Oncologic outcomes with and without amniotic membranes in robotic-assisted radical prostatectomy:A propensity score matched analysis

Objective:Placement of human placenta derived grafts during robotic-assisted radical prostatectomy(RARP)hastens the return of continence and potency.The long-term impact on the oncologic outcomes remains to be investigated.Our objective was to determine the oncologic outcomes of patients with dehydrated human amnion chorion membrane(dHACM)at RARP compared to a matched cohort.Methods:In a referral centre,from August 2013 to October 2019,599 patients used dHACM in bilateral nerve-sparing RARP.We excluded patients with less than 12 months follow-up,simple prostatectomy,and unilateral nerve-sparing.Patients with dHACM(amnio group)were 529,and were propensity score matched 1:1 to 2465 patients without dHACM(non-amnio group)and a minimum follow-up of 36 months.At the time of RARP,dHACM was placed around the neurovascular bundle in the amnio group.Continuous and categorical variables in matched groups was tested by two-sample Kolmogorov-Smirnov test and Fisher's exact test respectively.Outcomes measured were biochemical recurrence(BCR),adjuvant and salvage therapy rates.Results:Propensity score matching resulted in two groups of 444 patients.Cumulative incidence functions for BCR did not show a difference between the groups(p=0.3).Patients in the non-amnio group required salvage therapy more frequently than the amnio group,particularly after partial nerve-sparing RARP(6.3%vs.2.3%,p=0.001).Limitations are the absence of prospective randomization.Conclusion:The data suggest that using dHACM does not have a negative impact on BCR in patients.Outcomes of cancer specific and overall survival will require follow-up study to increase our understanding of these grafts’impact on prostate cancer biology.

Purpose:To compare cartilage tympanoplasty(CT)combined with eustachian tube balloon dilatation(ETBD)and cartilage tympanoplasty alone as a surgical treatment modality for adhesive otitis media(AdOM)in terms of graft healing,audiological outcomes,and impact on life style,using Chronic Otitis Media Outcome Test 15(COMOT-15).Methods:50 patients with AdOM were randomly classified into 2 groups:25 patients for cartilage tympanoplasty only(CT group)and 25 patients for cartilage tympanoplasty combined with eustachian tube balloon dilatation(CT t ETBD group).Clinical outcomes in both groups were compared at 3 and 6 months of follow up.Results:There was no significant difference in graft healing between the two groups.Postoperative COMOT-15 scores significantly decreased in both groups with a significant difference between the groups with regard to the decrease in COMOT-15 scores at 3 and 6 months of follow-up(P<0.05).Hearing improvement was achieved,as the mean preoperative ABG was 26.5±5.4 and 27.1±4.6 dB,and the mean postoperative ABG at 6 months was 19.4±4.4 and 14.6±3.9 dB in the CT and the CT t ETBD groups,respectively.The difference in the magnitude of ABG reduction in the two groups was significant at 3 and 6 months of postoperative follow-up(P<0.05)in favour of the CT t ETBD group.Conclusion:ETBD can increase the success rate of cartilage tympanoplasty in patients with AdOM by enhancing the audiological outcome and quality of life.

Synthesis of silver nanoparticles using seeds of Nigella sativa as a capping agent was evaluated in this study. Different concentrations of the aqueous extract of N. sativa with silver nitrate solution were exposed to sunlight;as a force for acceleration of the formulation. Then the silver nanoparticles were characterized by UV-Vis, scanning electron microscope (SEM) and X-ray diffraction (XRD) techniques. Antibacterial activity of the nanoparticles was investigated against Staphylococcus aureus and Escherichia coli by the disc diffusion method. The characterization of nanoparticles was detected by the change in color to yellow-brown which indicated the formulation of silver nanoparticles. Irregular shapes within range of nanoscale were detected using SEM and XRD techniques. The finding suggests that silver nanoparticles may be effectively used as antibacterial agent.

