seven patients had concomitant chronic viral hepatitis B or C. The 5-year and 10-years rates of overall survival were 92.6% and 89.5%,and for transplant-free survival rates 91.8% and 87.4%,respectively. Cirrhosis and possibly chronic viral hepatitis were associated with poorer overall survival. CONCLUSION There was a significant increase in the prevalence of Wilson's disease in Hong Kong. The prognosis was favorable except for those with cirrhosis or concomitant viral hepatitis.

Injuries to the central nervous system(CNS)such as stroke,brain,and spinal cord trauma often result in permanent disabilities because adult CNS neurons only exhibit limited axon *** brain has a surprising intrinsic capability of recovering itself after ***,the hostile extrinsic microenvironment significantly hinders axon *** advances have indicated that the inactivation of intrinsic regenerative pathways plays a pivotal role in the failure of most adult CNS neuronal ***,substantial evidence has convincingly demonstrated that the mechanistic target of rapamycin(mTOR)signaling is one of the most crucial intrinsic regenerative pathways that drive axonal regeneration and sprouting in various CNS *** this review,we will discuss the recent findings and highlight the critical roles of mTOR pathway in axon regeneration in different types of CNS ***,we will demonstrate that the reactivation of this regenerative pathway can be achieved by blocking the key mTOR signaling components such as phosphatase and tensin homolog(PTEN).Given that multiple mTOR signaling components are endogenous inhibitory factors of this pathway,we will discuss the promising potential of RNA-based therapeutics which are particularly suitable for this purpose,and the fact that they have attracted substantial attention recently after the success of coronavirus disease 2019 *** specifically tackle the blood-brain barrier issue,we will review the current technology to deliver these RNA therapeutics into the brain with a focus on nanoparticle *** will propose the clinical application of these RNA-mediated therapies in combination with the brain-targeted drug delivery approach against mTOR signaling components as an effective and feasible therapeutic strategy aiming to enhance axonal regeneration for functional recovery after CNS injury.

This study discusses the experimental result of the viscoplastic response and col- lapse of sharp-notched 316L stainless steel tubes with different notched depths subjected to cyclic bending. The tube bending machine and curvature-ovalization measurement apparatus were used for conducting the symmetric curvature-controlled cyclic bending. To highlight the viscoplastic behavior, three different curvature-rates, 0.0035, 0.035 and 0.35 m-1s-1, were controlled. Ob- servations of a certain curvature-rate reveal that five almost parallel lines corresponding to five different notch-depth （0.2, 0.4, 0.6, 0.8 and 1.0 mm） tubes were presented in the experimental relationship between the cyclic controlled curvature and the number of cycles needed to pro- duce buckling on a log-log scale. However, the slopes for the three different curvature-rates are different. An empirical formulation was proposed to simulate the aforementioned relationship. When comparing with the experimental findings, the simulation was in good agreement with the experimental data.

The global economic downturn caused primarily by the US sub-prime mortgage crisis in 2007 engendered revenue loss of the multinational corporations. Existing studies have yet to depict the detrimental impacts on city＇s command and control functions induced by the sub-prime mortgage crisis together with its residual wave of global economic recession on the global spatial economy. Recent and previous studies have produced an ＇instant history＇ of the global spatial economy before the global economic downturn undermines the global economy in late 2008. How- ever, the waxes and wanes of major cities＇ command and control functions on the global economic arena before and after the outbreak of financial crisis and its associated geo-economic transitions are still poorly understood. This paper attempts to contribute a new set of customized data to update and fill in the gap in the literature with the investigation of the command and control functions of cities arotmd the world from 2005 to 2009. Particular attentions are paid to the time-space relationship of the geo-economic transition that can capture the recent historical images of the com- mand and control situation of different cities in the world.

Downscaling and Disaggregating NAO-conflict Nexus in Pre-industrial Europe

Recently, the desiccation effect of the North Atlantic Oscillation(NAO) is found to be positively correlated with violent conflict in pre-industrial Europe, with agricultural shrinkage and its subsequent economic shocks to be their causal link. However, it remains unexplored whether the correlation persists if the study period is extended backward in time, a different definition of violent conflict is applied, or the relationship is examined at lower geographic levels. In this study, we based on 835 internal disturbance incidents in Europe during 1049–1800 to conduct long-temporal and multi-scalar examination on the NAO-conflict nexus. ′Time-series′ and ′panel data′ disaggregation approaches, together with Granger Causality, Multiple Regression, and Survival Analyses were applied to verify the nexus quantitatively. Results show that the positive NAO-conflict correlation was significant at the continent and physiographic zone levels. During the positive NAO phases, the annual probability of internal disturbance outbreak increased by 70.0% in the southern Europe and the Mediterranean, a zone most affected by the NAO-induced desiccation effect. Yet, the NAO-conflict correlation was rather inconsistent when it was downscaled to the sub-regional level. Moreover, the NAO-conflict correlation was inflated under the time-series approach, while the panel data approach demonstrated the region-specific nature of the NAO forcing more clearly. The associated implications in examining climate-conflict nexus are discussed. Our findings may be crucial in examining violent conflict in the northwestern Africa, a highly agricultural region affected by the NAO.
