Evaluation of Antibacterial and Synergistic/Antagonistic Effect of Some Medicinal Plants Extracted by Microwave and Conventional Methods

The purpose of the study was to evaluate the efficiency of the microwave (MW) extraction method by comparing it with a conventional method through evaluation of antimicrobial and synergism/antagonism activity of each aquatic and ethanolic extracts samples, which were extracted from the dried plants (Ficus sycomorus leaves, Lawsonia inermis leaves and Glycerrhiza glabra Linn.). Nine samples of each plant for both solvents were irradiated with MW at several power outputs (180 w, 360 w, and 540 w) in several interval times (1, 2 and 3 minutes). The antibacterial activities of extracts and the synergistic effect between plants and antibiotics were evaluated using disk diffusion method against clinical isolated Staphylococcus aureus and Escherichia coli. The result revealed that the inhibition zone for more than 50% using aquatic and ethanolic samples results (extracted in two minutes and MW power 180 w) had shown the optimum extract and better antibiotic activity for each plant. Also, the results of ethanolic extracts used against selected microorganisms showed antimicrobial and synergistic effect with most antibiotics better than aquatic extracts. Our results indicate the possibility of using MW apparatus as an extractor to obtain bioactive compounds from plants and thus used in the treatment of bacterial infections, and some results of this study were encouraging. However, the antagonistic reactions of some extracts with some antibiotics and their use in combination should be further studied for in vitro activities. It is clearly a need to be furthermore evaluated, to identify the effective components, the mode of action and the possible toxic effect in-vivo of these ingredients.

Malignant melanomas or lymphoma of the skin are malignant tumors of the skin and/or the mucous membranes whose uterine metastases are rare. The secondary uterine localizations, although rare, must be evoked in front of a pelvic tumoral syndrome, or diffuse metastases and a personal past history of melanoma, even after a long time of remission. In our observation, the evolution of the tumor in the pelvis extended to the muscular structures of the uterus and the sigmoid colon of a 72 year old patient, what made it an exceptional case. The diagnosis of these secondary localizations is a diagnosis of elimination, almost always post-operative, made on the histopathological and immunocytochemical study of the surgical specimen, supported by cytogenetics, even molecular biology. The treatment is based on chemotherapy.

Objective: To report ocular complications of Rift Valley fever (RVF) during its first reported outbreak in southwest Saudi Arabia in autumn 2000. Design: Cross- sectional study of patients in a referral hospital. Participants: One hundred forty- three consecutive patients with confirmed RVF serologic test results and ocular lesions were enrolled in the study. Methods: Hospitalized patients (n=30) and outpatients (n=113) with clinical symptoms consistent with RVF, positive RVF serologic test results, and ocular abnormalities were studied. Ophthalmologic examinations, including fundus photography and fluorescein angiography, were performed. Patients were followed up at regular intervals to determine the prognosis and outcome of identified ocular abnormalities. Main Outcome Measures: Visual acuity at initial presentation and course of anterior and posterior segment complications. Results: Among 143 patients (78% males; mean age,53.2 years), 212 eyes were affected, comprising 47 eyes in 30 inpatients and 165 eyes in 113 outpatients. The mean interval between the onset of RVF and visual symptoms ranged from4 to 15 days (mean, 8.8 days). Macular or paramacular retinitis was identified in all the affected eyes (n=212) at the time of initial assessment. Lesions included retinal hemorrhages (40% ), vitreous reactions (26% ), optic disc edema (15% ), and retinal vasculitis (7% ). Anterior uveitis was present in 31% of outpatients. Fluorescein angiography of the retinitis showed early hypofluorescence with late staining of retinal lesions and blood vessels. Initial visual acuity was less than 20/200 in 80% of eyes in the outpatient group; their vision improved, deteriorated, or remained the same in 13% , 15% , or 72% , respectively. Evaluation at the last follow- up showed macular (60% ) or paramacular (9% ) scarring, vascular occlusion (23% ), and optic atrophy (20% ) in the outpatient group. Objective. To evaluate our clinical practice for Group B streptococcus (GBS) screening during pregnancy and antibiotic therapy during delivery, Material and methods. We performed a retrospective evaluation of our SBS screening protocol, by vaginal swab, for a period of two years, including 1,674 asymptotic patients. Intrapartum antibiotic prophylaxis was administrated for GBS-positive women. Results. The rate of GBS carriage was 6.9% . Antibiotics were administrated for 79,3% patients with GBS-positive culture. Of these women only 39% had an interval greater than four hours between antibiotic injection and delivery. In the group of patients with positive GBS culture, no newborn was infected and only 5% were colonised. In the GBS-negative group with 1.3% of the newborns were GBS positive. Conclusion. We were surprised by our low rate of GBS carriage and the non optimal application of antibiotics during labor. We reviewed our results with all our team and a prospective evaluation has been initiated.

Long term outcome and quality of life after pelvic exenteration for recurrent endometrial and cervical cancers

Introduction: Pelvic exenteration can cure recurrent gynecological malignancies. However, treatment morbidity is over 50% in radiated pelves. We evaluated the outcome, the morbidity and the quality of life after exenteration. Methods: Patients who underwent an exenteration for recurrent cervical or endometrial cancer in our institution between 1999 and 2011 were retrieved. Survival rates were calculated according to Kaplan-Meier method and compared using the logrank test. The EORTC QLQ-C30 and QLQ-CX24 questionnaires were sent to patients alive in January 2012. Results: We identified 25 patients: 17 (68%) had cervical cancer and 8 (32%) endometrial cancer. 92% of them had received radiotherapy previously. All patients had a central pelvic recurrence, within a median time of 30 months [4 - 384] from initial treatment. Early complications requiring a re-laparotomy occurred in 9 patients (36%). Late complications included 2 (8%) fistulas, 2 (8%) occlusion and 1 (4%) ureteral stenosis. Complete resection was obtained in 92% of patients. Disease Free and Overall survival rates were better in cervical rather than in endometrial cancer (median DFS in months 17 [2 - 145] vs 9.5 [3 - 21], p = 0.064, median OS in months 20 [2 - 145] vs 13 [4 - 42], p = 0.019). 69% of patients answered the quality of life questionnaires. Mean global quality of life score was 45 on a scale of 0 - 100, none of the patients had a sexual activity. Conclusions: Morbidity of exenteration remains high and quality of life is altered. Endometrial cancer is associated with a poorer prognosis. In those patients, exenteration should be put in balance with best supportive care.

The paper is about how Lebanese auditors detect fraud in the course of their work and what they advise companies to implement in order to avoid fraudulent acts. For this purpose, various interviews were carried out from different experienced and well-reputed external auditors. This was also for their wide knowledge of all kinds of frauds. Data were taken from primary as well as secondary resources. The paper presents the theoretical and practical aspects. The theoretical part contains the accounting scandals, frauds, auditing processes, and auditor"s responsibilities and tools for auditors to detect fraud. The practical part consists of case study analysis and detailed research processes.
