Gemcitabine and cisplatin combination therapy (GC) is accepted as a standard treatment for advanced biliary tract cancer (BTC). However, little information is available regarding such treatment in the clinical practice setting in Japan. We retrospectively examined the clinical data of patients with unresectable or recurrent BTC who received GC as first-line treatment. The regimen consisted of cisplatin (25 mg/m2) and gemcitabine (1000 mg/m2) administered intravenously on days 1 and 8 of repeated 3-week cycles. Twenty patients were analyzed. A total of 148 cycles of GC was administered, with a median of 8 and a range of 1 to 18 cycles. Treatment delay and dose reduction were noted in 35 (24%) and 41 (28%) of the 148 cycles, respectively. The major adverse events of grade 3 or 4 included neutropenia (50%), leukopenia (45%), anemia (30%), and thrombocytopenia (15%). Nonhematologic toxicities included nausea (10%), appetite loss (10%), and fatigue (10%). Median progression-free and overall survival times were 6.9 and 12.3 months, respectively. Gallbladder cancer showed a significantly higher response rate than did other types of BTC (chi-squaretest, P = 0.002). GC was thus effective and well tolerated as first-line chemotherapy for Japanese patients with advanced BTC in the clinical practice setting.

While fatty acids play essential roles in the physiology of the myocardium,conventional culture media contain little *** previously revealed that rat neonatal myocardium mainly contains docosahexaenoic(DHA),linoleic(LA),and arachidonic(AA)acids as polyunsaturated fatty acids(PUFAs),and these contents in cultured cardiomyocytes derived from fetal rats were markedly lower than those in the neonatal *** this study,we first assessed the effects of supplementation of DHA,LA,or AA on the fatty acid contents and the percentage change of contractile area in primarily cultured rat *** on this assessment,we then evaluated the effects of DHA or AA supplementation on mRNA expression and further directly measured the contractile force of cardiomyocytes with the *** study revealed that percentage change of contractile area was maximized under 20μM DHA or 50μM AA supplementation while LA supplementation did not affect this contraction index,and that a widespread upregulation tendency of the mRNA expression related to differentiation,maturity,fatty acid metabolism,and cell adhesion was seen in the cultured cardiomyocytes with supplementation of DHA or *** particular,upregulation of the gene expression of cellular adhesion molecules connexin43 and N-cadherin were remarkable,whereas the effects on differentiation and maturation were less ***,the increase of the percentage change of the contractile area of cardiomyocyte clusters in culture dishes with the supplementations was significant,whereas the enhancement of the contractile force was *** results suggest that supplementation of DHA or AA to the fetal cardiomyocyte culture may play effective roles in preventing the de-differentiation of the cardiomyocytes in culture and that the enhancement of the contractile performance may be mainly attributed to the improvement of intercellular connection.

Between June 2015 and June 2017,two pressure-recording inverted echo sounders(PIESs)and five current and pressure-recording inverted echo sounders(CPIESs)deployed along a section across the Kerama Gap acquired a dataset of ocean bottom pressure records in which there was significant 21-day variability(Pbot21).The Pbot21,which was particularly strong from July-December 2016,was coherent with wind stress curl(WSC)on the continental shelf of the East China Sea(ECS)with a squared coherence of 0.65 for a 3-day time lag.A barotropic ocean model demonstrated the generation,propagation,and dissipation of *** modeled results show that the Pbot21 driven by coastal ocean WSC in the ECS propagated toward the Ryukyu Island Chain(RIC),while deep ocean WSC could not induce such *** the continental shelf,the Pbot21 was generated nearly synchronously with the WSC from the coastline to the southeast but dissipated within a few days due to the effect of bottom *** detection of Pbot21 by the moored array was dependent on the 21-day WSC patterns on the continental *** Pbot21 driven southeast of the Changjiang Estuary by the WSC was detected while the Pbot21generated northeast of the Changjiang Estuary was not.

Coronary Artery Bypass Surgery for Patients with Severe Cerebrovascular Atherosclerosis: Importance of Preoperative Assessment and Perioperative Management

Objective: Cerebrovascular atherosclerosis is known to play a crucial role in perioperative stroke in coronary arterial bypass grafting (CABG). This study is to identify the degree of severity of cerebrovascular lesions for which patients can still undergo CABG with an acceptably low risk in current techniques. Methods: Cerebrovascular atherosclerosis was evaluated and graded for 200 consecutive patients prior to CABG. Grading was initially based on the level of stenosis in carotid, vertebral, and cerebral arteries: grade-0: normal or mild stenosis in cerebral arteries or stenosis <50% in other arteries;grade-1: moderate in cerebral arteries or 50% - 69% in others;grade-2: severe in cerebral arteries or 70% - 89% in others;grade-3: occlusion in cerebral arteries or 90% - 100% in others. The grading was finally adjusted to a risk of regional ischemia by considering symptoms, number of lesions, and brain perfusion in scintigram. Therefore, some patients were up-graded. Off-pump CABG was scheduled for all patients. The lowest systolic arterial pressure during surgery was differently controlled in each grade: grade-0: ≥70 mmHg;grade-1: ≥80 mmHg;grade-2: ≥80 mmHg with intra-aortic balloon pump (IABP);grade-3: ≥90 mmHg with IABP;grade-4: ≥90 mmHg with IABP and administration of thyamiral. Results: Grade-1 and -2 included 38 and 29 patients respectively. Grade-3 initially included 36 patients and 14 of them were up-graded to grade-4 (extremely high risk patients). Stroke was seen in one patient (0.5%), for whom mild speaking disturbance occurred on the fifth day from CABG. Conclusion: Patients with severe cerebrovascular atherosclerosis can undergo CABG with a low risk of stroke. Intraoperative management of blood pressure may be critical for stroke prevention in CABG.

