Efficacy of omarigliptin,once-weekly dipeptidyl peptidase-4 inhibitor,in patients with type 2 diabetes

In this paper, the authors analyze the influence that connection of many distributed generators （DGs） has on the harmonics of the distribution network from the viewpoints of harmonic current, harmonic voltage, and voltage total harmonic distortion （THD）, when it is assumed that the harmonic voltages or the harmonic currents in a distribution network increases by connection of many DGs through the inverters. The analysis on the influence of DGs is carried out by using a standard analytical model of distribution network with DGs based on the practical data. The authors evaluate quantitatively the influence which harmonics generated from DGs has on the harmonics of the distribution network, and analyze visually about propagation of harmonics within the distribution network. In addition, the authors also verify about the restraint effect of the harmonics in the network scale by the active filters installation.

For square contingency tables with ordered categories, the present paper considers two kinds of weak marginal homogeneity and gives measures to represent the degree of departure from weak marginal homogeneity. The proposed measures lie between –1 to 1. When the marginal cumulative logistic model or the extended marginal homogeneity model holds, the proposed measures represent the degree of departure from marginal homogeneity. Using these measures, three kinds of unaided distance vision data are analyzed.

Amyloid fibrils are deposited in various tissues in the body, and are linked to the putative causes of serious diseases such as amyloidosis. Although the conditions of the disease would be expected to improve if the fibril structure could be destroyed, the aggregated structure is stable under physiological conditions. Recently, we found that the amyloid fibrils of lysozyme could be refolded into their active form by using a mid-infrared free-electron laser (MIR-FEL) tuned to the amide I band (corresponding to the C=O stretch vibration), with the MIR-FEL having specific oscillation characteristics of a picosecond pulse structure, a tunable wavelength within mid-infrared frequencies, and high photon density. In the study, we tested the usability of the FEL for dissociation of aggregates of pathological amyloid fibrils by using a short peptide of human thyroid hormone. The fibrils (after being placed on a glass slide) were irradiated using the FEL tuned to the amide I band (1644 cm?1), and those in situ were analyzed by Congo-Red assay, scanning-electron microscopy, and transmission-electron microscopy. All of the results obtained using these microscopic analyses indicated that the amyloid fibril formation was considerably decreased by FEL irradiation. Moreover, upon irradiation, a strong fibril peak at the amide I band in the infrared spectrum was transformed into a broad peak. These results imply that the β-sheet-rich structure of the amyloid fibrils changed into non-ordered or unspecified structures after the FEL irradiation. This FEL irradiation system, combined with various analytical methods, shows promise for the dissociation of amyloid aggregates.

In the future, the power quality will decrease by the introduction of a lot of renewable energy sources. The topic of this research is a new method of operation of PCS （power conditioning systems） in the future distribution system. The purpose of this research is development of PCS with a function of improvement of the distribution system. Therefore, the authors propose a method of the power quality improvement of the distribution system by PCS. In addition, the authors construct the control logic to use in PCS The control logic suggests adding harmonic restraint function to conventional control. These were verified by simulation and an experiment. As the results, we confirmed that basic operation of PCS being carried out, harmonics were restrained, and power quality had improved.

Air Purification Using Mists Generated by an Efficient Multi-fluid Mixer

Sadatomi and Kawahara developed a special twin-fluid atomizer called a multi-fluid mixer, which can spray mists by supplying air alone because water is automatically sucked by a vacuum pressure arisen inside the mixer. In the present paper, firstly, some applications of the mists sprayed by the atomizer are described. Secondly, the performance of the twin-fluid atomizer with best performance was compared with those of the commercial twin-fluid MMA100 type together with the single-fluid swirl type. In the CO2 adsorption tests, mists were sprayed five minutes by the respective types in turn in a test room, and time variations of CO2 concentration in air after the introduction of CO2 in the room were measured at the bottom of the room to compare the CO2 adsorption rates by the mists for the respective cases. In addition, diameters of droplets for the respective types were measured with a microscope. As a result, superiority of our twin-fluid atomizer was confirmed, because 40% droplets were 20 to 40 μm in diameter, and the CO2 adsorption rate by the mists with our twin-fluid atomizer was 25% higher than that with the commercial ones.

Effects of a Stress Management Program Based on Psychological Risk Factors of Cardiovascular Disease after Retirement in an Underpopulated Area: A Pilot Study

Background: To develop an effective health education program to prevent cardiovascular disease in middle-aged residents after retirement in underpopulated areas, we explored the effects of a stress management program based on the type A behavior pattern. Methods: This study was carried out in a rural city in Japan recognized as underpopulated and participants were civil servants aged 45 - 64 who joined a stress management program offered as part of staff training. Learning materials for the program were developed based on the type A behavior pattern. Measures for the impact evaluation were Bloom’s learning domains and stage of change for stress management practice. Measures for the outcome evaluation were KG’s Daily Life Questionnaire (KG Questionnaire), the Hospital Anxiety and Depression Scale (HADS) and the Framingham 10-year cardiovascular risk score (CVD risk score). We statistically analyzed changes in each item between time points. Results: Eighteen participants completed questionnaire surveys at pre-, post-, and 4 weeks post-program and eleven had complete blood pressure and weight measurements at pre- and post-program. In the impact evaluation, the Friedman test found significant differences between the three time points in all of Bloom’s learning domain scores and stage of change for stress management. In the post hoc analysis, a significant increase was seen between pre- and post-program and between pre- and 4 weeks post-program in cognitive domain score, psychomotor domain score and stage of change for stress management. In the outcome evaluation, a significant decrease in systolic blood pressure was seen between pre- and post-program. Conclusion: The present study suggested that a stress management program using learning materials based on type A behavior could promote stress management practices and reduce the risk of cardiovascular disease. This stress management program is expected to be useful as a health promotion activity for middle-aged residents

Aims: This study was designed to verify the proportion of Japanese adults with pervasive developmental disorder (PDD) who met the diagnostic criteria (other than E) for attention-deficit/hyperactivity disorder (ADHD) in the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, Text Revision (DSM-IV-TR). Furthermore, we examined to what extent adults with PDD think that they exhibit ADHD symptoms. Methods: We developed an original Japanese self-report questionnaire to determine the presence or absence of 18 symptoms from the diagnostic criteria for ADHD in the DSM-IV-TR. We administered the questionnaire to 64 adults with high-functioning PDD (45 men and 19 women) and 21 adults with ADHD (10 men and 11 women), aged 18 to 59 years, with a full-scale intelligence quotient ≥75. Target patients were evaluated for ADHD by their psychiatrists. Results: Twenty-nine (45.3%) adults with PDD also had ADHD. The percentage of these adults who had over six perceived inattention symptoms from the DSM-IV-TR was 96.6%. The percentage of these adults who had over six perceived hyperactivity-impulsivity symptoms was 65.5%. Thirty-five (55.6%) adults with PDD responded that they were aware of having ADHD symptoms at the level of the relevant diagnostic criteria. Conclusions: The present study is the first to examine the frequency of objective and perceived ADHD symptoms in adults with PDD in Japan. Our results show that both objective and perceived ADHD symptoms frequently appear in a large number of adults with PDD. This suggests that it is necessary to attend to concomitant ADHD symptoms in the medical care of adults with PDD.