3-Bromopyruvate(3BP) is a new, promising anticancer alkylating agent with several notable functions. In addition to inhibiting key glycolysis enzymes including hexokinase II and lactate dehydrogenase(LDH), 3BP also selectively inhibits mitochondrial oxidative phosphorylation, angiogenesis, and energy production in cancer cells. Moreover, 3BP induces hydrogen peroxide generation in cancer cells(oxidative stress effect) and competes with the LDH substrates pyruvate and lactate. There is only one published human clinical study showing that 3BP was effective in treating fibrolamellar hepatocellular carcinoma. LDH is a good measure for tumor evaluation and predicts the outcome of treatment better than the presence of a residual tumor mass. According to the Warburg effect, LDH is responsible for lactate synthesis, which facilitates cancer cell survival, progression, aggressiveness, metastasis, and angiogenesis. Lactate produced through LDH activity fuels aerobic cell populations inside tumors via metabolic symbiosis. In melanoma, the most deadly skin cancer, 3BP induced necrotic cell death in sensitive cells, whereas high glutathione(GSH) content made other melanoma cells resistant to 3BP. Concurrent use of a GSH depletor with 3BP killed resistant melanoma cells. Survival of melanoma patients was inversely associated with high serum LDH levels, which was reported to be highly predictive of melanoma treatment in randomized clinical trials. Here, we report a 28-year-old man presented with stage IV metastatic melanoma affecting the back, left pleura, and lung. The disease caused total destruction of the left lung and a high serum LDH level(4,283 U/L). After ethics committee approval and written patient consent, the patient received 3BP intravenous infusions(1-2.2 mg/kg), but the anticancer effect was minimal as indicated by a high serum LDH level. AIM To assess the levels of different immune modulators in patients with hepatocellular carcinoma(HCC),in relation to other hepatic diseases.METHODS Eighty-eight patients were included in the current study and represented patients with HCC(20),liver cirrhosis(28) and chronic hepatitis(CH;25),and normal controls(NC;15).Peripheral blood was isolated for immunophenotyping of active myeloid dendritic cells(m DCs;CD1 c and CD40),mature inactive myeloid cells(CD1 c and HLA),active plasmacytoid cells(p DCs;CD303 and CD40),mature inactive p DCs(CD30 and HLA),active natural killer(NK) cells(CD56 and CD161),active NK cells(CD56 and CD314) and inactive NK cells(CD56 and CD158) was done by flow cytometry.Serum levels of interleukin(IL)-2,IL-10,IL-12,IL-1β,interferon(IFN)-α,IFN-γ and tumor necrosis factor(TNF)-αR2 were assessed by ELISA.RESULTS Active m DCs(CD1 C+/CD40+) and inactive m DCs(CD1 c+/HLA+) were significantly decreased in HCC patients in relation to NC(P < 0.001).CD40+ expression on active p DCs was decreased in HCC patients(P < 0.001),and its level was not significantly changed among other groups.Inactive p DCs(CD303+/HLA+),inactive NKs(CD56+/CD158+) and active NKs(CD56+/CD161+) were not statistically changed among the four groups studied;however,the latter was increased in CH(P < 0.05).NKG2 D was statistically decreased in HCC,CH and cirrhosis(P < 0.001),and it was not expressed in 63%(12/20) of HCC patients.There was significant decrease of IL-2,IFN-α and IFN-γ(P < 0.001),and a significant increase in IL-10,IL-1β,and TNF-αR2(P <0.01,P < 0.001 and P < 0.001;respectively) in HCC patients.There was inverted correlation between IL-12 and IL-1β in HCC(r =-0.565,P < 0.01),with a strong correlation between p DCs(CD303+/CD40+) and NKs(CD56+/CD161+;r = 0.512,P < 0.05) as well as inactive m DCs(CD1 c+/HLA+) and inactive NK cells(CD56+/CD158+;r = 0.945,P < 0.001).CONCLUSION NKG2 D,CD40,IL-2 and IL-10 are important modulators in the development and progression of HCC.

