Background:Despite therapeutic advances,treatment of patients with axial spondyloarthritis(axSpA)continue to pose as a challenge as many do not respond well to conventional Western medications,such as nonsteroidal anti-inflammatory drugs(NSAIDs)and biologic diseasemodifying antirheumatic drugs(bDMARDs).Hence,acupuncture is a possible *** studies found electroacupuncture to be better than manual acupuncture,though no trials have been conducted in patients with *** clinical trial aims to evaluate the clinical efficacy,safety,and cost-effectiveness of electroacupuncture compared to manual acupuncture for patients with ***/Design:This randomized controlled trial will recruit 100 patients diagnosed with axSpA,who have active disease despite NSAIDs and *** patients will be randomized to receive either electroacupuncture or manual acupuncture in a 1:1 *** participants will receive standard rheumatologic care in addition to 20 acupuncture *** mean difference in Bath Ankylosing Spondylitis Disease Activity Index score between the 2 groups over 12 weeks will serve as the primary *** outcomes include improvements in other clinical,quality of life,and economic outcomes over 24 *** adverse events will be ***:Results from this trial may provide evidence regarding the clinical effectiveness,safety,and cost-effectiveness of electroacupuncture compared to manual acupuncture for patients with axSpA,and guide implementation into clinical *** of this trial include the lack of patient blinding,use of a repeated measures design,and possible variation in acupuncture technique amongst the various Traditional Chinese Medicine practitioners.

Self-expandable metallic stents for malignant biliary obstruction:Efficacy on proximal and distal tumors

AIM： To compare the efficacy of self-expandable metallic stents （EMS） in the treatment of distal and proximal stricture of malignant biliary tumors. METHODS： From March 1995 to June 2004, 61 patients （40 males, 21 females） with malignant biliary obstruction who received self-expandable metallic stent implantation were reviewed retrospectively. The stents were inserted by an endoscopic or percutaneous transhepatic method. We tried to place two stents in the biliary system in T or Y configuration in cases of hilar tumors with bilateral hepatic duct obstruction. The end points of the study were stent occlusion or patient death. RESULTS： The mean time of stent patency was 421 ± 67 d in the group of proximal stricture（ group Ⅰ） and 168 ± 18 d in the group of distal stricture （group Ⅱ）. The difference was significant in borderline between the two groups （P = 0.0567）. The mean survival time was 574 ± 76 d in group I and 182 ± 25 d in group II. There was a significant difference between the two groups （P = 0.0005）. CONCLUSION： EMS implantation is a feasible, palliative method for unresectable malignant biliary obstruction. The clinical efficacy of EMS in patients with proximal hilar tumors is better than that in patients with distal tumors.

BACKGROUND Complications associated with upper gastrointestinal(UGI)endoscopy are uncommon,and rarely involve those of cardiovascular *** report herein a unique case of spontaneous superior mesenteric artery dissection(SMAD)after UGI *** SUMMARY A 45-year-old man who had previously undergone UGI panendoscopy and colonoscopy during a voluntary health check-up at our facility was admitted to the emergency room(ER)at the same facility due to persistent epigastric pain with radiation to the *** the ER,the patient did not present notable abnormalities upon physical,conscious,or laboratory examinations apart from mild tenderness in the epigastric *** abdominal aortic dissection was suspected,and abdominal contrast-enhanced computed tomography confirmed *** was then transferred to the cardiovascular ward and treated conservatively with fasting,prostaglandin E1,and *** patient recovered and returned home soon after,and was symptom-free 6 months after discharge from the *** SMAD after UGI panendoscopic procedure is a previously unreported *** of this complication and associated sequela is warranted.

目的评介隆胸术治疗轻度漏斗胸的临床效果。方法2000年1月至2008年1月对15例女性轻度漏斗胸患者,年龄18～35岁,在双侧腋窝横皱襞内各作4cm切口直达皮下组织,随后向内分离至胸大肌外侧边缘,切开胸大肌肌膜,用手指钝性分出胸大肌后间隙,用乳房分离器插入间隙广泛分离至设计范围再置入乳房毛面硅凝胶假体。结果15例患者通过腋下入路行隆胸术后,随访1～9个月,均未见局部皮肤坏死、感染等并发症,前胸壁漏斗胸状畸形明显改善。结论应用隆胸术治疗轻度漏斗胸,具有操作方便,手术损伤小,效果满意等优良,可在临床推广应用。

BACKGROUND In the development of coronary stent technology,bioresorbable scaffolds are promising milestones in improving the clinical treatment of coronary artery ***“leave nothing behind”motto is the premise of the fourth revolution in percutaneous coronary intervention(PCI).Studies proving the safety and efficacy of the magnesium-based resorbable scaffolds(MgBRSs)include the BIOSOLVE-I and BIOSOLVE-II trials and the latest BIOSOLVE-IV ***,spontaneous retrograde dissection of a partially absorbed MgBRS may still occur,albeit *** SUMMARY We describe an unusual case of coronary artery disease in a patient who had undergone a successful PCI 8 mo earlier,where an MgBRS was implanted into the left anterior descending artery(LAD)and left circumflex artery with drug-coated balloons for a ramus intermedius branch stenosis to achieve the“leave nothing behind”therapeutic intention and was currently presenting with a gradual worsening of chest *** distal edge vascular response,during subsequent attempts with balloon angioplasty was performed ***,spontaneous retrograde dissection of a partially absorbed MgBRS in the LAD *** bailout stenting was performed with revascularization of the entry and exit sites created by spontaneous dissection and complete sealing of the intramural *** patient recovered well and was discharged after 2 d of *** followed up in August 2020(7 mo later),the patient showed uneventful *** Spontaneous retrograde dissection of a partially absorbed MgBRS was successfully treated using bailout sirolimus-eluting coronary stent strategy.

Intra-tumor heterogeneity(ITH) is a key challenge in cancer treatment, but previous studies have focused mainly on the genomic alterations without exploring phenotypic(transcriptomic and immune)heterogeneity. Using one of the largest prospective surgical cohorts for hepatocellular carcinoma(HCC)with multi-region sampling, we sequenced whole genomes and paired transcriptomes from 67 HCC patients(331 samples). We found that while genomic ITH was rather constant across stages, phenotypic ITH had a very different trajectory and quickly diversified in stage Ⅱ patients. Most strikingly, 30% of patients were found to contain more than one transcriptomic subtype within a single tumor. Such phenotypic ITH was found to be much more informative in predicting patient survival than genomic ITH and explains the poor efficacy of single-target systemic therapies in HCC. Taken together, we not only revealed an unprecedentedly dynamic landscape of phenotypic heterogeneity in HCC, but also highlighted the importance of studying phenotypic evolution across cancer types.