Carbohydrate Intake Is Correlated with the Glycated Albumin to Glycated Hemoglobin Ratio in Drug-Naive Patients with Type 2 Diabetes

Background: The glycated albumin (GA) to HbA1c ratio (GA/HbA1c ratio) has been reported to reflect postprandial hyperglycemia. Carbohydrate is the primary dietary macronutrient that causes postprandial hyperglycemia. Thus, we investigated whether carbohydrate intake was associated with the GA/HbA1c ratio in patients with type 2 diabetes. Methods: Daily energy intake and carbohydrate intake were estimated in twenty-two patients with type 2 diabetes who received no pharmacological therapy (18 men and 4 women, age 53 ± 11 years old). The energy index and the carbohydrate index were defined as the ratio of daily energy intake to body weight and daily carbohydrate intake to body weight, respectively. Results: The energy index was significantly correlated with the GA/HbA1c ratio (r = 0.451, p = 0.035), but not with fasting plasma glucose (FPG), HbA1c and GA. The carbohydrate index was significantly correlated with GA (r = 0.461, p = 0.031) and the GA/HbA1c ratio (r = 0.554, p = 0.007), but not with FPG and HbA1c. Multivariate analysis revealed that the carbohydrate index was independently associated with the GA/HbA1c ratio (β = 0.397, p = 0.046). Conclusions: The carbohydrate index was significantly correlated with GA and the GA/HbA1c ratio in the patients with type 2 diabetes. These results suggest that carbohydrate intake may be associated with the GA/HbA1c ratio through postprandial hyperglycemia.

Background: Capecitabine and irinotecan combination therapy(XELIRI) has been examined at various dose levels to treat metastatic colorectal cancer(m CRC). Recently, in the Association of Medical Oncology of the German Cancer Society(AIO) 0604 trial, tri?weekly XELIRI plus bevacizumab, with reduced doses of irinotecan(200 mg/m^2 on day 1) and capecitabine(1600 mg/m^2 on days 1–14), repeated every 3 weeks, has shown favorable tolerability and eicacy which were comparable to those of capecitabine and oxaliplatin(XELOX) plus bevacizumab. The doses of capecit?abine and irinotecan in the AIO trial are considered optimal. In a phase I/II study, XELIRI plus bevacizumab(BIX) as second?line chemotherapy was well tolerated and had promising eicacy in Japanese ***: The Asian XELIRI Projec T(AXEPT) is an East Asian collaborative, open?labelled, randomized, phase Ⅲ clinical trial which was designed to demonstrate the non?inferiority of XELIRI with or without bevacizumab versus standard FOLFIRI(5?fluorouracil, leucovorin, and irinotecan combination) with or without bevacizumab as second?line chemo?therapy for patients with m CRC. Patients with 20 years of age or older, histologically conirmed m CRC, Eastern Coop?erative Oncology Group performance status 0–2, adequate organ function, and disease progression or intolerance of the irst?line regimen will be eligible. Patients will be randomized(1:1) to receive standard FOLFIRI with or with?out bevacizumab(5 mg/kg on day 1), repeated every 2 weeks(FOLIRI arm) or XELIRI with or without bevacizumab(7.5 mg/kg on day 1), repeated every 3 weeks(XELIRI arm). Serum metabolome profiles characterized by patients with hepatocellular carcinoma associated with hepatitis B and C

AIM: To clarify the characteristics of metabolite profiles in virus-related hepatocellular carcinoma(HCC) patients using serum metabolome analysis. METHODS: The serum levels of low-molecular-weight metabolites in 68 patients with HCC were quantified using capillary electrophoresis chromatography and mass spectrometry. Thirty and 38 of the patients suffered from hepatitis B virus-related HCC(HCC-B) and hepatitis C virus-related HCC(HCC-C), ***: The main metabolites characteristic of HCC were those associated with glutathione metabolism, notably 13 γ-glutamyl peptides, which are by-products of glutathione induction. Two major profiles, i.e., concentration patterns, of metabolites were identified in HCC patients, and these were classified into two groups: an HCC-B group and an HCC-C group including some of the HCC-B cases. The receiver operating characteristic curve for the multiple logistic regressionmodel discriminating HCC-B from HCC-C incorporating the concentrations of glutamic acid, methionine and γ-glutamyl-glycine-glycine showed a highly significant area under the curve value of 0.94(95%CI: 0.89-1.0, P < 0.0001).CONCLUSION: The serum levels of γ-glutamyl peptides, as well as their concentration patterns, contribute to the development of potential biomarkers for virus-related HCC. The difference in metabolite profiles between HCC-B and HCC-C may reflect the respective metabolic reactions that underlie the different pathogeneses of these two types of HCC.
