Impact of postoperative sexual function on health-related quality of life after robot-assisted radical prostatectomy

Background: We investigated potential disparities in health-related quality of life, particularly concerning urinary function, between patients with preserved and those with impaired sexual function after robot-assisted radical prostatectomy (RARP). Materials and methods: Between December 2012 and April 2020, 704 men underwent RARP in our hospital. This study included 155 patients with a preoperative 5-item International Index of Erectile Function (IIEF-5) of >= 12 points and an assessable IIEF-5 at 12 months postoperatively. Health-related quality of life was assessed using the 8-item Short-Form Health Survey and Expanded Prostate Cancer Index Composite (EPIC) preoperatively and at 3, 6, and 12 months postoperatively. A logistic regression analysis and Wilcoxon rank sum tests were performed. Results: Patients were grouped according to the median IIEF-5 score 12 months after surgery: those with preserved sexual function (n = 71) and those with impaired sexual function (n = 84). The mental component summary of the 8-item Short-Form Health Survey was better in the group with preserved sexual function at 6 months postoperatively than in the group with impaired sexual function (p < 0.01). In the EPIC, the group with preserved sexual function performed better not only in the sexual domain but also in the urinary domain at all time points compared with the group with impaired sexual function (p < 0.01). In the comparison of the urinary subdomains of the EPIC, there were no significant differences in urinary function or incontinence, but there were significant differences in urinary distress and irritative/obstructive scores (p < 0.01). Conclusions: Patients with preserved postoperative sexual function after RARP showed better urinary function than those with impaired sexual function. Hence, preserved sexual function is closely associated with urinary function.

This paper reports the experimental and theoretical study of the surge occurred in prototyping an ultra micro centrifugal compressor. As the first step, the 10 times size model of an ultra micro centrifugal compressor having the 40 mm outer diameter was designed and manufactured. The detailed experimental investigations for the transient behavior of surge with several different values of B parameter were carded out. The experimental results during the surge were compared with those obtained by the non-linear lumped parameter theory in order to validate the effectiveness of the theoretical surge model for the micro centrifugal compressor. As a result, the quite different behavior of the surge appeared for the different values of B both in the experiment and in the analysis.

The major problems for the development of an ultra micro gas turbine system were discussed briefly from the stand point of the internal flow and the performance characteristics. Following to these, the development of ultra micro centrifugal compression systems for the ultra micro gas turbine is explained with the design and the manufacturing processes. The measured results of ultra micro centrifugal compressors are shown.

The purpose of this study is to suppress the fluctuations of surge by a simple passive control method. A bouncing ball system with different weight of the balls was tested. As a result, the bouncing ball system suppressed mild surge without the deterioration of the performance characteristics, and moved the initiation point of surge toward the lower flow rate when the steel ball of 15mm outer diameter was used. The present system was effective to suppress the surge and to widen the stable operating range of a centrifugal compressor when the ball was chosen appropriately.

背景与目的枕一颈．胸皮瓣在枕叶区有局部的皮肤蒂，皮瓣远端在胸肌区。枕．颈一胸皮瓣的一个缺点是皮瓣长度有限。为克服这一不足，可以使用加强灌注穿支的手法来扩大皮瓣的长度。本研究的目的是介绍加强灌注穿支的枕．颈．胸皮瓣在面颈部重建术中的应用，并讨论加强灌注穿支的手法和穿支解剖学。方法回顾分析所有面颈部重建术中使用的加强灌注穿支的枕．颈一胸皮瓣。结果所有9例患者中，第二乳内动脉穿支与枕一颈．胸皮瓣尾部连接，对对侧面动脉受体血管加强灌注。皮瓣的平均大小为22．6cm×6．2cm，无任何皮瓣损失。利用薄且柔韧的局部皮肤组织覆盖延伸至对侧较大范围的缺损是可行的。所有患者术后6个月对功能和美观均满意。结论内乳动脉加强灌注穿支的枕。颈．胸皮瓣可当做双蒂穿支皮瓣的一种类型，且可提供可靠的皮瓣血供。通过使用加强灌注穿支的手法，我们可以根据缺损的形状调整并扩大皮瓣的长度。（循证医学证据等级：Ⅳ级，疗法）

Numerical Investigation of Loss Generation Mechanisms of Flow in Turbomachinery by Using Curved Square Duct

The secondary flow within a passage of turbomachinery exhibits a complex flow pattern by the effect of the centrifugal and the Coriolis forces. The passage vortex in this secondary flow generates a major part of the losses. However, the mechanism of the loss generation has not been fully clarified yet. In this point of view, the passage vortex is closely examined by the computational method using the two-dimensional curved square ducts as fundamental models. The inlet boundary layer thickness and the inlet velocity distortion are considered to be the major parameters affecting the generation of passage vortex in the present study. The computed results revealed that the passage vortex gave the predominant effects for the generation of loss not only in the breakdown process but also in the development process.

Pancreatic cancer still remains a major cause of cancer-related mortality;however,there is a slight but continuous improvement in survival over the past 2 *** in chemotherapy has contributed to the survival improvement in patients with any stage of pancreatic *** this review,we summarize the currently available evidence regarding adjuvant and neoadjuvant therapy with a focus mainly on randomized controlled *** median overall survival in resected pancreatic cancer patients has significantly improved to 22.8 to 54.4 months with the use of adjuvant therapy from 11 to 20.2 months with a strategy of observation *** data from randomized trials support the use of neoadjuvant therapy for patients with resectable or borderline resectable pancreatic *** given a variety of neoadjuvant regimens and different definitions of resectability status,data should be interpreted with *** other trials are ongoing and will provide further evidence.