BACKGROUND Chronic liver disease,particularly cirrhosis,is associated with worse outcomes in patients infected with coronavirus disease 2019(COVID-19).AIM To assess outcomes of COVID-19 infection among patients with pre-existing hepatitis C with or without liver cirrhosis.METHODS This multicenter,retrospective cohort study included all cases of confirmed coinfection of severe acute respiratory syndrome coronavirus 2 and chronic hepatitis C with or without liver cirrhosis who were admitted to six hospitals(Al-Sahel Hospital,Al-Matareya Hospital,Al-Ahrar Hospital,ahmed Maher Teaching Hospital,Al-Gomhoreya Hospital,and the National Hepatology and Tropical Medicine Research Institute)affiliated with the General Organization for Teaching Hospitals and Institutes in Egypt.Patients were recruited from May 1,2020,to July 31,2020.Demographic,laboratory,imaging features,and outcomes were collected.Multivariate regression analysis was performed to detect factors affecting mortality.RESULTS This retrospective cohort study included 125 patients with chronic hepatitis C and COVID-19 co-infection,of which 64(51.20%)had liver cirrhosis and 40(32.00%)died.Fever,cough,dyspnea,and fatigue were the most frequent symptoms in patients with liver cirrhosis.Cough,sore throat,fatigue,myalgia,and diarrhea were significantly more common in patients with liver cirrhosis than in noncirrhotic patients.There was no difference between patients with and without cirrhosis regarding comorbidities.Fifteen patients(23.40%)with liver cirrhosis presented with hepatic encephalopathy.Patients with liver cirrhosis were more likely than non-cirrhotic patients to have combined ground-glass opacities and consolidations in CT chest scans:28(43.75%)vs 4(6.55%),respectively(P value<0.001).These patients also were more likely to have severe COVID-19 infection,compared to patients without liver cirrhosis:29(45.31%)vs 11(18.04%),respectively(P value<0.003).Mortality was higher in patients with liver cirrhosis,compared to those with no cirrhosis:33(51.56%)vs 9(14.

The Lie group method is applied to present an analysis of the magneto hydro-dynamics（MHD） steady laminar flow and the heat transfer from a warm laminar liquid flow to a melting moving surface in the presence of thermal radiation.By using the Lie group method,we have presented the transformation groups for the problem apart from the scaling group.The application of this method reduces the partial differential equations（PDEs） with their boundary conditions governing the flow and heat transfer to a system of nonlinear ordinary differential equations（ODEs） with appropriate boundary conditions.The resulting nonlinear system of ODEs is solved numerically using the implicit finite difference method（FDM）.The local skin-friction coefficients and the local Nusselt numbers for different physical parameters are presented in a table.

AIM:To assess the utility of an autologous CD34 + and CD133 + stem cells infusion as a possible therapeutic modality in patients with end-stage liver diseases.METHODS:One hundred and forty patients with endstage liver diseases were randomized into two groups.Group 1,comprising 90 patients,received granulocyte colony stimulating factor for five days followed by autologous CD34 + and CD133 + stem cell infusion in the portal vein.Group 2,comprising 50 patients,received regular liver treatment only and served as a control group.RESULTS:Near normalization of liver enzymes and improvement in synthetic function were observed in 54.5% of the group 1 patients;13.6% of the patients showed stable states in the infused group.None of the patients in the control group showed improvement.No adverse effects were noted.CONCLUSION:Our data showed that a CD34 + and CD133 + stem cells infusion can be used as supportive treatment for end-stage liver disease with satisfactory tolerability.

AIM:To investigate the effects of mutations in domain Ⅲ of the hepatitis C virus(HCV)internal ribosome entry sequences(IRES)on the response of chronic HCV genotype 4a patients to interferon therapy.METHODS:HCV RNA was extracted from 19 chronic HCV 4a patients receiving interferon/ribavirin therapy who showed dramatic differences in their response to combination therapy after initial viral clearance.IRES domainⅢ was cloned and 15 clones for each patient were sequenced.The obtained sequences were aligned with genotype 4a prototype using the ClustalW program and mutations scored.Prediction of stem-loop secondary structure and thermodynamic stability of the major quasispecies in each patient was performed using the MFOLD 3.2 program with Turner energies and selected constraints on base pairing.RESULTS:Analysis of RNA secondary structure revealed that insertions in domainⅢ altered WatsonCrick base pairing of stems and reduced molecular stability of RNA,which may ultimately reduce binding affinity to ribosomal proteins.Insertion mutations in domainⅢwere statistically more prevalent in sustained viral response patients(SVR,n=14)as compared to breakthrough(BT,n=5)patients.CONCLUSION:The influence of mutations within domainⅢ on the response of HCV patients to combination therapy depends primarily on the position,but not the frequency,of these mutations within IRES domain Ⅲ.
